    My Dad was a short-term stay resident in the rehab facility after a hospital stay. As soon as he…read morearrived there, I was received an email and a phone call to discuss his needs. The email had phone numbers for the front desk, the nurses station, and his social worker. The nurse gave me her cell phone number right away and was texting with me through the first day and evening to be sure everything was set up and to reassure a worried daughter. Everyone I came into contact with at Park Place was kind, helpful and caring. The daily COVID email updates as well as follow up phone calls regarding any cases were very appreciated. All safety protocols were followed and I felt confident that Park Place was doing their best to keep all of the residents safe and healthy. Dad was well taken care of and he was always very happy on the phone and during an outside visit. His therapy went very well and I was informed about his progress weekly. Everyone, including the director, nurse and social worker were very prompt to reply to any email as well as any voicemail. Upon his discharge, I was given detailed discharge information as well as all necessary medications and prescriptions. Park Place also calls the caregiver for a few days after discharge to check on his progress at his new living arrangement. I highly recommend Park Place.

I had a major spine surgery and was not able to get out of bed for at least two days. When I needed to use the bathroom, I turned on the nurse's alarm and no one came and 30 minutes passed. And I ended up urinating in the bed at three different occasions due to no one coming to help me. I just feel as an adult, and you're urinating in your bed that it is just a disgusting feeling. That should have never happened!!!! I gave them plenty of time to come and help me. At one time it was a forty five minute wait, and still no one came. I ended up urinating in my underwear and in my clothes, and so I had to put those in a bag for someone to help me with my laundry. Well that's another story, after a week of being there, I had a full bag of laundry. Every time I asked one of the aides, is anyone going pick up my laundry and they said, let me check and they never came back. So I had to leave my laundry sit in my room. Finally, I ended up giving it to an aid tossing it out into the hallway to her and asking her to take that to laundry. I had my name and my room number written huge, in black marker on both sides of the white bag.Well, needless to say, my laundry got lost!!! At that point I only had one pair of underwear left because I had limited amounts of clothes there. I had nothing to put on because everything was dirty. I tried my best to find out where my clothes were. And why they weren't coming back and went through many channels of talking to people. And nobody was able to help me or find my laundry when I finally went home after 2 days of being home. I called and talked to the administrator and inquired about my lost laundry bag. She said she would look into it. I did not hear from them. After another day, I called back .And they said, oh, we found your laundry. Well, I was surprised, and I asked them to please send it back to me and they insisted I should come and pick it up.Well, this is an hour away from my home.And I still wasn't driving due to my surgery and I told them I cannot. After practically begging them, two days later they sent someone to drop it off to my doorway. The worst part of the stay that there was... I was an extreme pain. I knew the pain medicine had to be given to me by a nurse, not by an aide. I knew I had a wait till specific time to get my medicine. So when that time came, I pulled the buzzer and the aid came about a half hour later, and I asked her for my medicine, and she said she'll get the nurse well about a half hour after that, the nurse brought me my medicine. So by then, it was an hour later. There were times that so much time passed that I was in tears.So I wobbled down to the end of the hall at nurses station three times during my stay and ask them for my medicine.Because no one came to answer my calls. When I got to the nurse station, they all looked at me like I was nuts.Why am I in the hall. I explained that i've been trying to get someone for over an hour. But when I got to the nurse station, I realized they were all just standing there talking.One person was on the computer. There's something very wrong with that picture!! I do not recommend this place at all! There are so many other things that had happened. I could not wait to get out of there! For any older people that are not understanding the way they are being taken care of and not able to speak up. But to just lay there and hope that they get help, that's bad. I would hope that a family member would be able to step in. Please do not go here.

    Why would anybody rate a nursing home 5 stars? you may ask. I think I can honestly do that. My mom…read morespent four years as a resident in the Alzheimer's unit, and from everything my aunt and I could see, she was safe, protected, and well-cared for. Finding a nursing home for an elderly dementia patient, especially when it's a woman on Medicaid, is not easy. I was literally having anxiety attacks, because so many places turned me down, either because of the Medicaid issue or because beds for women are scarce. When Sebo's said they had space available for my mom and could take her immediately, I did not check nursing home ratings, do an inspection, or anything. It was a bed. It was available. I placed mom there. Mom was in a dementia unit called Daisy Lane, and my review is only about that area and what happened there. I have no knowledge of the rehab side. My aunt, a long-time resident of Hobart, told me later that Sebo's hadn't had a good reputation in the past. Since that time, however, Sebo's has become employee-owned. We believe that made all the difference. You know all those pictures you see of dementia patients left sitting in wheelchairs in hallways, unattended, soiling themselves, all sorts of awful stuff? We never, ever saw that at Sebo's. Instead, the staff works tirelessly to get patients moving, even a little. I saw activity sessions at Sebo's led by an enthusiastic young woman, playing with and encouraging patients to participate, even a little. There is an activity board that lists everything going on, and there is a lot. Have you ever walked into a nursing home or rehab center and been greeted by some awful smell? That never happened at Sebo's. It's always spotless. And we dropped in at all sorts of times. I think my mom was a staff favorite, because she was the happiest nut job you ever saw. She was not paranoid, hostile, or abusive. I did see patients who were very difficult, and I saw the staff respond in appropriate ways. How the staff handled my mom's aversion to the shower demonstrates how clever and compassionate they are. At first, Mom saw the shower sign, and she'd turn around and say, "I already had one." So the staff covered the shower sign with a towel, and they were able to get her in. Sebo's will notify you of the tiniest change. Mom has a bruise on her arm. Mom has a rash. They do call at all hours. I had a message once at 1:14am. Trust me, I'm not answering my phone then. But it shows that nothing gets past them. At one point, Mom had a skin rash. Sebo's called and told me about how they were treating it. They asked me to purchase bath soap for sensitive skin for her, which I did. Apparently the rash was bothering her on the bra line. I got a call from Sebo's: "Miss Frances has informed us she won't be wearing bras any more, so could you get her some t-shirts?" I did, and all was well. One time, Mom fell and actually got a cut on her forehead and a black eye. The nurse, Stephanie, called me, because she was the staffer who responded when Mom's roommate came over and said my mom was laying on the floor for some reason. Stephanie and my mom had become great friends, and she was very worried that mom had been hurt. She called me and gave me a complete run-down of what happened, what she did, what mom said, and how they were responding. I got follow-up calls over the next few days. When mom went into her final decline, they updated me with condition changes, and talked to me about hospice. Each step of the way was handled with professionalism and compassion. Mom was very lucky that Sebo's had a bed available that day, because, frankly, I would've taken anything at that point. And I was lucky, too, because I never had to worry about what was happening there (I live in Illinois). As the head of the Alzheimer's unit helped me carry out mom's few possessions after her death, I said something about, there's probably another old lady waiting for this space. He said, yes, we have a waiting list. I can believe it. I highly recommend Sebo's for dementia care.

    I knew I came to the wrong place when they took X rays of my hip considering I was there for my…read moreknee. It seems they take X rays before you see a doctor. Yes I have problems with my hip but nowhere near as severe as my knee. The X ray tech only offered an unstable support device on my left side pushing me to the right. The device had 4 legs with a tall handle with two legs on the floor and two elevated on part of the x ray machine. I was in excruciating pain try to stay on my feet to prevent falling to my right. The support device should have been on my right where the bad knee is sitting on a level surface. Next I'm called into a room by a nurse who also seemed more concerned about my hip. My knee has been hurting for 3 years with the hip starting to give pain about 7 weeks ago but with mild pain. My right knee will no longer hold my full body weight. I asked about treatment for my knee but was told we're not dealing with that today. Knowing this wasn't the right course to get the ability to walk again and the dangers in X ray it was time to leave. didn't get to see a doctor yet I'm being billed for a missed appointment which I have no intention of paying since none were available for the primary reason of being there. The various puddles of urine at different levels of evaporation was a sign before X ray. There had been urine on the floor for hours. Hopefully I won't have to wait another 3 weeks to see a professional.

    So my Mom has been at Brickyard for five days now. Wednesday, my Mom called me in horrible pain in…read moreher knees. It had been a long time since I'd heard her cry from pain. So I emailed Jemai, and did my best to explain to her my Mom in regards to her pain management. I know as a Director of Admissions, I would have been like who the heck does she think she is! Just kidding! But seriously, when you brimg your loved ones to a place you know nothing about, you're VERY nervous. It's understood that people have to become acclimated to systems, but sometimes there's a right away thing that comes about. And with our Mom, that's about what it is. Everything else, she's cooler than a fan. But Jemai acted right away! She emailed me back with the Director of Nursing, Kathy, a part of the email letting me know she passed along my concerns to the appropriate party. Then Kathy emailed me directly addressing my concerns with solutions!! Then Rhyle, the unit manager, responded introducing himself, letting me know to reach out with any concerns. Now I'll say this much in comparison to Ignite Medical Resorts, which I still love, Brickyard is A1 in response time in email form, which I appreciate, because I am an email kind of girl!!! Also have to thank Nurse Yasmine for getting the ball rolling, getting control of Mom's pain. She even got her setup with an orthopedist for her knee pain. So appreciative! And I met Jasmine, whom my Mom loves!! I asked about laundry, and she took Mom's clothes right down. Hope they make it back. Lol TBC She also talks of how much she helps her, along with TaNiyah. When you find genuine caring people, it means the world. Then there is therapy, which my Mom talks about alot. She even has good nurses on the midnight. Thank you to the midnight shift. She doesn't remember names well. Lol She talked about how much she's getting up and down in therapy. Will admit, haven't seen it yet, but says she be wore out!! Lol That's a good thing. So thank you LaTisha, Sue, and whoever Rick?, hope that's right. She talks about them often, with a smile. Thank you to Ms. Bianca, the Unit C manager. She made sure to ask what Mom needed, especially a walker, which is what we said. She said she'd get it ordered, with a list of other things. Everyone has been so helpful. ADMIN AND UNIT C ROCKS! It is so appreciated. The things you hear are sometimes often misconstrued!!

Covid lockdown in IL in 2020 blocked…read moreall local physical therapy (hereafter 'PT') options so I've driven 100+ miles round trip 1x/week for the past 6 months to Crown Point. Dr. Art is phenomenal - he quickly, accurately diagnosed underlying cause of my shoulder pain, explained it and then prescribed a relatively simple PT cure - stretching & progressive strengthening exercises. It has produced amazing results - my shoulder pain is virtually gone and as an added bonus my upper arms look great! Next my wife strained her lower back 2 mos. ago so I brought her with. Dr. Art diagnosed and remedied her back pain. But much more importantly he convinced my wife she might be able to overcome her balance issues (resulting from suffering a stroke 17 yrs ago) & possibly eliminate her reliance on a cane if she undertook a PT exercise program (again stretching & progressive strengthening exercises). In 8 weeks she's made great progress so we're very optimistic! Finally Dr. Art's been considerate & flexible re our stingy insurance provider - since they won't cover our PT sessions we pay Dr. Art directly. He charges us less than the trainer at our health club would charge while delivering exceptional results! We greatly appreciate Dr. Art's help! If you have an issue that could be helped by physical therapy I again highly, highly recommend seeking treatment from Dr. Art!

This place gave a very convincing tour…read moreof a well ran facility full of light, love & activities to create a home like environment. None of which was true. Ice cream parlor, hair salon, movie theater.... In 2 months those doors were NEVER open until the day we were removing our loved one. Convenient right? The amount of neglect, mental/physical abuse & disregard for the well being of the elderly they are employed to protect is despicable. The nursing staff was unorganized & unprofessional. The administration & leadership staff was unhelpful & dishonest. And genuinely behaved bothered & irritated when asked for assistance. Our family member went in for small fixable issues & came out no longer able to walk, only using diapers, dehydrated, bed sores & a full body rash.... All of which were NEVER an issue before. Left to sit in their own waste for an obscene amount of hours to the point of straining a large area of the skin, cold showers (rare if at all) & sanitation of the room was very poor. IF the necessary medicine/treatment was being given is unsure, but doubtful given the declined condition of health upon leaving this facility. Director & social workers close their office doors on you once they realize you are not going to tolerate their BS. A member of the admissions staff even said.. and I quote "I wouldn't even put my own mother here & I only work here cuz they offered me more money." THAT right there should of been enough of a red flag. Day of departure... Staff members stood nearby gawking as we struggled to help our family member into the car. We asked for help & got told they were "on break"... But they kept watching. If you love your family members, do not place them here. They may not be so lucky.

In my very own…read moreopinion from my experience this past month, this facility should definitely be evaluated/inspected by state agencies. The care and services I witnessed were horrendous. I am an out of town grandson of a recent resident of this facility. My family member suffers from Alzheimer's and Dementia. She has also had mobility issues and was sent here, recommended by insurance company for REHABILITATION. Rehab DID NOT happen. In fact, my family member worsened while in the care of Dyer Nursing and Rehab. Here is what I witnessed in just a few days: - Poor security. While they did have someone at the front concerned about checking temperatures, this person did not guide guests to the residents room, just pointed in the direction. I wandered the facility looking for my family member's room. Each area had it's own nurses station with plenty of staff around. Not one of them greeted me or even asked what I was doing or who I was there to see. I could have easily wandered into any residents room. Eventually I found my family member. - Horrendous behavior from staff. While I was in the room with my family member I could hear an employee (who was was in scrubs, in the hallway) cursing left and right. "Mother F*** this and that, F*** so and so etc." This was not just one sentence. This was on and on and very loud. I walked to the hallway to see who she was speaking to like that, and it appeared she had an ear piece in and was talking to someone on the phone. Meanwhile, my family member as well as other residents and guest could hear this nasty language clear and loudly from their rooms. This made my family member very anxious. - EVERY time I visited my family member, she was soiled. This told me they were not routinely checking/changing her. - At one point a nurse came in the room and handed my family member a cup of pills and said, "time to take your medicine." Didn't greet me or my family member. I asked a question about my family member's mobility and capabilities as I was visiting and I wanted to know her progress. The nurse responded in an extremely rude manner saying, "Don't you know who she is?" "Don't you know her capablities?!" I replied, "Yes, she is my grandmother, but I am visiting from out of state and want to know her restrictions with mobility." The nurse didn't say a word. Walked to her chart, turned around and said, "SHE'S A ONE ASSIST." I reply, "I'm sorry ma'am, what does that mean?" This aggrevated her and she replied, "IT MEANS SHE NEEDS ONE PERSON TO ASSIST HER." That was the end of that conversation. - While visiting, I heard a resident across the hall yelling, "NURSE... NURSE... I need a bed pan." She yelled this for 20 minutes. Bottom line is the staff does not show their residents basic dignity, care, or respect. - My family member was only here for a few weeks and left in worse shape than she arrived. Bruises all over her body. - I witnessed lunch/dinner being dropped off. It was delivered COLD. Cold meat, cold vegetable and cold coffee. - My family member would be in the same attire for days in a row. She was only changed and cleaned up because I did so. If you have a loved one, please look for other facilities to put them in. From my first hand experience, this place is a MESS! The lack of BASIC care and respect for the patients/residents is mind blowing. Extremely unprofessional. I'm grateful my family member was moved to another facility in hopes for better care. I DO NOT RECOMMEND! I highly encourage county and state agencies to investigate the ongoings of this nursing/rehab operation. - A concerned family member.

    This is not a family-friendly facility. My 14 year old tried to come with me as a guest to try it…read moreout (my 16 year old had come once). They said the 14 year old had to buy a package for kids which was $30 for 10 visits (no smaller packages just to try it out). When deciding on it when she was already with me in her workout clothes, we told the worker that she was going to sometimes come with her friend's family, so we would get it. Nothing was said about any rules against this. A month after that, I decided to cancel my membership because of several reasons (class times not convenient for a teacher. Rigid rules about teen memberships, etc). When I canceled, they called to tell me Tessa's membership would also be canceled since she has to come with me. I told them my understanding was that she just had to be with an adult. They would not budge and told me absolutely no refunds for any reason. This is very poor business. Also, their policies are so rigid...a 14-15 year old must work out with a parent. I don't know many teens that want to be glued to their parent's side when working out. Other gyms have much more flexibility. Other businesses would be more flexible with miscommunications. I'm glad we are ending our membership there.

No mouthwash in the locker room any longer! We didn't forget! Fitness Pointe did! Poor leadership that would rather blame someone for complaining about a problem person, in hopes one of them would cancel their membership so Mr Coward manager didn't have to take a stand. Missing leadership! Arrogant! Excellent at being pompous and arrogant! Then there's the therapy pool piranha and the school of disgruntled, mean followers that believe this is their own personal warm water swamp. The pool piranha feverishly splashes about from side to side hogging up three lanes is a message to everyone...I OWN THIS POOL! I COME FIRST! You must kiss the ass of the selfish pool piranha and follow it around, back and forth like a good little obedient fan. No respect for others! You can't enjoy the therapy pool alone because the pool piranha lacks attention and needs your constant validation. Then there's the employee with the strawberry blonde brush haircut that hates everyone including itself. Mean, belligerent and can easily grimace, flip you the finger or tell you to SHUT UP! Excuse me....Is one of it's tired, dated lines. When management lacks the balls to use their authority properly, you end up with sacred cow staff members, laziness and a few angered members calling the shots over the majority. Fitness Pointe isn't the best place to work out or enjoy your leisure. DON'T WASTE YOUR TIME HERE! Not worth the aggravation!
