This was my first experience for my father here and it was beyond disappointing. The work ethic is terrible. Most of the time the employees are on their phones which should not be allowed except on break. The play pass the buck no one takes blame for nothing being done. Never brushed dads teeth, changed him when they felt like it wearing others clothes sitting in a dirty pullup with the door closed. How long was he sitting in pooped pants. He has dementia and does hassle you but you give up so easy to not take him to therapy so he may be able to walk again after his fall in April Do you bill medicare for therapy? I believe you did whether he got it or not. Did I not request finger food for him and he would have soup, mashed potatoes etc and make a huge mess. Did they ever clean the rooms they we so ugly and dirty. The more drugs they can give them to make the Zombies and bill medicare for, the better for them. Sarah was a caring nurse and he had a nice nurse thats name started with a T that understood dementia but for the most part no one cared!!!!!! I was lied to a few times there which made me very upset. I learned real quick it was all about the $$$$$$ not the care of the patient or concerns of the family. They could wait to get him out of there because he wasnt the perfect bed ridden patient. I wanted him out of his be each day. Maybe it was me they wanted to get rid of because I expected way better care than he got and I always was asking why things werent done to help him and treat him like they cared. I'm mom never got a meal on time and they never could get the she eat reg food with no restrictions they never checked on her this place is a true disappointment can't wait to free her of this horrible experience. But they would give her several huge pills made her take them at once. read more

The physical therapy department is excellent but there is a communication gap between staff. I know that 2 of their aides are excellent: Alexia and Laura. My sister was in the facility and I communicated with staff every day. I would speak with a member of the medical staff but then other staff would not follow the recommendation. There was constant miscommunication with the kitchen staff and I heard other staff members complaining there was a shortage and lack of training with kitchen members. The administration was also a problem. They reached out to me and encouraged my sister to come to their facility. Once she was placed, I asked to see Mariann Gornal but she was too busy and told the receptionist to find out what I wanted. The social worker contradicted information I was told multiple times. She did not even know my sister's diagnosis and I had discussed it with medical staff. Have they heard of documenting? That information should be available to staff members. If you're going for physical therapy and it's a short stay, you'll survive. But if your loved one has a serious condition, you may want to look elsewhere. If they ever improve communication between staff and between departments then they could be an excellent facility. read more

I just spent 5 weeks there where they restored my mobility and physical strength and health.I was admitted there after 7 weeks of total immobility due to a severe case of pneumonia where I was flat on my back and became a total invalid, could not,lift my head,roll over or sit-up and had to be Hoyer lifted in and out of bed.The nursing care and physical therapy are top notch and second to none, in 5 weeks I was discharged using a walker, no longer on supplemental oxygen and gaining my strength and stamina while losing 49 lads of muscle prior to admission.I highly recommend this facility and faculty for recovery from any debilitating situation. read more

CNAs shutting off call lights, being rude, ignoring pleas for help. If you can't be there daily, don't do it.

They did a great job with PT and OT. My mom was able to get strong enough to go back to her home. Twice a day therapy and riding the exercise bike daily as well. The nurses were great but, probably overwhelmed. We just figured they were short staffed. The food service was not good. Over almost 2 weeks maybe 2 meals were warm. Just so poor considering the kitchen was 100 feet from the room. read more

This place is NOT #1. It changed names from Symphony to Ignite. The best thing they could do with this building is IGNITE it on fire. The place is terrible. My mom was there less than 24 hours before I got her discharged. The bed was waaay to small for her, the only light in the room that worked was a small table lamp, we killed 3 bugs in her room, saw one near the kitchen while we were walking to her room, the staff doesn't answer call lights in a timely matter (more like 45 mins) the walls are banged up. The place has a horrible odor. I wouldn't ever step foot back in this place. The only staff that care is a nurse Vickie and a nurses aid Emily M. I told them both they are too good to work at this dump and need to explore their options where they'd be more appreciated. I'd never recommend this place. Zero stars read more

Ignite was great in the beginning. My dad spent 2 weeks in rehab. There were hiccups but I overlooked them because they were short staffed. He moved to assisted living. I have weeks of texts from myself and the director Marjorie asking to please provide food my dad can eat. She swore it would be taken care of but it wasn't. All he wanted was grilled cheese and soup. I had to buy him food because they wouldn't give him food he would eat. Can't get rid of the fruit flies or gnats or whatever they are. Room is not cleaned. We brought silverware from home and put it in a drawer for him and it was taken. His room phone has never worked. So he's been in hospice for the last month or so. They threw his denture away. Sure it was accidental but they won't do anything about it. They said because he's in assisted living that they are not responsible. My dad can't even lift himself to put his teeth on the tray table but they said that's what he did. Now that he has no teeth...NOW they bring him grilled cheese!! Which he cannot eat. So I had to buy him oatmeal and soup and things he can eat. This man has lived his entire life being good to others. Hes a veteran, a Legionnaire, and a Mason. And this is how he is treated. I have many more texts about the care he didn't receive. I always believed they were trying and I'm not the demanding sort of person. There are a few really special people...like Arnie and Miley and Jasmine... they really try their best. Sadly, this facility puts money over doing the right thing when they make a mistake. Staff at Ignite Crown Point would not go in a private room to discuss neglect issues which is a violation of HIPAA rules. My father missed multiple Doctor appointments. He was left with poop in his pants for hours and did not get his body cleaned or a shower causing him to get sores all over his body. The bedding sheets, pillow cases and floor where dirty. I do not recommend having your loved ones at Ignite Crown Point for care because it is not there and staff rig the records to cover up their neglect. read more

I just have to say this place is the absolute worst!! My elderly neighbor had to go here for rehab after the hospital and let me tell you what a joke!! He had been having problems breathing and he called them in to the room he was told take a drink of water seriously what the heck!! And he has now passed away and they called his phone they asked if they could speak to him I said I'm sorry he has passed the lady goes oh well I wanted to do a survey like are you serious right now what kind of B's is that not hey I'm sorry for your loss BUT WANTED TO DO A SURVEY!!!!!! I do not recommend this place at all such disrespect!! So if you have family don't take them there because this is not the first person they have done this to!!! read more

This place by far is the worst and nobody should ever take their loved ones here. They are also extremely shady. My mother was discharged March 5, 2025, and they are still reporting to Medicare that my mother is an in-patient resident. I have called numerous times to get this fixed, and I get nothing but the run around and have been told that Amy will call me back, which she never has done. Call lights stay on for hours at a time with not one person coming in to check on the patient. I asked my sister to have the Nurse Practitioner, Sandy to call me and she said to my sister "why", to which she never called me and when I did meet her, I told not to do something and just went right ahead and did what she wanted. The main doctor is beyond ignorant when I tried to speak to him regarding the NP and his reply was "are you a doctor", beyond unprofessional and he sounded like he shouldn't even have a medical license. There so many other things that are wrong with this place. I have filed numerous complaints with the Indiana Department of Health and now, I will be filing a lawsuit against them. I wouldn't take my dying dog to this place. Absolutely, the worst place to be. read more

Never again!! No communication between departments. Call lights sometimes up to an hour. Imagine sitting in the bathroom for one hour! Food service terrible(wrong meals, or no meals because they didn't know she was still there.)Poor discharge planning, didn't have the equipment needed when discharged home.Left messages on main telephone number several times with no callbacks. Was told there wasn't a code to retrieve. Basically came in around 5 o'clock at night and said you're denied and need to leave tomorrow. Tried to find out who denied the appeal by a third-party,Lavinta and I was told the doctor was anonymous( if it was an actual Doctor who did the review and determined she could go home) going off notes from the prior week before she left. No proper preparation or explanation to family until I spoke to the administrator. Very poorly run. Highly recommend not putting your loved one in that facility. Seemed very short staffed. Your loved one needs an advocate for sure if they end up there. Also had to self-pay for one day as she was going out for a doctors appointment that day and there was no time for preparation for the transition to go home. Though the nurse practitioner was helpful in answering our questions. I also noticed employees giving positive reviews since this was published. My review is exactly what is happening there. The staff and facility itself make you feel like you are at home. There is always someone who is willing to help and give you answers that you need! The nurses are very responsive and have effective communication to family members - they truly want the best for their residents & will help/encourage them to push themselves. The facility is kept clean and there are many activities going on that help residents get involved & up and moving around. There are different choices for everyone when it comes to breakfast, lunch, & dinner and the kitchen staff are more than happy to welcome guests to eat with you. I would highly recommend this facility for your loved ones if you want the best of the best care. read more

This is an update on my Mom's progress at the rehab facility. Since my last update, my mom could not lift up her bottom without help. She could stand with help, but she could walk once she got up with the walkers. She can now get up with supervision on the side of her bed. NO MORE HOYER!!! She can move from bed to chair with help. She was also EXTREMELY excited about going to the bathroom in the bathroom, although she needed help. She was SO HAPPY!! She has gone from dreading therapy, to looking for her therapy team, although she gives them a hard time she is very grateful. Michelle, Ashley, Tracy, and Rick, they all have their training strategies and techniques, but they all have helped her to regain quite a few of her abilities!! Then there's Jada, Maddy, and Alexis. They are wonderful. I also want to thank the midnight shift. I am not there, but I hear alot about you all as well. But my Mom is not good at remembering names. Thank you all though! I love the nursing staff as well, Ms. Tracy and the other two nurses. They continue to make sure she is comfortable and continuously communicates concerns to Dr. Sandy. My Mom's health has for sure done a 360!! Then there is Dr. Stone and the wound team, Colleen and Nurse Emily. They are awesome. Her wounds look beautiful if wounds can look beautiful! Ignite has been ABSOLUTELY AMAZING in regards to her care. The only problems that exist is the frustrating process of insurance companies. We are prayerful that Mom will be able to continue rehab. She is almost there, although she only has a few days left that Medicare will pay for. The simple things we take for granted, like putting on clothes, getting out of bed, showering, using the toilet, bending standing, walking, can be taken from us in the blink of an eye. For facilities like this to be around for people is a God send. As with any business, there are issues, and I'll be honest. The issues I've seen are all due to limited staff, which sometimes cannot be calculated! Employees have lives, and sometimes, life be life-ING, and we can't control those things. There are a lot of good things, and they definitely outweigh the bad!!! Even the janitorial staff, the new young lady, the gentleman, and kitchen staff, Ms. Hershey, the two gentleman, and Ms. Vicky go the extra! Can't forget Emma, the lady on the weekend, Mariann for making this all happen, Amy, Shelby, and Ms. Gina, they're a wonderful team!! Oh last thing, thank you Vick for your kind words about my Mom.
