Do NOT send your loved ones here. My family member spent 6 weeks in this facility. She came in with an infection that was not treated through its course; she finished her course of antibiotics, but her symptoms returned shortly afterward. She got weaker and weaker, and complained of abdominal pain. The staff did nothing; one nurse even had the gall to tell me there was nothing to do for stomach pain. I had to ask them to take blood and stool samples, and when the results finally came back positive for C Diff, I learned they had not even started her on a course of antibiotics despite having the results in hand! In that moment, I insisted that they transfer her to an ER immediately that night; she was so weak she could no longer stand or use her arms and hands. I regret so much not taking this action sooner; it's unbelievable to think you can't trust medical professionals to take basic care of your loved one. Prior to that, my family member reported that the folks leading her physical therapy appointments were often on their phones, and disengaged with her care. The food is also the worst of the 5-6 hospitals and facilities my family member stayed at in the area. The staff is largely unresponsive. The social worker at the time (Joy, winter 2025) set up a good informational meeting with the care team when my family member was first admitted; then she promptly disappeared. She would not return phone calls; she was always "in meetings". When I finally would connect with her, she would promise to stop by to visit my family member to communicate with her. Days and days would go by, no visit, no returned phone call. Absolutely unprofessional and shameful. Search previous low-star reviews, and you will see a pattern. Re the care my family member received: she was in such bad shape when she left Glendora Canyon and went to the ER (colitis, sepsis, then she had a stroke) that she spent 2 weeks in the ICU, and then another month recovering in a hospital. Glendora Canyon, her health was in your hands and you deeply failed her. SHE ALMOST DIED in your facility. And finally, adding insult to injury, when my family member was taken by ambulance to an ER, they lost her personal items that were in her room. I tried in vain to find them; they first gave me two bags of someone else's clothing and items, and then tried to push someone's else items on me. At the hospital my family member went to, I met family members of a different Glendora Canyon TCU patient who had his phone and iPad go missing when he was also transferred to a hospital, also for not being cared for adequately at Glendora Canyon. March 2026. My Brother is very sick and his Heart is very weak. He just back to Glendora after another Hospital stay, It's his 16th Hospitalization in less than 1 year. Two new amazing Nurses on the 2nd floor Isaiah & Gavin along with CNA's Dublin & Alyssa. All of them are Fantastic. I can't believe the quality of care they have given to my brother these last 2 days he has been back. Glendora Canyon staff & facility is a BLESSINGS. My mother went from Foothill Presbyterian Hospital to Glendora Canyon Transitional Care. She was in the hospital 11 days and had surgery. She was in Glendora Transitional for 14 days. The Staff: The nursing staff was awesome. Very helpful and did whatever was needed to make her comfortable. She had a wound on her lower back. The wound specialist comes in every day, checks it, changes the dressing, and makes sure she is comfortable. Mom is Incontinent, is not aware if she has had a bowel movement or urinated. That was on mom's record. Someone would come, check, and do what needed to be done without my mom feeling embarrassed. Then she needed to be bathed, could not have asked for better care. The maintenance crew was great. Several of them spoke Spanish, as does my mother, and they would visit with her and see how she was doing. They do their job and ensure the patients' needs are taken care of. God Bless them. Therapy: Each physical therapist who worked with my mom was kind, helpful, and spoke clearly to you. She has 98% hearing loss and 5% word comprehension. Very respectful of her and to her. The Director, David, makes sure he has a hand on everything that is going on in the Facility. On the first day my mother was there, he made sure to check in with me to see if we needed anything or had any concerns, so I would contact him. At the end of the 2nd week, I spoke with the Social Worker, Joy. She was polite and courteous, but when it came to trying to have my mother discharged (she had met everyone's expectations of how well she is progressing. The PT Director indicated she was ready to go, as well as his staff, except Joy. She insisted she had to stay longer. I spoke to David; he looked into the concerns I had and agreed that she could go home. Her body was healing, but her emotional state was getting worse. My mother is 101 years old, and I was looking out for her best interest. Thank you to the Director for understanding, the nursing staff for making mom feel at home, the physical therapy staff, and the maintenance crew. If I could give this place 0 stars I would, I am very disappointed with the care they provide for their patients; they let them be soiled for hours and they do not help immobile patients with their feeding. This facility did not take my family member to their dialysis appointment, it has been 5 days and she has ammonia in her blood, this is a problem which could have been avoided with adequate care. This facility had lied to my family about said dialysis appointment and were not able to give a straight answer about scheduling matters. It is very difficult to get in contact with someone as they keep giving my family and I the runaround. We have caught them in several lies. Looks are deceiving! My mother was admitted around 6:00pm on 2/16/25 and we pulled her out around 5:30pm on 2/17/25. She was in the facility less than 24 hours. Around 7pm my mother pressed the nurse call button to see if she could get her medication. Audrey stuck her head in the room and said she would be right back, and she did not return for at least 30 minutes. My mother had to press nurse call another 4/5 times. They brought her dinner and the only thing they fed her was a very dry piece of chicken with two slices of bread. Around 1:00am she pressed nurse call to see if she could get help going to restroom and a male nurse came and said he would help. She was very uncomfortable with a male helping and asked if he can get a female nurse to help. He stated that she could go on herself if she didn't want him to help or wait until the morning. (Nestor Mariano told me that there was a female nurse on duty during the time my mom asked for a female nurse. Nestor stated that there were two male nurses during the time of the issue, and they were Alex and Carlos) Around 4-5am she pressed nurse call button, and the same nurse came in and she asked if they could change her bedding because she couldn't wait and she was cold from being wet. He did come back in a few minutes with a female nurse. The female nurse helped my mother change, and they changed the bedding. Later in the morning she asked for help to use restroom, and they came back about 30 minutes later to help her. They brought her breakfast, and it was cold scrambled eggs, oatmeal and toast with cranberry drink. She was only able to eat the toast. My wife brought her lunch around 12:00pm and she needed a fork for the salad. She had to press the nurse call at least 4/5 times and nobody came. She heard voices outside her room, and she called out loud to get someone's attention and a male nurse came in to see what she needed and said he would bring her a fork. The fork never came and she ate the salad with her fingers since she was starving. Two nurses came in to do a skin assessment. She asked if they could clear the food and trash from her table and they said they would send someone. They didn't even do the skin assessment and after about 20-30 minutes my mom pressed the nurse call button, and a nurse came to see what she needed and when she asked if the table could be cleared the nurse left and had someone come to clear the table. Two individuals came to help with physical therapy and they are very friendly and helpful towards my mom. Bernard and Irene were very good to her. She was very hopeful that they were going to help her with her PT and the only positive thing about here 24 hour stay. In less than 24 hours my mother had to urinate on herself twice due to nobody came to help when she asked for help. Nurses came in just to turn off nurse call and not do anything to help her with her needs on all the occasions she asked for. The food was not even warm, and they just showed up whenever they showed up with nobody giving her a schedule. At the end Nestor stated that "he was sorry that they could not meet our standards". I felt he said this in a tone of us having unrealistic and high standards. The nursing staff treated my mom like trash. They wouldn't even remove the trash when she asked and just left it on her table. She was not a bad demanding patient. She wanted to be treated with dignity and respect and letting someone urinate on themselves is not being treated with dignity nor respect! I would like to relate to your staff that nobody makes them come to do this kind of work. If they are not happy coming to work or feel that they are not getting paid enough then they should go find something they like doing. Spend time learning more to get what you want in their work life. I work in a hospital, and I know the work they do is demanding and hard, but it doesn't give them the right to mistreat anyone. They should look at every patient as if they were their direct family member. Bernard and Irene were very professional and treated my mom with dignity and respect. They were very helpful in showing her how to properly do the PT. Glendora Transitional Card Facility showed above and beyond excellence service in caring for my brother who was diagnosed with Parkinson's And needed rehabilitation. Coincidently, my mother was admitted too and we were able to visit both at the same time. This is not legal advice, and you should not use this information to decide whether to use these facilities. Make you research. In my very personal opinion, I would not use this place nor would I recommend it. It was mistake to come here where one registered nurse for the entire fifty patients in floor and all the nursing staff could be finding difficult to supervises all of them. There is no physician here. You need to utilize your primary care physician. Many times was very difficult to reach our physician since all communication were done through the nursing staff. It took a lot of time to get an answer from the physician. We came to this place encouraged by the person in charge of recruiting patients. This person helped us greatly by advising us how to proceed in this my relative difficult health situation. Therefore, we decided come to here. It was sad to know that recruiter cannot know exactly how the patients are treated. I stayed overnight with my relative, so I know what I am talking about it. I believe but not sure that the nurses and attending physician made several errors, which caused my relative to return to the hospital in very delicate condition. First day here and all medications delayed over 24 hours. Also From the first day we arrived, some nurses did not know how to handle the feeding tube my relative had. Some assistant nurses also did not know that residues in the tube could cause no digestion. My relative was in pain day and night for the six days because she was not digesting the food or medications provided through the tube. My relative had so much undigested liquid in her stomach that she vomited and this caused her to aspirate and lower her oxygenation. During six days, my relative had active bleeding. Dr. and nurses said it was hemorrhoids. Nurses and doctor did not do anything but to have my relative in observation for a while. There was very little coordination between registered nurse, physician and all the associates. On one occasion, she was about to be given a medication that was on hold, and the assistant nurse did not know about it, so I had to let him know. My relative became sicker and sicker every day until it was necessary to return to the hospital, but it was too late. This place should be more strict who they hire to work here. I do not know how some people give to these place five stars. I lived there for six days, and what I saw warrants less than five stars. I got the saddest experience in my life. You decide after your research if you want to come here but do not use this my personal opinion for your decision. read more

Be very careful with this place. At first view it looks different than other skilled nursing homes until you get to the second floor. Worst care that I have ever seen provided for a human being. A close family friend ended up here due to cancer and a quick decline. This is not where you want your loved ones if they are getting confused or cant advocate for themselves. Going to share with you yelp review readers things that would set anyone off. 1) the multiple times that I got there and found that the call light had been moved out of her reach. 2) The time I showed up and she had a bowel movement right before dinner. I asked the nursing aide assigned for help and I was willing to help clean her up but was told the dinner trays were now on the unit and no one could be changed. The nursing aide told me that per their policy my loved one now had to sit in her poop till everyone on the unit was done. I asked to see the policy....... Turns out they made it up! 3) There's also that time I got there at 11:30 and she was in a diaper that had been stuffed with washcloths and completely covered in poop. I asked to have her cleaned up. The nursing aide told me that he had just cleaned her yet he didn't even know that she had a bedsore right on her butt. So did he ever even check on her? The world will never know. Although he did immediately blame it on the night shift. Said they were suppose to change her even though they had been gone 4.5 hours. SO IF YOU ARE CONSIDERING THIS PLACE, DONT DO IT! Not unless you're bringing an alert and oriented family member. If not you'll come and find your loved ones covered in poop and bed sores. read more

My MIL was transported there for physical therapy after she had a stroke. Only to find out this place had a COVID outbreak as of two weeks prior to her being admitted. No one informed us before she was admitted to the facillty. Before entering the facility, all visitors must mask up and take a covid test. No idea why a facility continues to accept new patients when they have a current covid outbreak. Ridiculous. Rating one star because I cannot rate negative stars. read more

My brother is in this place. Everyone here is wonderful. Will go out of their way for you. Came in to visit his TV had no signal. There was no maintenance until Tuesday. They made a phone call maintenance came down and fix this TV they went above and beyond to fix the issue even got a call from the administrator David very caring very concerned and I love this place. I made a yelp review a year ago on my friend' who received horrible care post surgery resulting in facility acquired wound infection which she had to be returned to surgery to be taken care of, and would you believe it, they sent her back to this nasty place with appalling care and neglect, resulting in her death from further neglect. The staff and the DON including the doctors should have been held accountable and their licenses revoked. Biggest falsification of documentation and coverup. read more

Horrible.....this is a rating from last year. Had my Mom here in the past and things were fine. NOT even close this last time. 1st- a limit of visitors ( which you made an appointment for) and you required to wear a mask ( even though a few of the nurses and the maintenance workers didnt) and no restroom service for visitors. 2nd adjustable beds that didn't work correctly. My poor Mom's bed was frozen in a very uncomfortable position that was stuck for hours....it forced be to be loud and rude to finally get one of their maintenance persons on it. He ended up beating on the motor with a rubber mallet to get it to work to return the bed to its proper position. (Inferior equipment) Next they had a COVID outbreak which cause a quarantine. This caused my Mom's physical therapy ( which was so needed) to be canceled. After this, when allowed, I went in to find the patients ignored and the nurses on their phone more than doing their job. Next I went in to find my mom's bed lowered to the floor. I interrupted one of the nurses on her phone and was told this was done so she wouldn't try to get out of her bed and hurt herself. This was a hellish horrible place and there should be investigation from the state of this place read more

Hello my name is Rafael O and I was a patient at Glendora Canyon canyon TCU back in October and November of 2024. I highly recommend you think twice about sending your loved ones here or admitting yourself to this place. The service was horrible. LVN Michael Ruiz was the worst He had the audacity to tell me that I wasn't in pain when I would ask for pain medicine because my blood pressure was in the normal range. Had he paid attention in nursing school He should have known that people have different pain thresholds and people who suffer from severe nerve pain can present with normal blood pressure and still be in pain! I complained about him to his manager and they had him apologized to me which apparently meant nothing to him because he got worse and refused to come to my room and bring me any of my pain medications He says his boss told him that he didn't have to come and see me anymore. I asked Marion who was his boss if that was true and she said yes so I asked her if it was normal for the nurses to pick and choose who they help and she said no. If you ask me it's a severe case of the tail wagging the dog. I also brought this incident up to David gunnell who is supposedly the big boss and nothing got resolved They just said that he didn't have to come to my room. One night I wasn't severe pain and I asked for my pain medication for over an hour and a half and it turned out that Michael Ruiz was working that night and he wouldn't come bring me my medicine He told me to come and get it myself. My blood pressure was so high I asked him to call 911 and he said that no they would not call 911 He had someone come to my room and tell me to calm down that I was working myself up. I called 911 and was taken to foothill Presbyterian hospital where I was told in the ER that I was suffering from a gallbladder attack and then upon further diagnosis They found a blockage in my colon. I had been complaining about lower abdominal pain for about a month I told my doctor in charge there and all they did was give me laxatives and more laxatives but I still couldn't go to the bathroom. The service here was horrible!!! To top it all off when I return to the facility to pick up my stuff all might belongings were stolen!!! I had a little over $1,800 worth of sports gear including Dodgers jerseys and Dallas cowboy jerseys!! When I asked what happened to my stuff they said they didn't know they couldn't find it. I gave them an itemized bill and ask them to pay me for my stuff It was a little over 1800 dollars They came at me and tried to negotiate with me and try to blame it on me saying why did I have such nice stuff and they had the audacity to call me a liar and said how do they know I actually had this stuff!! It took over 2 months to get paid for my belongings and I had to settle at $1,200 only because they were threatening to not give me anything If I didn't settle so that was what we agreed upon.. please please please stay away from this place it is bad news!! They were a few nice people that worked there. If you do have to go there please don't take anything nice and if you do make sure they log it in!! read more

I would like to thank Glendora Canyon, Transitional Care Unit for taking care of my mom while she was in recovery of a broken hip. My mother was there for two weeks and I would recommend this facility 10/10 for any care. We took a tour with the administrator, David, he was very professional and caring. Anything we needed throughout these two weeks he was there for us. I also want to thank 3 staff members that went above and beyond daily taking care of my mom. Delia, Diana and Miguel thank you so much for all of the care, patient, & love you gave to my mom. Thank you for being such dedicated caregivers! My husband was on the rehab floor for 15 days in Nov 2021. They lost his Ray Ban $400 glasses in the 1st few days. I have been following up on the reimbursement many times, after 2 months request finally went to corporate, now its Feb 2022 and still no reimbursement, I called social worker and admin. only to get voice mails over and over. Very disappointed. The nursing staff was great exception of 1 CNA with an attitude. One evening was attempting to get ahold of any nursing station for several hours then when a nurse picked up, he left the phone on nursing station never to return however I was able to their voices, my husband was sick that day and wanted to make sure he was feeling better. I had to make sure CNA kept up with 3 bathing days, bedding was kept clean, tv was broken 1st 2 days, room was run down but clean. read more

My family member was just recently admitted to Glendora tcu. Helping Hands Senior Foundation was instrumental in finding placement here and I'm so grateful for that. This first week has shown staff that is caring and concerned and there is a positive atmosphere that feels like a family community. The physical therapy and occupational therapy also has our family member working hard to get better. My father was a patient here for several months. He was very ill with cancer, and while it's never easy to have a loved one in a skilled nursing facility, the staff at Glendora Canyon did the best they could for him. That's all you can ask. They were very kind and attentive to our needs and if any issues arose, they always did their best to address them. One thing I especially appreciated is that David, who runs the facility, would always take our calls, always listen to our concerns, and always did his best to help. He's awesome. WORSE NURSING HOME EVER. THEY KILL PATIENTS. IF YOU LOVE YOUT FAMILY MEMBERS Do not bring them here. Not only do they not care for your loved ones but they never pick up the phone even when you're not allowed to visit. And when I say never I mean never. I have called this location 30 times in a row before and no one has answered my phone call. I put my mom in here where I thought she would be safe and they got over 60 of their patients infected with COVID from their staff. They weren't allowing visitors so it came in with their staff. Over 60 of their staff have COVID. My mom had a bedsore so bad then when they had to send her to the hospital they could see her bone. I was never told about the bedsore that she had. The hospital told me the bedsore was so bad it was past stage 4. They couldn't even name it anymore. If you think your loved one is being monitored by a doctor guess what they're not. The doctors might come out every few months but during COVID at least the doctors are being consulted by the nurses on the phone and not even visiting the patients to make sure they're OK or handling their care correctly. The joke is if they have 60 to 70 staff member members with COVID that they have to have all these temp staff members here helping out. So these are the people that are taking care of your loved ones and reporting back to the doctors. Obviously, they're taking COVID very seriously that's how the whole building is infected. I warned them ahead of time my mom was very high risk and would die for sure if she caught COVID and 100% she did. When they finally told me my mom had COVID I did everything I could to keep her alive and safe. The biggest problem is no one was there to monitor her, the nurses were overworked and no doctors were really overseeing her since they weren't coming into the facility. One time one of the CNA's called me with my mom telling me she needed Tylenol and asked if I could call the front desk to get a Tylenol since the nurses were too busy for her. Her Exact words were that the nurses there aren't prompt. I believe their facility was so bad because of the temp workers filling in since the rest of the staff all had COVID. Look I appreciate all the people that risk their lives to take care of other people but if you're working at a nursing home, with elderly people, or people that have lower immune system's you have to be more careful than your average person. My mom's long-term care insurance paid almost $10,000 a month for this facility and this is the kind of care she received. My mom died at the hospital right across from the nursing home last night. My mom has been fighting to stay alive for years with the cancer she was battling and she was finally cancer-free. If we never went here she would still be alive. read more

My sister has CP and is still in Glendora Canyon after having phenomena and needing physical therapy. They have been so kind to her and very attentive . And always made me feel welcome. Kristin who is at the front desk most of the time knows me by name and is always welcoming. My sister has been in for several months now because she also needed surgery on her hand. I know most of her nurses and CNAs by name. My sister of course had her favorites. They have taught her to walk again better than she has in 4 years. The administration is the best. Never have I seen someone so caring about the patients there. My sister goes to activities everyday and enjoys it. We are working on placing her into a long term now. If Glendora Canyon was long term I'd love to have her stay. Yes we have had a few up and downs but they were so easy to work with to get things resolved. I do have to say that they need to work on the laundry, lots of lost clothing. like anything else you sometimes you have to be an advocate for your loved ones. It is nearly impossible to reach anyone by phone at this facility. Whenever I try to call, I either get placed on hold (once for 30 minutes), or I am sent to a random voicemail inbox that has no recipient. When I finally get through to a human being, I get placed on hold AGAIN, and then, I get cut off. So, I have to start over and call again. Sometimes it takes me 6 calls to get through to someone. It's exhausting. This is especially difficult when I'm trying to speak to my mom, who is a long-term patient there. They will sometimes have her speak to me through the phone at the nurses' station, but she has trouble hearing through that phone (that phone has problems), and is unable to alert the nurse. So then, I cannot get through to let the nurses know. Also, my mom is unable to use her room phone on her own, so I have to call to have someone help her with the phone. But, when I call, I get put on hold or cut off, and have to call back. AGAIN. When I leave a message with a particular question or need, I rarely get a call back. I am sure that the employees at Glendora Canyon are doing their best, but this is entirely FRUSTRATING. read more

We had an amazing experience with Glendora Canyon transitional care unit. Absolutely impressed! The patient care was outstanding, "it was exactly what I was looking for." All the way from start to finish! I was so pleased with the service my mom received from this facility. The staff worked together to put forth a high level of effort to deliver results. They went well above and beyond to help my mom feel safe and well taken care of during her 60 day stay at this facility. The support staff was very professional and pleasant. The facility was well equipped and designed for patient care and privacy. They also helped us the family with getting her on the ALW Waiver which provided our mom a way to financially pay for her future care at an assisted living facility. We are so grateful for the help and support that was given to us (the family) and the best part was the caring director, David, he was OUTSTANDING! Very compassionate and patient a 24 hour care and availability on his behalf. We will forever be grateful. Great experience during hard times. read more

El equipo de trabajadores de atención médica realmente se interesó en mi recuperación. Inicialmente vine a Glendora Canyon desde Queen of The Valley Hospital , después de mi cirugía. Básicamente estuve postrado en cama durante un par de meses. ¡Los equipos de terapia física, ocupacional y del fisico en Glendora Canyon son los MEJORES! Como equipo, planificaron mi tratamiento de rehabilitación y todos los días trabajaron juntos para garantizar que alcanzara mis objetivos diarios. Me mantuvieron motivado y me empujaron con seguridad a alcanzar mis metas. I highly recommend bringing your loved one to this facility. I brought my mother here where she stayed for physical therapy. I have to say I was extremely happy with how she was treated. The facility is great, clean, and they have great staff. With my mom having dementia they were very patient with her and made sure to update me constantly. Highly recommend. read more

My mom transferred from another skilled facility to Glendora Canyon and let me tell you Glendora Canyon was by far a great facility. The nursing staff was excellent, whenever my mom needed assistance they would come right away. My mom never had to wait . I was very impressed with the inside of the facility as well. It was clean did not smell foul like most facilities do. My mom needed rehab due to a back surgery and the therapist got her up and going and prepared her to come home. They have the biggest rehab in all the Glendora, Azusa, Covina, and West Covina area. I was definitely pleased with the care and services. The facilities Administrator Jason is very helpful and addressed any concerns we had. He is always there greeting everyone with a smile. read more

My grandmother was here for a few weeks for physical therapy and IV antibiotics. She says she was very happy with the care she received here. She really enjoyed the activities that were available because it made her stay here go fast. At home unfortunately we all work so we are not able to be with her all day and none of us are nurses or therapists so we wouldn't have known how to give her the proper treatment. I highly recommend this place to anyone looking for good nursing care and physical therapy. read more
