The biggest take away after having my grandfather here for a month is that he lost every last skill he had for ADL and got worse than he was when he got there. He got an infection due to neglect, an open pressure sore, his clothes were only changed 3 times in a total of 4 weeks, and he had two showers in a month. I was very clear with them about me doing his laundry to ensure nothing was lost. Even still, they took his laundry and lost his clothes. I visited him every 2-3 days and spent most of my time at the nurse's station harping on them about lack of care and why he had become wheelchair bound after several weeks there when before he got there he was walking. Myself and other family members would find him sleeping bedside on his wheelchair on a regular basis. He couldn't reach his call light from the side of the bed, and he shared they never checked on him. He got an open sore on his tailbone from sitting in his wheelchair 24/7. The photos they posted are fluff; the place is a totally apocalyptic like. Dirty, old and broken, wreaks, and residents all over struggling to get around on their own. Getting parking in the bottom lot or the garage was usually a challenge. The outdoor areas were also filthy and gross. No real place to visit with residents. And sadly, the employees do not care (aside from Bong and Alex). Social services - useless.. she was aware of these issues and agreed that there was a lack of proper care at a level she wouldn't allow for her own self or family member and nothing changed. Even after a meeting with the ombudsman, the neglect continued until I discharged him because I was terrified he would die there. He went to the hospital for SOB and was sent to post-acute to gain cardiovascular strength and improve his ability to move around (in and out of bed, up and down stairs, walking longer distances). He was admitted to this facility relying on only a cane to walk short distances, he left unable to walk AT ALL, even with a walker. read more

Do NOT send your loved ones here if you value their health and well-being. The images on their Google profile are blatantly misleading and do not reflect the facility's actual condition, which is shockingly filthy, neglected, and run-down. I can't imagine where they got those pictures, as the reality is truly appalling. The entire place reeks of urine and waste, the rooms are dirty, and staff routinely ignore residents' pleas to contact family members. My mother's health rapidly declined under their so-called 'care,' culminating in a severe blood infection caused by an untreated UTI and E. coli. The infection became so advanced that she was left unresponsive, unable to move or speak. If we hadn't intervened in time, she would not have survived another day. They not only failed to provide basic medical attention but also seem to overmedicate residents to keep them subdued, sparing themselves the effort of proper care. Please, if you're considering this facility, take the time to visit and witness the lack of cleanliness, compassion, and basic human decency firsthand. This place should not be allowed to operate. For the safety and dignity of your loved one, stay far away--they deserve so much better. read more

My dad got traumatic brain injury 24 years ago when he was 50 years old. After 24 years of dealing with trauma units, residential care, Physical Therapy facilities, post acute facilities. Country Hills Postacute is ran like a well oiled machine. It is literally a city within a city. Every single resident is treated with and kindness, and love. My dad was released today after a months stay. it was truly bittersweet, not only did my granddaughter, and I developed close relationships with the staff that works there but also the residence. There's recognizable love, understanding, dignity, humility, and respect handed out at all times. Historically, my main concern has always been the patient and or residence care, with all due respect to the residents I met, it's not you I'm concerned about I'm not worried about, it's the staff. The human servants running this facility, are Saints, the complaints, listening to endless chatter, unexplainable yelling , all the while, doing their jobs, smiling, engaging and reassuring the residence as well as the visitors your love ones are in good hands. I have so much love and adoration and respect for every physical therapist, nutritionist, social worker, doctor the human resource, let's not forget the receptionist etc. etc. my father's care was impeccable and he was treated with dignity and love. Keep in mind, I have 24 years of experience with these facilities, I have a history. This has been half of my life as my dad's only advocate(scary) . I left each evening knowing my dad was in the best hands possiible. . Leaving my dad at night over the years, has never been easy, there is always going to be chaos, impatience, misunderstandings, but I assure you it's not from these employees it's from the residence who are handled with the utmost care. 10+ stars. Country Hills Postacute deserves to be recognized. I would never want their job yet. it's not pretty! Kassandra, you managed my dad's recovery journey after a one month hospital stay impeccably. This place has bedbugs. Food awful as well. Your loved one shares a room with multiple people. Do not recommend.

An abomination of a place. Everyone who works here suffers from an awful attitude. They are consistently rude and cold to the guests. The nurses pretend (sometimes) kindness to the patients (probably forced to do so) but most of the staff are otherwise rude, especially when they answer the phone. My brother is a patient here and I pray that this review does not lead them to abuse him even more. The building is ancient and decrepit, a lot of the televisions do not work, there are stains on the ceilings and most of the time the place stinks like a bowel movement. One particularly noxious patient goes up and down the hall, leaking fecal matter. I was standing outside, waiting for someone to change my brother and some 'nurse' tried to imply that I was stealing something. All I was doing was killing time. There are no bed pans. My brother had to live in diapers. We were going to buy a bed pan for him but they magically found one after I made an issue of him having to go to the bathroom in his diaper. The elevators are terrible. They take five minutes to move between floors. The nurses are cold, it takes an hour for a nurse to respond to the call button even when they're doing nothing but sitting at the station, the CNAs are used to do virtually everything, the food looks like it slithered out of a Chernobyl cafeteria, there are people screaming and moaning and yelling at all hours. You can't stand outside or the Uber Eats delivery guy gives you hard stares like you're going to steal his car. The garage is tiny, dark and cramped, like a cave. We drove in one day and a woman ran out, afraid. They moved my brother from one room to another and in doing so broke his glasses, threw them away, and then somehow mysteriously lost his laptop charger. In one fell swoop they blinded him and left with him nothing to do. Many of the people who work here seem evil. Sorry I have to say such a thing but it's true. A lot of people who do nursing are clinical sociopaths and enjoy causing pain to those in their care. Nothing I've said here is false or wrong. An awful place where they send poor and indigent patients. By the way, they take forever to rehabilitate the patient's ability to walk, quite possibly to milk Medicare. I wouldn't send my dog here to die. If something happens to my brother in response to this review, I will call the cops and a lawyer. read more

Country Hills , What a nightmare, and that's an under statement. I am the caregiver for my client Bruce. He went in for a hip replacement, the surgery went well. After he was transferred to Country Hills for rehab, and instead of getting better he died. He was perfectly fine when he got there, then they started giving him all these different medications for depression, to the point of turning him into a zombie. he stopped eating, stopped talking and began drooling , and his body starting to shake very bad. He was not himself any longer. He didn't have heart problems or any other major issues, but suddenly had all these issues, So they sent him to the hospital to get him back to a normal state, then went back to country Hills and that's where he passed away. He was over medicated so bad that he could not even get up to do any type of physical therapy to begin walking again, due to being over medicated. Neglected in every way possible. Myself and his family noticed the rapid change in him and called the doctor there for a call back more then 2 dozen times leaving messages for him to call back and not a single response. We wanted to know why he was like this and not even a word back from them. I wanted to take him out of there, but was told if I did I would be charged and put in jail for taking him out. I feel this place killed him due to over medicating him and neglect. At what point do people say he's lost almost 100 pounds from not eating, and does something about it? I cant even believe this place is still in business this long. not to mention the place was gross and unsanitary to say the least. this place should be shut down and investigated on a serious note. The messed up part is we don't really have the money for a malpractice and wrongful death attorney, but are gonna find one, no matter what the cost is. so where's the justice in that. If you value your loved ones DO NOT PUT THEM AT COUNTRY HILLS OR YOU MAY NEVER SEE THEM AGAIN ALIVE... This place is not even a one star rating read more

If this place has availability and you're looking at yelp to see what's up; here is the lowdown. Don't send your loved one here. There is no such thing as quiet hours. At 12:05 AM my patient was forced to tell other patients in the hall to keep their voice down. He was then woken up at all hours of the night by the staff when he should have been sleeping. They have no idea that people that need to heal actually need to sleep at night. Patients randomly walked into my friend's room while I was on the phone with him. A man stared at my friend blankly & wouldn't say a word until he was returned to the nursing station. This is like the zombie apocalypse. People just roam the halls and go into anyone's room that they want to. Patients do not have phones in their own room. You have to call the nurses desk and they will take the cordless cell phone to the patient. Yes, there is only one phone per nurse's station and the patients have to share it. The food is absolutely disgusting and there are no substitutions or exchanges you eat what you're given. You can't even get a cup of coffee unless it's with a meal. There are broken vending machine machines that swallow your money so you may or may not have only snack food to eat. My patient was given a hotdog casserole dish that tasted like formaldehyde. He ate it and ended up getting food poisoning from obviously rancid food and threw up all night. I tried to send him food by Uber eats the next night and the driver stole his pizza, cheesecake and soda instead of delivering it to his bed. I work as a patient advocate, and I have never once been called by somebody and begged to get them out of a nursing home. I was begged multiple times a day starting at 6:45 am to get my patient out of this place. On Day three I finally had time to come get him and take him home. He can't walk. He can't take care of himself as he awaits an 8 hour surgery. He said being home alone is better than being here. The head nurse seemed completely unfazed by a patient, signing out AMA. They had the form for him to sign the night before I could get there. I told him don't sign it now because as soon as you do you're no longer a patient and they can & will tell you to get out. He did not know that. I'm sure they would enjoy having less work to do. No one bothered to address any of his simple needs for food & sleep. I wouldn't be at all surprised to find out that the nurses kept his pain medication for themselves. They wouldn't give it to him and he's in excruciating pain, and his surgen didn't call in anything to the pharmacy when I told him our patient was leaving. It was just nothing but horror stories. One thing after another made this a hell hole my patient couldn't wait to get out of. I couldn't even reach him. I would call 14 different extensions and just get nothing but voicemail. My mom went to Carlsbad by the Sea for six weeks. It didn't matter that it was a 42 mile drive for me. I put her in the best place I could find and paid an extra $80 a day for her to have a private room. God help those that can't get accepted there. I didn't notice any odors of feces or urine while I was there. I'm sure that comes and goes This place is a total "no go." read more

I was once again a patient at Country hills and received great care. While I was there this time I discovered that they not only have a Hairdresser, they also have a Podiatrist and a Dentist that come in once a month, the hairdresser is there three days per week. You can get a haircut, shampoo and blow dry once a month for free. Theses are some of the niceties. The nursing staff is great they will answer any and all questions about your care. The CNAs are some of the best and very giving to their patients. Therapy people are not going to give up on you or let you give up. They will make sure you are safe to be home. Hello. I spent the end of october 2020 to january 14 2021 here at Country Hills. I ended up there due to heart issues. But mainly mental health issues. The people who worked there were great. Im working at CH since 2021. I gave 4 stars to this facility cause this is where I found a true meaning of dedication. Administrator is very approachable, supportive with employees and also helping on the floor especially during Covid pandemic. I did not see him transported Covid resident to Covid unit before but heard it from many of my co- workers and when I heard it, I was amazed that he did it. My DON is very dedicated in his job, he will make sure that everything is done and checked everything to make sure that care is provided for residents with safety, compassion and dignity. His very supportive and help staff do PASSR and IDT notes. His very involved in every situation especially the one that needs urgent intervention and not afraid of transporting Covid resident in Covid unit during pandemic His very knowledgeable in his position. Our ADON's are very helpful and thorough in following up all concerns and needs of our resident and they are hardworking. Our MDS coordinator is awesome, very knowledgeable in her position and very supportive of all her staff. All our staff here are friendly and hardworking. I gave 4 stars cause we still need space for improvement. But honestly Im very happy to work here at Country hills.
