His job requires this paperwork to be filled out in its entirety, before he can return to work. There is a statute for this paperwork to be completed, and after this statute the FMLA is denied and he will be terminated for time missed. The surgeon is William Curtis, and he only works out of this office 1 day a week or less, and HE is the ONLY person who can fill out this paperwork. After finally getting this surgeon to fill out my boyfriend's FMLA paperwork, it was done INCORRECTLY and only signed 1 of the 3 papers needed for him to return to work. We have called, we have shown up to the office and cried in the lobby, we have done everything we can for these people to get this paperwork sorted out and are consistently given roundabout responses with no action. My boyfriend will be terminated from his position in 4 days, because of the negligence of the surgeon and the people working this office who are not making this an absolute priority. Real people trying to just pay their bills and survive will be severely impacted by the sheer unprofessionalism given from this practice. I've heard corporate dentistry is bad, but I am now learning first hand how they don't care about their patients in the chair, they only care about money in their pockets and insurance companies that pay the best. They got him in, extracted 7 of his teeth under general anesthesia, took our money, billed the insurance from our yearly max, and now my boyfriend will lose his job AND his insurance because they cannot simply fill out paperwork. If you want an office and surgeons who don't care about the livelihoods of their patients day to day, and who use AI to filter their calls, then by all means, this is the office for you. I have never been more disgusted with a health care practice in my whole life. Corporate dentistry does not care about your dental needs, your financial situation, or your fears of losing your career. If I could give zero stars I will, and I will be posting this review everywhere I possibly can.

The office staff was so pleasant and professional. They answered all my questions and was so patient with me. The oral surgery went better than I expected. I wasn't sedated since I didn't have anyone that could drive me home afterwards. I decided on nitrous oxide to reduce the anxiety and fear. Dr. Melendrez gave me the novocaine and stayed in the room and chatted until it was time to get started. It was a very nice conversation and it put me at ease. He worked quick. The extraction and the bone graft took less than 15 minutes. There was barely any swelling or pain. I highly recommend Dr. Melendrez and his staff especially if you are anxious going to the dentist.

But, my wisdom teeth were removed a few years ago by Dr. Muff, so now I can remember the experience for what it really was: GREAT! I was referred to Dr. Muff by my dentist--twice. I kept putting the surgery off because again, no one likes getting work done to their teeth. But it had become serious. I was teething just like my toddler daughter. And just as she would get feverish and catch every bug, I would, too! The ever-opening wound in my mouth was NOT good for my health. My favorite thing about Dr. Muff is that he never made me feel bad for putting off the surgery. Ok, and that he laughed at my jokes! His bedside manner is superb. I was comfortable and confident from the consultation to the surgery to the follow up to the recovery (aka Netflix & Popsicle marathon). Dr. Muff also was cool and collected when I announced I did not have health insurance. The office worked out an easy payment plan that worked great for me! His willingness to work with my finances was a major indicator that he really CARED about my oral surgery. He WANTED the best for my health--even if it meant some patience on his part--THAT was refreshing! The office staff was great. They supported Dr. Muff so he could excel at what he does best--surgery! So we went on 12/16/2025 when we got there the price changed my portion $2,566.40 on the statement I told her I could not pay that what happened to the $1,500 as the highest. she told me that the person I made the appointment with was let go. I should have left then with the high jump in price. so she said let me see if she could get my daughter a discount she came back with another statement showing discount $80.30 discount $80.30 discount $ 300.00 for total $2,242.00. So I said to okay and paid we had traveled 45 min to get there. Dr. Falke came out 30 minutes later and said my daughter was very emotional. My daughter was wheeled after him crying crocodile tears and laughing at the same time. I was upset because there was no recovery time very strange I did like it. I told the nurse because he left 2 seconds later. Today 4/28/2026 I get a text from Dr. Falke's office saying I owe $307.40 So I called she said it was for anesthesia they billed insurance 45min and they paid 15 min so I owe for 30min of anesthesia even though they gave me some discount on the anesthesia. And put my daughter out literally 30 min later not 45min as a basket case for the next 20 minutes. I would never recommend him you don't treat people this way he tries to get as much money as he can. 4 teeth impacted- partially bony 2 impacted - completely bony with unusual surgical complication according to him more money. She was only 19 years old and never had a cavity. Pediatric dentist twice a year from 1yrs until 18

    I went to my dentist of over 25 years due to a toothache. She discovered that my upper last molar…read morehad a metal filling that needed a new crown. As she uncovered the old alloy, she saw that my tooth had badly cracked in to the roots. She put on a temporary crown and referred me to a nearby endodontist. She also prepared me to know that I might need to have the tooth extracted if the crack extended too deep in the roots. The endodontist prepped me for a root canal due to a dying nerve. As she drilled and examined the area of the crack she noticed bone decay. Due to that she said the root canal would fail if she completed it. She recommended extraction. The first dentist referred me to a dental surgeon, Dr Aaron Adamson DMD. His front office staff prepped me and got me to fill out the needed paperwork. I was given an appointment the next day for my extraction. The office is clean and has some nice landscape paintings. The billing rep took my payment and I was off to my seat to have the pesky tooth removed. His assistant explained the procedures and post op paperwork. The doc was kind and gentle. I had some trepidations as my mom had all her teeth taken out at age 42 due to poor oral hygiene. She died the next year from cancer. My father also had poor oral hygiene and he died with only one tooth in his mouth. Hopefully, brushing & flossing will keep the rest of my teeth healthy so I can take the rest of them to the grave. As a dentist once told me... "Only brush & floss the ones you want to keep..."
