The staff was really nice, supportive, comforting and helpful. They treated my grandparents really well and made my family feel welcome. They would bring us carts of beverages and snacks and they let my mom and aunt stay overnight with my grandma. The staff really seemed like they genuinely cared about us and helped make my grandma as comfortable as possible. They have an ice cream parlor and they allowed my whole family to get ice cream. And we all played bingo in the dining area with my grandpa. There is a sheet they hand out daily listing all the different activities and meals for the day. They have movie nights, bingo, singers, nail salon days and other things to do. There's also a barber shop where my grandpa was able to get his haircut. Overall, I was really impressed with all the staff and the cleanliness of the facility. The only thing I'd say that I can see needs an improvement are the rooms. They could use a fresh coat of paint, new carpet, shower tiles, little things like that. And we had trouble with the power on my grandmas bed, with being able to move it up and down. But I think this facility has what matters most, great caregivers, and I appreciate everyone for showing us all care and compassion during such a hard time.

Regional Ops Directors are not approachable or accessible either by voice mail messages, emails and texts. Wellness Director resigned and was walked out that day. No one to communicate with regarding loved one's care plan. 2. Shortage of staff in EVERY department including CNA's and Caregivers. Agency staff were brought in and many noted that they did not have access to care plans and were advised to "do the best they can". My mother was scheduled, per her care plan, for showers on Wednesday's and Saturdays at around 3. I would say in the 6 1/2 months she lived there she may have had 10 showers that someone actually came in her apartment on their own and "watched" her shower. The remaining times I had to have her press her pendant and await someone to respond to request her shower. Most every time we were met with "we are short-staffed" so not sure we can get your shower in today. I paid out of pocket a few times to have some give her a shower. Her care plan noted she was to be assisted in the shower with bathing and washing her hair. Never happened. Staff would sit on the closet toilet and "watch" her shower. Fecal matter was present on the shower seat. Never offered her deodorant, lotion or q-tips as noted in care plan. Came in once and she was sitting on her walker with a towel over her, they just left her alone. Thank goodness we came at that time. Her pendant was hanging up outside the restroom. 3. Call response time. Upon her arrival call times were about 5-10 minutes. Shortly thereafter the call times would range anywhere from 20 - to an hour. 4. My mother fell in her apartment on July 13th. She called me at 5:45pm to advise she was going to bed and headed to her restroom. At 6:40 I received a call from Carestaff that my mother had fell. After my mother was transported to the hospital and I spoke to Cardiologist, I notified the then Regional Exectutive Director of what transpired and that my mother would not be returning. NO ONE from Astoria called me to inquire what transpired or offer their well wishes. Sadly my mother ended up on Hospice and transitioned to her Heavenly Home a few days later. I notified Astoria and again , no one offered their condolences. I asked the Regional Director of Ops to advise what the call response time was at the time my mother fell and pressed her pendant and if the fall was reported to the state since she broke her elbow and heel. NO response other than "can we set up a time to meet or call you"? I was not in a position to engage with them at this time and again sent an email requesting information. NO response other than can we set up a time to talk. Please, read all the reviews. Do your due diligence and think long and hard before placing your loved one or yourself here. Clearly senior communities will have turnover, but when every department head is let go or resigns, that's a major red flag. Promises are made and never kept. They hold town hall meetings addressing concerns from residents, but nothing changes.

We received texts, calls, and remarkably quick responses--even on weekends--as they worked to find the right placeMent. Emily, in particular, advocated fiercely and compassionately, making sure our family member was placed somewhere she trusted. She thoughtfully considered the whole picture of his needs. The care, relationships, and sheer humanity of every person we encountered at Haven is unsurpassed. He was always clean, treated with dignity, and genuinely connected with the people who cared for him. The Haven Hospice Team didn't simply provide hospice services; they gave our family safe passage through an incredibly difficult time and remained deeply caring and present through my dad's final days. The staff at Elk Ridge is -- mostly -- very nice, particularly their on-staff nurse; however, just…read moreremember, they're still a business and their business is to make a profit. (I always felt like the truly helpful staff members were doing their best to make up for the staff members who were consistently less than helpful, notably the maintenance staff, the surly receptionist, and the operations director.) If you're in an apartment that's been renovated, you may enjoy living here. However if you're in an apartment that has NOT been renovated, go somewhere else. The prices are the same for the renovated apartments -- prices that predictably increase every year -- and the incompetent staff won't permanently fix anything that's broken in the non-renovated apartments, for example, a perpetually leaky faucet; a stained ceramic sink; a perpetually broken disposal; a refrigerator that produced no ice; and cheap vertical blinds that also often broke. These were all very basic things that greatly would have improved my parents' quality of life, but Elk Ridge refused to replace these items which, in the grand scheme of things, are relatively inexpensive. In the winter, they also inexplicably do not put down salt on the sidewalks -- it's Nebraska in winter...Putting down salt should be a daily SOP. While walking, I slipped and fell straight down on my shins and kneecaps -- several witnesses, thankfully, and thankfully I only sustained severe bruising that lasted ~three weeks. Elk Ridge is lucky I didn't break anything; I would have sued, no question. One of the residents told me she moved in, in circa 2019, and has watched the quality and level service decrease steadily, which is unfortunate. I got the sense she was strongly considering moving. Overall, I would not recommend. Live there at your own peril.

Mandy's expertise in both areas (neurological disorders and orthopedic rehabilitation) inspired total confidence in me and is largely responsible for me successfully walking today. I truly cannot thank Mandy and everyone at Horizon enough. They really help change lives for the better every day. And it is not just because they are friendly and engaging people (which they are), but because they also push each one of their clients to work their hardest and to get better as soon as possible. From personal articles coming up missing to two nurses in particular involved in an abuse situation that required an ombudsman and literally zero actions taken by the facility other than to cover their collective tails. I would take someone literally anywhere other than ambassador health. I am giving one star because of the two rock star nurses (they know who they are) and respiratory. The facility should be shut down. It is quite literally that bad.
