My husband and I came as patients feeling heartbroken and frustrated with recurrent losses and Dr. Miranian and the rest of the team listened to us and treated us with the utmost empathy and consideration. He took time to get to know us, educate us, and really understand our situation. I feel extremely confident in the care he provided us, and he was always there to listen, explain, or ideate with us - all while being both realistic and warm. While some times in our journey have been really challenging, every single interaction with the team at RMA - whether it's Dr. Miranian, or one of the other REs there that day (Danielle, Lange, Guttman, etc.), nurses Annette or Brooke, or Abby (phlebotomist) - have given us the best possible experience. We are now pregnant and graduated to OB care and while we are happy we are moving forward, we will miss the team!!!

I am so sorry I started my fertility journey here (and did not educate myself more). Nice people but very disorganized and this disorganization can lead to mistakes.

That's fine, not everyone is in network, but the way they said it made me think that I have zero fertility coverage, not just that they specifically aren't covered (the clinic I switched to DOES accept my insurance). I should have checked with my insurance but they definitely should have been more clear. Anyway, so every time I had an appointment I checked out at the front desk and asked what we owe. My intention was to pay out of pocket as we went, every single time. Yet after my first and only failed IUI, we were told we had a surprise $700 bill. I repeatedly asked to speak with the finance team that never called me back. Also, after the failed IUI, I didn't get a SINGLE phone call or message in the portal indicating sympathy. Nothing whatsoever, just something to the effect of "let us know when you want to come in again". We pay thousands of dollars out of pocket and you can't leave one voicemail saying we're so sorry this didn't work? Nothing?! And instead just "hey you owe us another $700" I'll leave a single star because their front desk person seemed nice.

Regardless of what they tell you you will owe or what is covered by insurance, do not believe them. Get absolutely everything pre approved in writing down to individual tests and visits even when they tell you they are covered because they will fall back on a general waiver they force you to sign at the beginning. They take no responsibility in providing accurate cost estimates and will absolutely blame you for their lies. We were told that we would have no costs for a given cycle and even with insurance coverage for the procedure, they claimed that we should have known that they were not contracted by the insurance provider for the specific treatment. When we asked for the contract details, which would have been the only way to know this in advance and determine that their guidance was wrong, they refused and said it was private. So their position is that even if they tell us something wrong and provide a service they are not contracted to provide, it is on us to know the truth they refuse to disclose and because we signed a waiver, they can charge us anything they want. Do not go to this clinic. If you do, don't trust any numbers they say or quote. And get ready for the most irresponsible and unprofessional example of exploiting patients.

I have already spent hours coordinating between MLF, the pharmacy and insurance. MLF forgot to order half of my stim medications, twice - what in the world???! Once, they straight up forgot and once, it got fallen through the cracks. This is unacceptable and all they could do is say they will have a serious conversation with the nurse who made this error. I had to already correct them with when my baseline monitoring appt should've been scheduled. Definitely feel like just a number due to the lack of communication, transparency and coordination. I have so many friends who did IVF nationwide and none of their practices acted like this. The prelude app is atrocious. You can't even see the full messages in the messaging portal, which is also super frustrating. You can't even attach attachments in a message. Feels like you have to fend for yourself, educate yourself and be your own advocate. The fact that they can't provide any itemized financial estimates for an investment like IVF, is mind blowing. I hope to have a better experience as we continue with our cycle so I can come back to this review and add some positive aspects of MLF.

    As I review I have a caveat when writing posts such as these because there seems to be a trend to…read morepost in an (naturally) emotional scale, when I'm posting as 1-a healthcare provider-I am a registered nurse, but NOT in this specialty-not every body knows everything and this is NOT a lane I ever desire to be in and avoided in my 15 year career; 2-with being a nurse the understanding and knowledge that most pregnancies even without issues with fertility the pregnant individual has a 40 percent chance of SUCCESS; 3-the healthcare delivery system SUCKS, so no matter what accolades a storied institution has, they are NOT the boss in terms of healthcare delivery-I've been healthcare for 20 years in this area and enough that the reputation that people expect has been DESTROYED because of it-so when that happens it's unfortunate but put that in PERSPECTIVE-scheduling, billing, those skills I know that is supposed to be efficient break down-and have been for some times now with less hospitals in the area-more patients more workload all in order to meet the needs of the patient population. 4. We are fortunate to be in an area where we have access to the specialty of reproductive medicine-however it is STILL a small amount of providers that can do this; so in that perspective expect a marathon. I have been a Penn Medicine patient for over 5 years, most likely longer so my GYN (Dr. DeSousa-Sanders which I love through Penn Medicine) referred me to them (naturally) to review my ability to have children in 2018/2019; Dr Butts guided me through this process amazingly and gave me the confidence to start this journey-there was an expectation that a cycle or two may not be successful due to my extensive health history-trauma patient and abdominal surgery, found causes and came up with a plan; due to COVID I was unable to start, after standards were set to resume preparing for a cycle, my care has been resumed with Dr Edwards-he is amazing, and both physicians have answered questions, able to think of the possibilities and even give you the space and time to answer questions; with fertility you have to think of EVERY aspect-from even an ailment a relative may have can have a genetic factor-which was my case that was found and I scheduled my fourth cycle-I had to cancel the first due to complications they expected, however my body still decided to have ovarian hyper-stimulation syndrome-the fellows were AWESOME in guiding me through such a scary time-I had to be observed during the night for my case; the second cycle I ovulated so no go for that one, and the third passed, and due to a possible gene that causes an auto-immune condition that causes clotting factors-my grandfather, mother and one of my aunts have this; it was able to be pinpointed, my health protected and a few medication adjustments to start again. And above ALL-my fellow nurses-Alex and Aubrey are AMAZING at this...the reproductive cycle and the body is a machine like no other-they really try to get it RIGHT...but even then be gentle to it-it's only going on HOW it operates, not how YOU want it to BE. I am satisfied with my care, because of the caveats; I will also post that I had issues with the "business" side so I called patient relations and was able to handle that and get people in charge of that so any issues of how things were ran I had a point person-I suggest anyone who may feel anxious, upset or feel if they are being discriminated against to go this route-they will handle the issues; they handled mine so well it seems that when I call they know who I am which is fine by me-I and anyone is to be treated with dignity-and this is such a very sensitive subject that reminding those who may not have those skills perfected to have a recharge and meet people where they ARE during a difficult era of a pandemic with fertility in the mix, is appropriate. I would also suggest that patients who specifically those "soft skills" or needs on a higher lever-I may not needs as much-to add on the psychosocial aspect-there are counselors, or even your own therapist for this journey. There are so many moving parts handling the mental as well as the physical and "putting it out there" so that the team will better see to your needs is key-they're pretty transparent and they let me be, especially where I expect a high standard as a nurse-I think sometimes more so than the public, we will out Type A ANYONE because of reason and rationale-and we do consider when taking care of patients financial, emotional and past triggering stressors-they've seen it all and we're willing to meet me when I was when I had MY LIMIT when my second cycle was cancelled. Dr Edwards called me personally and talked to me for 90 mins and I let me just TALK , and really broke things down on a Saturday afternoon-NOT ON CALL. That's how personable they are-and that fits THIS patient. Baby dust to you ALL!!

    Our family has been patients w/ PennMed since 2020,& our experience at other network locations has…read moregenerally been positive.This review covers our personal experience at Penn Fertility Care specifically, navigating family planning w/ a known genetic risk (FMR1/Fragile X syndrome),through IVF & PGT-M.Before starting,we asked whether the clinic had hands-on experience managing Fragile X cases.We were told yes, & our care was handled by Dr. Anuja Dokras.All 5 consultations were virtual, w/ occasional tech delays one stretching to ~40 min. Gaps of 1-3 mos between appts left us feeling short on time to unpack complex genetic questions & fully grasp next steps amid the uncertainty. We asked twice to switch physicians (at intake & again early on, requesting someone specific), but were turned down both times (see att.).After genetic counseling & 2 stimulated cycles,we had pressing questions about realistic odds of getting a healthy,transferable embryo using our own eggs.Cycle 1:16 eggs retrieved,2 embryos formed,neither cleared PGT-A.Cycle 2:14 eggs retrieved,1 embryo formed carrying the full mutation.Only after repeated follow-up,once a 3rd cycle was already on the table, did we finally get an individualized estimate:roughly 10% odds per cycle.Layer in a ~50% embryo transfer success rate,standard freeze/thaw risks,plus a 94%+ chance that a 133-repeat maternal premutation expands to a full mutation (ruling out transferring a female carrier embryo something never clarified upfront), & the cumulative odds were startlingly low.Chasing a single healthy embryo at any cost, on an open-ended timeline, felt like a losing strategy absent full counseling from the start.We respect clinical judgment & understand IVF guarantees nothing.Still,we never got an early,plainly explained picture of how age, the FMR1 premutation,& PGT-A/PGT-M results combined to shape our actual prognosis.A candid,timely talk about alternatives donor eggs included before 2 costly,grueling cycles could've spared us significant financial & emotional strain.In a meeting w/clinic leadership,we pressed on strategy & communication gaps.We were shown general IVF stats from genetics,but felt these broad figures didn't capture our complex case.We were offered a $2,500 partial reimbursement for administrative issues appreciated,but it didn't touch our core concern: poor communication & lack of personalized planning.At our grievance meeting w/ the Division Chief of Reproductive Endocrinology & Infertility, the clinic manager opened by asking about our jobs.Through the mtg, we felt our case was being framed as bad luck, sidestepping accountability for the level of attn given to a case like ours a ~40yo patient w/ Fragile X.It closed w/ a genetics-lab branded mousepad citing general IVF stats.Overall,we felt unheard,w/our core points seemingly dismissed.To be clear, this isn't a claim of formal medical error just a candid account. Penn Fertility Care,Dr. Dokras,& the HUP Patient Experience dept have real room to improve:explaining individualized prognoses sooner,surfacing options earlier,& building a more empathetic communication culture for complex genetic cases.We remain open to dialogue & sent full documentation to Penn Med's CEO & VPs,requesting an independent grievance review & a fair resolution given incomplete informed consent.Other issues worth flagging:Embryo storage, materials stayed in cryo by default even when unusable,& we only learned about active fees after getting an invoice.For cycle 2,we had to proactively file paperwork ourselves to stop storage of the affected embryo (see att.).Procedure concern during a follow-up visit, a fellow attempted catheter insertion twice unsuccessfully,causing real discomfort, until an attending MD stepped in & completed it; this was documented & raised w/ leadership. Finance dept,non-routine requests took 2-4 wks w/ repeated follow-ups; a simple itemized bill needed for an employer grant faced major delays;we were referred to ARC Fertility (13-18% APR) despite much cheaper personal credit options;recurring billing errors required insurance-side audits;& before prepayment, we were sent the wrong contract package,raising further coordination concerns (see att.).All of this eroded our trust.We're giving 2 stars specifically to recognize the dedicated nurses,clinical staff,& individual physicians who showed real empathy & care throughout.Fresh perspectives & strong leadership could genuinely move this clinic forward.Sincere thanks to Kathleen O'Neill,Marissa Weiss,Alexandra Taylor,Laura Swartz, the procedure room nurses,& the front desk team

It's…read morehard for any practice to achieve perfection in fertility treatment - there are so many interactions and variables. I visited RMA over 25 times during my care across 3 different locations. But overall, it came together well and I left feeling like RMA was in my corner and delivered on their promise. I'm very grateful to them for helping us. Doctors Our initial interactions with RMA doctors were not good. Some of the older doctors in the practice are very condescending and controlling - one said some very offensive things to us. Stuff that would make your jaw drop. We considered terminating care but ultimately contacted the practice manager who took our concerns very seriously and we switched to another RMA doctor who we liked right away. This doctor created a very personalized treatment plan and helped us make the right choices at each stage of the process. He also kept me safe through very close monitoring during a very long IVF stim process where I was at very high risk of OHSS and had 10 visits in 14 days. Financial counseling RMA has a robust financial counseling department that gave us accurate information about our costs and took swift action with preapprovals and insurance. In addition, I had to submit some appeals to my insurance and they produced supportive materials / letters swiftly. I received an insane bill from a third party for the anesthesia and they were on it right away - it was a mistake. Our personal financial breakdown: Procedures and appointments $25 copays PGT testing $3000 (optional) Embryo biopsy $1700 (optional) Meds $2200 Insurance paid for everything else Nursing and medical assistant care We had good interactions with nurses and MAs and some of the MAs started to treat us like friends over time. One area that could use improvement is the prior authorization process for drugs - this is managed by MAs and there is no direct line to patients for questions and concerns. "You can't speak to them but we can pass along a message." I had a complex situation (two insurances with different rules) and really needed personalized help. I eventually - through sheer willpower - figured out who to speak to and their email address (and they were happy to help) and got them to submit the right authorizations but by then it was too late. I paid $2200 out of pocket for my meds and the next week an insurance approval letter arrived. Nothing was reimbursed because the approval came too late. Focus on patient comfort IVF treatment is not comfortable but it doesn't have to be traumatic and you should be in control. At RMA in my experience, if you don't advocate for yourself, you may find yourself in physically uncomfortable situations, usually mild but it was definitely upsetting for me at times. For example, if you prefer to insert the ultrasound wand or if you want Valium for procedures - you will have to know to ask. I expressed that I didn't want to do 9 weeks of IVF progesterone shots and my doctor agreed to do a natural cycle - had I not asked, it might not have been offered. Some doctors at RMA don't do any natural cycles. I also turned down the hysteroscopy procedure because of anticipated discomfort and they were not happy - I had to really put my foot down. Eventually they offered to do it under anesthesia to try to convince me, I still said no. My mock transfer and FET transfer were also tricky, taking almost 20 minutes each, and they did present me with options like Valium and even anesthesia for the FET after the mock transfer left the doctor sweating but I had to ask. One other thing I noticed that really bothered me is that the receptionist asks male patients dropping off sperm if they've abstained from sex SO LOUDLY that everyone in the waiting room can hear. Give the guy a little privacy, come on. Communication I was pleased with the communication processes from RMA. There are multiple ways to receive your results and there is always someone to answer your questions. If they say they're going to call, they always do. Instructions for medication administration are thorough and you always know what to expect. Overall result RMA delivered on their promise and got me pregnant with one round of IVF, which is what they expected (they said it was about a 70% chance per round if you do PGT testing). I would recommend them to other patients with the caveat that you should have a perspective on what you want, pick the doctor that suits you, and stick up for yourself.
