I was a resident/patient at Hope Village for about a year and a half and just moved out in June…read more2026. There are many worse facilities. This establishment is clean and the CNA staff is good-but there are not enough of them-ever. A few of the nurses are excellent-many are half baked and not on the ball. There is a TV connected to each bed, however each TV is tuned to a different station-too loud. There is a physical and occupational therapy gym but the PT staff is not the best team. If you need physical therapy, I suggest you make an appointment with ProActice Physical Therapy found about 12 blocks down Ivy Street. ProActive is the A team. Food at Hope Village is basic institutional fare-think what you got in the high school cafeteria. They do make a great carrot soup. Ask for it. Rooms are way crowded with often three beds per small room. So conflicts with room mates are frequent and not resolved well. The facility is so crowded that if there was a fire, earthquake or flood- the residents could not be evacuated fast enough. If I were the Canby fire chief, I would demand fewer patients per room. Hope Village says they conduct fire drills- they are Micky Mouse fire drills. Ask the management when they had a real fire drill, where everyone was removed to a safe place. Sign up for CAT (Canby Area Transit) bus services-excellent little wheelchair transport provider that costs a buck a ride and well worth it. There is a limit to how far CAT will take you outside of Canby. Canby has a good little library and small independent bookstore, The Book Nook- both worth visiting. The CNA staff are good but frequently there are temporary staff. The MD in charge of Hope Village has two other large facilities he oversees. You can depend that he treats the management of Marquis, not you. Keep you own doctor and Hope Village will often follow your MD's directions. Sometimes not. I had to use Providence/Urgent care twice-once for a bladder infection. There are many adult care facilities far worse but that's not saying much. The upper management of Marquis is unaccountable and does not make patient care a priority. They are out for your money. Family and friends should monitor and supervise and your loved one will get better care. Best wishes.

I enjoyed my time working in the office with the team and also appreciated it when given the opportunity to take on some caregiving shifts as well. I was greeted by name and even one of the residents was holding me a place for lunch. The executive staff is very professional but warm and kind. The support staff is equally amenable. I have never had anyone be short with me upon a request. The activities director is amazing in the number of activities she organizes. If you are looking for a place for a family member or yourself, this facility is a nice size. Not too small, not too big. I would definitely return here in the future if necessary for my well being. One of my best friends just retired as an ombudsman for retirement centers. Oswego Place did not know who she was of course and she said to me "the atmosphere here is positive and welcoming."

In…read morethe year+ since my great aunt moved to Oswego Place, there have been 6 (yes six!) different executive directors. She has lost count the number of activities staff that have come and gone and it's a revolving door for care staff. Staff are literally there one week and gone the next. There is no continuity whatsoever and understaffing is a constant problem along with theft. There have been many residents, including my aunt who have had precious items stolen, and although there is a rumor that one of the elderly residents is the thief, my aunt believes it is the unscrupulous staff. Bonaventure leadership does not care whatsoever and gaslights those who report theft. Bonaventure leadership has also been entirely unresponsive to the issues raised by residents. My aunt has asked repeatedly for fresh fruit for breakfast but has never been served anything other than cheap canned fruit. She also asked for more cultural activities outside of the building, but that almost never happens. Our family knows Dwight Dunton, the owner of Bonaventure, and that's why my great aunt was moved to Oswego Place. But it turns out knowing people in high places is no assurance. Dwight rakes in millions of dollars off self-paying residents and Medicaid, living a rich man's life while his employees and his customers suffer under poor management and a lack of investment in business operations and facility maintenance. Sadly, Oswego Place is just another example of a multi-millionaire living off the backs of good, hard working people.

I was in Porthaven short term to recover from an injury. The physical therapists and other…read moreprofessionals in the gym are fabulous. Very competent, and I felt like I was "on a team with them" for my on care, which is what the medical profession expects between patients and providers. These therapists' competence and care are the only reason I gave this facility three stars, instead of only one star. Porthaven is described as a skilled nursing facility. But it not that, as the nursing staff is short-staffed. Skilled nursing facilities are described as being for short term care, where patients are given therapy and expected to go home soon. But it seemed to me like there are people who actually live there.....the rooms of these people had some personal furniture, and personal pictures hung non their walls. One woman told me she had been there for four years. And as a whole I did not feel like there was teamwork between myself and many others there -- as a whole I felt like a lost my identity as a competent adult, and decisions were made FOR me and ABOUT me, and not in a timely manner, without my knowledge or consent. Fortunately, I have a health advocate, and she worked to make sure I was getting what I needed. CNA staff to patient staffing ratios, I was told by one of the CNAs at Porthaven, are determined Oregon State Board of Nursing. She agreed with me that Porthaven is understaffed. I subsequently researched this ratio and discovered that the state regulations for the bare minimum staffing for safety, and that facilities are able to hire more staff. The only reason I agreed to stay at Porthaven initially is because they allowed me to have my own room, without a roommate. Initially, I had a roommate, who left her TV on day and night. This is totally unacceptable for what should be healing environment, and I told Porthaven this, that I refused to admit myself, but then they assigned me to a private room at my request.

My mother was admitted and on her first night, she called for assistance to use the restroom--no one came. She fell, fracturing her sternum and three ribs, and the facility waited eight hours before calling an ambulance. She is now crammed into an 8x6 ft partitioned space with two other patients, separated only by curtains. In the weeks she's been there, I only received a single call from a doctor about her care, this was only after I threatened to report them. I had left multiple messages and even emailed the administrator and head of nursing--no one responded. This facility is dangerously understaffed, unresponsive, and negligent. I wouldn't wish this level of "care" on anyone. Avoid at all costs.
