My mother is staying at this facility. And she is complaining about the the meals that are very bad and they don't pay attention to to what she should have on her tray. But when visitors are coming they get all nervous. I find it very odd. Also they don't want any visitors without an appointment and when they have appointments it's also a problem. I'm not sure who runs this place but they need to change this. Another thing is that everything you bring them disappear.

Where do I even begin? My grandma was transferred here for care. The nurses were amazing! Albert and Lesley was amazing. Of course because of covid we could not go in to see her but she had a window and were able to see her through that. A month or so passes and we end up putting her on hospice. According to their website we would be able to visit her in person. We went one night, no issues. Next morning, no issues. That night before she passed they made it a HUGE issue to visit her. We entered with no problem, everyone said hi, and then my brother and I were removed from the building (he's only 9, and I, 26). My brother and I did not get to see her for long. They said "until she is transitioning" we can't see her. How will they know the exact moment she is reaching death? Anyways, that night they called us around 10pm to let us know she's "transitioning" and allowed us to go visit 1 by 1. Great. She's no longer talking at this point where as earlier she was talking. So we didn't get to say a proper good bye. Fast forward to 3am we get a call that she has passed away. We can come visit. We get there and then nurse in charge of answering the door is asleep!!! We had to wait until they woke up to answer the door. "Normita" is a nightmare, she threatened to transfer my grandma to another facility two days before she passed. Update: her dentures have been found by the mortuary. What should have been a peaceful time was a nightmare because of their lack of communication and compassion during this time.

The staff at Sierra View Care Center are helpful and resourceful. Most recently while visiting the center I was able to enjoy their new exterior court yard. Was pleasantly surprised to be greeted and taken care of by staff while I was waiting. The facility is clean and most importantly does not smell like some of the buildings I've gone into in the past. The nurses and social services staff were warm and friendly helping me navigate some next steps I need to take to help a loved one in the area. Thank you Sierra View. I'd highly recommend sending a loved one here if they could benefit from a short stay!

Generally, Rehabilitation Care Centers and Skilled Nursing Facilities have a difficult job and have to deal with many different kinds of people. The staff of Sierra View Care Center does an outstanding job in all of the areas my 91 year old Mother needed with her 2 weeks of Skilled Nursing and Physical And Occupational therapy. The nurses would call me with updates on my Mom's progress. The PT staff would allow me to work extra with my Mom, using some of the equipment and instruct me how to use it. The whole place was clean and orderly every time I visited. I have been to a few skilled nursing facilities in the past and "the smell" would always hit you when entering, but not here. Her room was big enough and shared with another resident. Flat screen TV's for each. Competing tv noise was a problem so I bought my Mom a tv headset which worked well. The bed was comfy and she did not complain. She was well taken care of in her room and not ignored. The food was good. I joined my Mom on several occasions. There is a dining room employee who made sure my Mom got her coffee and had her bread buttered each day. He was so kind to all the residents, some who needed tremendous amounts of patience. Although I forgot his name, I won't forget his gentle smile and helpfulness. The office staff kept us informed of all necessary paperwork and meetings. I and my brothers were very thankful our Mom was treated at Sierra View Care Center, truly a well run business, that cares about their clientele.

This facility is better then few I've visited in. This staff is more so willing to help and are very friendly but there are a few that act like your bothering them. The few that i have visited in are just the same I just cant get the smell of urine out of my head when you visit these facilities this particular facility is not that bad but at times will have the odor. The rooms are a fairly decent size but could use a minor touch ups and maybe a little bit of decoration to kind of give it a home feeling. they have the basic TV no satellite dish so it is do able if you just like the major network channels. My dad was only here a short time I just hope he doesn't have to come back ever.

Please this is a serious warning, I just removed my mother from this run-down facility on May 8, 2025, after the RN in charge took it upon herself to allow a complete stranger in the facility at 11pm on May 7, 2025, to visit my mom who she did not know. The RN did not require the stranger to sign in nor did she have a family member at the facility. The RN told me she let the strange women crying hysterically in the center because she claims to have lost her mother and wanted to visit anyone at the facility. The RN then brought the hysterical woman to my mom's bedside and stated, "you have a visitor" !!!! What RN does such a thing? The strange Women had access to my mom's cell phone and called my mom's sister crying hysterically. She told my mom's sister she was her neighbor/friend visiting her and said how much she loved my mom! The stranger obviously was not in a clear mental state, and she was insistent on feeding my mom. My mother kept telling her she was not hungry. The strange woman then told my mom that she was my mom's niece, my mom responded, "You are not my niece." The stranger then probed my mom for person information about her family, which my mom provided being vulnerable in the medical state that she is in, after several strokes and very weak. This went on for a bit until my mom's roommate told the charge nurse to remove this stranger from the room, because my mom did not know her. (Thank God for her roommate) This RN put my mother's life in danger as well as the other patients there. The medical staff are not adequate and make horrible decisions there, and there needs to be a serious investigation regarding this matter! These so-called professionals will be reported as well by me to make sure they don't hurt other vulnerable patients. We filed a report with the local police department in Baldwin Park CA. and with the proper authorities for example Public Health, and other entities. If anyone wants to ask me questions, please feel free to message me, I will be very honest about our experience and those of other friends at the facility. I will also note that the staff is very unprofessional and require serious training. The only good thing I can say about the facility is that 2 of the CNAs Frederico & Hambee took good care of my mom when they were on duty only, otherwise my mom had a difficult time with other staff who were unprofessional, rude to her. (This enter is more like a nightmare, don't let their pre-recorded message on the phone system fool you !!!!!) The loving caring model is all fake!!!!! The decision to abruptly remove my mom indirect response to the breach of safety protocols was Absolutely necessary; I could not allow my mother to remain in an environment where her well-being is at risk. The facility has never called to apologize for this horrid incident nor tried to explain the negligence that took place at the facility. I know that other patients were upset, including concerned family members as well. This facility should be under an extensive investigation for such negligence. Additionally, my mom had fell a couple a weeks ago and she is on a blood thinner medication and a different RN fail to call 911 which is standard procedure when my mom was not feeling well per the treating physician at the hospital ER the doctor recommended to report the facilities poor judgement and lack of knowledge by the RN at Victoria Care Center. KEEP YOUR LOVED ONES FAR AWAY FROM THIS HORRIBLE FACILITY!!!!!!!!!!!!!!!!!!!!!!!!! Don't risk your loved one's health or life!

I'm so blessed to have my 94 year old mom residing here. It was the most difficult decision to…read moreplace her in an assisted living facility. Mom resided with my family for over two decades. As her dementia and alzheiner's has progressively worsened and needed 24/7 care. I had a meeting with her physician who recommended and assisted getting mom placed here. It's been over three months since she began residing with Mayflower. I'm estatic with the care, procedures, documentation and treatment she receives from the entire staff. In addition, the cleanliness of the facility is outstanding. Visiting hours for guests are flexible with appointments made within a few days notice and family is allowed to take her out for the day outings - with approval of their physician. I just had a 3 month review with her social worker and she informed me that mom saw a dentist, optometrist in addition, to receiving a flu & covid booster shot. The facility also has activities like game activities, physical therapy, dancing and even threw a Halloween costume luncheon - which mom couldn't stop raving about. If you're considering care of a loved one - I would highly recommend Mayflower Care Center.

My father stayed at this facility for approximately one month, and during that time it became clear that the staff lacked sufficient training and experience in managing patients with dementia-related behaviors, particularly sundown syndrome. On one occasion, I received a phone call at 1:00 a.m. asking me to come to the facility because my father was awake, talking, and bothering the nursing staff. About two weeks later, I received another call at 5:00 a.m. with the same request. During that call, I was told that if I did not come in, they would transfer my father to another facility. As a family member, I understand that caring for dementia patients can be challenging, but these behaviors are common symptoms of the disease and should be expected in a facility that accepts patients with dementia and Alzheimer's. It was concerning to feel pressured to come in during the middle of the night because the staff could not adequately manage his condition. My disappointment continued after my father's discharge. The discharge coordinator, Ms. Wang, informed us that she would submit the paperwork to begin the IHSS (In-Home Supportive Services) application process on our behalf and told us we should receive documentation within one to one-and-a-half weeks. After 1.5 weeks passed without receiving anything, I contacted her and was told she would look into it. When another 1.5 weeks went by with no update, I personally visited the IHSS office to check on the status of my father's application. To my shock, I was informed that no application had ever been started. As a result, we lost three valuable weeks waiting for a process that had never been initiated. Had we been told the truth from the beginning, I could have submitted the application myself and avoided the delay. Families caring for elderly loved ones already have enough stress without being given inaccurate information that affects important support services. Overall, my experience was extremely disappointing. The facility demonstrated a lack of preparedness for dementia patients and failed to follow through on important discharge-related responsibilities. I hope management takes steps to improve staff training and communication so that other families do not experience similar issues.
