My mother-in-law was brought in on Mother's Day. We were originally told she was only approved for 14 days of hospice, no mention on what would happen if it took longer than 14 days. It took a week to hear from social worker regarding her situation because social worker was out of town. no on-site chaplain, but one was available upon request. Social worker promised support with FMLA and a note for work, never received either. My husband was promised bereavement/mental health support, but only after she passed. it's been over a week and nothing. We have to track down the social worker to do her job. The last week of her stay, several loud families were visiting patients in the hospice house, with no enforcement of rules, rowdy guests, or inappropriate behaviors after we were told the teenagers in our party need to be on good behavior. The last few days of her stay, visitors, nurses and staff were talking loudly outside of her room about their personal lives. Completely inappropriate. Have had to ask staff several times for pain meds for MIL, she would be moaning and saying "ow" - nobody would check on her. Would tell staff at desk to get her more meds, they said I had to tell the nurse in a closed-door office. Why is it so hard to identify who is actually providing care? We were told by staff to plan funeral arrangements and they would contact our provider. After she passed, we were told she was a coroner's case and had to go to the Cook County Medical Examiner. We were NOT told this and have no record of this conversation or procedures (but a handout with funeral homes/crematoriums). Staff insisted they told us she was a coroner's case. We have no record. She passed on Memorial Day, the nurse on-site was unprepared for this and kept us at the hospice after she passed to talk to coroner. Did not release us, PCN staff told me it was okay to clean out her fridge and go home. We did not know we would have to talk to the cops, nobody told us that anyone who is seen for a fall and passes within 1 year has to go to the ME. No clue what to expect. More trauma after a traumatic day. We have now had to contact the funeral home on our own and hope they transport my MIL from Cook County to their facility without incident. This has been botched from the get-go. No continuity in training, communication, or actual support for families. The social worker here does not care about the families at all.

They visited him every day and talked with him. They also spoke with the staff, so he would no longer be carted off too the hospital each time his condition got a little worse. He was 92, had serious dementia and trips to the hospital were both terrifying and pointless. Hospice made him comfortable, made sure he had strong pain meds when necessary and could be in familiar surroundings. They let me know when they thought he was living his last few days, which was nice. Their services really made his passing peaceful.

We opted for the hospice option because it was already spreading around her body. Life Choice Hospice has been there for us with a caring and professional intake session the morning after we contacted them through their website. That evening, Life Choice had delivered the equipment that my wife would need and started transitioning her to her new medications. We have a core care team of three. The names I have been using for them on our CaringBridge page are: The Amazing Nurse Julie (the primary caregiver who expertly changes the tumor's dressing and provides on-point answers, advice and support for both of us three times a week), The Fabulous Jessica (social worker whose visits, calm assurances, and insightful suggestions are truly priceless), and Stephanie the Magnificent (our Certified Nursing Assistant - because like a magician, she can turn my wife's soiled bed linens into clean ones really fast without my wife ever having to leave the comfort of the bed). When my wife woke up in pain at 2 am, we called the Life Choice Hospice 24-hour line, expecting to get an AI or an answering service. A Nurse Practitioner (NP) answered, (yes, a real person), and had the on-call person here in less time than we usually had to wait in an Emergency Room. The on-call (I forget if they were LPN or NP) was professional, caring, understanding, competent, and thorough who took care of her and did not leave until she and I both understood what was going on, why, what they were going to do about it, and what I had to do and watch for as they change her medication dosage and frequency to adapt to her increasing pain levels. We have needed the Life Choice 24-hour services at other odd hours between 11 pm and 4 am, and, while the NP on the phone and the LPN or NP that showed up were different, the pattern of professionalism, care, concern, and thoroughness was the same. When she noticed that I was getting overwhelmed with caring for my wife, The Fabulous Jessica suggested a "respite" meaning that my wife could visit a 24/7 care facility for 5 days to give us both a break. I did not know such a thing existed or could be so beneficial and timely. All three of my wife's core care team continued to care for her while she was in the 24/7 care facility. Life Choice had already contracted with a service award-winning facility, arranged transportation, got my wife settled in there, provided continuity of care by keeping their previous schedule, and coordinated the care my wife received from the 24/7 facility when Life Choice was not there. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. We shared feedback regarding the weekend doctors communication being highly misleading/confusing for our family, due to coming in so early, saying so little, and focusing on what he thought we wanted to hear instead of what we needed to know (what's best for the patient). The other challenge we faced was related to the Home care side of the business systems not talking to the in-facility side of the business, so we had to repeat information such as funeral home. The worst fall-through with that was receiving a call from the home team to schedule equipment pickup while we were still in hospice, before a death....
