The groves is a nice place and my father has made the adjustment and now feels very much at home here. The thing that strikes me the most is the entire staff has done everything imaginable to ease my fathers transition into their care as well as our family's transition of placing him into their care. The grounds are behind locked gates, both my wife and I received keys and are free to visit 24/7. This is in sharp contrast to most other homes that have rigid limited hours of visitation. In addition this unlimited access gives us great peace of mind that nothing weird is going on after hours and that our family member is safe and having his needs met. When not providing resident care the staff is always busy cleaning or cooking and always seems to be on the move. The absence of the odor of urine at the front door is a tell tale sign that your loved one is being properly cared for. The floor plans are wide open and large, but still warm and almost cozy in many ways. Institutional residence Vs. jail would be a nice way of saying it. I had my father in independent living for many years and when the time did come that his ability to self care was over and the time was upon us to upgrade the level of intervention and assistance we toured over a dozen places before deciding on the Groves. It's a choice we are happy we made without regrets. The staff has been very helpful, and while all the staff has been exceptional, Jennifer in particular has been fantastic with my father. And Mary sets the standard of great care. I think the greatest fear we have in placing a loved one into a care home is that are we doing the right thing? I think the need to do the right thing over shadows our need for caution out of fear we might do the wrong thing. Both my wife and I are happy we have made the right choice in placing my father at the groves. My father has made it his new home and has adjusted well. We pick him up numerous times a month and go to breakfast, take him to his appointments and after granting them permission, he often leaves in his electric chair to get out and conduct his personal business. While everyone's need are different, our needs have been met and again we are very, if not extremely satisfied with our choice in placement as well as the care and interaction my father receives at the groves. I do wish it was easier to load my wheel chair bound father at the curb, but since the staff and the owners seem compelled to park closest to the building it force me to load him into a vehicle from the gravel areas Vs. at the sidewalk.

The hallways on the first day were lined with what appeared to me broken, old or outdated equipment, stacked chairs, and lots of flattened cardboard boxes. By day 2 that had all disappeared. With the exception of most, if not all of the PT and OT staff almost no one was wearing any sort of identification. Few people introduced themselves to my dad or my sister and I. We had to ask them who they were & what their position was. The is pretty basic...you ALWAYS wear ID. Shannon was one of the people I remember being friendly and bubbly and welcoming. Mike, dads nurse, was also friendly and wearing his ID. Frank was great with my dad, but no ID. We ignored the large (volleyball size) sticky spots on the floor of my dads room and increasing about of litter that was on his floor along with the previous days clothes. We tidied up ourselves. When we asked an orderly, aide, cna, (whatever he was as he didn't have ID on) to help us fix our dads bed he said he was too busy. I was shocked. He was right there in the room with us as the time. My dad was not Shannon's patient that day, but when I went to ask her, she kindly helped him with his bed issue. The social services office was a nightmare to work with. They sat on the paperwork from the memory facility dad was going to transition to for over a week and his release was delayed because of it. I spoke to them face to face daily, & left phone messages that were never returned. Request after request to do a TB test on dad as required by the memory care facility was met with delay after delay. We knew dad would not allow the test if we were not there, so my sister asked to have them do it while she was there at 4 pm on Monday. The nurse, Misty, refused saying she was busy charting and would get to it before the end of her shift at 10 pm. My sister explained that with his dementia he would not allow it if we weren't there to sooth him and assure him it was ok. She still refused. At 7 pm Frank called me 2-3 times in row and allowed me to speak to dad as he was refusing the test. Talking to dad on the phone did not work and Frank was unable to give him the test. Around 8 pm Misty called and said Frank tried 3 times and failed and dad would not be getting the test. I asked her to personally go and attempt it and to put me on the phone with dad. She said I'll try do it before I leave at 10 pm. Trying and doing are 2 different things and I could not bear to leave him there another day if she failed. So I drove over there and simply spoke to my dad and Frank was able to give him the test. All of this could have been avoided. I left voice mails with the social services people, the director of nurses and PT/OT. Only PT/OT ever returned those calls. Also called the administrator after hours and left a message. I have not yet heard from him. The memory care facility had tried repeatedly over the past week to get the paperwork sent to them needed to admit my dad. At 3:45 pm yesterday 7/8, after over a week of requesting it from them, they notified me that dad would not be able to be admitted today, 7/9. I saw red!!! I left messages with all offices mentioned before, social services, director of nursing, administrator as all had left for the day (before 4 pm!) I was told. I then asked to speak to whoever was in charge of the facility at the moment and it turned out to be the nurse, Frank. I didn't know he was a nurse as he didn't wear any ID. When I spoke to him he said he would get on it. At 4:30 I arrived at Sabino Canyon rehab to make something happen. NOTHING was going to keep my dad in here any longer. Brittany in social services was in the office and said she was working on getting the paperwork sent. I asked her why all the delays and why didn't she return my messages. She didn't have an answer. I then spoke to the director of nursing and went through the laundry list of complaints with her including the fact that she was not wearing any form of identification. I am removing my dad today, thank goodness, The place is dirty, and you can tell people don't like working there, no one greets you or smiles. PT/OT were absolutely wonderful. I don't remember everyone's names, but Paul and Dora were great! Do not send your loved one here!

Nursing care was excellent as well. Cleaning staff worked to keep the rooms clean. Cons: Limited focus on healing environment. Food was barely edible, hot food mostly cold, really unsafe, often couldn't tell what i was eating, they offer extra items on the menu, just check them off, yet don't provide little pencils, so that is a fail. The time i did order other items, not fullfilled. Erratic staff performance: often left the lights on after night checking vitals, often not all vitals taken, roomate left their tv on 24/7, so i had to listen to old tv shows, high level of noise from the hallways, Too many times Vitals equipment not working, broken. Communicated to management via web site response, no response to my note:,Fail Given i was in rehab did spend time watching netflix on my phone, took days to understand they had wifi, and to obtain the account name and password. So many little items to easily fix, to improve the experience, management not paying attention or maybe don't care. Changing the little things would improve the experience big time.

A housekeeper was dragging a large bag of laundry? garbage? through the hallway. The rooms were small, even for two people, and noted that some rooms had three people crammed in. I saw residents sleeping on mattresses on the floor. The plaster was falling off the walls. Baseboards rotting. The sign for the Utility room was missing letters.     this skilled nursing facility /rehab & long-term care / nursing home is horrific in every way! the…read morecare from CNAs, nurses is a travesty! personal care, if one is confined to bed and cannot go to the bathroom themselves, they are changed by an aid a CNA actually. that stands for certified nurse assistant! where they got trained I do not know!?, as each and every time, and each and every cna does not know how to do this properly. urine on your butt can cause bed sores which can be extremely serious! if they do use the wipes, they never and I mean ever wipe the cheeks on your butts! also, there were a few times where the CNA decided not to use any wipes at all! when I pointed it out to her, she was defensive. I watched how they rolled the chuck/pads under you and the brief how they taped it and how you rolled from side to side. so, from there on, I changed myself! they may not do personal care for me any longer! it boggled my mind! I was a patient there, I'm in a wheelchair for 10 years now but I broke my leg in a car accident and had surgery. so this is what I experienced above. it's difficult to write this in a review. but people need to know the personal care, and how they do not care! I am compassionate and realize they have many other patients, too many in fact, and some CNAs and nurses work double shifts. it has become the last couple of months, like a revolving door. people are quitting and I'll see a new face each and every day! this is difficult on the patient even though I do feel sorry for the employees. they should be paid more. also, they should be able to hire more medical professionals so they do not have to have 25 to 30 patients for one employee! therefore, the patient suffers as your call button will be going for one, two even 3 hours without help. suppose you're choking? suppose you're having a heart attack? after all, we are in these rehab facilities after coming from a hospital from many illnesses, falls and fractures, some are on oxygen etc. secondly, they seem to only care about money, about filling a bed! the meals are awful! they come cold, & every day three times a day the trays and the silverware are incredibly filthy! if you are one of the 10% of elderly that 'speak up', advocate for yourself, your own body and mind. watch out! they do not like that. so be very careful. it's like walking on eggshells. I lived it for 7 and 1/2 months. I didn't even have a way or the time to grieve for my husband that passed away only months before I entered on June 4th 2025. I left January 16th 2026. if you have a loved one, please do NOT bring them to Park Avenue health and rehabilitation Center. do not be fooled by fancy decor, as chandeliers and such as you walk in the front door..( at any place). looks are deceiving. and you will be greeted kindly by the staff and office people. you have to visit your loved one or be a patient moreover to know what you got yourself into! if you really love your loved one you will be aware of where you are taking them to live or to heal. and never sign anything before reading every word!

I was somewhat worried after reading some negative reviews in Yelp, but my experience was a good one. The staff were very cheerful, friendly and helpful, especially Nicole and Mila. Anytime I needed help, medication, or had questions, the staff quickly responded at the push of a button, day or night. The food was varied and generally good and nutritious. The kitchen gave us two options for our entree, and allowed us choices of beverages and dessert. We were given 2 showers weekly, along with plenty of towels and toiletries.The shower was clean and I never saw any cockroaches or other insects. My room was cleaned daily and my bed linens were changed twice a week in contrast. I had therapy in the facility gym twice daily Monday - Saturday. My two main therapists, Andy and Dave, were kind, patient, and helpful. My leg was non-wait-bearing, so they showed me how to use a walker with one leg, and taught me how to use a wheelchair to get around. They also demonstrated some "hacks" for getting around and give me a list of excises I could do on my own. Foothills Rehabilitation Center thoughtfully provided tv headsets on request so my roommate could sleep and not be disturbed by noise from my television. Finally, there is an activities center where residents can meet people, play games, have sing-alongs near the gym. The only down point was that it was quite cold in my room the first few days of my stay. However, I requested extra blanketsand I was fine from then on. All in all, I was extremely pleased with my stay there, and highly recommend the Foothills Rehabilitation Center.

While much of the common areas are kept nice for visitors, the individual rooms are filthy, with old food under the beds and trash cans that stick to the floors. The evening and night staff are self-absorbed, treating the residents as an annoyance. Some are actually psychologically abusive as they behave in a threatening manner. Other staff are simply not trained to do what is required of them. In short, this is a very unsafe place to stay at.

The lobby looks nice, but patient rooms have paper thin walls and lousy plumbing. The staff had lousy attitudes and were unprofessional. The male LPN was sleeping with both of the nurses aids ( Why did I need to know that?), and accused one of the aids of stealing drugs from a patient. Keep your loved ones out of this place!
