I refused such a request and asked who is asking for this? I was told by the front office. A few minutes later a woman came out and asked me if my son was adopted and then requested my guardianship papers. Of course, I pushed back and refused to provide any paperwork. I was then informed that all adoptive parents are asked this question. I refused once again and they completed doing his cleaning/checkup. This entire experience felt like a slap in my face. X-rays, exams, and consultation. 2nd appointment is the cleaning. The exam was very thorough whereas mentioned X-rays were performed. She recommended removing the wisdom tooth. I have not had any issues with it but it was recommended. The front desk staff was nice and helpful. They explained the process of the first and second visits. I was there maybe 20 min before I was called back. The exam area was clean. The equipment looked fairly new Dr. Carey came in about 15 min after X-rays. She reviewed the results went over what she saw and recommended. They put me on the schedule for a cleaning next month.
