He's only at this location one day a week, I think, and they use another doctor's office/optometrist (?) facility. There's really nothing that tells you he's there and when you're standing in line to sign in, there's a sign for a dermatology office and another for St. Clair Eye. We had to ask, "Are we in the right place?" So they took us back to a room within a few minutes - less than 30 minutes in the waiting area - and then... more waiting. We ended up waiting almost 2 hours and then finally someone came in and they admitted they totally forgot we were there. Ok, fine. They owned up to it and apologized, it's ok - we can move on from that. Dr. Mallempati was great, very thorough, and great bedside manner. He schedules an MRI and wanted us to come back after for a follow up. MRI was done and we return one week later. This time we saw him in his main location and thankfully, wait time was much better. His nurse practitioner saw us first and she was very thorough, but she interrupted us when we tried to tell her something and we had to make her listen. We saw Dr. Mallempati again, and he was great. He wanted us to schedule a nerve block which would be performed at Surgical Institute of Alabama. We immediately scheduled the nerve block for Dec 23rd, 2025 to be done at Surgical Institute, BUT they (Surgical Institute) would call us the day prior to confirm the time. No food or drink the day of the nerve block (prior to). Also, a 3 week follow up post-nerve block would be scheduled with Mallempati. December 22nd comes and by 3:30 p.m., we still had not received confirmation. We call Surgical Institute and the lady who answered the phone said, "What's your name? Oh yes, we have you down for tomorrow and they WILL be confirming this, but we're just really backed up. But you will hear something." We go to bed at 9 pm, still no word. December 23rd comes and we've taken vacation from work and are sitting here, waiting to hear. At 7:30 a.m., I call Surgical Institute and the lady who answered said rather rudely that that she had no idea what we were talking about, no appointment was scheduled for us. We repeated the story about how someone at her office said we WERE on schedule, and she kept repeating, "You need to call Mallempati's office - no one from there has ever sent us anything to do with your information." I called Tracy, Dr. Mallempati's nurse, and left a voice mail to PLEASE call us ASAP. By 9:30 a.m., we gave up hope and decided to eat/drink since obviously the nerve block wasn't happening. It's April 19th, 2026 and Tracy nor anyone else at OrthoAlabama Spine has yet to call us back. We've been completely ghosted by this facility, as if we were never there. We've told everyone we know to avoid this place - which is sad, because Dr. Mallempati is great.

He saw I was pre-diabetic and asked when my next diabetes test was because the Cortizone shot would produce a sugar spike. He even told me that a bone that I broken 2 years ago had healed. He was also a good communicator, showed compassion, calm demeanor. He treated me like a person and when he injected my hips, I felt almost nothing. All other bursa hip injections that I have had have hurt immensely (APP, UAB higlands ground floor). Either medication has changed since then or he is a highly skilled Doctor who can inject without even hurting you. Amazing! I would have liked the receptionist to be much more polite and speak more clearly. She seemed impatient. I don't think it was because the practice was busy, when I got there an hour later it was empty, but who knows. The waiting room was comfortable, clean, cool with cold water bottles to drink! The trainers at her high school recommended an x-ray so we brought her here. Located in the relatively new and very nice and eclectic "Brocks Gap", OS1 is in between a ballet studio and Moe's BBQ. Also in the same strip are The Trak Shak, Whole Scoop Ice Cream Shop, Santos Coffee and Tre Luna Bar & Kitchen. A very small front door leads to a modest and simple waiting room with a complimentary Keurig and canned sodas. There's a small counter with a receptionist. OS1 calls themselves a Sports Injury Clinic and indeed they are. They have trained sports medicine specialists on site. They have convenient hours closing as late as 7. They claim to be less expensive than the ER with decreased wait times. Finally, they can take X-rays and have MRI equipment on-site! We arrived with our injured daughter about 40 minutes before closing. They normally don't see new appointments past 6, but we were greeted with a friendly face and after checking w/ the medical staff, they immediately took Sarah "to the back" while Tammy filled out the paperwork. In about 10 minutes, we were back in the exam room where Dr. Beidleman was already examining her ankle. From start to finish, we were walking out of OS1 in about 30 minutes. Dr. Beidleman was kind, sincere, sympathetic and most of all professional. His nurse who came in to help show Sarah how to use her fancy boot was also fantastic. Everyone who was there was extremely friendly to the point that it kind of creeped me out... in a good way! So having said all that, I need to add that at 7pm, the NCAA National Championship game Georgia vs Alabama was going to be broadcasted and their entire staff might have been trying to get us out as quickly as possible. Also, since it was the last hour before close, there wasn't any other patient there so we were their center of attention. Would our experience be any different if the NC game wasn't coming on at 7 or if we were there in the middle of the day with 5 other patients?!? I don't know. But I do know that this experience was exceptional and we will be returning. In fact, we have a followup appointment in a week. I'll write an update...

The science of Ketamine Infusion makes too much sense for insurance companies to my help pay for such a thing. They would much rather pay for pain medicine. He was the only doctor in the Birmingham area who was willing to refill my implanted pain pump. Before finding him, I had to drive to Slidell, LA every month to have it filled. He and his staff work together as a team. It is comforting to see teamwork in one's medical care. They are also very good at what they do, and each one plays a part in your care. I have suffered from hip and thigh pain since I had a defective hip replaced with a non-defective one. I thought that the pain was caused by all the surgery. Thank goodness Dr. Varley had other impressions. He discovered that the pain was emanating from my spine and that Radio Frequency Ablation (RFA) was the solution. I had been having it performed on the other side of my spine for several years, with good results. Walking without pain is truly amazing, after suffering with it for so many years. I highly recommend Southern Pain Specialists for anyone with back/spine problems.

She is knowledgeable, professional, and someone I have felt confident following medically. However, over the last couple of years, my annual appointments have consistently been with a physician assistant rather than with Dr. Leong. Unfortunately, my experiences with this provider have been disappointing. I have felt talked over during visits, not fully listened to, and at times treated as though my concerns were an inconvenience. I raised these concerns previously and was told I would not have to see this provider again, yet at my most recent visit I was scheduled with her again. The most concerning experience occurred when I became extremely ill. My wife called the office to request medication for nausea and was told that if we did not hear back, we should check the pharmacy. There was no follow-up call and no prescription sent. For a patient of more than two decades, that lack of communication and care felt dismissive and unacceptable. I still have respect for Dr. Leong, but the overall patient experience has declined. Long-term patients deserve to feel heard, respected, and cared for -- especially when acutely ill. I hope the practice and anyone thinking of joining takes this feedback seriously.

Doctors are excellent, but the support staff and billing process have been extremely frustrating…read more I received a "First and Final Notice" for a bill more than two years after my visit. Despite multiple attempts, I have been unable to reach the billing department. When I called the office for assistance, they were unable to help and simply transferred me back to billing, where no one answers the phone. To make matters worse, the note on the bill threatened collections. I attempted to pay the balance online, but the Phreesia payment system froze and would not complete the transaction. At this point, I have tried every available option: The billing department does not answer calls. The office staff cannot answer billing questions. Calls are routed back to billing with no resolution. The online payment system is not functioning. The physicians may provide good care, but the administrative and billing experience has been unacceptable. If patients cannot get answers to billing questions or make payments through the available channels, there are serious organizational issues that need to be addressed. Unfortunately, this experience has earned a well-deserved one-star review..
