My 97 year old Mother spent the last 3 weeks here recovering from severe hydration which caused her fall. She was here 2 years ago. We were happy with the care she received, that's why we brought her back again. First of all, it's a very clean facility, small and personal, but most of all great staff. The nurses my Mom had were professional & helpful. The CNA's were amazing. The ones I will mention were the very best. Mario, Smile, Rain, and one special CNA on swing shift named Michelle. Jessica on the weekends was also an excellent help with my Mom, Nursing director Teri was extremely helpful, always available to help. Vanessa social worker was also fantastic at assisting with so much. The physical therapists & occupational therapists were also great. I hope my Mom never has to go back, but if she ever did, Legacy would surely be our number 1 pick. Much love & thanks to the staff. Horrible communication and lacking in patient care. I would come to see my mother after lunch to find that she didn't have her hearing aid in her ear and her dentures were still in the case. which had dirty water in it. I had to complain to severl people before anyone would do anything about the moldy water they were storing her dentures in. Very few people here care. Good luck trying to get someone on the phone "They're in a meeting and will call you back as soon as it's over" was the standare lie I was told. There was no set level of care at which my mother would be released so as long as they could bill for her she was stuck in a hostage situation. My mother's health declined horribly while here, she lost a lot of weight and she died a month after they FINALLY agreed to release her. read more

From the moment I left the hospital your staff made me feel comfortable and at ease. Every single Legacy department was professional, caring and compassionate-not only with me but with my visitors too. I would rate this ZERO stars! This review is to give my feedback of the facility my brother is being held in. First and foremost, I am hurt and saddened by the amount of trauma that my brother is being put through at this facility. He experiences emotional distress everyday here and tells me about it. I, myself have been to the facility to see for myself and believe every word he has told me. He has complained to me about the nurses never being around to assist my brother when he needs something. I have asked the nurses myself questions about my brothers follow ups on how his day went or his medications and the nurses tell me that "they don't know because they were on break" or because "they just started their shift" I had an incident where my brother just last night was extremely hot in his room and had opened his window around 2:45am to let some air in and he says a hug air of hot air hit his face when he opened it and he felt like he was being choked and couldn't breathe for 15 seconds. He lets me know that there is no hot water when he showers twice a week at the facility and says by the time he's done showering the hot water starts coming out. He's always complaining about how cold it is in the room, and I've had to bring a heated blanket to make sure he's comfortable. This is a basic necessity and impacts my brothers well-being and hygiene. It violates his rights to having a safe and habitable living environment while he is staying here for the time being. I urge potential residents and their families to consider these issues when evaluating this facility. No one should be able to experience any of these after a major heart surgery! I ended up contacting the CEO of the company (Joseph, Tom),. I did not know what else to do at that point, and brought the issues to him. He said he would call me back and he would give a few calls (assuming to staff). I called him back an hour later to follow up and it was a completely different tone. He sounded frustrated and told me I needed to complain to the other people at the facility. He said there was a chain of commands all before him and why don't I take it up with them. It was very heart breaking to hear this from him (literally cried after out of frustration). He kept saying I needed to attend the conference and address my issues there, but my mom had already brought up these issues to the staff at the facility. The CEO was saying how hard his staff worked, and I was not picking at his staff on purpose, but just mentioned how frustrating it was not having the nurses around when needed especially after major heart surgery. I have came across one of the worst encounters with this CEO and was hoping to look for answers or help for my uncle, but left back at square one with nothing. At this point if this is what a CEO is supposed to treat others then I am completely in shambles and worry about my future healthcare and what that will look like if I ever end up in a facility like this one. read more

My mom was sent to Legacy after a week in the hospital with the flu that made her extremely weak and unable to get up or walk. She was in isolation for 5 days and remained for about 4 weeks overall. I can't speak more highly about the staff and quality of care they provided her. They were patient to an often difficult patient. She was difficult to motivate for physical therapy but they had a lot of little tricks they used to get her to participate. She was extremely happy with the food and her care. They work with me when issues arose (like her lack of cooperation). She came to the facility with a small sore and the staff immediately informed me of it and the care they would take for it and then updated me on it. Additionally, they helped coordinate her aftercare at home. Can't speak more highly of the entire staff including reception, admissions, physical therapy, occupational therapy, daily care and dietary (hope I didn't miss anyone). I have had both my mother and father at other facilities and will now only use Legacy. Can't give a stronger recommendation! read more

The nurses, CNA's administrative, just the entire staff has so much compassion and love for the patients. They have shown so much care to him. I came to Legacy Post-Acute Care after my hip replacement, and I'm so glad I did. This was my first time in a skilled nursing facility, and I didn't know what to expect. When I arrived, I couldn't walk. But thanks to the incredible therapy team and the support of the entire staff, I'm now leaving fully independent. The care here is amazing. The staff are friendly, attentive, and always willing to help. Any time I pressed the call light, someone was there right away--usually more than one person. The food was tasty, and the therapy team went above and beyond to get me back on my feet. I would recommend Legacy to anyone. When first coming into the facility, the first thing one notices is the clean and fresh smell! After that, I found the attitude surrounding me was joy, the willingness to be of service. The staff was so eager to help in anyway possible. The Dietician, Raquel, was certain to meet my dietary needs within her scope of possibilities. The food was very tasty, well prepared and served within reasonable times stated for meals. I was most often delighted with my meals... delicious! Cnas are often overlooked they are the core of this.facility making the flow of everything else work. Their job is not simple with each one dealing with patients with so much respect and patience.With first hand experience watching their tenderness towards handling some difficult situations I have learned that the happiness of patients start with the CNAs. I have watched the bonding of CNAs with patients so much so that I have heard the laughter and even happy tears show when a patient sees or hears that special CNA. Their jobs of keeping patients clean and SAFE are just jobs. It's the tender caring, understanding and patience that TRULY are the core of making the flow of care work here. I cannot say enough good thing about this facility. My mom has been there a few times for rehabilitation after a hospital stay. The nurses are so caring and attentive. The kitchen is so accommodating to my mom's celiac disease that requires a special diet. And the regular meals are also fantastic! The social worker keeps you very informed with regular meetings. This truly is a fabulous place for aging parents who may need part time assistance to recover, or full time when the time comes. I highly recommend Legacy. read more

Legacy has great staff. When you walk in the facility you're greeted with a smile. The place is clean and well maintained. Whenever there is an issue, their staff are quick to respond. The call lights may go off during the busy hours, but you will see every one of the staff members answer them. They may be the only family-owned business out there, it's not like a corporation where they say, "see ya later". They care for their patients, even resources to help out when my grandpa was ready to come home. When I leave this place, I know that my loved one is well taken care of. I'm going to be very honest about my 7 week stay at this facility as I feel it's very important. After 4 serious breaks to my right leg and hip over the last 5 years, this was my 4th stay at a physical rehab facility, and my 1st time at Legacy. I had been laying in my bed at home for almost a year, gaining 70lbs which made me severely obese. My muscles were in very bad shape and I could barely walk. By a big miracle I found out from Stephanie, the Director of Admissions, that Medicare would cover my stay without spending 3 days in a hospital, due to a special Covid law that ended 2 weeks later. She had a bed waiting for me with an amazing view of the garden from my sliding glass door that opened onto the patio. At my age of 64 I was a little afraid of being in a strange place, not knowing anyone. From the moment I was checked in, the staff was so friendly and comforting that I was at ease on the 1st morning I woke up there! The physical and occupational therapists (Hazel, Ace, Nicole, Stephanie, Marivek, & Myla) began my intense therapies. This is by far the the best physical therapy I've ever received! I had it 6 days a week for 1&1/2 hours per day and it has changed my life! The medication nurses (Lulu, Jean, Sonny & Tamara, and a few names I can't remember) went above & beyond to make sure I had all of the medication I needed at the right times so that I could continue all my physical therapy each day without being in terrible pain. My room & bathroom were kept extremely neat & clean (which was very important to me) by 2 of the nicest&friendly ladies, Kimberly & Lena! But the CNA's were where this facility really shined (Angela, Moria, Nora, Evelyn, Larissa& a few I'm forgetting) quickly learned & remembered my quirky needs&wants. My big fear of having to be given a shower was overwhelming, but during the first one I was laughing and joking with her and the other ladies. They even put lotion all over me to get rid of my dry skin and anything else I needed! I was especially surprised by the Facility's Director Jeremy, who would jump in during very busy times, to deliver meals, come in to see what I needed & then take care it when I pressed my call button. His sense of humor and caring nature made me immediately feel comfortable during uncomfortable times! I would have given this facility 5 stars instead of 4 because of the 2 following reasons: In my opinion the food was not very good, sometimes very salty& the meat was dry and over cooked, and some times the meal on the pre printed weekly menu, was not the meal I received. But the kitchen manager made sure I was given only the specific foods that I liked and went along with my self imposed diet. Snacks were readily available and there were many options. My other complaint is that on 5 different occasions they ran out of my medications that were very important to me, this was because the contracted pharmacy had issues. Other than that I highly recommend this facility, as after 7 weeks I regained my muscle strength, my ability to walk and stand, and when I got home today I prepared my own meal which I have not done in over a year! read more

I've been here two weeks as a patient and my time here so far has been wonderful. The staff are so friendly and helpful! The administration staff is so accommodating! The kitchen staff are accommodating as well! I have so many wonderful things to say about everyone here. *this is my first stay in a SNF (skilled nursing facility) as a patient but I have been to two others where my grandmother was the patient and I was terrified to be a patient in a SNF because of the lack of care/cleanliness/friendliness. I saw with my grandmother with her experience. It was so bad we had to move her from one facility to another that we thought was better but by my standards was far less in what I found acceptable. She was begging to leave even days in because of the horrible care she received in two other SNFs in the Bay Area. Legacy has changed my mind about SNFs! It's so clean and they give you multiple showers through the week, and the food is good, the kitchen staff are so accommodating if you don't like what was ordered you can always ask for something off the main menu they have the best grilled cheese. You can order out and they'll bring your food to your room, and I love how everyone is allowed to roam the facility through out the day. We're not locked up in our rooms. I love the patio looking at the beautiful hills. They have a wonderful activities director named Sharon. She's so nice and has so many fun things planned everyday. One of my favorites is the live music with Andrew! He's great. The Physical therapy and Occupational Therapy is AMAZING!!! Better then outpatient and better than John Muir acute rehab! And that's saying something! The pt and ot's really listen to you and watch your body so closely to make sure you're doing as much as you can't but not pushing it to a limit where you're wiped out the next day and you get Pt and ot almost everyday. Which in my case I needed. I have felt I have gotten such better care here than I ever did in the hospital and I love John Muir Walnut Creek. They're a wonderful hospital with the best hospital food (just in case you were wondering) The care here is outstanding! The CNA's and Nurses are always on top of their work and never have I waited more than maybe 3 minutes to get someone to me after pressing the call button even when they're fully staffed. The only thing I would have to say about my stay that was by the best was breakfast is at 7am and I am NOT a morning person.. but I mean if that's the only downfall you NEED to visit this place! The rooms are huge in the short term hall. They have two people to a room or three people depending where you're staying in the facility but I've been moved three times because of needing room for male patients, a roommate not liking the same (dark and cold room that I need for healing). Some people just need it to be light in their rooms and want to see outside and I don't blame her. I just need it dark and cold for my recovery because I cannot regulate my body temperature and the staff here understand that and have accommodated me to a cold dark peaceful room. I have loved my stay so far and would recommend this place to Everyone looking into a Skilled Nursing and Rehabilitation Facility. You will not be disappointed! read more

I spent 66 hours there. Wednesday evening to Friday noon. Here are some scenes. I arrived after day shift left, so there was no one to greet me. The night shift left me in a bed that was too short, too narrow, and too hard, with a mug of water and no walker to get to the bathroom. At 2 AM the screaming started. NO, STOP, DON'T. I tried to leave by Uber the next afternoon, but was too weak to make it. I had to go back in and make nice. That night the screaming started again. And then my Spanish-speaking roommate had a crisis I couldn't understand, moaning and thrashing. I called for the nurse and we waited 21 minutes for help who were none too gentle when they did show. That's when I decided I had to leave. That could have been me. Around noon the next day I was gone. I went there expecting a rehab clinic, but it's a rest home. Maybe all the stuff that happened to me is normal for rest homes. I wouldn't know and hope I never find out. read more

Great staff. I was there recovering after a pretty severe ankle break. I had a roommate who was normal during the day but couldn't take any meds cause they made him sick. So after 10 nights of freak outs, cursing, breaking things, kicking down the bathroom door, ripping the curtains and rod off the wall, basically becoming a completely different person, I was removed from that room "for my own safety" and given a new room with a new roommate who had a nice family who would visit him daily and feed him. The nursing staff and doctors were great. Very nice, and the cooks in the kitchen made decent food, but then again they had to accommodate so many diets and special requests were always met. This was after a very bad experience at another place in Pleasant Hill which led me to call 911 so I could go back to John Muir hospital. AND THEY TAKE MEDI-CAL! read more

My father just got released from Legacy after 12 days there. they did a great job getting his strength back after covid. the food was fairly good and the did a fairly good job (except for the last night) getting his high blood pressure under control. No one felt he should be discharged but the insurance company. But they did have some advice for me Incase he needs to come back. I was so relieved he was there and they got him up and walking and able to do some stairs by the time he came home I just can't say enough good things about this place. read more

Beware of the care, especially if you have a family member that's a stroke victim. My mother was there for eight weeks. She came out in worse shape than when she got! I have come in there several times, and her oxygen isn't on. She's in bed with wet clothing and her tray with her water and Kleenex is across the room. They left her in wet and feces covered drawers, and they were kind enough to put them in their dirty clothes for me to take home and wash. She got a sore on the bottom of her foot from laying in bed against the bottom support system so long it caused a sore. She also had a sore on her bottom . Everybody was all concerned as we were leaving the facility. Unfortunately, the nurse practitioner that packed my mothers medicine did not send home her rescue inhaler and at 8 o'clock that night she's having a breathing episode at home and I have no rescue inhaler. I must mention there were a couple of people that gave outstanding care that would be Stephanie, the physical therapist, Bobo, the nurse, and a tall young lady. That's a housekeeper there that got the Director of nursing to listen to my complaints and concerns. read more

I was overly stressed to put my mom in the rehab. We had a very bad experience with the one in Walnut Creek a few years ago. On top, my mom doesn't speak English. From the very first phone call through the second week my mom is at Legacy our experience was nothing but pleasant! The staff is attentive and tries their very best to accommodate all the requests and keep the family informed. The social worker Stephanie is very responsive and approachable, shared her cell phone for direct access! Dr. Anthony Allen is regularly there, checking and adjusting whatever is needed. The nurses, physical and occupational therapists, CNAs are all working hard to make sure the needs of their patients are met. They are outstanding and amazing! I was called to find out my mom's diet preferences and activities she enjoys! My mom does not need to wait long after she pushes the call button. And to ease the language barrier the main words and phrases are printed along the pictures and could be used for reference at any moment. She is picky with her food choices but can eat the food served at Legacy which is a very good sign for me. I'm very grateful to all the staff at Legacy who made my mom's and my experience the least painful. Thank you! read more

I have been here at Legacy for two months and the staff is amazing! A variety of activities are available and the food is good. The care I have received here has been outstanding and the therapists are all excellent. The housekeeping crew is right on The job- the place is always clean and odor free. My father spent 3 years with Legacy Nursing and Rehab center, and I want to say they were all so incredibly kind, nurturing and caring for my father. There are horror stories about nursing homes and skilled nursing facilities, and I wasn't sure what to expect. My father came there via hospice declaration by Kaiser Walnut Creek. He got better, and it was determined he at a severe urinary tract infection. So kudo's to them right off the batt, nursing my father back from dear death! My dad is an incredibly difficult person to deal with, and once he recovered, is old self surfaced, and he quickly became what I am sure was a huge pain in the A.. for Legacy's entire staff! I could not have done it, for sure - dad was a nightmare my whole childhood.. I want to give a specific shout out to Jeanne Eastridge, Legacy's Social Services and Discharge head person. She is that kind of incredibly warm hearted person that an employer is lucky to have. You cannot train such incredible customer service and attitude!!! Jeanne was constantly harassed by my father to do things like drive him places, to call me and my brother to do so, on and on (things she need not have even paid attention to). She handled my father with grace and dignity, and was incredibly effective in calming him. My dad ultimately moved to join my mom in a skilled nursing facility she just transferred to that is much closer to us kids, and dad will miss Jeanne, and Dipa, the head administrator at Legacy, along with the entire, wonderful staff. My husband had been in Legacy in 2016 for recovery from an amputation. He was there for 6 weeks. This was the first SNF we had ever been in and it was old, in need of renovations but they were caring and the food was OK. We had nothing to compare but over the course of the next 6 years we unfortunately ended up in 3 other SNF's, two of them for more than one stay. We knew that we would never go back to any of those if it was necessary again. Well recently, he needed to be sent to another SNF and it was decided he would be going to Legacy. WOW!! Surprise!!! The center has been fully renovated, he was well cared for, the therapists worked with him daily. The staff was great. It was clean, no smell (a lot of other places were just nasty) and the food was good. It has been beautifully landscaped and had an outdoor covered patio and a family room. Should we ever need post-acute services again, Legacy will be our first choice. read more

I would not put my dog in that place! My mom was in there for 3 days and then she died. When I went to see her all they had on her was a sweat shirt and diaper. Nothing on her legs.The last day she was in there I was up there with her and I kept telling the nurse that something was wrong with her and she needed to go to the hospital.After a couple hours of telling them that they finally took her blood pressure and it was low and the nurse finally said yes she needs to go to the hospital. So they called tha ambulance and by the time they got there she passed away. If you want your loved one to die fast that's the place to put them!!! read more

Ended up having an awful experience and should have written about this right away. (This all happened just before COVID). Short version is there were a litany of mistakes that added up to a big problem. Things like the physical therapy department refusing to do PT and OT for 9 days, just letting our sweet 99 year old loved one waste away and get weaker and weaker; nursing staff not following up on a STAT order from a doctor to rule out pneumonia (after they called for an ambulance - it was that serious), repeatedly giving her the wrong food or food she was not able to cut/eat on her own, and would outright ignore her call button unless a family member was present and went to search for a nurse or nurse's aid. Many missed communications about orders between the facility and the doctor, with the facility saying it was the doctor's fault. After a worthless attempt at working with the ombudsperson, we finally reached out to the doctor directly and miraculously the problems went away. Spoke to the Director three different times and was left with promises of improvement with no action. She would smirk at me in the hallways, and go out of her way to say "hi". Frankly, I could care less about insincere greetings. Patients deserve the care and attention they are paying for, not lip service. Icing on the cake was at discharge: Our loved one's regular doctor had retired while she was in rehab for 2 months and she was having to be transported via medical transport which was extremely painful for her. We asked for meds for 45 days so that would give her time to heal some more before we had to find her a new doctor. Nurse (Supreet) said that the doctor would only prescribe for 30 days. Ok, that would buy us a little time. They ended up giving us only enough for 3 days without even telling us. When the pharmacy called the prescribing doctor, he said he couldn't extend the prescriptions since she had already been discharged. As we were waiting for med transport, PT refused to even teach us how to transfer a newly bound to wheelchair person in and out of bed until they realized how unsafe that would have been to discharge her, even though they had known two days prior that she would be discharged. My elderly mother received great care from the staff and Administration during her six week stay. The Martinez Facility was one of only two care facilities in the County without and Covid cases among patients or staff. Although, at the end of the day, the most important issue is care and respect of the patients, I gave 4 out of 5 stars rating due to a non-working, almost non-existence phone system. During Covid 19, where patients cannot have physical visits from family and friends, it is important that they are able to stay in touch by telephone. read more
