Dr. Teruel is the epitome of professionalism, compassion, and expertise, as is her entire staff without exception. Additionally she's Board Certified, the highest of achievement in the profession. My (severe) TMJ is a combination of genetics and decades of extreme teeth gritting, or bruxism during sleep. The Head Pain Institute was formed to address the needs of people in my exact position and I'm forever grateful. My wife was referred to this organization for jaw pain. After we arrived we were escorted into an examination room. A MA asked several questions and then asked for a credit card payment before the exam could continue. We paid and some images were taken follow my the Pain Specialist (Dentist) came into the room and explained options including trigger point injections and a replacement mouth guard. We passed on the $1000 mouth guard and then was asked for a second payment before the injections could be administered. We are now at a total of almost $1,300. I injections did absolutely no pain relief. The paid institute also billed our insurance and was paid an additional $800 plus with a note to us that we owned only $104.00. I requested a reimbursement for the excess paid. NO response from the provider. The whole experience was a sales job. read more

Appointment was extremely thorough. It took me two hours to fill out the paperwork at home and then there was more paperwork to check in. It was another hour of answering questions with the nurse. Love the staff. Doc explained why I may have had throat pain, unrelated to TMJ. She was not helpful in relation to my TMJ. I know I look young, it would have been helpful if she read my file before assuming my age. I'm 34, I know perfectly well why I need new nightguards often. Found the appointment frustrating. I personally haven't experienced any of the problems with billing and insurance as many of the other reviewers. As a matter of fact on my most recent visit, they wouldn't do a scan for me because it had to be pre-authorized by insurance. Are treatments and appliances expensive? Yes. If you haven't met your insurance deductible and out of pocket costs, you're going to pay. It is the way things work now. Sadly I trusted my daughters dentists referral. I thought that we were being directed to a reputable specialist that could help with my daughters popping/locking jaw issue. I'm pissed at myself for not reading these other reviews before proceeding. We abruptly left our appt after spending $614 on a 3 min exam and a simple set of X-rays. The decision to run away came shortly after bite splints were the "recommended treatment plan." How much are the splints you might ask? Only $3000!!!!, followed by~ "..and your insurance won't cover any of it." Now my jaw hurts, as it hit the floor. A few years back, I went to Dr. Farrell. When I made the appointment, they told me that it would cost $400, which was steep for me at the time, but I was in so much pain. WHAT A SCAM!! Their aggression at collecting the $400 consult fee the second I walked in the door should have been a red flag. After a 2 minute consult, Dr. Farrell tells me that the exam would be inconclusive without another $600 for imaging. They stick a card reader in my face. $1000 was an impossible amount for me at the time, but I gave in to sunk cost fallacy. He then describes the treatment plan that includes injections, a mouth guard, and such. He then sends me to another room where some woman gives me paperwork to sign that says I understand that there is no guarantee of success. That's when I see treatment plan's price tag: $4-5k more! Insurance wouldn't touch it. I had spent more than my mortgage that day only to walk away with zero treatment. As I bawled in the parking lot on the way to my car, feeling helpless and scammed, I see a brand new sports car with the vanity plate "TMJ DOC" mocking my pain. Corvette, if I remember correctly. Way to twist the knife, bro! What kind of horrid human boasts about profiting off a broke mom working two jobs? Thing is, I had listened as he described how the mouth guard would hold my jaw, so I started holding my jaw in that position for a minute or so a couple times a day. My flare-ups instantly went away and I've barely had issues since. I didn't even need his stupid scam treatment plan. If I could give a ZERO I would. Although the staff was very pleasant, they don't tell you want your estimated balance would be if your splints would not be paid for by the insurance company!! I had been a patient of Dr. Farrell's for over 10 years. I was worried about transitioning to Dr. Tereul. I had seen her before and knew that she had been under Dr. Farrell's tutelage for a number of years. I had nothing to worry about. I didn't think it was possible but Dr. Tereul is significantly better than Dr. Farrell. She has great bedside manners and her injections are gentle. I am the biggest baby when it comes to needles. I used to cry and scream with Dr. Farrell's injections. I didn't cry and didn't scream as loudly with Dr. Tereul's injections. She is Amazing!!!!!!! Medical Assistant, CJ is also Amazing!!! I told them that I need her as my medical assistant whenever I go in. Dr. Farrell could not have left his practice in better hands. Kudos to Dr. Tereul and CJ. The only negative is the woman from the office who was trying to pressure me into getting injections that my insurance did not cover versus the ones that my insurance did cover. CJ made me feel better after I expressed that I did not like how the woman from the office was trying to pressure me. The negative reviews are not incorrect. I am still suffering from a dislocated jaw after months of following their treatment plan. I have seen minor improvements. Although I would have looked in to other specialist first. The recommended treatment is extremely overpriced and I have pretty decent health insurance. The out of pocket price was just ridiculous. Also most of the staff is disorganized and rude. My second appointment, I waited three hours past my appointment time to be seen for a rather painful procedure. It was not a good experience. PLEASE find someone else to go to. Although Dr. Ferrel knows what he's doing, it's not worth the torture from their billing dept. nor the money. I've spent thousands of dollars on this treatment that really wasn't even necessary. I 'needed' 2 different splints, and honestly only one of them was needed. Besides all that, their billing specialist or department is horrible. if you have any changes to insurance or need to adjust something, it is worse than pulling teeth. I would get phone calls about xyz not being covered, but turns out they didn't bill the right insurance company. I really wanted to give them a chance, but after I was over charged what was discussed after some changes to insurance, (and they promised me I would be reimbursed) I have yet to see that reimbursement and it's been 2 months. My biggest pet peeve is that when you call and someone isn't available to help you, they will tell you someone will call back by end of day, but no one will call you back, I had to call about 6 different times before someone was finally able to answer my questions. As someone who works in an industry that prides themselves on customer relations, it is unacceptable to not call someone back and have your client/customer call numerous times. read more

They were always willing to set up an appointment even at a moments notice. After receiving their treatment and following their recommendations and path forward I was finally able to regain control of my jaw problems and get back to feeling normal! I've been in a mysterious chronic facial pain since 1997 and my last major facial surgery in Oct. 2018 left me with increased pain, ten fold. I was impressed with the CT Scan available at the first visit and viewing the results together with the doctor is nice. They discovered a metal fragment in my face, exactly where the pain is, left over from a surgery in October. **update** Super glad i got to meet Dr. Ferrell and his skills on reading the ct scan and telling me there is hope in removing it, it is still in the soft tissue. I know this update to this review is a long time after I originally wrote the first one, but I had to think about what I was going to write. First of all, I will say that since I stopped seeing Dr. Farrell I've noticed a few things: 1. I can finally chew solid foods again. I went SIX months on a soft foods diet (i.e. couldn't chew anything harder than a BANANA). 2. I stopped using the mouth guards that were custom made for me and reverted back to the NTI guard my dentist had made me originally. 3. Pain is gone. 4. I have about $3000 in medical bills that I'm paying off for services that DID NOT HELP ME WHAT SO EVER. Dr. Farrell is out to get your money and preys on desperation. How do I know this? Because I was desperate to feel better and instead of helping me, he made things signficantly worse. Prior to visiting Dr. Farrell I had facial tenderness, jaw popping, the occasional headache and jaw tension. Nothing that was unbearable and something that I just dealt with on a day to day basis. Visited Dr. Farrell and I went from managable pain to excruitiating pain as soon as he gave me the anti-inflammatory shots. I'm pretty sure those shots just immobilized my jaw for 6 months. I was literally in hell for 6 months as I couldn't chew anything. I was on anti-depressants, muscle relaxers, insomnia medication, pain meds, etc. for all the issues that insued after the injections. Fast forward to now...I'm off all medications, my jaw feels normal when I stopped drinking caffeine and cleaned up my diet AND I can finally chew solid foods and not be in pain. People - save yourself pain and money by not coming here. This is just the tip of the iceberg. read more

As far as DR Farrell goes, he's wonderful, very knowledgeable and seems to care about your prognosis. I became a patient in July 2018. The staff was intially wonderful! I received injections and meds and a soft food diet on my first visit and my pain on the left side had decreased significantly. And I'm truly grateful they worked after suffering chronic migraines for the past 15 years (20-25 migraine days a month). I spoke with their billing specialist, who later I find out doesn't do billing, (confusing I know), we agreed to payments of $358/mo for 4 months. She stated these payments could not be extended past that point. So I agreed because I want a normal pain free life again. So the following day she calls me and demands $358, the payments start day was never clarified and I only had $50 to give her at the time, and I did. During the conversation she was very rude and would not even try to work with me. I ended up taking out a small loan and she was paid $358 within 7 days of my first visit. So this continues for another month and in the mean time I get my explanation of benefits from my insurance. In my profession I've done medical billing and coding and currently handle quality control for a health insurance company. I notice my first visit I was charged for several different "x-rays". At the visit we did 1 panoramic x-ray, which they called a CT. I was billed individually per x-ray view, than just billed for a single panoramic code. I can only assume this was a money making tactic as the multiple charges added up to $2401 in x-rays vs the couple hundred for a single code for panaramic x-ray. That's when I started paying attention to my bills. So I paid the next month and then on 9/12/18 I went in for another visit for split adjustment and injections on the right side. Dr Farrell was out of office and the Nurse Practioner was there and she administered the injections, trigger point injections. These were quite more painful than the first injections administered by Dr Farrell. The first time I received injections I was numb in the locations for a good 24 hours and then pain free since. Now on the right side, they were more painful. I was NEVER numb, and I've been in increased pain ever since, and literally out of work for close to a week now. Now I'm not claiming any malpractice or anything, maybe the injections just didn't work. The card they had on file was a temporary debit card, and it was cancelled when I got my new card activated, which I had honestly forgot to inform their office of the update. I get another explanation of benefits and I'm going over the charges and I see a "diagnostic x-ray " charge -$250. Now at the 9/12 visit the billing specialist came in and asked if they were performing an ultrasound with the injections in which she was told no, not by 1 but 3 different MAs/Nurse and one was not done. But I was charged anyhow. So when the billing specialist calls me she was very rude. I had been working 60 hrs a week and in school 25hrs/wk and wasn't able to get back to her during business hours. She leaves me a rude voicemail threatening to send me to collections because my cancelled card declined. I called her back and she was very rude, accusing me of trying to "scam her", and demanding payment because I've "been difficult to work with". At this point due to all the billing red flags, I refuse to give them my financial information, and I asked for an itemized bill and refuse to pay until I can audit the billing and make appeals with my insurance company, who has paid out on services not rendered. During this conversation I explained the failure with the injections, which she said was a separate issue and condescendingly told me to stop getting the injections. At some point she put me on hold and returned to tell me I would get an itemized bill, my last visit would be comped ($122, my portion after the insurance paid) and Dr Farrell would like to examine my pain concerns himself. I appreciate her trying to make the situation "right". Here's where I stand now. I do not wish to go back to that office after being treated so rudely by Becky, on almost every encounter. I'm not sure if the NP isn't specialized enough to perform certain or any injections due to the outcome of this last set. I have a hard time stomaching the billing ethics as it appears they bill to make the most money instead of fairly and honestly. At this time Im auditing my itemized bills and appealing and working with my insurance company to recoup any overpayments made to AZTMJ. This is disappointing as I finally thought I had found the solution to my pain, and will be looking for another provider. read more

SCAM. I was referred here by my dentist for TMJ treatment. I was transparent with them and told them I would not be able to afford treatment without prior authorizations through my insurance. They assured me they would submit PAs and get back to me. I had to call them 3 times over 3 weeks to ask if they were approved. Each time I called I was told that no PAs were initiated, but they likely wouldn't be covered. I asked how much it would be for my treatment plan and they said they "couldn't give me a quote over the phone", and insisted I'd have to book/pay for another appointment just so they could tell me pricing that I couldn't afford. Disclosing out of pocket costs is not violating HIPPA. After asking a few more times how much it would cost- I was told "since I was insisting" - anti inflammatory injections, teeth impressions, and Botox would be $2500. People are in real pain and looking to places like this for relief, not to be finessed and for an even bigger headache. read more

They took my money upfront promising to get rid of my migraines as I have no cartilage left on my jaw. Then got COVID called the office and they cancelled all my injections to have them rescheduled at a later time. When I was well enough I called to reschedule the injections and their office said I needed another evaluation although I was in excruciating pain and what I really needed was the injections. Anyway, when I showed up for my appointment that is when they mentioned the second evaluation, although I got tired of explaining my situation. Dr. Antonia Turel showed up an hour later and was upset at me for trying to explain my situation to her staff and scolded me for cancelling my injection appointments (even though I called her office to notify them of me having COVID). She talked down to me as if I was the problem not her office and the horrible service and fired me as her patient. When I got my remaining balance they had deducted $2,000 + for a scan and a couple of injections that didn't help with my pain. Ive had severe jaw pain for many years so I booked an appointment with Dr Farrell and I'm so glad I did. The treatment can be a little pricey but it's worth it. So far it's only been 3 weeks of splits and injections and I'm almost pain free. I scheduled an appointment with another doctor for my second round of injections because Dr Farrell was booked and he still popped his head in to check on me and make sure I was alright. Was not happy with this office at all. I felt they run a pill mill operation here in a way. I do not like to take pain medicine (i.e Muscle Relaxers, Narcotics, etc) and told them this. They kept insisting that it's part of the treatment but they wanted me to take 2 a day for 6 months!!! I don't think so! You guys aren't gonna get me addicted to that stuff. I felt offended that no one offered me an alternative. They simply said " well we can't help you if you don't follow the treatment". Also for a TMJ place you would think they would use innovated technology like electric pods, bite analysis technology, and other new age stuff proven to help. Or offer teeth shaving referral to make sure teeth is aligning and balanced. All they wanted to do was prescribe pills, and a bite splint. Which they claim insurance pays but really it doesn't. Then they want to object you with shots. There is so much more to TMJ relief than overpriced bite splints! I feel this place is a ripoff and the only people writing good reviews are the ones who are loving the pain meds and are loopy from its eurphotic state. This was the worst doctor's office experience I've ever had. First off, we're still in the middle of a global pandemic and the entire office was mask-less. As in, they have a no mask policy for staff and patients. Um...persons in the medical field acting as if nearly 700,000 Americans haven't died of Covid? Very cool. Second, when I called to make my appointment, I specifically said I needed to get botox for my TMJ. (I've been doing this twice a year for five years and it's the only thing that has made life manageable.) I was told I needed to have a consultation with Dr. Teruel first and *then* I'd get the injections. (I even got the price breakdown for everything: $250 for the consultation, $1200 for the botox.) When the consultation with the doctor started, I made it perfectly clear I was there for the botox because it's the only thing that gives me relief from the TMJ symptoms and migraines. I was then forced into a CT scan of my jaw (for an additional $350) because I was told this, too, was the only path forward for botox. After the scan, the doctor told me she wanted to give me steroid specific injections in my face, jaw, head...and that *this* had to happen first. As in, before the botox. I expressed that I didn't want steroid injections in my face, jaw, and head and that I had just moved to Scottsdale leaving my Los Angeles ENT behind and the botox is what I needed. She would not do it. I reiterated the botox injections were the reason I made the appointment. She wouldn't budge. To recap, I had been told I'd be able to get the botox *after* the $350 CT scan--after being told, initially, I'd be able to get the botox after the consultation. This was a massive bait and switch and I feel completely defrauded by the doctor. I was led on and lied to. I have an absurd bill for absolutely nothing. Meanwhile, I hope I don't get Covid. Not ONE safety protocol was in place. (No hyperbole.) It's still mind-boggling. My advice if you're reading this, please find another doctor for your headaches and/or TMJ. While they cured me of my TMJ pain, I am reminded every time I come back of why I hate this place. Come on time for your scheduled appointment and sit in the waiting room for 35-45 mins. Complain about the wait time and get sat in a room for an additional 15-20 mins. It's fucking ridiculous. I thought the office looked nice and organized... they had lots of bells and whistles that made it look like it would be a promising visit. Dr. F really never spoke directly to me, but through his MA. I found that odd and quite rude. He asked me NOTHING of my symptoms but rather the MA read a synopsis to him ... I didn't even have a chance to ask questions or clarify my symptoms. Which makes perfect sense now - he doesn't care to treat YOU, it's about throwing nearly the same plan at everyone! I would be cautious!!! And yes. The finance gal...I'm not even sure where to begin. It wasn't smooth at all. Really kind of rude if you don't buy into their protocol or plan. The receptionist, Kalee is not very knowledgeable, as to how insurance with Medicare works, therefore she gives out the wrong information. I spent over an hour trying to call around and figure this out. Be sure you complete your new patient paperwork immediately. It is so very important for them. She has no customer service and all they are concerned about it how they get their money. Not a place that I would recommend. read more

Wow I had the exact same experience as Nicole M. I have been twice here and told every time that my insurance doesn't cover it. I called my new insurance before I made the appt and they said it was all covered however when I went in it felt like a joke. They make you do a full work up but say insurance doesn't cover it. Had to do the ct scan and pay the $350 like the other posts here. I was in alot of pain and they stated they had to submit to insurance again to see if injections were covered. I asked if I could get a few muscle relaxers but they won't they want me to come back and pay again to see the doctor. Come to find out they never called the insurance nor submitted the paperwork. It feels like an absolute scam and insurance fraud. read more
