Today, February 3, my girlfriend filled out her menu as she does for each meal. She cant eat the eggs because she is allergic to soy and notes it every time. (the eggs contains soy) Despite this, she gets served eggs. When she lists cold cereal, she gets served cream of wheat. For dinner, she ordered a cheeseburger and gets served a turkey sandwhich with miracle whip, which contains soy. The nurse was nice enough to go go the kitchen and get her what she ordered and she was given another turkey sandwich, being told that they had nothing else to eat (despite the fact it was an allowable substitute on the menu she submitted.)The kitchen also told her they were unaware of any soy issues, even though she writes in it big letters on the menu she submits it. Also, she keeps copies of all her menus so there is ample evidence that this place doesn't care about what the patients want or need. Stay tuned for what's going to go down tomorrow....I'll keep you all posted. Once again, no response from Courtney, the hospital administrator. He doesn't have the courtesy to talk to me about any issues I have raised. I finally got a call from Heidi the interim administrator. She was very nice and dressed all my concerns. It is still a work in progress but at least someone is finally listening. read more

I never expected to be one of the people who say "THIS FACILITY ALMOST KILLED MY FAMILY MEMBER" but here we are. My mother-in-law was in their care for only FOUR days after a severe stroke and she ended up in the hospital in septic shock and severe dehydration. It's from a suspected UTI, due to the utter lack of hygiene and care at this facility. We were there every day advocating for her care, so it's shocking that this could still happen. On top of that, Coronado did not want to send her to the hospital. We had to demand it after learning that her blood sugar was 600+ and she was unresponsive all day long. They told us she was in no danger and it wasn't worth the stress of transport to the hospital. Two nurses and the doctor agreed she did not need the hospital. They didn't even bother checking her vitals until we insisted she go. At that point they determined she needed supplemental oxygen and was struggling to breathe. They then proceeded to use their private transport company, which took almost an hour to arrive instead of calling 911 "because it's a pain". They assured us they were keeping a close eye on her oxygen while awaiting transportation, but when we arrived she was alone in her room in terrible shape. By the time she got to the hospital my mother-in-law was rushed into trauma, her blood pressure was dropping, and her organs were failing. They confirmed she would have died if we hadn't intervened, and she's still not out of the woods. This is all on top of many other issues we've had with the facility over the past several days, including bed bugs. They all seem too trivial to mention at this point, but the lack of care and communication is appalling and shameful. This place is garbage, the nurses and CNAs do not care about their patients and have zero urgency, even in an actual emergency. The case managers don't return calls. The list of complaints is about a mile long. read more

A family member got sick enough that they were released here for respiratory and physical therapy. They recovered strength, returning home after several weeks. Doors are locked at 6 pm. Later visitors get in via the nurses station door. The staff were friendly. 8 pm is last call. Food is served in-room or in a Community Room. There are also activities offered. Our very weak family member could barely move on arrival. They received physical therapy daily, walking out under their own power on check out day. Goal met. Family was allowed to bring in foods craved by the patient. They had lots of snacks bedside in a drawer. Coronado filled the requirements. read more

Stay away. EVIL. My elderly roommate has been in there a week. The PT was only there once, and we have tried to move her since Wednesday, it's now Monday because they didn't respond quickly enough to have her transferred by Friday so they could collect the insurance money. Very sweet check in gal, but no qualified nurses after 7, hours waiting to go to the bathroom, nurse said my friend was a liar when she said she was trying to transfer to a different center. The restroom next to the Case Managers office stinks so bad she apologized, but I was thinking, "why don't you clean it?" , rude staff other than the nice check in lady. Just a horrible experience, this place should be closed down. Not humane or professional. read more

It's definitely not a pretty place. It's run down, and in fact, you could call it a dump. But the staff! They actually care. My stepmom, after a fall and hospital stay in ICU, went to a beautiful facility for two weeks. I once was there for five hours before somebody stuck their head in to check on her. She had another fall while there, another quick stay in ICU, and then we sent her to Coronado. What a difference. People were checking in on her all the time, even on the weekends. Yeah, the food wasn't super appetizing, but she ate it. The Physical, Occupational and Speech Therapists were on it, making sure she kept improving. She got moved closer to the nurses station so they could keep an eye on her. I got calls from the nurses (thank you Niviene) about occurrences. SHE GOT BETTER!!!! A lot of patients are moving around the hallways, chatting, and stray staff just stopping to chat or get them to the right room. I got led to my step mom's hall by another patient in a wheel chair. There's a liveliness that I found to be great, gritty as it may look. AGAIN, IT'S ALL ABOUT THE STAFF!!! Sometimes they are stretched a bit thin, but I never felt lost in the shuffle. Today was our last day there, and we moved her closer to us in California, but I'll never forget some of the staff. Niviene, Andre, Letisha (RN's), Francine (CNA), and Pat (cleaner) all went out of their way to be friendly and take care of my step mom. Thank you all!! read more

The CNAs are very helpful and do their job well and they seem like they was busy. My occupation therapy went very well and my physical therapy is going well I'm now standing and walking with a walker and when I got here I could not do any of that and the nursing staff do very well just make sure that you see your doctor first so you do not have any problems with your medication when needed it. Maintenance housekeeping and laundry does a great job. The kitchen is average and they need to do a lot better. read more

Some of the staff just don't care. My mother in law was gotten out of bed by therapy not changed and forced to go to the therapy room while still wet. DO NOT EVER GO TO Coronado Healthcare on 19th ave. I drive for a rideshare company. Coronado just had me pick up an elderly man who could barely walk and was confused. They put a garbage bag of stuff in the back of my car along with a clear bag of medication. I thought I was taking him home where someone would help him out of the car. I didn't realize until we were there they had me taking him to the homeless shelter on 12th street!!! I helped him get inside, but there is no way this man can take care of himself. I called the elder abuse hotline, but they said there was nothing they could do since I didn't even know his name. read more

My loved one was there in July 2023. They received the worst care. When the needed assistance and depressed the call light, they waited for over an hour. The place stank of cigarettes smoke and constantly had patients arguing out in the hallways. The food was inedible and almost 3/4 of the time they were there, I brought food in. Now discharge....because they were taking their time, making us wait....we just started to walk out...as we were getting in our vehicle, then staff came running out to us. Sadly, my lived one was scheduled to be there, per the doctor....we left after 6 days. I did a better job!!! DON'T USE!! read more

If you love your loved one DO NOT place them here. This facilty is not attentive to those that are placed in their care. Mom had only been with them about six days. I should have listened to the reviews of patients and not the five star rating from the CMS. They have orders that mom is a fall risk and a floor mat is needed. There is a sign advising that she is a fall risk of five. They did not place one in the floor last night and mom tried to get out of bed and fell. She had a swollen eye but was okay. I accepted their apology with a guarantee that the staff will be informed of her fall risk that they apparently didn't know about. Nurse claims they were unaware she was a fall risk. This is BS as they tell her she had to use the bathroom in her briefs and they will clean her because she is unable to walk in her own. She is there to get stronger to walk. An hour later they call to tell me that PT placed her in a wheel chair and left the room for about ten minutes. The team that knows her limits decided to leave her in an unsafe position.They did not place a mat on the floor and mom again tried to get up and fell. She is now on her way back to the hospital for evaluation. I admit to being angry and loud with the nurse. I asked how is it possible this happened again after being guaranteed a mat would be around her when she is alone. The nurse was rude and asked are you going to let me speak now. Instead of addressing the concerns she was full of excuses and i wasn't going to waste time. After some more rude comments i told her I was done talking with her and requested her charge nurse. Instead of honoring my request she hung up on me. I called back and spoke with a manger and told her my experience and admitted i was loud with the nurse but anyone would after they negligently left her in a compromised situation. All she could do was apologize. An apology was accepted the first time. An apology does not change the fact that they are unable to care for those in their care. I reminded they train their staff to care for people or close their doors. I requested that she be discharged within the hour and i will pick her up. She then tells me they are sending her to the hospital to be evaluated. This is a major thing that the first nurse decided didn't need to be told since she hung up on me. I'm here medical POA and they know this and should legally be advised if all information.I requested they send her stuff to the hospital and she would not be returning to them. If you choose this facilty know that you are choosing the following for your loved one: 1. A team of individuals that cannot do the minimum to keep your loved ones safe. 2. Difficulty getting anyone to answer the phones. 3. Risking your loved ones life. 4. Team of people that do not spend the time answering your questions or concerns. 5. Unprofessional nurses that do not have the patience to care for individuals that are in compromised health. 6. An unorganized corporation that will put profits over the safety of those in their care. 7. A team of people that can only apologize and make excuses for their failure. An unwillingness to fix their broken system. Again if you love your love ones do NOT send them here. They will eventually cause harm or death of your loved one. Especially if they require a lot of attention Twist the reviews and not the five star CMS reviews. They are good at faking what they do when the government is evaluating them. I would not recommend this place to anyone. I spent 2 weeks. The first day, an RN named Adam refused to supply me with ice packs after I had hip replacement surgery. He said it was not necessary, so the doctor that works there thinks the same. I told him my surgeon thought it's a must, but he said my surgeon had nothing to say. The food, especially the breakfast, came cold. Sorry, I can not eat scramble cold eggs. Nurses and CNA were too slow to provide a service. Sometimes, it took 3 hours to get pain medication. You need to ask 3 people if u need something. My roommate was a Spanish speaking lady and most of the CNAs spoke Spanish she always got first, like extra food or drink. I had to ask a few times to get a cranberry drink. The staff was sitting and often talking and not providing a service. The table next to the bed was never cleaned in 2 weeks. If you can't go to the bathroom by self, it's not a place for you unless it's okay to use the bed. I was lucky enough that I was able to use my walker. I also was over medicated, they gave me pain medication and muscles relaxer and I slept nonstop for 2 days. I asked not to give me muscles relaxes and sleep medication. So be careful what u get there. read more

We discussed his meds how hes doing and what treatment he has been getting. Thank you Raquel. 1st post: So far I'm disappointed with this care facility, getting through their phone system takes forever. Then you may get lucky to have someone answer the phone. I have received no doctor calls telling me how my brother is doing since he is severely handicapped and can not speak for himself, i am his advocate and try to do the best i can for him. Since this is during COVID i can not go see him to check on him and discuss his treatment, medications etc. With the nurses and doctors this is Very frustrating. During these times of covid I know they are doing their best in these difficult times, but for a person that can not advocate for themselves they need to be more diligent in getting in touch with the family. read more

My grandma passed away on May 30th. She was at this facility (Paloma East) for almost 8 years. She had Alzheimer's plus a Ileostomy and needed special care. She didn't speak English. However- the staff was amazing and learned to speak her language in order to communicate with her. She was happy and clean everytime that I visited. One of the CNA who took care of her, Tiffany, was amazing and took care of my grandma and put in great lengths to celebrate her birthday. There was a particular week where my grandma fell ill where the staff took care of her night and day and made her feel comfortable. Special recognition goes to LPN Carey and CNA Maria who were attentive and caring. Thank you to all the staff at Paloma from the bottom of my heart. I strongly recommend this facility. read more

When I was in their care, it felt like they had kept me dehydrated, and constipated. I had immense pain, and i was crying alone in my room, and they finally came in over an hour later to get me my medications. It was the worst place I've ever been. They set multiple things up for residents at the same time so it is impossible to get a service because you miss out on one. Do you want your biweekly shower or do you want PT? This place should be shutdown for the horrible way they take care of the patients! They neglected my family member to the point where they almost succumbed to their injuries. They never let me speak to my family member over the phone, and when I went in person it was so hostile in the building. When I visited, I politely asked a nurse on the floor directions to my family member's room. This lady literally laughed in my face in a cunningly tone , then proceeded to point to the appropriate hallway. PLEASE PLEASE PLEASE do not bring anyone you truly care about here! This place needs to be closed down by the state. The list of in fractions are immense from housekeeping, not wearing gloves when when throwing the trash from Covid patients not wiping down surfaces with cleaning products. Housekeeping going from room to room continuously, not wearing gloves when throwing the trash, therefore spreading Covid contamination throughout the facility. Doctors orders for patients not being followed by evidence of bedsores, medication not being given at due time the nurse was not even seen the days I was there. I have been there visiting my friend whose been at Coronado for almost four months, Overall she has pretty good things to say about this facility. I've always seen that it is clean and organized. they seem to answer her questions pretty well and if I am there will respond to me as well with no issues. Her major complaint is the food. This is almost always the major complaint any health care facility. When I have eaten with her, some days the food is better than others, so I can't disagree with her on that. read more

My loved one was sent here from Chandler Regional. I've just tried to call 11 times over the past 4 hours and couldn't get a nurse's station to answer: when they finally did, they set the phone down and I could hear someone crying "help me, help me, please help me" in the background. He just went 3 days with no wound care, because they are so short staffed. His cell phone was stolen by another patient, and nothing was done by the facility despite knowing who took it. He has missed critical appointments at Saint Joes because of the disorganization here. Send a loved one here if Hell is full. read more

Horrible. This place is disgusting. My father was sent here for physical therapy after leaving the hospital. He was supposed to stay there for 4-5 days. I walked in and it smelled like piss and dirty diapers. My dad was in a very small room with another man. I was scared to sit down because it was dirty. The floors are filthy. This place is a dump. I was there a total of 15 minutes and took my dad out immediately. I refuse to let my dad stay in a shit hole such as this. The employee I spoke to seemed nice but other employees seemed offended that I didn't want my dad to stay there. Who cares. I was scared to leave my dad for even one night. I thot he might get robbed or hurt. F*** that. I would never leave my dad in a filthy place like this one. read more

My husband was sent by Banner Thunderbird with papers to Coronado, so that the doctors and nurses could review his medical information. In the morning I was able to see him. He was listless. I asked did he get his insulin? Their response was they didn't know he was diabetic. Unbelievable! It was the same for other meds too. They didn't give him water when needed, monitor his kidneys when he was finally on meds for Mssa, and when my husband pushed the call button, they would send in helpers that couldn't give him the needed medications. This is not the place for anyone. Also, when I was visiting, more then one person was screaming to get them out. The only good thing' at the place was OT and PT, because they took their job seriously. They actually helped. In the end my husband was sent to John C. Lincoln(excellent)) but when he came in, they did not receive the medical info needed from Coronado. I called Coronado very upset and they hung up on me. Their rating should be no more than 1/2 star. read more

I was very concerned after reading the reviews about what I might find when I came to visit my niece who is here for medical rehab. so far things have been okay she says she's been being treated well she said the food is not so bad and her room is fairly decent. I have not had too much interaction with the staff but the nursing staff seems to be pretty friendly. if you visit after 7 p.m. the lobby is locked and no one will tell you how to get out unless you ask that's a little intimidating. I feel pretty secure that my niece is getting at least decent care and she is healing well. don't be expecting a 5-star hotel just good solid care read more

This place is terrible. They aren't sanitary. They don't clean up. Especially the walls. My brother was left in their care and his walls were filled with blood spots and other things. My brother witnessed someone not use hand sanitizer and then went yo go help a patient. Calling them for help is even worse because no is EVER at the phones. read more

My uncle was there and was absolutely the best care of any facility that I've ever been in... Sadly after he rehab he went strictly downhill and ended up passing away there and they were amazing. . Hospice was brought in and they stocked the room with food and drinks so that we could be with my uncle in his final days. For once the family ruled not money...
