I happen to arrive here right after they moved from across the street. The old facility was much smaller and they're having a lot of growing pains. I have visited the Grossmont Post Acute Care facility, which is located in La Mesa, very close to Grossmont Hospital, a number of times over the past years. There is a large parking lot that surrounds the entire building, so parking on your visit should not be a problem at all. My Mother stayed here a number of years ago. in addition, my Aunt Virginina from Point Loma was also recently staying here ito recover after a fall in her house. Most recently, my step grandfather's wife was a resident at Grossmont Post Acute Care to recover after back surgery. It is very easy to visit, and there is a little sign-in book in the entry reception area. The buiding itself is located in a beautiful Mid-Century modern structure fashioned in the shape of a star. If a patient contracts COVID-19, protocols are followed and the patient is isolated. read more

The care is subpar. Ask any staff member a question and standard response is I don't know. The promise to find out but never return. The food is not suitable for a dog. You don't even recognize what it is. One night my dad's roommate was actually given my dad's medication. New building looks nice but care is same as old building. Handicap parking is nearly 50 yards down a hill and the location is not ADA accessible. Parking is relegated to another hill with nearly 70 steps downhill to get to the facility. Huge liability risk. Turnover of nurses is nearly 60 percent in first year of employment. They seem to only have 1 doctor who visits daily for a 90 bed facility. Your best bet, when possible, is to recover at home. Go their at lunchtime and see the food. read more

My mom and our family loved this place. Everyone was very nice, and very professional. My mom stayed at Stanford Court in Santee in the past, and they were a disaster. They were always very understaffed, and the patients suffered because of it. The exact opposite is true at Grossmont Post Acute. Anytime my mom needed anything, they were there relatively quick. There were always very nice to my mom, and to the family when we needed something. One afternoon I just walked around and watched the staff talking with patients, helping them, feeding them, etc. and the staff was genuinely caring with every interaction I saw. And anytime my mom had a health issue like her blood sugar dropping low, they would always call me to keep me updated. I can't say enough positive things about Grossmont Post Acute Care. We are very appreciative that they took such good care of our mom. read more

Went to go visit my father who was recovering from a previous illness. He was in a ventilator for over a week and currently doing dialysis 3 times a week. He is also a fall risk because of what happened and very fragile almost skin and bones. Tonight (11/28/2024) around 6:20 PM, he asked his CNA for a bed pan because he needs to have a bowel movement. His CNA, said he should just use the bathroom so she used a wheelchair and pushed him in the bathroom. After he was done, he pressed the call button and waited for 15 mins and no one showed up. Good thing we showed up, he was trying hard to get up and get back on the wheelchair without anyone's help. I was livid so I pressed the bedside call light 10 times and then waited outside the door. When I was outside his bedroom door, I saw four other call lights were activated by other patients. After waiting for a long time, a CNA came and assisted our father. She said she was not the CNA assigned and the CNA assigned to my dad just took her "LUNCH BREAK". Yes, she said lunch break at 6:20PM. The CNA assigned to my dad is a FILIPINA. She could've waited for my dad to get finished in the bathroom before taking her precious break to ensure my dad's safety. Or at least informed another CNA that her patient is in the bathroom. If a patient who is a fall risk asked for a bed pan, they should have provided it to him and cleaned him up instead of putting them in grave danger from falling. If you wanted a safe environment for your family member or friend to recover, take them somewhere else. Day crew was all over the place and very attentive to their patients, night crew seems like they don't care at all. This FILIPINA CNA should be fired for putting a patient in danger. I'm pretty sure this is not the first time she did this kind of thing. I'm also sure she did not want to go to any trouble in helping clean up her patient or go and flush her patient's feces in the bathroom. This place is just waiting for somebody to get injured or maybe die from the lack of patient care and then they will change their ways when legal action is taken against them. Again, TAKE YOUR FAMILY MEMBER somewhere else. Yes, I do know how hard medical personnel works. I used to work in a VA Hospital where we wipe their feces, vomit off of a patient. My dad stayed here a while for physical therapy after being in the hospital for a while. His nurse aid was really funny and laid back I liked her. The food they gave him didn't look that good like it wasn't cooked good. When you go in I didn't really get a welcome feeling in the front desk. If I could give no star I would.. my mom was there less than 24 hours after being in the hospital for two weeks and she was very sick with pancreatitis and has a specialized diet my father and I work full time so we were hoping it'd be a good place for her to regain her strength but that was not the case, she needed to use the restroom and called for someone and no one came for 30 minutes my mom has a very weak bladder and almost wet herself her little roommate in the same room as her wet herself because of how long it took to get assistance. On top of all that they didn't give her food accommodating her diet which could make her even more sick we took her out immediately and brought her home. read more

I broke my hip on Thanksgiving and this is where I did my rehab. The Physical therapists are outstanding. Liz, Brandon, Suzi all of them were fantastic. Not only did they give me support they encouraged me that I could recover and I have. Today less than 8 weeks since surgery I am walking with a cane only. All the CNAs were kind and well trained. I highly recommend Grossmont Post Acute care for your loved ones, my 93 year old mother took a fall September 2024 and needed therapy to bring her back home. Dr Smith was very attentive and provided a high level of care along with therapist, nurses, CNA's and every single staff member we came in contact with. I was very pleased with my mother's care, it was also good to hear my mother express how well she was treated from day 1. This was our first experience having mom away from home but I highly recommend also for their cleanliness, food and the list goes on. read more

This place is absolutely amazing. My 87 year old grandfather needed therapy after having blood clots in his leg causing him to feel very weak and unable to walk. He had an unparalleled Physical Therapist. He cared for him and did a great job showing us what we needed to do, to help him continue gaining his strength. The nurses were always so attentive especially since my grandfather has dementia. I recommend this place for anyone who's needing care for an elderly loved one. read more

Honestly we were really worried to send my 84 yr old mom to a rehab facility after falling and breaking a rib, fracturing her pelvis and hitting her head, because some of the north county ones have terrible reputations.. one is even know as a "kill center" by paramedics so we decided to choose this one even though it was 45 min away but I can say our family is so happy we did. She was there 20 days and they just released her yesterday. She couldn't move when she got there, was put in a room with 3 other women and at first it was she was scared but they moved her when space opened and she met two beautiful woman in her new room and she finally successfully walked out with a walker. The care, especially the kind nurses that work so hard and the amazing PT and OT therapists worked tirelessly and you can see the results, they are amazing. We hope she continues to get better but we are so thankful to this staff for getting her this far. Thank you read more

Sad to have to update this. Matthew continues to be amazing. Tia is also awesome. Kerry is a sweetheart. But where oh where is Dr. P? My mom has asked to see him for three days straight now to no avail. The nurses have called on her behalf. Dr. P is no where to be found. Meantime- my mom's foot/leg is swollen the size of an elephant trunk. Very good chance could be a blood clot. Was told they don't perform "dopplers" (Ultrasounds for blood clots) after 5pm. So in the meantime- it could turn into a pulmonary embolism while we wait to see if Dr. P ever shows up. Nurses and staff are great- but they are bound by the doctor who clearly is in dereliction of his duty and oath- and what's worse- HE DOESN'T CARE. So Dr P- if she is not your first patient tomorrow- we are going to have a problem. The center has my number. I will be expecting your call first thing. read more

I was not going to do a review but I did read some bad reviews about this place so I wanted to add about the experience my mother had. She was release from the hospital on June 2nd to this facility. She wrote here at around 5:30 pm. The first day, I was really ready to take her out. I think I was just upset and a bit emotional. My husband and niece said, let's just give it a try and if it doesn't work out, we take her out. Well, two weeks later, I am glad I didn't take my mom out of this great place. The workers, nurses, cnas, and staff were excellent as well as the therapists. The food for the most part mom enjoyed. After a few days, she was able to join the activities and she even won once at Bingo. Today, she is being released and I want to thank Grossmont Post Acute for the excellent care given to my mother. My dad was admitted to this facility on 5/26/24. At first I thought we're on a good path to recovery being he needed intravenous antibiotics. Kaisers hospital social worker said patients request this facility and when I was visiting another patients son said it's a great facility. It may depend on what is needed regarding recovery. My experience: Upon arrival I told the staff my dad is gluten free. That seem to have fallen on deaf ears, when I came during lunch, his lunch was not gluten free and it happened again thereafter. I finally spoke to someone to change it in their system. I feel there is a lack of communication. My dad has a Foley catheter and while I was in the room the staff put his catch bag on the floor. I was in disbelief and told the CNA that's a big no. You think this is a given being she's a CNA. Next day I return only to exhibit the same behavior from the same CNA. I told her I already spoke to you about this! Today (5/31)... I come to visit and now I see my dad's mid--line, hanging out! I will not use this facility if we need future nursing care. I made it clear someone needs to be held accountable for the mid-line mishap. read more

I was a patient recovering from hip surgery and was treated fairly well. I feel they need more CNA's to add the patient to staff ratio. The food is pretty good. They do their best to customize your food choices. They need to upgrade their outdoor areas from the patients rooms. They have a very good physical therapy department. Physical therapy and care center for post hospital visits. Furniture and light fixtures are outdated. Rooms are not well lit, cramped, and musty. Not enough seating for visiting family members and - as mentioned - rooms are claustrophobic so it doesn't help that they cram two patients into one room. Nice outdoor patios with grassy areas which are great for escaping the cramped indoor areas. Food is pretty decent it sounds like. My dad at least had no complaints and mentioned that it was quite good. It didn't look extremely appetizing to me though. Way too understaffed and nurses and CNA's are a bit overworked. This is reflected in the way in which some treat visitors although I found some who were also quite pleasant. A lot of untrained staff members who do not know how to work their already rickety devices. On multiple occasions, my dad was nearly dropped by the 'crane' that was lifting him from his bed (he couldn't get up without assistance). Staff is very disorganized and forgetful probably because they are stressed. It also sometimes takes them a long time to answer a patients' call. This is especially the case at night when the hallways are a ghost town. What I especially dislike is how quickly they try to 'kick out'/ discharge a patient. They try to so within 7-10 days after a patient arrives and really push a patient during physical therapy sessions. In this light, I do not feel like they give the patient the ultimate care and attention they need. Physical therapy takes time; it cannot be rushed, but the higher ups here don't seem to take that into consideration. Probably the most affordable option out there in comparison to others, but just some things to consider. read more

My mother fell and broke her hip on March 8th, she had fracture repair and on the 9th. She had a catheter inserted on the 11th. She was transferred to Grossmont care on the 12th. I ask on the 13th for the catheter to be removed. Dr Pecherskiy, said we would talk about in on the 14th. I did not hear from him and noone at the facility knew anything about removing it. I contacted her PCP on the 15th, she said I could request it be removed. I talk with Dr. Pecherskiy, he told me it would be likely that she would fail at urinating on her own and they would have to reinsert a catheter which is more likely to cause infection than leaving it in. I don't believe this is true. I ask again on the 16th to have it remove, no one at the fancily would do it. They told me it would be remove on the 17th. It just kept getting put off because they did not want the bother of taking her to use the restroom. I wrote a letter to Dr. Pecherskiy and the Case Manager on the 18th that we were very up happy with her care and would be taking her hone on the 21st. The Catheter was finally removed on the 20th ( 10 days after its insertion) "Catheter-associated urinary tract infection rates increased with each additional day of catheterisation." National Institute for Health and Care Research. At home she continued to have more pain than we expected, she was taken to her PCP on April 3rd. She did have a urinary tract infection and it was also discover that the nail had come out of her hip and she would now need hip replacement surgery. She has had that surgery and did not even need a catheter. Other complaints we have is that she was left sitting in her wheelchair with wet hair by herself crying and unable to get help after a shower. We were not able to watch PT to see how she was doing. Her roommate had COVID. Also, her next roommate was left in her room with her oxygen turn off and could not breath or get anyone to help her. My brother turn her oxygen back on for her. Don't leave your loved one here. read more

The staff was great. It's primarily a Rehab facility good PT & OT.Food was adequate for that type of care. The BIG DISAPPOINTMENT was not getting the clothes & underware I arrived with. The laundry or some where in between lost all my clothes. No explaination no appoligies ? I went home in a backless hospital garment. I E-mailed 3 unanswered requests & stopped by to leave a written request that was ignored. They could have paid to have my clothes replaced but I got no response. My advice would be to not wear any clothes that you really would like to keep! Kaiser returned all my clothes after surgery but GROSSMONT PAC TOOK MY CLOTHES. Phil Thielen read more

Recently my Mom had a pretty major stroke and I had to do some reconnaissance on post acute/rehab centers in the east county where she would need to spend a few weeks. Quite frankly I was freaked out reading reviews about this place AND others. It was a mixed bag, where you will really need to just use your best judgement after visiting each place and drawing your own conclusions. After reading reviews, and health reports on other sites - we landed on Grossmont Post Acute. I almost gave this place 4 stars, but could not based on one major issue. Just remember people, this is basically a medicare/insurance rehab center -- this is not the Ritz Carlton and if you want private care, spend the money and get your loved one that level of care... be fair when drawing conclusions based on the ratio of patients to employees. Let me start with the good though. The place is clean, small (90ish beds) and the front desk staff are VERY friendly, welcoming and nice. I found the room that my Mom was located in to be clean and comfortable, with lots of sunshine. My Mom loved her CNA's and Therapists. We did too. They really looked after her, and were kind and gentle with her. In particular Irene (both of them because there are two) and Judith. The CNA's and therapists are the ones who are really doing the hands on stuff and caring for your loved one. What I did not like was and found concerning was the attending Dr. (Houser) and the higher level nursing staff who did not seem to take issues with my Mom's condition seriously. Without getting into details they were made aware of relatively serious issues that took them 4-5 days to address appropriately and by that time she had to be readmitted to the hospital. This is not an exaggeration as I personally kept advocating for my Mom each day and was pretty much blown off. The higher level medical staff - LVN/RN/MD are what makes this place 3 star v. 5 star. I am thankful that the CNA and therapist staff looked after my Mom to the best of their ability. If you can stay on top of the other people that work here -- you will find this place to be a safe enough place for your loved one to rehabilitate. read more

This is a skilled nursing facility located in La Mesa, near Sharp Hospital. They offered physical and occupational therapy for a post hospital visit. The staff helped care for my Mom after a week long stay at the hospital. She was took too weak to come home. Grossmont Post Acute Care, helped her with physical therapy and occupational therapy to help get her stronger to be able to come home. The staff was super friendly and warm towards my mom. She even seemed to like the food that they fed her, however given the fact that she typically eats my food, this does not say much for their cooking. LOL The room that my mom stayed in had a TV for her and one for her roommate. The staff helped keep my Mom on track with her medications and treatment. The facility is a little older however they still have up to date equipment and treatment plans. The staff was organized and kept us up to date on the progress of care and then discharge information, along with post treatment needs. There was plenty of parking and I did not have to pay for it since it is a private lot. Not too far off the freeway for easy access. For me, I liked that they were in a location near the Grossmont Shopping Center, so that I could stop by and run errands and grab food. read more

I have been to 5 nursing facilities and this one is at the top of the list..Five stars is really not high enough..I spent 12 days here and arrived in pretty bad shape, 12 days later, i WALKED out..The entire Staff, the PT and OT Staff, the Nursing Staff, the CNA Staff, the Kitchen Staff and Cleaning Staff were ALL outstanding. The many helpful students there were a plus. The Doctor and PA I had were incredible and it all started with the Case manager and Social worker. I am truly appreciative and my quality of life is greatly enhanced thanks to This facility. Outstanding people here. jc read more

The 1 nurse I did encounter was very nice & she looked totally sincere when I went there to pick up my Uncle last night. I didn't get to go inside because of the pandemic we're in. But it was right on Center Dr. Like driving up the little hill on Center Dr. (don't go all the way up because that's where iPhone map took me.) the first lil turn in on the right & you swirl kinda down into the one way in & one way out lil building with lots of parking there. read more

Their facility is nice. I especially liked their nurses center and shower room. However, security is non-existent. Anyone can come & go as they please. I was shocked by the food they served. Almost all fruit & vegetables are canned. Instead of juice, they served us red punch. Even the oatmeal was either too runny or lumpy. I was disappointed with their physical therapy. It was over staffed. Many therapists were constantly talking to each other and eating (with ants crawling around the room). They had me repeating the same exercise within the same hour, even though they had observed that I could accomplish it with no problem. The rest of the facility is extremely under staffed. The CNAs are all very nice but stretched far too thin. Each with too many patients to serve. They are running back and forth but response time for assistance is way too slow. The final straw was when I asked repeatedly for my pain medication. After three hours of waiting, I fell down trying to get the medication nurse's attention. I checked myself out the following morning. Please don't let your loved one to be placed in this facility. read more

I do rounds to most of the skilled nursing facilities in San Diego county as part of my job. I have read a lot of reviews for a lot of rehab centers like this facility. Couple of things I want patients and family members to understand, Skilled Nursing Facilities or rehab centers ARE NOT places you can leave your loved ones for long term care. If and when you have a chance to choose rehab centers after a hospitalization of a loved one, please tour at least 3 facilities for comparison. Comparably, this facility is very decent, hence 4 stars for me as far as quality of care, cleanliness and food. These rehabs are also profit organizations just like hospitals and though ARE tightly regulated by the state, there are criteria and allowable days covered by insurances, if you have HMO, chances are you will only be covered up to 2 weeks, medicare, 20 days without co-pay. So, if you want full time care givers for your loved ones, YOU MUST PROVIDE your own at your own expense (pay privately for private care givers) If you have an expectation that your loved ones can be left in this kind of facilities for them to take care of because you cannot care for them, that's not part of the role of the rehab or the skilled nursing facilities anywhere. These facilities SHOULD be able to make your loved ones appropriate for discharge back home or assisted living facilities for more on going care BUT these facilities are not obliged to house your loved ones for long term. It always depends on your insurance. read more

The worst place ever skilled nursing my ass I do not recommend this place to nobody. If you really love your loved ones do not take them to this place. They treat them horribly instead of getting out of there better they get out feeling worse. With anxiety and depression and on top of all have to recover from the traumatic experience They had. They leave the patients waiting and waiting to go to the bathroom to take their pain medication to sit them up to put them down on their bed. You can call the nurses 50,000 times and they still won't come. They don't care about your loved ones they treat your loved ones like nothing Of course it's not their family it's not their blood it's like anybody else for them. It sucks that people that work in these places don't really care about their job the worst place ever by far Theyshould be ashamed. My loved one could not take it anymore always crying and complaining since day one that they never listen always ignoring and Forgetting about her and making her wait for for a long time for her to even get her medication go to the bathroom putting her to bed it sucks that places like this are still open. They don't even deserve a star unfortunately I had to give them one in order for me to post this how sad read more

NOT EVEN WORTH A SINGLE STAR! wow, where to start....if you want your parent to get worse and never be the same again, send them here. I reeeeeally tried to put up with this place. It was hit or miss with the staff, obviously very short staffed. The care is VERY inconsistent and very unsafe at times (such as checking/not checking Blood sugars and giving insulin an hour after a meal). I also noticed that when my dad's catheter was taken out and reinserted, they had back dated the catheter to the admission date, as if it was inserted then (which was several days ago, but it wasn't). They left a heplock IV in for DAYS from the prior hospital and it wasn't being used and we kept asking everyday for someone to take it out and it never happened until we were angry. No one returns my emails or voicemails, and we tried to reach social worker and case managers on several occasions but kept getting told that "they're coming" or "they're in meetings and on their way", and never showed up. We waited 8 hrs one day and checked at the front desk every couple of hours and no one came to give us answers as promised. We were told the DAY before being fined (after begging for answers) that we were to pay $160/day for the stay, after being there for 20 days...and we have been trying to get someone to talk to us for weeks.. People, ESPECIALLY the MD (who we have never seen nor met) NEVER listened to my dad. Our dad has history of stroke and poor comprehension, but no one seemed to care and assumed that he understood or meant what he said, even though that wasn't what he meant/said. Now they must be mad at us for demanding answers because they're dumping my dad back at home with a catheter and diaper dependent, incontinent...and he was NEVER like this before being here. We asked SEVERAL TIMES to trial without the catheter, but one day the Md claimed my dad refused for it to be out...but really my dad wanted to discuss more about what was happening because of all the trauma he received from the last trial, and I also believe that they didn't wait the full 6-8 hrs to allow my dad to void because he quickly had a catheter back in in no time. Then days later they said that they would take the catheter out...and now they're saying, "well you're dad can't have the catheter out for A VERY LONG TIME and you have to accept his condition is worsening and figure out what you're going to do". I told them that I cannot be with my dad to care for him 24/7 because I have a family, child, and work full time night shift, yet they don't listen. I told them that our family is not comfortable with this discharge, and we are not on board with this. They told us that a nurse can come to our house 2x/wk for 1 hour. What is that going to help? My dad is wheelchair/Walker dependent and bowel incontinent with diarrhea and an indwelling catheter. NO ONE IS LISTENING TO US! This is not safe! Our dad has one kidney left and history of UTIs..this is not good. We've never seen this MD who loves avoiding us. Dr. Pecherskiy. I'll update this later with all of the names of everyone. And a follow up... pray this never happens to you or your family. Update: Went to dad's follow up and his primary told us that there were no notes about him having a catheter, and there was no indication for it, so they took out his Foley and he was able to void and he had a UTI! Home health is now following us and they were shocked at the condition my dad was left at, and that they sent us home with NO wound dressings, nor education on anything about the Foley or the wound dressing changes. Luckily, I know the basics because I'm an RN. My dad also has foot drop in his left foot, which he never had before the nursing home. And no sensation in his left foot at all. read more

My husband has been admitted to this facility three times and each time he has received the best care I could ever ask for. The staff is very responsive, caring and competent. The patients are treated with the utmost respect. I can't recommend this center more highly. I especially commend Irena who treats my husband as though he were her own family member. read more

09/18 - I initially stayed at this facility after a broken ankle surgery, even stayed in hospital for an extra day to wait for an opening as I heard such good things about this place. The food was very good, nurses were pretty good responding to call button, the physical therapists were wonderful and knew how much to push and time for you to rest. The only negative is they would come to your room without notification so get dressed early (before breakfast). I also want to mention how clean they are when mopping (every day) and making your bed. 02/19 - Another ankle surgery to fuse ankle and I wouldn't go anywhere else. I knew what to expect from the first visit and had made friends with everyone from first visit. Loved ones can visit at anytime. To date: I may need another surgery for re-do of fusion and would go in a heartbeat but now I have Kaiser. read more

This place is wonderful! EVERYONE was so attentive and caring, from nursing to PT to Dr Lai. My father"s situation was especially difficult as he was 96 and had fallen recently.
