It is difficult to rate skilled nursing facilities because people go there out of need not want. However, [[HIGHLIGHT]]when I walked in, I noted it smelled clean and the rooms also were clean and the staff were very friendly.[[ENDHIGHLIGHT]] I am also realistic, that things can happen in an environment in which there are many people to be cared for with too few staff. My loved one was evaluated by OT and PT, physician and LVN none of which knew he had very poor eyesight until after I divulged that information, which makes a big difference in a skilled assessments. They also were not aware of his baseline before his hospitalization, which was fully independent. This Made me question chart review, and how much information was passed on to the facility from Kaiser hospital. Unfortunately, he fell that night after being provided medication that made him confused even after we had discussed it. If I had a loved one, that was there, I would be questioning what type of monitoring is taking place after medication's are administered, have they reviewed the medical history and discuss the goals, intermediate and long term to be sure everyone is in a realistic and shared place. This fall could have been avoided and is not looked on as positive for any facility but reality is it can happen. What they do to prevent these events is important. Terrible. My grandma was a patient here and she was so neglected. We told so many nurses/CNAs and even put a sign over her bed that she won't drink water if it's not cold. We came everyday, multiple times a day and she NEVER had cold water. Furthermore she wasn't eating and they kept giving her the full dose of insulin even after we requested otherwise MANY times, leading to her having many hypoglycemia episodes. We even requested her having a sitter at meals to ensure she eats, but of course we didn't get that. The cherry on top is us bringing my grandma home AMA & her having a pressure injury on her heel... how this was missed & caused is unacceptable. I had to ask them to turn her more often because she was laying in the same position every time I visited her. But she still has one. I checked out several facilities for my wife's therapy. I chose this one because of their longer visiting hours, and no foreseen issues with on-site/ nearby parking. This is a well-managed facility with caring, dedicated staff. Victoria Post exceeded our expectations when it came to providing patient-specific equipment, needs and services. Physical and Occupational Therapists were skilled, professional and accommodating. Physical therapy room was well-equipped, cheerful, and engaging. This is an old, but clean facility, and well-maintained. The outside common areas are park-like settings. read more

The night my husband was brought to Victoria Post Acute, he let the nurse know that he needed a cpap machine because of his acute sleep apnea. The nurse asked him what that was, before bringing him an oxygen machine. It was the beginning of days of nurses who didn't seem to know how to properly check blood sugar; meals that were not always diabetic friendly, and tiny, unapetizing portions. [[HIGHLIGHT]]When I asked to speak with an administrator, I was directed to the marketing director, who was immediately adversarial, was dismissive of my husband's concerns, and frankly rude.[[ENDHIGHLIGHT]] My husband ended up leaving against medical advice because he didn't feel safe there. read more

Worst experience! Kaiser case worker mentioned this rehab center was very good. Lied. Mom went there after massive stroke, they wouldn't feed her because she slept through breakfast. Forgot to change diaper, neglected changing her. She got a rash. She was on soft foods, they served a HAMBURGER! They overlooked her insulin! They were giving her Norco every 4 hours! Without pain! When instructions said as needed, unfortunately my mom is nonverbal because of stroke. She couldn't defend herself. We decided to take her home. As that place is not suitable for humans. Staff is incompetent, rude and LAZY! I can't believe this place is still running. These people are on the wrong business, they don't care about humanity. read more

This place is disgusting and nasty! It is completely delapitated. There are holes in the roof and when it rains there are leaks and they have to put buckets out to catch the rain leaking from the roof. Aside from the cockroaches in the bathroom the water from the sinks in the rooms trickle and I couldn't wash my hands after I tried to help my dad get out of his dirty diaper. [[HIGHLIGHT]]Don't bring your love ones here, the staff completely ignore them and leave them in their diapers with urine and feces.[[ENDHIGHLIGHT]] My dad ended up with bed sores down to the bone on his butt for leaving him in his bed for 2 days. I was a patient at VPAC for four weeks after major spinal surgery. The care and compassion I received from staff was outstanding. I arrived unable to move my leg and left being able to walk 60 ft with a walker and stand for over a minute. I was amazed at the hard work and competence of people like my OT therapist Kim who not only challenged me to do my best but also reinforced the need for being patient and took the time to train my family, Kirsten and Joyce who also helped me and gave me encouragement, nurse Melinda from Kaiser who was my advocate and checked on me periodically, doctor Mirza also from KP who kept on top of my condition, nurse Beth who found time to dress my incision daily, nurse Claudia who also gave me excellent care, nurses Bethany and Mike who cared for me and gave me medication and catheterization as well as many overnight nurses whose names I did not get. My PT team was also great, if overworked. I got great care and encouragement from Bessie, Mike and Miranda. My input for management would be to give them the staffing so they can provide their excellent care instead of spending so long bringing patients to the gym. The food was great and I could see the care put in by the kitchen staff. The cleaning staff, on my case doña Maria, was attentive and detailed as well as friendly. The CNAs are caring and compassionate. They helped me with empathy and as fast as possible. Again management may want to provide more staff so that a brief change can be done within half an hour...I witnessed how attentive the staff is but they just have too many patients. I want to single Giancarlo, Hazel, Ashley, and David for their support, competence and friendliness. The activities staff was superb and made sure we were aware of what was going on like VR demo, bingo, and socialization. The local cat Tinkerbell walking back and forth in front of my window was a source of joy and I hope management lets her be...she is the closest to a facility pet. The garden with the fountain was a beautiful place to sit and relax with family and friends. The front desk staff was always kind with my visitors and would nicely bring me items I occasionally ordered through DoorDash. Another input for management is to make sure there are enough slide boards, and wheelchairs so the therapists don't have to scramble to find them. I was impressed at how seriously COVID masking precautions were addressed I came to Victoria scared and vulnerable and the staff there embraced me and helped me get better. and I will always be grateful. read more

Horrible - just horrible. Food: execrable. My room and bathroom were dirty and full of bugs. Many of the staff acted as though they were doing me a favor by doing anything at all for me, i.e. food trays, assistance to bathroom, bringing meds, etc. Best: CNA trainees - they were terrific; dealt with them whenever I could. Some good PT folk; they helped me a lot. Worst: Had to request 4 roomies in 4 weeks!!!!! Snorers, criers, talkers, TV 24/7, loud machines, walkers. My favorite: roomie accused me of stealing her bed, very funny, but she was serious - unfortunately. All in all, it was a pretty ghastly experience! Never so happy to leave a place in my entire life. I've been a Home Health Liaison who gets to help patients come home from VPAC. Many times I get to meet patients in person. I get to work with many of the SNFs in East County. What I love most about VPAC is that they have had a consistent director and clinical team. The Admissions Director, Administrator, Social Services, Case Manager and the Rehab Director have been at VPAC and taking care of patients together for years. This is a collaborative team with the goal of getting patients taken care of. When I speak to patients who come home from VPAC, they are happy with their care and strong enough to come home safely with Home Health :) The BEST results. read more

Imagine for a moment abuse and mistreatment of your loved one during a pandemic. There is no rescuing them. You feel utterly hopeless. For weeks you hear a barrage of heartbreaking complaints. You call and advocate for them, but the majority of time it's fruitless. Imagine they fall from grogginess/sleepiness all because of a certain night shift worker that they were afraid of. Then they fall into a deep depression, cooped up in their bed dreaming of better days because they shouldn't have to live their last days like this. And yes, those were his exact words, his last days indeed. He died without dignity here. Imagine the fear of your loved one having respiratory issues along with their roomate. You wonder about Covid-19 but there are no tests performed even after several pleas. And rudeness plus a lack of sympathy from the care handler after your loved one just expired. You hang up the phone in disbelief that a person could even be this cold. The grief hits you like a thousand shards of glass to your heart. Imagine a sentimental artifact of your loved one, a beloved cross necklace that was promised to you by the same caregiver on more than one occasion, but you never received it. And your calls dodged when you wanted answers or even answers for just peace of mind in general. please please read this review before you choose your facility. do not send your loved ones, friends or family to this place... if i could give them negative stars I would give them a thousand... where do I start??? 1. my mom was discharged to this facility on Aug. 16th 2021. she was in covid unit for 14 days, then released to a reg room. she was on 5-6 liters of oxygen at this time. you have to make an appt to see your loved one so they can have them ready to be brought out to the tables in front of the facility. several times we had our appt scheduled and waited 30-45 mins for them to bring her out to see us. why is an appt made if she is not ready??????? several times when she was brought out for her visit she did not have her oxygen tank with her... remember she is on 5 liters of o2. we had to ask the front desk where is her oxygen??? this is something that is extremely important that should not be missed in care. there answer was "oh we forgot" 2. I visited my mom from Aug. 17th - sept 26 every day... i never every saw them wipe down any of the areas that people met with there family or friends. when i checked in pens were never wiped in between visitors. the check in process was never consistent there were times that i put my own temp down and gave the clipboard back to the staff. 3. Mom complained to me on several occ. that she was not getting a bed bath consistently. she ended up with bed sores and a huge rash on her back. m - f she was very well she was walking with her walker and doing PT, OT. the staff / nurses on the weekends is inconsistent. every time mom went backwards in her weekday progress was on Saturday and Sundays. I can prove that Friday she was walking and Monday at pt she couldn't even stand up with assistance. she told me weekend staff missed her pills one time. ect her urine bag had sediment in the tubing we had to mention it to the staff. this should of been monitored by the nurses. 4. i was called on Sunday sept 26th that mom was being taken to the sharp Grossmont hospital. i asked why what's wrong?? they had no answers they said we think she might be having a seizure but we are not sure that's why we called the ambulance... when she was admitted to icu at Grossmont hospital i received a call from the dr. on staff with a report of how my mom was doing... he said she is recovering well then proceeded to tell me you know with the heart attack she had there is going to be some recovery time... i said what????????????? heart attack??????? he said yeah that's why she was brought over here... you didn't know that? she was given cpr at the facility and was without a pulse for approx 2 min... soooooo how the "f" did you not know she what was wrong if she was having cpr??? the dr. said they should of told you why she was being transferred that is there job. 5. they are breaking hippa violation when people outside of her contact list calling and requesting information re : her status. i had to explain to them that they should not be giving any info outside of who is on her chart contact list. 6. I paid moms copay for the month of sept. this amount would carry her until sept 30th. when she was rushed to the hospital Sunday, Monday am the facility called me asking for 308.00 "to hold her bed." I said how you going to ask for more money when she is paid at least for 3 more days... so seemed to me all they cared about is the money couldn't tell me why she went to hospital but could call for $$. didn't even ask if mom was ok or anything that showed concern... finally I have called 4 times now to speak to a supervisor and the director of head nursing in the facility and still no call returned... so to me they have no care for my concerns while my mom was there at there facility and i will never recc anyone here. read more

A friend was admitted here so I stopped in to visit. From what I could see she was well cared for. The lady that was manning (or was she womaning) the front desk was friendly polite yet proffessional and helpful. The facility appeared clean and well maintained. My only complaint is that I was pinned in by an ambulance when I went to leave and it took a lot of manuvering and skill to exit the parking lot. As I sit, writing this review, my father, who was sent to this facility by Kaiser, is gone. We spend our days settling his affairs, and the rest of our time missing him, wondering if he might still be here if things had gone differently. What I can say, with absolute certainty, is that this facility was of minimal to no help in the matter. Rather than take you through a long and drawn out account of what happened, I'll give you the short version. The two things to take away from this review are listed below. 1) Kaiser is, in my opinion, not the best insurance option (Sharp Rees-Stealy is fantastic and a great choice from my personal experience with them), AND 2) Find a better option for a skilled nursing facility (I'd recommend Grossmont Post Acute Care instead) - The staff was, for the most part, unresponsive and had little to no accountability (they regularly neglected to call us back when they said, if at all, or couldn't provide answers that were helpful and/or relevant) whatsoever, including Robin, the social services director, and Victoria, who had no idea why our father was there, or the even the most basic facts of his case. There was also the omsbudsman, Sharon, who used the phrase, "It's difficult" to respond to many of our questions, the receptionist who told us when we showed up for a meeting with Dina (another social worker) that there was no such person who worked there and then watched her come out of a room, and Merle, who had difficulty with being punctual or returning a phone call. - There was a lack of communication between staff members; we got conflicting stories from the staff, or had to explain the same things OVER AND OVER again to different people (Why weren't these things being noted in his chart, and if they were, why was nobody bothering to read them?). - Other health concerns were left unaddressed, after they were mentioned numerous times to staff, who responded as though it were new information each time. In the end, it was put on the back burner so that someone else/some other department could handle it! - By the time they figured out that he wasn't getting better and was declining, and sent him to the hospital with a high fever, it was too late. - When my husband went back to pick up his personal effects, he was ignored at the nurse's station. They were discussing personal gossip stories which had no relevance to health care or a patient. It was a few minutes before he was acknowledged by a passing nurse, who then managed to get the attention of the people at the nurse's station. Then no one knew where his personal effects were kept, nor his file so he could sign off on receiving the items after they were handed over. No one asked about how he was doing, except one man, who after being told, did not seem to care. They also had no idea that he had passed. My wonderful, brilliant, charismatic, loving father, who was tough as nails and survived World War II, a car accident of serious magnitude that would have killed the average person, and 38 years of marriage to a woman who could be politely described as "difficult", went to the hospital after less than a week, and he never came home. After living through all those things, he didn't survive Kaiser, and he didn't survive this place. read more

I think this place should be called house of Angels. My mother had to stay here for 2 months after a hip fracture and then hip replacement, and I really can't say enough about this facility. My mom received the best care here, the entire staff, from nurses, doctors and front reception did an outstanding job. Anyone would be so lucky to have rehabilitation here. read more

If I could leave this review with 0 starts I would. This place is unsafe, inhumane and down right disgusting! It took all but 2 weeks for my grandma to pass away once arriving to this horrible place. Two weeks of them not feeding her, two weeks of them drugging her so she could stay asleep, 2 weeks of keeping her dehydrated. 2 weeks of no one giving a single ounce of care to her. This place truly needs to be shut down, the smell is horrible (you can tell they don't regularly try and change people), none of the nurses know anything you ask a question and it's always "I don't know" or "I don't handle that" or "you'll have to ask someone else". To leave someone malnourished for WEEKS is beyond me and I don't know how these people live with themselves. All I have to ask them is if it was your parent, your grandparent or your loved that was being treated the way you treat all these other peoples loved ones who are stuck in your care? I KNOW they would be LIVID because as someone who's in the medical industry you should know better. I hope this place gets shut down! My grandma passed two days, TWO DAYYYYSS after her birthday because of this incompetent company. DO NOT BRING YOUR FAMILY HERE, take all of these bad reviews you see as a sign that this place is horrid! And if my post gets deleted I WILL post another review! read more

The rehab team at this facility is one of a kind with a caring and compassionate attitude that your family members will love. My grandmother enjoyed her stay here and really connected with Sam and Jacalyn who demonstrated the kind of patient care that you look for when you send your family member to a nursing facility. The culture is improving under the new administrator Colton, who you can tell is doing everything he can to make this building live up to its potential. The rest of the administration is caring and also provides a customer first attitude. Which is an improvement from the last administration based off previous reviews. They were supposed to care for my grand mother and instead drugged her and served her foods she couldn't eat and then finally when they got it right no one cared to sit there and help her feed herself they didn't pick her up flip her side to side they just let her die off if they had cared one ounce she'd still be alive and she just passed away 2/25/24 two days after her birthday if she had never come here she would have gotten the love and care from her family don't trust this place with ur loved ones if you care for them. This place here has no type of care besides letting there patients sit and die away and sit and smell in there own bodily fluids they don't keep up with there records and know nothing when it comes to there patients they do less than the bare minimum. Lilly is an advocate for patient care from day one. She gave my husband a thorough and detailed tour of the facility while I was still in the hospital. We had a horrific experience at another rehab center and were very leary - Lilly listened to our concerns and addressed each one. She came to Sharp to meet me and introduce herself before we had decided what facility to come to and that sealed our decision. She coordinated my transfer personally and came to the hospital again the morning of transfer to let me know what time to expect the ambulance- she let her nursing staff know the issues I had previously getting my pain meds filled and administered in a timely manner and they were prepared with my meds list upon arrival and made sure patient care and comfort was in the forefront. Any questions and concerns were immediately addressed and Lily even gave me her cell phone number before she left the facility for the weekend in case I had any issues or questions. This true concern and accessibility eased my anxiety of being in a new environment. My CNA Care has been empathetic , timely , thorough, comforting, helpful and caring - stand out CNA's are Maria, Andrea, Selina, Gabriella, Amanda, Jawana and Rick. When you need assistance with every aspect of self care you feel vulnerable - the CNA staff helps with a smile and a hug and normalize a very difficult transition to help you heal. They notice and encourage your progress and treat you like part of their family- and care for you with that depth of attention. My nightingale med nurse Laverne starts your day with a song and her smile and comforting persona show you each day how much she cares for you as a person - not just the patient in xyz bed - she takes a personal interest in being your caregiver. Emily is a treasure , her excited, happy, persona shows you she loves her job and her attention to detail administering meds in a timely dependable manner make her more of a rock star. Joseph is a rock - any situation that arises he handles and keeps his calm comforting persona while making sure every I is dotted and T crossed. Josie is the director of nursing and is an empathetic listener and promptly resolved a staff issue we encountered, quickly and efficiently. She practices hands on patient care and checks in on patients and knows our names. Victoria is a true delight each time you see her - you feel her energy and commitment to excellence. Sweet smiling Jen our case worker helps navigate the insurance process and helps in any way she can. Morgan the dietician will give you menu options and adapt the menu to meet weight loss goals, dietary restrictions and give fresh, tasty options. Robin in social work has a heart for helping people and you can feel her advocacy on your behalf. Wendy is Wonder Woman - when issues with insurance or coordinating transport, supplies or anything arises she acts quickly and thoroughly and resolved any issue with a smile. The PT staff truly care about your progress and work to keep you motivated and moving forward. Michelle in OT has so many helpful lessons to adapt to living daily life and she instructs to help make you stronger. Michael treats and teaches, he wants you to learn to improve and loves goal setting. His encouragement and heart for patient care shows in each treatment. I was the passenger in a violent painful car accident - a careless driver ran a red light and t-boned my family on our way to a doctor appointment. This one moment changed our lives forever. We did not choose this situation and the pain of physical healing from injuries, emotional and traumatic pain and family separation- The Staff at Victoria Post Acute finds ways to assist in any way they can. I would have to say one word about Victoria Post Acute Care.... Phenomenal. Victoria Post Acute Care is the embodiment of what "Phenomenal" represents. Their nurses shows phenomenal unconditional care and love for their patients. Their doctors show phenomenal attention, patience, and respect for their patients. Let's not forget the workers who also phenomenally dedicate their time serving VPAC such as their phenomenal Physical Therapists, physical therapists assistants, occupational therapists, occupational therapists assistants, speech and language pathologists, rehab/aid technicians, Activities director/coordinators, the medical records staff, and others who have been in the front-line during these times of crises. I REGRET SENDING MY ELDERLY FATHER HERE My father was 88 years old and got the Covid vaccine this past summer and a couple of months later he got covid. He went to the hospital and he was in a weakened state and couldn't walk from the bed to the bathroom. So we had to get him help by using victoria.acute care center. I use the words Acute care sarcastically. My dad was supposed to get physical therapy to regain the muscle he lost while sick in the hospital but there was no evidence he ever got out of His bed. The staff kept saying that he couldn't support himself but never mentioned what treatment they did like massage, muscle manipulation etc. Finally I was able to see my dad through a window and talk to him via phone. On the last time I saw my dad he had a mustache! My dad in my 55 years never had any facial hair, he couldn't stand it not to shave every day. I was shocked! I got on the phone with the nurse practitioner and said I wanted my dad to get off the covid ward and move to another room so we could have contact with him since he had been there 3 weeks. When we went to visit him through the window again He wasn't responding to us other than mumbling and was so lethargic I knew he was going down hill. The nurse told me the same day within a couple of hours that I demanded that they move my dad that my dad wasn't doing well (duh) and that they needed to get him back to the hospital. I said ok but then I said that I wanted to be notified when he was transported. They didn't call me. I called to check on my dad's status and they didn't know where he had been taken. They said the ambulance would know and then gave me the wrong company. I was furious I finally got the right company and find my father. The ambulance transport had to divert to another hospital than the one they had told me he was going to. I spoke with the hospital Dr and he said that my dad was #1 dehydrated #2 had a fever #3 had pneumonia now in his lungs. He told me that my father was a very sick man and they were doing everything they could to help him. It was too late for him and he died 5 days later. I believe that if he had Gotten better care at Victoria he may still be alive today. I write this long review so no one else has to go thru what we did as a family. To lose your only surviving parent is heartbreaking. I would give this place -5 stars if possible . Please don't use this place read more

DO NOT SEND YOUR LOVE ONES TO THIS CLINIC! the staff was rude and did not respond to our concerns regarding our parents who were admitted to this clinic during the holiday season. Both mom and dad were diagnosed with covid. After being released from the hospital, they were admitted to the care of Victoria Post Acute Care. During the two week stay, we learned that the clinic administered a sleep aid to them every night (without consent). We concluded that this was to keep them asleep so they did not "bother" the night shift. While face timing with mom, we noticed how filthy her room was and we don't believe they showered the entire time they was in this facility. When we called the nursing station for answers, the phone would just ring off the hook. On the rare occasion someone answered the phone, we was told "I don't know, I just got here" or "I'll check and get back to you." which they NEVER DID! Our family was VERY unhappy with the care our parents received and to make matters worse, once we insisted on the release of our parents, YES, we pulled them out of the clinic," they lost mom's dentures!!! we are filling a claim against this clinic due to the missing dentures...if they find them please keep them as we don't want them back at this point!! (nasty). I will say that the physical therapy staff and speech therapist was awesome. However, the administrative, nurses, CNA's and receptionist at the front NEED WORK! They should not be taking care of people!! Very unprofessional group!! read more

My daughter in law's grandfather was sent to Victoria after his stroke. The nursing staff were great and very attentive to his needs.. They also have an awesome therapy team, who worked hard to get him up and walking again. I highly recommend this place. read more

Although there were a few staff members who cared, they seemed understaffed and SO slow to answer call buttons, even for emergency bathroom needs, forcing people into diapers just because they couldn't get there fast enough. There was an onion stuck to the floor for about two weeks until I actually saw someone cleaning the room and asked them to take care of it... if they don't get an onion off the floor who knows what other things are caked on there. They would give "as-needed" meds regularly...even when they weren't specifically asked for. They lost clothes all the time even with names on the clothes, and when you'd go to talk to them about it the door would be locked... you could hear them in there but they didn't want to talk to you and wouldn't answer. Overall, it's a terrifying thought that some people spend years in there... read more
