The worst place to have your loved one at. Some of the nurses don't care on how your loved one is they don't check up on them if you call they take forever to come check up on them me and my family had told them my dad couldn't eat on his own they didn't care they just went in to the room leave the plate and leave, today the nurse taking care of him I had called for him to get cleaned out she said she couldn't do it because she was passing breakfast she told him to eat and after she was going to clean him I called once he was done she came in with an attitude didn't say a word changed him and left. He had physical therapy come by today they took him brought him back next thing you know the nurse comes all mad shaking her head saying they told her something because he's not at dialysis when it's her job to take him, after he got back she put him in bed crushed his feet and didn't even say sorry after that she dumped the pee from his catheter spilling a lot on the floor and than she left, I called her back she come in raising her hands with an attitude asking what we want that she was coming back I asked her to heat up his lunch she left slammed the door came back the food is cold she left again once again slamming the door mind you almost all her shift she has been using her headphones blasting music loud that even I can hear it with the tv on, I also asked her for her name she didn't say it she had it covered so I wasn't able to get it. Another thing my dad uses oxygen and it's the third time they take it of and don't put it back on I've been doing it. There was another time where I saw this guy nurse taking him to get his dialysis they pick him up with a machine since he can't get up on his own he told my dad to hold on the bars and once he was in the chair to get the dialysis his arm was stuck in between the bars and the guy told my dad that why was he holding on to it and that's when I told him he had told him to hold on from there and he just laughed than he asked me if I speak both English and Spanish and I said yes and I told him I understand both maybe he thought I didn't read more

This skilled nursing facility is wonderful. My dad went there after some time in a hospital. They were so professional, so kind, and incredibly caring. They treated him with respect. The facility is very clean, the food is well above hospital food. I would not hesitate to use them again. The nurses were quick to respond. It felt more like family than medical staff. My friend arrived on a Thursday evening. Barely give information about the basics. Another family member had to go to the front desk to figure out how to get his phone on the internet. The bed wouldn't go up and down...reported on Friday- every nurse, assistant or anyone else who entered the room.Try to help fix it...4 days later come on monday it's now working. The remote for the TV was filthy and had paper tape on it that had to be full of germs. The particular remote wasn't well marked and you had to guess how to use it till we got help on Monday afternoon. The person who was so helpful was going to try to get a different remote.That was better marked. They should have some kind of handout for new patients that tells them many of these things and how to get help. Right now my rating for them is very low although they were recommended. From the outside it looks good. The lobby looks good. Checked my sister in and it was downhill from there. Response time for needing help Is 30 min to an hr at best. Some of the staff were very rude. They feed you and give you your meds, that's it. No one ever checked if she needed anything! We've been told this facility isn't that bad. That's unacceptable. Some of the staff went above and beyond, Hallie was very helpful when the DON wouldn't return my calls about arranging transportation for a medical appt. Another receptionist assisted me when the social worker was hard to get ahold of. We tried to be patient and understanding, always said thank you when tasks were completed. When my sister was being discharged they practically kicked her out of the room because someone was else needed it. We'd been on their time schedule for 2 weeks and suddenly they want us to accommodate them?' I'm a 75 year old woman pushing my sister in a wheelchair to return to home and not ONE person offered to assist me in loading her in my car. If you like to be neglected, eat crappy food and are lucky enough have someone attend to your needs, this is the place for you. read more

Do not send your loved one here. This place is HORRIBLE. My boyfriend had a stroke in Feb 2023. He was in Memorial Hermann TIRR for a few weeks and he had to transition to a skilled nursing facility before going to a neuro specific facility. The hospital recommended this place, but unbeknownst to them it was a horrible 8 days. My boyfriend is unable to speak clearly or walk by himself, so therefore he is bed ridden unless someone helps him get into his wheel chair. The first full day was okay. The second full day his daughter walked in to him soaked in his own urine and then he bursts out in tears. Over the next week, I walked into his room and sat next to him. He was lying in a sheet with dried urine one it. One day when I went the power was out due to the storm that just blew threw. There was a contract CNA that came to speak with me when I pushed the button requesting for assistance. When he came into the room, I was asking him a bunch of questions that he obviously did not want me asking. I asked him when the last time he was changed and he could not tell me due to the computers being off. I asked him if he had a log and he told me that due to HIPPA they were not allowed to log anything and leave it lying around. Although, in his room at that time the nurse from the previous shift left all the stats of all the patients living in the facility in his room. The CNA, Devyn, began to raise his voice, roll his eyes, and shake his head at me as I was the person in the wrong. When I called him out on his attitude and raising his voice at me, he walked out and said that he was going to get the nurse. When the nurse, Lauren, walked in, she was completely polite and the only person that I cared for at the facility. Lauren is the only person who had the patience and understanding of those temporarily living there and their families. The speech therapist would come into the room and speak to my boyfriend as he was stupid yelling at him in his face. My boyfriend would complain that he had not done much in therapy and pretty much lost 8 days of work after he worked his butt off in TIRR for four weeks. There was a second speech therapist, I cannot remember her name, who was amazing with him, but unfortunately he only saw her his last day there. On Easter weekend, he had a wet diaper. I hit the call button requesting for a nurse and 20 mins go by. I began playing a show on YouTube that was 55 minutes long. I understand that they have other patients, but no one ever came to the room. At this point, he had soaked his sheets and he needed an entire bed changing. I hit the button a second time and no one came. I waited a bit longer and I walked out to the nurse's station, no one was there. About 20 minutes later, I walked back to the nurse's station and I spoke with the head nurse, Holly, who said she would send the aide. I went back to the room and waited another 20 or so minutes, and finally he asked me to change him. I walked back to the nurse's station and when I asked for sheets to change him myself. She responded that it had only been 20 minutes. I explained it had been longer as I continued to push the button and walked to tell them. I requested fresh sheets a second time and she gladly gave them to me. Holly then turned around and walked back to her desk to sit down. She never offered to come help me or send someone to come check. After this incident, Holly never once looked me in the face as I walked through the halls or had anything to do with me or my boyfriend. On Sunday, he again waited over 40 minutes for someone to change him. An aide walked into the room and once she found out he needed to be changed she turned off the alert button, said she would send his assigned aide, and then she walked out. The time went on and over 40 minutes later, again, he asked me to change him. I walked out to the nurse's station where I told Ebony that I changed him and they do not need to send anyone. The look on her face was surprised. Prior to this change, I explained to Ebony what occurred the day before and she responded that she would let the Nurse know. I explained that it was Holly and she responded "well, I guess I will let someone else know". I had a dayshift aide tell me that they do not get the bed pads for their patients because the other shifts are lazy and do not check them to clean the patients up when they're suppose to. This is proven that they do not check as myself and my boyfriend's daughter explained to the nursing staff that he needs to be checked on every couple hours due to him not pushing the button, he's new and does not know these people changing him, and he's recovering from a stroke so he may forget to push the button. I was with him every evening after I got off work and this is only a tid bit of the experience that I got. This is not even including his daughter's experience with them as well. This whole experience here was horrible and a complete set back of his progress. read more

Simply outstanding. Staff is simply outstanding. The food is top notch. Facility is very clean. I have been here two weeks, there is a young man here named Polo. His very presents brightens my day every time I see him and he says hello. I'm a disabled veteran, and I get treated like royalty. The place rates a SUPERB from me, and I'm known to complain. we were required to pay in advance for an entire 30 days at this facility. our loved one did not live the entire 30 days and when we requested a refund for the remainder of the funds paid in full we were told it may take from 2-3 weeks! we are in need of these funds immediately to pay for pending funeral arrangements but not even the facility administrator will return our calls concerning this matter. not to mention a nurse who walked in my relative's room with no name tag, refused to identify herself or write it down and then slammed her fist down in defiance when asked again! our loved one was found in a urine soaked bed when we came to visit him. read more

She always treated my Aunt with kindness and compassion and made sure she always communicated with me and the rest of my family. We selected this facility because it was one of few allowing family visitation. Limited visits and time periods allowed for a safe environment for patients, family and staff. Their Covid protocols made us feel protected and safe during recovery. Quality staff from start to finish. Administrator- Admad very attentive to patients needs as well as the family. Our nurses were so understanding and compassionate, took very good care of my aunt. Tammy with PT was awesome. Anthony, Janet, Anna and Vanessa were caring and so accommodating. This place is awful! Medical care and communication with families is a disgrace.

NOT RECOMMENDED! My loved one was admitted for post-stroke rehab to HTC on Jason Street. In a mere 32 hours, a nurse had clogged his GI tube with pills that had not been crushed properly. In addition, nursing staff was not trained in recognizing a UTI. Staff response times went as long as 45 minutes and delivery of nourishment and meds were delayed well past acceptable times. Hopefully, these problems were addressed after my loved one left in an ambulance to have his GI tube replaced at the hospital. We did not return. Don't let the beauty of the lobby deceive you. Ask about response time and nurse training before considering this place. read more

This place is awful. My Mother was there for five days and it was a constant battle to have her cared for. The nurses that work there during the week do a passable job but, the people tasked with caring for our loved ones on the weekend could care less about them. My mother was in this facility one weekend and that was one too many. She was not given her medication on time and sometimes not at all. Called to and spoke to her nurse and she became hostile. Only a call from her doctor resulted in her getting her medication. No controls for the bed and no call button to request help should she need it. I spoke to the nurse who was supposed to be caring for her at 9am and she hadn't even been in the room to check on her and she came on duty at 7am. When my mother first arrived at the center, she did not possess many of her physical faculties due to a stroke. Through your diligent work, she continues to see daily improvement physically and in her spirit. Now, she can talk, move both hands and walk slowly with help. We definitely feel a part of the collaborative team in my Mother's healing and it's the Therapist that have made that possible. I would like to thank Mr. Brawn (Administrator), Mr. Michael V. It's a nice new nursing facility. Doctors are knowledgeable and along with other staff members they take good care of the patients.

When my dad was discharged from the hospital to go into Houston Transitional Care (HTC) for rehab/physical therapy, he was able to walk about 100 feet with minimal assistance using a hemiwalker down the hospital hallway and back. After two days at HTC, he had declined to maximum assistance with two therapists, and he could not move his right leg by himself and walked only about 15 feet. The staff at HTC did not get him better; instead he grew weaker and weaker by the day. They kept him in bed all hours except during his therapy time. They left him unattended and he was eating his meals alone in his bed, with the bed not in the upright position. He had a Foley catheter from the hospital and I was told they would remove it, do bladder training and monitor his urine output. It took them 10 days to remove the catheter (at my asking the nurse practioner about it) and then they did not do any monitoring or bladder training. I finally got him out of HTC and into a rehab hospital after 2 1/2 weeks of his poor treatment and rapid and steady decline. Less than 24 hours after he left HTC, the other facility's labwork indicated my dad was in kidney failure and septic. Because HTC did nothing about making sure he was emptying his bladder, it was discovered in the ER that he was retaining urine and was admitted back into the hospital for another 5 days. If my dad would have stayed at HTC, we'd be going to see his grave at the cemetery today. By God's grace, he got out of HTC when he did and got proper medical treatment. The HTC doctor never contacted me about my dad's care. The phone in my dad's room did not work. Sometimes when I called I was able to speak with someone who would help me communicate with my dad but other times no one would answer the phone and I got the voice menu. One of the office workers hung up on me. When I expressed my concerns that my dad was getting worse instead of better, the director told me I was ignorant and didn't know what I was talking about. Do NOT trust your loved one to HTC. It, as is most skilled nursing facilities, is a horrible place with awful medical care. If I could give this place negative stars I would. If you or a loved one needs physical therapy, you're better off getting home health care. read more
