My mom was at this facility for about 11 days after having a stroke. I say first couple of days when mom Needed higher level of care staff was attentive as cna's followed rn's orders real well. When her care was less critical I would say it was bordering on neglect . A cna said she was in charge of 15 patents and almost half had to be spoon fed including my mother. Hence, myself and Family made sure to be there so she could eat. My observations were patients who didn't have family step in for them might of not ate well. I heard one rn supervisor talk extremely rough to like a dementia patient to be still and stop moving. It appeared as work burn out. There were many times mom was soiled and staff would not come to help. I had to change mom myself and do best I can to get her to the restroom, changed and brought back to bed. They had no sense of urgency in helping despite telling me to not take mom alone. I waited but when it started approaching two hours I had to do it . I also Had to help cna put mom To bed as she had no help as mom was too weak and a two person job. They got mom's diet order wrong as moved up to solids and kept on doing a blender diet for days. We had to Go out and buy food so she can eat as she hated blended diets and I don't blame her. I was told this happened cause dieticians use a different system and it don't data share with the facility but nobody could make a phone call. There was like a comittee but I don't think they were proactive (except supervisor of nursing) as made excuses For some of my concerns . I am not going to say this facility is the worse but definitely not the best. I was told this place was the best in town. I am Just going to say it's like any other nursing facility from what I hear. When I thought mom was well enough to go home I demanded she leave immediately as they wanted to keep mom longer. I overheard admin mentioning mom was a "good" patient as had dual insurance. I told them at my house mom is not neglected and will get better care as I had all her bags packed. I don't recommend this facility for your loved one and it's definitely not the best in town but can't say the worse cause I haven't seen many . It missed my high expectations and them trying to say get better care cause they are small is garbage. read more

My mom has been pleased with the service she has gotten. They provide PT, OT, dietitian, speech therapy, and social worker to address health needs. The aides come in a timely manner when the button is pushed. She said that the food has been tasty. They provide activities for the residents. When visitors enter, you must wear a mask, get your temperature from a machine, do a brief survey, plus check in & out. It is very secure & a hospital is across the street. read more

We chose this facility because of reviews but it is NOT the same as it had been. POOR CARE, ROUGH HANDLING, NEGLECT. Letting my mother come to this facility after she fell killed her. My mom was afraid of the staff physical therapist! She was neglected. Not given care. The staff treated family visitors as a joke. The young girls staffing would burst out laughing when we walked out the door. As for requiring Covid tests, they made us wait to take the test but their results took a couple of minutes. They faked Covid tests. That may have been why they were laughing. read more

If I could give this place Zero stars, I would. When you walk in, it appears open, airy and almost charming. But then you peel back the very first thin layer quickly. The two love seats in the lobby look like they came from a crime scene. See pics. They keep it absurdly warm in the front and if you're menopausal or just sensitive to heat, you will feel it within just a few minutes of being in the lobby. Especially crappy on hot summer valley days like today with a high of over 100. You have to ring the doorbell to get in so there may or may not be someone at the front desk who will let you in. So sometimes you're stuck out in the hot weather waiting for someone to do their job. From my experience the social care department is complete crap. The only time they were communicative is when they had a discharge date and they told me she could stay beyond that at $450 a day. Never. Gonna. Happen. But they called me several times to tell me that nonetheless. Basically they seem understaffed and completely disorganized. Now remember this is not a nonprofit...this is a for profit "we make so much money...we make more money than God" type of business. And this place is considered top-notch in Stockton. This is really a nice rehab facility. It's clean, bright, cheery and everybody is helpful right down to the administrator who can be seen walking the halls answering call lights, helping nursing staff as needed. Call lights in this place are answered within 5 minutes and the patients needs are taken care of. Nurses will stop what they are doing when told a patient needs a pain pill and will take care of it right away. Now that is what I call caring, along, with the CNA's who are always right there to help the patients. The rehab staff is second to none. Very caring and very professional. read more

Our family took a serious blow when my father suffered from a massive stroke a little over three months ago. We've been through the roller coaster of feelings and experiences; sadness, uncertainty, fear, loss... And I have experienced (as a patients family member) the process of the medical system; emergency rooms, emergency room intensive care units, ICUS, CCU's, skilled nursing facilities, acute rehab department inside of hospitals. We are blessed to have found Lincoln! This facility is clean, feels like a model home (if there was such a thing for skilled nursing facilities, jk!) From Alexa who facilitated the logistics rundown and paperwork, to the entire nursing staff, I wish I could remember all of their names, but needless to say there is a level of professionalism, competency, warm and friendliness that is outstanding. And I have to mention the therapists, they are skilled, caring, professional and warm. A huge plus, is I actually enjoy the visits and feel comfortable hanging with my dad. Everything is clean, organized and everyone is so helpful. :) Great news is my dad is making amazing progress and gains everyday. Highly recommend! read more

I'm born and raised Stocktonian and my Dad had A recent back surgery at Stanford. He wasn't quite ready to go home So he went to Lincoln Square since my family resides in Stockton. We were not sure which Post Acute Care to choose . After Lincoln Square was suggested we were really happy with the service , facility and ironically my dad thought the food was better than Stanford. I was really impressed when I would come to visit And how clean and new everything looked. Also the PT program to help My dad walk again and build strength in his legs. The case manager mentioned to visit before deciding. We didn't beforehand and really happy we went with LIncoln Square. Take a tour if considering or my word for it.......... read more

I have been a customer service trainer and evaluator for 30 years. In addition to that my son is a physician in another state as well as I have several other family members in multiple medical fields. More importantly, I am a daughter of a mother I so desperately would do anything for. I would move heaven & earth for her. I cannot speak highly enough about LINCOLN SQUARE SKILLED NURSING FACILITY. No matter whether it is the nursing department, therapy department, administration, maintenance, any of the departments, the members of the staff go above and beyond to not only assist the patient but the family as well to accommodate whatever their needs may be. They realize each situation is different and the standard guidelines do not always provide the best care and support for patient and family. They continually review the medical, physical and emotional needs of the patient and make revisions where needed to come up with the best plan of care for the patient. read more

My mom recently had open heart surgery. This was a very scary and difficult thing to go through. We were so incredibly pleased with the care she received at Lincoln Square. My mom lives in Stockton and I live in LA. The day that I brought her from the hospital to the nursing facility, I had to head back to LA. It was a relief to feel that she was in good hands. I returned 2 days later and she had improved greatly! The therapist were knowledgable in her individual case and created a routine for her specific needs She soon began sitting outside her room in what came to be known as her "office". Everyone knew her by name! The entire staff were so friendly, kind, professional, and truly CARED about my mom. This really is a SKILLED Nursing Facility! This is a most incredibly caring facility that I have ever experienced ! My Mom has been here twice and each time has been a pleasure. The staff is friendly and the care supreme! No other hospital or place gives you service within 3 minutes of pushing that red button! They also have goals for each person and go over them with the family. read more

My father was transferred to Lincoln Square on 12/31. My mother received a call from the staff doctor to communicate about his condition on 1/1. A social services consultant called my mother one time, approximately 1 week later, for very brief and preliminary communication regarding when/if he could come home. As of now, 1/28, we have received no further communication from a doctor or social services. My mother and I have called requesting an update from either the doctor or from social services once weekly. But, the only phone calls my mother gets (4 thus far) are automated updates that a 'staff member' and/or a 'patient' has tested positive for covid... the message goes on in female robot voice to offer sympathies. read more

Great therapy and nursing services. My mother is able to talk and walk after a stroke. My father had a speddy recovery after a broken leg. Both are home now and very grateful for all the services provided. read more

This facility ROCKS!!!! I was a patient here and the care I received was top-notch. I had never at a facility like this before so I was a little apprehensive. The staff here eased my apprehensiveness with their excellent care and kindness they made my stay there very nice. The physical & occupational therapy I received there helped me when I went home. The food was pretty good too. The whole time that I was there I didn't have to want for anything. And the place smells and looks clean too. If you need to rehab from a surgery this definitely the place to go. You won't be disappointed in the care that you or family member receive. read more

I am here now recovering from leg surgery, and this is the best place in Stockton. I've had dealings with some of the other places like this for my mother, and this place beats then all. Wonderfully clean, no odors, very kind staff, the PT department is to notch. The staff go out of their way to make sure that you are comfortable. I wouldn't hesitate at all to recommend it highly. The rooms are small and the staff is VERY hit and miss. Sometimes they respond to requests and alarms in a timely manner; other times they are completely forgotten or totally ignored. My dad went this place for his recovery after neuro-surgery, I read the reviews thinking he was in good hands but he wasn't. Their staffs seem to be not around, or when we ask questions on his care they would respond by I DONT KNOW. I asked about them cleaning the area around his stitch but they response was idk or yes we clean it but by the looks of it, it wasn't clean. We had questions about his showers, they respond by oh idk I just started or it was the morning shift job. Very unprofessional!!! And the worst part is, they didn't keep us updated on his care, and the fact they call the wrong hospital to contact his doctor in Stanford, SMH they called Davis hospital! When my mom provided all his paperwork from his doctor from Stanford hospital! Now my dad is currently back in the hospital for abscess brain. Their staff here seem to work for a pay check and not care for their residents. If you take your family here make sure your on them because they will tell you what you want to hear. read more

I had surgery in June 2018 and I could not go home right away- my doctor suggested I stay at Lincoln Square- I must admit at 60 yrs old I was a bit scared- but when I got there immediately I was treated warm and kind. The smiles at Lincoln made my heart Happy and my spirits were lifted right away!- The Physical Therapy was extraordinary they helped me to get stronger and believe in myself that I could do it!- They take us as individuals and work with us with care and compassion!- The nurses and CNA's are wonderfully warm- caring- attentive to our needs! They are so friendly and make you feel as important as family!- they have activities too- and all who run that department are just exceptional they want you involved and they want you to be happy because they care!- I felt so wanted and cared for at Lincoln Square!- it helped my recovery 100 percent and when I got home I missed everyone At Lincoln Square because of their kindness- their caring and concern! I hope if I ever need help again my recovery will be at Lincoln Square!- Even though I live 2 hours away- it was so worth it!- Thank You All at Lincoln Square from the bottom of my Heart- I will always remember my excellent care!- and I do not want to forget the people who run it too- they know what we need and keep a clean and happy place all ready and waiting for us! This facility is wonderful. The nurses , physical therapists and other care-givers have been professional, knowledgeable , kind ,gentle and respectful to my dad. They have been very responsive to my dad's changing health problems and my panic . The administration was also helpful and solved any issues. I am very grateful for all their help in navigating my way through the health system and managing my dads needs. 6/21/2016 It has been one month since I wrote my review. My Dad is going home tomorrow. I truly am grateful and appreciate that he would not be going home but for the expertise and care of the nurses and physical therapists at Lincoln Square. I have been meaning to write a review based on the experience my father had and now I feel compelled to share what our experience with this facility has been. He was brought to Lincoln Square after his stroke in December 2013. We were impressed on the clean rooms and the friendly staff. However during the end of January, he was coughing excessively and we were concerned. Their doctor on staff prescribed cough syrup and did blood work. When he did not get better, a chest x-ray was ordered. On January 27th, he began to vomit and needed to be placed on oxygen. The facility contacted the Dr. who looked at the Xray and his bloodwork which was a few days old and said it wasn't pneumonia but if we wanted him transferred to the hospital, he would order it. We agreed, when we went to the ER, the doctor was like "Are you kidding me, what are they doing over there! He has severe pneumonia and has had it for awhile." He didn't think he would make it and asked us if we wanted CPR to be used or if he was DNR. We were shocked, we trusted in the facility and this is what happened. My father did recover and since the other facilities on his insurance had more violations than Lincoln Square we decided to return him there, but we would be on his care. The facility agreed to meet with my sister who wanted to know what went wrong and we were to review his medication and stayed on top of his care. We did stay vigilant and made sure no other further incidences occurred. Yes, we did ask a lot of questions and asked to see all the records regarding our father's care. The administrator was accommodating and said that whatever we needed, he would work with us. He did exit the facility in March. When he was at Lincoln Square, we were told that he was unable to be trained to use a urinal or a toilet. Within 2 days of being home, he was using the urinal and going in his commode. I wondered if they were just too busy to try with him and out of convenience use a diaper. Now my father has a medical condition that the hospital feels he should be put in a skilled nursing facility for recovery. We decided to go back to Lincoln Square because they were willing to work with us and my father's insurance has limited places he could go. We were turned down immediately. My sister was a little suspicious. The doctor at Dameron called over and was told that it was not my father but the family that was difficult and they had a right to refuse a patient. Excuse me? We were never rude, we just asked questions on his care and wanted to make sure no other mistakes were made. We could have made a big fuss after the missed diagnosis but we were willing to work with them to make sure nothing like that happened again. I guess if you care about your loved one and want them to be accountable, you will be blacklisted from returning to their facility. read more

My mom recently stayed at Lincoln Square to recover from hip replacement surgery. After several back surgeries, my mom has seen her fair share of after-care facilities and this was by far the best. If I could give it 6 stars I would. The staff is phenomenal; her nurses Cheryl, Russel, Sing, JC and James (just to name a few) worked very hard to keep her and other residents comfortable. The food was outstanding, I paid a little extra just to eat with my mom each day (it was that good!) Best part tho, this place does not have that typical nursing home/hospital smell. It is kept clean and feels just like home. If your loved one is looking for a place to stay, my family highly recommends this location above all others in Stockton. read more

I have had experience with skilled nursery facilities in Stockton in the past for my parents, and this facility is superior to any place in Stockton. I am recovering from a knee replacement surgery and the physical therapy department is outstanding. Everyone from the administrator to every CNA who has cared for me have been amazing. I just can't say enough good things about this great place. They have some of the best staff around . They have supper caring staff that have always big smiles so pleasant There is no way I could call out my care givers by name but there is not one person that I could complain about. I had the very best physical therapy team a person could ask for . The CNA staff are the most kind caring bunch you will ever find they know how to treat an care for you while all the while allowing you to keep ur dignity. The nursing staff always took time to really listen an always acted on the information. The housekeeping staff the best. The laundry staff know they did a super great job moving those bins around no holding ur nose at this place. You just won't find a better place to recoup from surgery or other medical conditions. I am a blind and physically disabled woman who was sent there from a local hospital for post-acute care. I can say without a doubt that my time there was the worst experience of my entire life. Though the facility was clean and neat and the staff was very friendly, the overall level of care I actually received in my brief stay there (less than a month, though the order was for a full 30 days), was extremely sub-par. Both facility and staff regularly failed to accept responsibility for mistakes beginning with dietary and specific care issues and ending with errors in both paperwork and protocol at discharge, which were blamed on me though I'd done nothing but remain in my room until it was time to be discharged. They indicated to other parties displeased with the manner in which they'd handled, or failed to handle things that they'd had to scramble as I was "just leaving". I admit I did actually did say during the "Care Conference" where we were supposed to discuss the quality of care I'd received so far, or lack of it, that I was at the end of my rope and considered just leaving rather than continue to be so completely mishandled. However, the discussion between them, myself, and two of my family members concluded with agreement that they would look into how to safely discharge me, with safety being the key, to a home health agency and I would remain there until it was successfully achieved. Instead, they took short cuts and dropped the ball in several areas, then lacked both competency and integrity afterward to admit their mistakes. Nothing in my experience suggests this is a place I could ever refer anyone else to. Instead, I'd strongly advise you all for your own good to go just about ANYWHERE ELSE. read more
