Absolutely awful experience. The room was dirty, sheets were never changed, and the machines beeped nonstop all night. The food was just as bad -- they can't even get a simple order right, especially if you're vegetarian. At one point they couldn't tell the difference between a hamburger and a hotdog. The whole place feels like a cement prison. I honestly would've rather been back at the hospital than stuck here. Also, good luck getting physical therapy or anything to actually help you. You're on your own here. Everything I came here for I didn't get. Last thing - Completely unprofessional. I requested medical records over a month ago and was told they were "mailed with no tracking." Nothing ever arrived, and the staff's only response was to "wait longer." Mailing private medical info with no proof or accountability is unacceptable. read more

I'm currently a resident here.I've been here for almost 2 months.I have the issue with the head nurse , as well as a regular lowlife nurse Jennaba and Favor they mistreat patients as well as talk about other staff instead of checking on their patients that actually need to be checked upon, including doing vitals and checking on their well-being, I communicate it with favor that I'm schizophrenic, and I'm also verbalizing that I've had an episode the last 3 or 4 days, and she continues to wake me up out of my sleep, very aggressively and threatening to call the police if I don't stay awake and have no visitors which I had no visitors. And I'm also dead asleep. They have it in my paperwork that I have night, and she continues to harass me and then threaten to call the police for the visitor that was not in my room randomly at a random time, he was in my room during visiting hours hurt and the head nurse continued to pick at me and discriminate against my husband for some odd reason. Ever since his first visitation, here I've confided in my case manager, and I've also been verbal about these ongoing issues and nothing seems to have been dying. But I am sick of being harassed by this bald headed nurse , and this manly ass head nurse. read more

I was inpatient at Haven in Phoenix, AZ in 2024. The hospital was horrible!! The nurses gave me the wrong medication and thankfully I caught the error or otherwise I might have been hospitalized elsewhere. I am a professional who has a speech problem, I stutter. The psychiatrist and social worker had no understanding of stuttering. They thought I had cognition issues. What stupidity!!! One nurse was incredibly unprofessional and yelled my diagnosis down the hallway of the hospital. It was the most horrifying experience of my life. They keep you at the hospital longer if you have good medical insurance, which I did. I did not recover my mental health from that hospital-only got PTSD from the experience. read more

if I could give this place zero stars I would Do not ever bring your family member to this place. My mother was placed there and when she got there, she was good mentally as she stayed there. Her mental mind declined. They weren't giving her her medicine we had a meeting and they said they were but they were not. She ended up back in the hospital for the doctors to say she has not been taking her medicine which confused her a lot they neglected her at that place. They weren't feeding her. Her tray would just be placed by the door just to be mean They kept her in the room all day and when we told them she needs to get out they end up just keeping her in the day room all day in her wheelchair. They were mean to her we walked in on one of the staffs, a young black man with braids, pulling her out of her bed by her arm we immediately took her out of that place. *this review is for Haven Health Sky Harbor on Van Buren Street* If I could give this place ZERO stars I would. Let me tell you,the people that work at this "facility" and I use that term very loosely, don't give one damn about the patients in house. I was a patient there for 10 days for rehab after my total knee replacement surgery, and I am still not walking. Two therapists (Hayden and Jeff) actually worked with me, but that was it. I got 30 minutes of therapy for 5 out of the ten days. The CNAs except for two (sorry, I do not recall their names) could care less. Patients will use the call bell for something they need and the resident will wait between 30 minutes to an hour for someone to come help them. Then, when they come into the room, they act all bothered because you rang for them. The nurses weren't much better. I was to have meds every 4 hours, and when I would call for my meds, again I would have to wait between 30 minutes to an hour for my meds. Many times this occurred between 1 and 4 a.m. which made me lose sleep. Again, just like the CNAs, the nurses acted as you are a bother because they had to do THEIR JOBS. Finally, let me tell you about Saturdays and Sundays. Nobody bothers to check on you and the call lights beep incessantly for hours. They come by to feed you and give you meds like you are some damn animal and not a human. If you are thinking of putting a loved one in here, DONT. If you have a loved one in this facility, please do yourself and them a favor and remove them ASAP.That is unless you put them in there just to get rid of them, in which case, you are just mean. This place discharged me today (Monday) even though I appealed that decision, but they still discharged me at 10 am. As I arrived back home in Kingman, I was notified that I won my appeal and could stay. No thanks, I'll take my chances of doing therapy at home. Whoever runs this facility should clean house and replace everyone with people who actually care about people, or resign and let somebody that ACTUALLY CARES ABOUT PEOPLE run the facility! read more

A dear friend of mine was places at the Haven at Sky Harbor after she was discharged from the hospital. This place accepted her knowing the amount of oxygen she required. Almost every place we looked at couldn't take her because of the amount of oxygen she was on. A representative name Jon Dale came to see her and jus ranted and raved about this place and how the food was very good, therapy was fantastic, and the staff was very nurturing and compassionate. NONE OF THIS TURNED OUT TO BE CLOSE TO BEING THE TRUTH!!! Only one nursed showed her any type of empathy. Respitory sucks! Knowing that they did not have what was needed for her oxygen wise, which was obvious by the several machine and oxygen tanks she was using, they continuously ignored her cries for help. I was usually on the phone with her for 8 to 9 hours a day because she was so scared, and this woman didn't scare easy. She refused for me to make a complaint or talk to anyone in fear of retaliation from the staff. Her call light would be on for 30 minutes and even longer. The nursing staff and doctors are a joke when it comes to care. We always, for everything no matter waht, get the excuse that they are short staffed. Funny how my friends room was right across from the nurses station and their breakroom and the eating area for patients, and especially at night you can hear them laughing, talking about patients and other staff, etc. On the 10th (a week ago) I spent a lot of the day with her, and saw very little of the staff and being there through 2 meals (My dogs eat better than this!) I was disgusted. She had been struggling all day with breathing but pretty much knew what to do to get her levels back up. THE STAFF WOULD LET HER MACHINE THAT MEASURES HER OXYGEN AND PULSE TO BEEP CONSISTANTLY AND NOT CHECK ON HER! She died a little later on. I got a phone call at 6:45 that they found her already deceased at 6 am while doing routines. I had just left her at 10:30 the night before. We came to retrieve her belongings around 3 that afternoon and everything was already bagged up in a trash bag, including snotty kleenex's and bloody cotton swabs. We finished cleaning up and I decided to check some of her belongings. I have a grocery list of items that are missing like money, phone charger, batteries, pulsometer, etc. It seems what at that time, that everything that was missing was not on the inventory list. NO ONE TOLD US THAT WHAT WE LEAVE WITH HER HAS TO BE ON A FREAKING INVENTORY LIST!!!! We had groceries and all kind of things delivered to her there. There is so much more, but enough has been said. If you use this facility or any of the Haven's, beware and pay attention. You're loved one obviously by the posts I've seen on here tonight, is not going to be taken care of and it may be the last time you see them. I dispize this place with so much hatred and hurt for my friend that it is unbelievable! I would remarry my ex husband happily before I send another loved one there. I hope this helps! What a beautiful experience my mother in law has had here so far!!! The staff all seem to be SO happy. The vibe is so positive!!! In a world where everyone seems to be short staffed, this isn't the case here! We feel so comforted having our family member here. They're attentive, compassionate and all around WONDERFUL! They treat her like their own family member, included her in social events and are continuously working to get her stronger. Our family can sleep better at night knowing she is getting such great care. So grateful for Haven with her care. Today it's almost noon and I been at the hospital since around 630am. Reason Haven Health ran out of oxygen. All 02 are st in the hospital on those horrible ER beds. I'm in excruciating pain. I haven't had had my morning meds and I haven't even had breakfast. For the last 3 or 4 weeks the care has declined greatly. Meals up to 2 hrs late needing to be reheated. Meals actually missing parts of the meal. Very slow response from the kitchen to get Meals correctly. Last week twice the main part of My meal was missing. Nurses notified kitchen. Kitchen said they would bring it up. They never brought it either time. read more

I was there from Mid May 2019 till May 31, 2019. I was treated GREAT. The staff was very helpful and thoughtful. The food was terrific and I was ALWAYS treated with respect! I enjoyed my stay totally. When I went out in to the courtyard, I was greeted by the other permeant residence with happiness and joy. The were all so kind to me. I loved my time in the courtyard and would spend the majority of my time out there. They're where so many birds, and hummingbird to enjoy.I Have and app on my phone that would show what kind of birds they were and I would point them out to the other residence and they enjoyed that very much. Run!!!!! Run like you or your family members life depends on it!! The photos on website are not correct, they cram 2 or more patients in tiny rooms. No covid protocol followed by staff or residents. NO physical therapy to speak of, unless sliding a patient into a wheel chair and rolling them into the dining area to sit for hours is therapy? my mother was there 5 days, she was on the mend and only needed to gain strength to walk a few steps when she arrived. We were there every day and evening, wondered why she was so hungry all the time, I will go into that on my next post. No one communicated anything to us that she was having any issues. We walked in on the 5th night to her hanging upside down in bed, non responsive. Transferred her to hospital, pneumonia, Covid, renal failure, major bedsore, malnutrition and not being treated for any of it even tho supposedly a doctor saw her earlier that day, clearly a day too late, and no one told us even tho I called facility while at work to check on her. I have waited awhile before posting trying to work through the anger. This was moms first experience using a facility and last. We lost our mother a few days later despite the wonderful care she received after she was rescued. I can only say she passed being cared for and treated by loving nurses and doctors that honestly were horrified at her condition when arriving , they did the best they could. Overall, Haven did a good job of caring for my 80-year-old mother as she continued to recover from Covid-19, although there were a few items that I'll mention below that kept me from giving it a 5-star review. BACKGROUND: My mom had been hospitalized with Covid for a little over two weeks and was discharged to Haven's Covid unit at 4202 N 20th Ave. She was at Haven for 12 days (July 8 -- July 20, 2020). According to the hospital's social worker, Medicare's rating for this facility was five stars (some other facilities for Covid skilled nursing were only getting four stars, and another was a two-star outfit, so Medicare's five-star rating meant something). Haven Health had transitioned this location to being exclusively dedicated to skilled nursing (hospital step-down) with a special unit geared for patients with Covid. POSITIVES (5-star material): I was very pleased with the asst. director of nursing (Jolly) and the two nurses I spoke with (Josh & Michelle) as well as the secretary of the Covid unit (Sam), all of whom were very understanding and caring with the family's concerns about my mom's needs. For instance, I'd been shocked to learn at the end of my mom's hospital stay that she had not been receiving a daily multivitamin there, and her hemoglobin was starting to suddenly drop at the end of her hospital stay (the body needs iron, vitamin B12, folate, riboflavin and copper to produce hemoglobin). So I spoke with Jolly before my mom had even made it to their facility to see what needed to be done to ensure she would receive a multivitamin with minerals, and Jolly ensured the Haven doctor had it prescribed from the get-go. I was also very pleased that upon admission Haven tested her zinc level (a critical element for the immune system and something the elderly are notoriously deficient in)--something the hospital never did--and found she was severely deficient (even by lax American standards, it should be at or above 70) and hence the multi-vitamin with minerals was critical along with a zinc supplement that was added. In this regard, Haven provided better treatment than the hospital. As for Jolly and her Covid nurses I spoke with, it was clear to me that they're heart-centered and doing what they can to care for patients and interact with families who can't visit. When I asked my mom for her assessment upon her return home, she said all the nurses and assistants were supportive and caring with the exception of only one (who performed her tasks okay but my mom wasn't thrilled with her grousing, "she had a negative attitude"). My mom said the food was good and received plenty of it. MINOR CONCERNS (downgrading from 5 stars to 4 stars): I was informed that patients were to get a shower twice a week, and my mom was scheduled for Saturdays and Wednesdays. Upon learning she did not get one on her first Saturday there, I spoke with Jolly (asst. director of nursing) about it and agreed that my mom may have declined it at the time (Covid's a very fatiguing illness and my mom even told me as we talked about it a day or two later that she'd just rather stay in bed). By Weds, though, she was doing better and got her shower as scheduled. By the second Saturday (just two days before being discharged home), she was looking forward to another shower but never got it. Especially after it had been raised as an issue earlier in the week, you'd think the ball wouldn't have been dropped there, but it was. Meanwhile, it doesn't sound like my mom got much in the way of physical therapy. As she described it after getting home, it was just once a day for about 15-30 minutes but nothing on the weekend. I understand the fatigue and what not for the early part of her stay at Haven, but as she was doing better, she and the rest of us (her family) think she would've been better served with more (either longer sessions or a second session later in the day). Next, my mom said that when she pushed the call button for assistance, "it sometimes took awhile" before the help arrived. Finally, the nursing staff was occasionally difficult for family to get ahold of with questions. All in all, I think the Haven staff was pretty incredible and my mom was in very good hands, but it seems they may be a bit understaffed (which is nothing the staff can do anything about but the executives can). read more

I was sent to this rehabilitation place from the good ratings they had from my health insurance from banner Estrella hospital and it was a horrible experience I ever had to live with. The staffing is extremely low and I never knew being so neglected in my lifetime. I laid in urine and poop for hours and the cnas were so mean to me and my roommate friend. She went home because she was being abused. She had a stroke and couldn't move the right side of her body and she was tossed around and the fat cna hurt her and yelled at her. I felt so bad for her. I was on my back because Estrella banner had dropped me so I needed rehab to be able to walk again. So I was scheduled for the doctors and they had hoit lifted in my wheelchair and the transportation couldn't get my bariatric wheelchair into the van it was too large so he wheeled me back out and put me in a smaller high up wheelchair in which was working out and he lifted me up in the van with no problem but the wheel had caught up on something so the guy went in the front of my wheelchair and was loosing the wheel, so he never strapped me in and kept saying we're going to be late for he was in a hurry, while I was lifted up and the wheel stuck and then the whole wheelchair went forward and went back and flipped over and I flew back over to the asphalt on both knees and right arm and both feet curled under me. Nobody couldn't pick me up from the ground and they lifted me up with the hoit lifter and put me back in the wheelchair and took me to the room to examine my body. Then they X-ray me and said no broken bones. I was laying there in excruciating pain in the whole right side of my body all the way up my neck in allot of pain. The pain lasted another 2 weeks on my knees hurting badly. Then they wanted me to go to physical therapy for walking and I couldn't even stand but 5 minutes and couldn't take a step. With a lady in front of me.m, one in back and me trying to pull myself up from the monkey rails I couldn't take the pain. Then they said I had to leave because I was refusing my rehab, plus they said my Medicare ran out or if I stayed they would charge me over 6 thousand a month. So I had to leave to another place but they don't do rehabilitation here. So I am still on my back bedridden and still in pain on my knees and feet. All this pain and horrific drop caused me allot of pain and agony. I had gotten a crotch rash from being wet and pooped too long from neglect and a very mean cna. She worked for the graveyard shift. I give this place not even one star. I wouldn't put my family here at all. read more

I simply do not understand all the positive reviews of this place. Unfortunately my father is there for rehab. Except for physical therapy sessions he gets several times a week, he is expected to stay in a wheelchair or in bed all day long. Nursing staff is indifferent. Long, long waits for someone to answer the call button. There have been several times no one answered the call button and my father sat in his own urine for hours. The food is unpalatable and monotonous. Visiting hours end at 6 pm - how can people who work visit their loved ones with such restrictive visiting hours. This place is so poor my father's health is actually getting worse rather than better. I wouldn't recommend this place to my worst enemy. read more

My adoptive father went there and came back in worse condition than he was in before then. The CNA's sat they will do something and don't. The nurses can barely get their jobs done and it is near impossible to find one. They put him on a medication that was on his allergy list, and he had a bad reaction to it. He wound up back in the hospital less than two days of being home. They have tiny rooms that are being shared by two people. Jack and jill bathrooms being shared by four people and you never know if you are going to walk in on someone. There isn't even a shower in the bathroom. The place is severely understaffed and nurses under worked. And I only found a couple nurses with great bed side manner and one CNA or tech. There are two TVs in the room and it's like a blaring contest. This place is horrible and needs some work in order to provide proper care for the clientele they are assisting. read more

NO ONE has ever walked into the door of a Skilled Nursing Home and felt happy to be there, let's be honest. Haven of Phoenix was not my first choice, but wasn't too far from home. After 3 weeks I cannot say enough great comments about the care that my mother received, the caregivers were exceptional they kept an upbeat attitude with my mom. The therapy was also great taking time with her keeping her therapist consistent and encouraging. Nurses who helped me as well as my Mom. They helped us when we had questions to understand because I don't know everything about health care. Give them a try as I did and they won't let you down. read more

##600 Review## I wanted to make my 600th review a little special, was thinking about this for a while. Then I realized, I never written a review for the place my father spent his last days. My father spent the last six months of his life either at the hospital, or at a rehabilitation center. There were some good times and bad times during this span. Even though his life ended in a bit of a surprise. This place really took care of him. On March 8th, 2013 I received that dreaded call at 3:30 in morning asking me to get there soon as possible. When I got there, he was already gone. So I was in there in the room by ourselves. Really didn't know what I was suppose to do? First time I was responsible to arrange everything? Now I wished I could remember their names, but the young lady at the nurse station really took the time to help me. She told me what I need to do, and gave me references. Luckily my fathers brother called me and requested the mortuary. So during the time I was waited for the mortuary to pick my father up. There was a shift change, so when some of the workers started to file in and found out what happened they came by the room. So I sat there and watched some of them pay their respect, I was realizing how much this place really cared. One of the young man who was his nurse. He started to clean the room, changed his clothes, made sure everything was good. You could tell he had a passion for his job, work places need more people like him. Watching this really made me feel better that day, knowing that my father was in good care. For that I will never forget about this place. read more

My previous review of Grace HealthCare is "currently not recommended." However, I want to add that Grace HealthCare, Phoenix AZ, essentially functions as a lockup facility, even though, with one exception, no one has been sentenced by a court to be there. A small handful of residents are allowed to come and go, but, for the most part, no one can leave the building without a doctor's, nursing staff's, and administrator's combined permissions. I talked with one resident who said she had not been out of the building in more than 3 years. I asked Activities (Jessica) if I might be included in trips to the grocery store. She said, "Oh sure!" and then proceeded to ignore me and simply blow me off. Case Management (Nikki) did the same thing. However, the licensed physical and occupational therapists there were excellent and a stark contrast to the direct care staff. I improved markedly due to the therapists. read more

My mom has been here for a week and the staff have been very caring and patient. There are a good many activities that the residents appear to enjoy. I have also observed many residents strolling the halls engaging each other in conversation. My mom's favorite activity is having a pissing contest over who is enduring the most pain and she can talk endlessly about what medications she is on and so she is enjoying her stay (hopefully a short one) at Grace Healthcare. Also, she gives a big thumbs up for the food at this facility. read more

We have had our mother in a few rehab facilities over the years, usually following surgeries. She recently spent time at Haven Health on Drinkwater Rd. in Scottsdale. This was our first experience with this facility. I say, without reservation, that this was best facility she has ever been to. It is a smaller facility and much more personal than anything we have experienced in the past. The entire staff, be it the CNA's, the therapists, nurses, administration, social services, dietitian, and even the cleaning staff, all appeared to sincerely care about those they were taking care of. I think one of the things that brought such peace to my siblings and me was the upbeat attitude of EVERYONE, and the way everyone worked together. When it came time to leave Haven, we were left with the challenge of finding our mother a group home. Jessica Murphy in the social services department was able to walk us through the options and even recommended a placement company that did a fabulous job helping us find the home Mom is now at. Should our mother ever need this type of care, Haven Health will be our first choice, and it will always be the only facility I would recommend to anyone else. read more
