While it's not the Ritz the care here is wonderful. I've been a patient here twice and the staff is pleasant, caring and attentive.

Cam Jones and team, notably Matt and Jean who go above and beyond as activity directors and really care about the occupants have made what my mom calls "the not so golden years" easier. My mom has made friends and Cam has facilitated transportation when we needed it. Cam is also accessible anytime I have questions or requests. It's not a fancy place but my mother is comfortable. The food is just ok, but they are happy to make my mom a tuna sandwich if she doesn't care for the hot meal. When I was told a few years ago that I had two hours to choose a skilled nursing facility I jumped in my car and quickly toured a few. I chose La Casa Via where my grandmother stayed several years ago as well. I give this facility 5 stars not for being perfect but because this type of care is very hard and the facility is clean as well as the care and attention I mentioned above. read more

Our family is deeply grateful for the exceptional care and support we received during our time at La Casa Via Transitional Care where my husband was cared for. A special thank you to our Case Manager, Victoria, who guided us through each step of this process with compassion and professionalism. My husband was fortunate to work with a dedicated Physical Therapy & Occupational Therapy team led by Charlene. Through their dedication, patience and persistence, he was able to sit up, stand and walk - things he couldn't do when we first arrived. With their help my husband was accepted into the Acute Rehabilitation Unit. An amazing blessing! We spent a month and a half with this wonderful group, during that time the nursing staff, CNAs and executives Cam & Tim were consistently welcoming and helpful. It felt like a family setting. My dad was transferred here after his stay at the hospital for a spinal infection. This is one of incompetent people I have ever met. My dad needed help getting back into bed. He pushed the nurses button 6 times and they showed 45 minutes later. Rude people! Javier one of the "nurses" swore at my dad and called him an old man and lazy. This is not ok. If you are looking for a transitional care facility DO NOT pick this place. The only thing nice about this place is the landscape!!!! Keep looking. The staff here needs a lot more training on how to treat the patients there!!! read more

Awful place. not enough staff. night time CNAs were totally unresponsive. Had to wait 90 minutes to help me with my oxygen. Finally called the front desk to have them notify the staff of my critical need. apparently there is a class action lawsuit against them for failing to employ an adequate number of nursing personnel. not surprisingly. once when the power was shut off to businesses and residents in a few block radius for several hours, their generator didn't work. physical therapy was a joke. got Covid while there. I could go on and on... have steered many people in need of skilled nursing after hospital stay away from La Casa Via. read more

If I could rate 0 stars, I would. My grandfather stayed here for a couple months. The care he received here was the WORST. Javier is RUDE, the lady at the front desk is RUDE. Nobody cares about the patients here. Even at times, they would "forget" to give my grandfather his meds after losing his wife, something traumatic and they forget?! What kind of human decency is that?! Just overall, the care, compassion, and nurses need extensive work here. I would not recommend this place to ANYBODY. Also, not to mention, the sense of urgency for an emergency is definitely not up to par. This is Part 3, the (maybe) final part of my review of La Casa Via. My experiences with Physical (PT) and Occupational Therapists (OT), and members of the glorious staff was remarkable. In fact, I was surprised at how quickly and ready I was to go home. My physical and occupational therapists are the people who helped me learn to walk again in just over three weeks after I broke my hip and pelvis. Here I must mention the PTs with whom I spent countless hours. Accolades go to Roop, Deb, Imonie, Cynthia and Abigaile (a rehab aid). Cudos to my Occupational therapists Mios, Deb and Charlene. There are so many others to recognize here. Susana, Jit, Thomas, Joey, Derik worked hard to wash and dry 'tons' of laundry and to get it back to every patient at La Casa. Faith, Shana and other people cleaned the rooms and the everywhere floors. Hurrah to Cash, Maise and other 'food' workers in the kitchen. And my final thanks go to Gupreet, the amazing 'Nurse boss,' sweet Dana at the front desk, Lupita who processed my admissions forms, Cynthia (Director of Social Services), lovely Tanveer (Director of Nursing), and Arnell (Case Worker Manager). read more

This place is horrible for anyone on end of life care. The employees barely speak English, they don't follow medication protocols, and would rather jack up my father in law on morphine to kill him. They even said that the morphine is to slow his breathing hence in collapsing his lungs. Completely unacceptable. I would not recommend this place for anyone with advance or end of life care needs. We rushed him to the ER and he is thriving. read more

La Casa Via is truly exceptional when it comes to the staff. After my 82 year old mother broke her femur, the care she received was outstanding. Director Camalin Jones goes above and beyond her job description, personally catering to my mom's needs and making sure she felt comfortable and supported. The nurses, CNAs, and therapists were compassionate, professional and attentive. The rehab team helped my mom regain her strength with care and encouragement. I need to preface this review by saying that there are some exceptional workers at this facility. Namely, Alice and Melinda were kind, attentive nurses who truly seemed to care about how my mom was doing, and went above and beyond to care for her. Some context for my review: My mom was in Stage 5 kidney disease, suffered from anemia, and had diabetes when she fell and broke her hip on May 8, 2024. She was not yet at the stage of needing hospice care. So we decided to admit her into La Casa Via for rehab. We knew there was a chance though, that the need to go into hospice care would happen before she could walk again. Here is a list of the things that went wrong while my mom was at La Casa Via: 5/19/24: My mom was left alone in a closed room, in a braked wheelchair, without access to a call button for 1.5 hours. The PT had her sit up starting around 10:30am and told her they'd return at 11:30am. But the PT had failed to leave her with or near a call button, and also closed the door to her room. After an hour passed, my mom started to feel nauseous. A few minutes after the hour, she thought she was forgotten, started to panic, and tried her hardest to move herself closer to the door in order to open it and ask for help. Unfortunately, she was new to wheelchairs and did not know how to turn off the brakes, so she ended up exerting a lot of energy trying to move. Finally, at around noon, someone else came to give her lunch, saw her struggling and helped her back to bed. By that time, my mom was so nauseated and exhausted it basically messed up her appetite and energy for the rest of the day. I learned later that the PT had decided they would return to my mom after she ate lunch, and had communicated this to the lunch delivery person. *But no one communicated this change to my mom.* Plus, that means that the PT decided it was a good idea to extend the time to 1.5 hours without checking in on my mom. 5/23 & 5/30/24: The social services director failed to show up to two of my mom's "care" conferences. I had to hunt her down both times. The first time, she had no apology, only stating that she "lost track of time." 5/27/24: While doing a PT session, my mom experienced nausea again and started dry heaving. The PT encouraged her to keep going, saying to just use a wastepaper basket if she needed to throw up. This pushing only served to make my mom feel nauseated, making her not want to eat her lunch, and causing her blood sugar to plummet. My main complaint here is that the PT for this session did not seem to understand that my mom had other complicating factors, so just pushing her to keep going wasn't the most medically safe approach. 5/27/24: My mom requested a diaper change at night, and the attendant got angry with her for some reason, telling her she should have requested that change at 10:30pm prior to the change of shift. My mom did not know the shift schedule. Late night on 5/30/24: My mom fell again. This was entirely her fault, as she was trying to get to the wheelchair to go use the bathroom herself. I include it here, because it's important information for what follows. A few days after this fall, it became clear that my mom would need to transition into hospice care. 5/31/24: Even though my mom fell, a PT came and said it was time to do a session. This makes zero sense to me. No new assessment had been made on her hip. Shouldn't PT be held off until we knew the status of her hip after the second fall? 6/1/24: After waiting almost two days, we got x-ray results. But it was for the *wrong hip.* I was the one who happened to catch this error while reading the report. So we had to wait another day to get correct results. 6/4/24: In the morning, asked social services director for hospice care resources via text. No reply all day. This was bothersome because we were given a deadline of when my mom needed to be out of the facility, so time was truly of the essence, and I was told texting was the primary way to get in touch with her. Turns out she wasn't the person to ask (how was I supposed to know), but even a "try asking _____" would have been helpful. 6/10/24: Upon discharge, La Casa failed to include my mom's insulin in her med pack. I had to return to pick it up. Zero apology. There were additional minor issues (like nurses/PTs calling my mom "post-surgery," when she never had surgery on her hip) that I don't have room to spell out. My assessment is that this place is ill-equipped to handle patients with more complicated health profiles. If you/your loved one has to stay here, be watchful and ready to advocate hard on your/their behalf. read more

Sadly my dad has been at La Casa Via four times in the past four months, following falls, then trips to John Muir hospital, then across the street to La Casa Via. Normally his stays are two week stays. Dad really likes it there (when he's not able to go home), the care and attention are great, the food is way better than the hospital, and the staff is marvelous. It's also right across the street from the hospital. There is OT and PT at the facility. Cam, the admin, is super to work with and very responsive (text messages, phone calls, talking in person) read more

In my professional life working with hospice, I visited a lot of rehab centers and skilled nursing facilities, so when my mom fell and broke her hip, I was concerned about where she would go after leaving the hospital. But La Casa Via is more than I could have hoped for! Its staff is outstanding, starting with the Director of Clinical Care, Cam, who is always responsive to my in person questions or phone calls or texts regarding Mom's progress. The nurses and aides are wonderful, too, as are the physical and occupational therapists. La Casa Via deserves its 5 star rating. read more

My husband was there for 10 days and I never knew who his nurse was. He was never rolled over and ended up with 2 open sores. After he signed a DNR they took away all his medication and started giving him morphine. He would died the 2nd night except my family had him moved to the hospital. He was on a norcan drip for over 8 hours because of the amount of opioids in his system. He also received emergency surgery that saved his life. read more

This is my first experience at a skilled nursing facility. I have never been in the hospital before. I am very pleased with the staff here, in fact, with my entire experience here. In my opinion the skilled staff is by far the most critical element involved in how well a patient will do. The staff is very friendly, skilled and they respond really quickly to every situation. There are more situations to respond to here than in a normal hospital. Patient needs are many and always look to be very important to the patients. The patients all seem to be well cared for and considering their circumstances, happy and are getting all of their needs met. Believe me, that is a huge responsibility for the entire staff. The facility is quite clean and well equipped and well maintained. It's not perfect but the maintenance supervisor, Ronnie, is running his tail off solving dozens of problems every day. All with an outstanding attitude and big grin. I have seen a few things that need attention and I'm sure they will get to them as soon as time and budget allow. None of these issues bother any of the patients. Myself included. My roommate is a stroke victim and needs an enormous amount of constant care and understanding. A lot of it. He always seems happy and is very well cared for. Watching him throughout the day (and night) here in our room he looks like he is a member of a big, caring family. Really. It takes special people to make this happen. Every one that comes into our room does so much to see he is really well taken care of. I ask if I can help or get something for him many times every day. He always says no. He feels whatever he needs is going to be taken care of without my help. That has almost always been the case with all of the other patients. They are all so nice and I want to help whenever I can. I keep offering but nobody takes me upon my offer. That says a lot about how good the staff is. The managers are extremely good and are very friendly. They have accommodated all of my needs (quite a few) pretty much on the spot. Cam, has gone way beyond what I have expected to take care of me. Thank you so much Cam and thank you to all of my new friends (staff) here. Every one of them deserves a hug. I really mean that. The kitchen does a very nice job providing healthy and appealing meals and snacks for a huge group of people who all have different needs. As far as I have seen, they are really responsive to every patient's needs and a bunch of special requests. This is not an easy job, believe me. I am very happy with everything in the kitchen. I have not been the easiest guy to feed. I have made special requests and changes multiple times every week and the staff in the kitchen has been completely understanding and accommodating every time. And, they keep smiling and treat me like someone who is a guest for dinner at home. Expected from managers and supervisors but everybody has been just that nice. Nobody has been difficult and they always do whatever they can to help me out. Just like all of the nursing staff, they always make me feel really well taken care of. 10 out of 10 for all of the staff. read more

The facility needs a good cleaning and remodel. We were told they were remodeling but never saw anyone working and we went there everyday. Sliding glass doors did not lock, years worth of dirt in the sliding glass door track, bed table was rusted at the bottom and didn't work properly, bathrooms were old and the floor moldings were lifting off the walls, back patio was never swept, no patio furniture outside they used folding tables and chairs from dining area outside, trash cans were not emptied and were overflowing, the janitor that cleaned the floors emptied the mop buckets right at the back doors to patio area and people had to walk and roll wheel chairs through the dirty water, there was old broken equipment in the halls and on the back patio. The facility needs a good cleaning and remodel and new beds and bed trays. Facility is old and not properly maintained!! The food was awful. One night they served a tomato and mayo sandwich for dinner. One woman ordered a grilled cheese sandwich and they brought her 2 pieces of toast with a slice of cheese between the toast. We went everyday to deliver my family member food or they would not have eaten. Yes it was that bad!! The staff was ok. The nurses do not help eachother out. The nurse bells rang continiously without any available nurse checking on the patient. I timed it for my family member and response time was 12 minutes. That was about average. The rehab staff only did 1 hours a day and then patient was left in bed the rest of the day. How are they supposed to get stronger and go home with 1 hour of rehab. My family member is did better with R/N, P/T, O/T and speech therapy with home health. I wish we had brought them home instead of going there for 6 weeks first. Very depressing!! My family member was considered high level supervision meaning they could not get up to use the restroom alone. They wanted to put them in a diaper which my family member refused. They tried to get out of bed without help because the staff did not answer the call button for long periods of time. My family member took a fall while using the restroom unattended. The staff DID NOT call the family to tell us about the fall. The P/T mentioned it to me days later and then tried to say the reason that did not call is because my family member is of sound mind to be responsible for their own care. The social worker originally told us we would meet weekly to discuss patient progress. She never scheduled anything and after several inquiries and phone calls we decided to pull my family member out. Doctor release patient but we needed a wheel chair. They needed to send doctors order to get chair. It took them 1 week to get it orders for the chair. We could have taken my family member out the week before but we were told the insurance has not cut off the coverage so they can stay until the insurance says no more. I got the impression they wanted the money from medicare more than letting the patient go home. If you leave your family member here, you need to also be there everyday to make sure your family member is being taken care of and eating something besides the slop they serve. Yes the food was that bad!!! read more

After Breaking both bones in my right leg, the professional, exeptional, and Great Care I recieved from Casa La Villa was a truly a Wonderful Experience! Kudos go to My Nurse Larney, my Physical therapy gal Samantha, and my Patient Advocate Cam--- and JC ,,,,,,,,,(I sware he walks on water) !!!!!!!!! I was released 1 month after surgery! La Casa Via has outstanding patient care and an excellent physical therapy program. I'm a retired healthcare consultant, and I chose LCV after a long stay at John Muir Hospital. The CNAs are very kind, attentive and respond promptly to my Call Light, when pressed. The Nurses are good about answering questions and keep up with medication schedule; it works best when I keep track of when my medicine, so I know when pain medicine is due, so then I can ask for it timely. The Medical Director Dr Sheikh is excellent and responsive when I have questions and concerns. The Wound Nurse stayed on top of my surgical incision, and was sure to monitor my incision for improvement. She mentioned that she thought my surgical staples had been in for a while, I quickly called my surgeon's office to ask if the LCV staff could remove them, she authorized their removal, and it was done promptly and with no pain or issues. The Physical Therapists and Occupational Therapist were fantastic! They encouraged, help me set goals, ensured I was safe, understood the difference between pain and fear, and often times said "well done!" which really supported my mindset of positivity! One day, after many consecutive days of PT & OT, I was having a lot of pain, I asked for a day off, and they agreed; I had been working very hard and attaining my goals, and was just beat. They said "see you tomorrow!" and I rested up, and indeed I saw them the next day and felt even better, and continued to smash my goals. Whenever I ordered "necessities" from Amazon, the Front Desk Staff quickly delivered them to my room; We became fast friends! A girl needs her Moisturizer and Yoga socks! Now, the Food here is bland, overcooked and simple, as to be expected, because the rest of the facility is a Nursing Home, but this was the way to ask friends to bring me take out (great Chinese Food restaurant and Burger place in shopping center nearby; also, there's a Panera on Treat and Bancroft). I had a picnic with many friends over my my three week stay here. One last thought: the physical plant is aging, and needs some touching up, but the important stuff, the STAFF, is OUTSTANDING and honestly, when it comes to great care, it's the STAFF you want to have the best, and they are. Every patient room has sliding screened doors from the room to outside, so if you like a little Fresh Air and a chance to take in the outside, you can! Newer places don't have that! Lastly, Cam is the Facility's Executive Director, who is smart and adept at problem solving. Whenever I had a concern, I would text her for quick resolution. She is caring and supportive of her staff, which is likely why they're all so terrific; also, who would want to work in a place where many patients are elderly, cranky (because they're not in their own home, etc) and in pain? Well, these CNAs and Nurses do because their Leader (Cam) is there to support them and problem solve, so they can do their jobs! Thank You Cam! I cannot recommend La Casa Via Highly enough, not only as a Medical Professional, but as a patient of three-plus weeks there. read more

I'm a only child who's been managing my mother's care. From the moment I first inquired about her staying here, to the day my mom was discharged. Cam's level of care and communication was ABOVE and beyond. She was able to talk me down from many frazzled and frustrating moments with external medical personnel asking things I didn't understand, she took care of it all. She manages an amazing staff, one of her nurses, Marc was just the absolute sweetest and best with my mom. I cannot say enough good things about this facility for care. No one wants to be in a skilled nursing home. But if you do end up in one, you want to be in La Casa Via. It's so close to John Muir the trip was easy for my Uncle. The liaison was so helpful in helping us (as the family) understand what was happening and what we could expect. The staff, the room, all aspects felt good. Even when it was time to leave, the staff let us know we would have a nurse come visit the house... What a relief. My father suffered from two broken hips and was not improving at another facility. Cam was able to step in during a time crunch, transfer my dad quickly and safely, and my father improved within days and is now home. My mother was at your facility in 2020 and she is still very much alive and being well taken care of at home. Unfortunately she stopped walking after her stay at your facility because the PT was ineffective. She also got a a bad open sore after the 2nd day there because her diaper was not being changed in a timely manner and it only got treated because I pointed it out to the CNA's who did not notice it. read more

Very pleased with the care and services my mother received while being cared for at La Casa Via TCC for the 4-weeks she was a patient this past February/March 2023. The facility is very clean and well run by leadership and case management. The nurses, aids, caregivers and administrative staff were incredibly professional, friendly, attentive to my mother's needs, addressed any issues we requested assistance with, transported my mother to/from her doctors appts and made certain I was promptly made aware of any changes that'd affect her situation (meds, physical therapy, in-house doctors updates, etc.). My mother stayed at another SNF in the Concord area back in December, and the difference between said SNF and La Casa Via were night and day. I'd recommend this SNF for your loved one... read more

This is the worst place a family member passed at 6 pm and didn't notify us until almost 8 pm .. days before his passing he fell and the nurses had no idea how he fell or what he hit his head on she said she had pictures and when we asked to see them they no longer had them. My brother and my family are very happy with the care he received here. He was a resident of this facility for about 6 months, recovering from a traumatic event. The staff was very kind and accommodating, even during this pandemic. It was even hard for him to leave because the people made it feel like home for him. I especially want to thank Jean Hernlem, who went above and beyond in motivating my brother to be better. When we weren't able to see him, she was there for him in our place. She'd encouraged him and often offered a listening ear to not only him, but also to us (the family). Now that he is out of the facility, my brother is doing much better on his own, and he wouldn't be where he is at now, without her help. read more

My mom spent around 3 months at this facility recovering from surgery complications. Her medical conditions were challenging and, at times, made her unwilling to cooperate with the staff. Overall, I believe my mom received good care, as the majority of the staff was helpful and caring. My family wouldn't hesitate to use this facility in the future.
