My mom was recently in this facility for six weeks at first I was super nervous because of the bad reviews. What I experienced was a group of kind loving staff. My mom was very well cared for. As you first walk in, you are welcomed by a spacious, well-maintained lobby. The facility is clean, and the environment smells fresh. A view through the glass doors reveals a beautiful fountain courtyard where recovering patients can walk, relax, and sit in covered areas. The Physical Therapy department has an outstanding reputation for helping patients regain balance, strength, and mobility. Progress notes and medical records are kept in thorough, detailed order. I was very impressed with the physician assigned to my care. One of the first things I look for is compassionate, caring, and helpful nurses and staff. I found this to be true in most cases. Daily activities are thoughtfully organized by an excellent activities director. Nothing is perfect, as some might say, but it is important to look at the whole picture and decide what matters most to you. My positive first impression began when I met Sherwin, the Admissions Director. He worked hard to coordinate everything and genuinely cared about my needs. His assistant, Stephanie, was also very supportive--together, they make a great team. I observed many staff members who truly cared. Dirty, smells bad upon walking in, low quality rooms, they don't seem to care about the patients there, they are forcing so many people into rooms, looks like a nightmare movie,people are left alone and I would never recommend bringing your family here. I was discharged to this facility after surgery for a broken hip. Upon arrival I was immediately struck by how shabby the facility was. I was plopped in a disgusting triple room. During "intake", I was informed that it wouldn't be until morning that the facility MD would "approve" my medications. WTF? By 3am, I was in a full blown cardiac incident. 911 was my rescue from this death camp. All the staff did was stare at me, rendering no assistance whatsoever, until the ambulance arrived to rescue me. WHAT A SHIT SHOW!!!! Under no circumstances go near this place. read more

Weekend receptionists have zero interest in welcoming visitors. They are always on their personal devises. You'd think a compassionate, caring soul would be there to greet visitors at a nursing facility, instead we are met by employees doing as little as possible while on the clock. My mother spent a little over a week recovering from an illness. The staff was amazing, from the occupational therapist, physical therapist, nurses & support staff. They were so great to work with. communication was key to my moms successful recovery. Anything from changing the times she received PT, extra condiments or assistance using the facilities, with solid communication and flexibility on both sides my mom was able to heal and be released a week before the expected timeframe. I was very skeptical because I had heard negative feedback in general about rehab facilities but I am no longer a skeptic, every day I witnessed employees going above and beyond for patients and residents of this facility. read more

I give this ratting for many reasons. One; the staff from top to bottom are the best of the best. Weather they were supervisory or staff. Two; they always made me feel comfortable in my situation. Three; They brought my meals, my meds and anything else that was required. Four; The time that I was required to spend in the rehab room was fun as well as rehabilitating. The crew in there were great. Five; I could go on for days with great cements, but I would be just redundant all the way around. So all I can say is thank you all for a super great job. My husband is dementia, currently stays there for recovering of 5 days. It is well known that nursing essential to recovering and health. During the five days, his bear was shaved, showered. And his gown, sheets were changed almost every day. They check my husband's diaper often, feeding him each meal, and much more... I like all the nurses are professionally, nicely, caring patience! This place has a very bad reputation in the medical and long term care community. It's horrendous when it comes to care, communication, food, and the overall place is falling apart. The furniture is rusted, broken and can be dangerous, dressers are broken with drawers hanging, broken trash cans, they use power strips which have multiple plugs in them which is a fire hazard. A lot of the residents appear to be homeless and they park their wheelchairs in the front of the building with trash piled on top of them covered with a tarp or trash bag. Nursing and administration does not communicate with family members. My mother inlaw was in this place and she never received a shower in over a month. People come out sicker than when they go in. I wouldn't send my worst enemy here!! I've seen positive reviews on this place and believe me they are probably done by someone that works there. The turnover is like a revolving door at this place also, and a lot of the nurses and aides lack training and communication. The overall feeling of the place is very unfriendly, no one acknowledges guest they just keep their back turned to everyone. The place also always smells like urine, very strong odors. they do not know what is going on with their patients. my mom was there june/july of 2024 and i was not informed they sent her to the hospital and in addition they sent her to the wrong hospital. then when i arrived to see her 2 days later i asked where my mom and they had a look on their faces that they had no idea where she was and was like oh whos your mom. and i was like wtf. then one of the nurses had the nerve to call the nurse that was on staff when she went to hospital. Heres the kicker, i called friday evening and no one ( the nurse who sent my mom) never siad anything about her not doing good and may have to go to hospital. and then they sent her 2 hours later after i called i had no missed called or voicemail. in additon, they had the nerve to send her to send her to the wrong hospital which it wasnt an emergancy so they could have taken to the correct hospital and then the hospital never called me either so i was so pissed. i have german in me and damn that temper surfaced so fast i was mad even her roommates she had there was like why didnt they tell you, sue the hell out of this place. in addition during the time she was there she said she didnt get to eat much and in the week and half she was there she lost like 12 pounds which wasnt good bc she needs help to eat and they wouldnt have anyone do it until everyone esle was serviced and by then she wasnt hungry and her food would be cold. i complained and they still did nothing to fix it. i would never recommand this facility to anyone and i hope hospitals stop sending people there because they dont care for the patients as they should. read more

I was only able to tolerate being there for one awful night. Called a taxi first thing in the morning to come get me. I went there after being in another, excellent facility following hip replacement surgery, so I had a basis of comparison. Going to the La Jolla Nursing and Rehabilitation Center was a big mistake. I'm amazed a facility this poorly run and this run-down is licensed. It shouldn't be. They put me in a room that faced the back alley and which had a curtain separating me from the other "inmate" who was probably praying to die. They took me to physical therapy which was only parallel bars at the end of a tiny office where other people were sitting at desks and talking on the phone. No separate rehab facility or supervised help. Then they made me take a shower sitting on a bench with an attendant spraying me with a hose. It felt demeaning and creepy. They claimed they had to do that to check for bruises. When they brought me supper, it was some sort of egg thing that had tiny balls of gray matter in it that I assumed was some sort of meat, but I have never seen gray meat before. For breakfast, there was another egg thing with the same tiny balls of gray matter in it. There was a small glass of some sort of brown liquid. When I asked what it was, I was told it was pineapple juice. I have never seen brown, pineapple juice before. Obviously, the food was not just inedible but probably expired. I had brought some protein bars so just subsisted on that. I had thought a place in La Jolla would be decent but it was horrendous. I decided I would be better off at home recovering where at least I could sit on a bath bench to bathe in private and have access to edible food. I couldn't get out of there fast enough. Horrible experience. If you or a loved one needs to go to a nursing or rehab facility, don't go to this place. It is a nightmare that shouldn't be in business. read more

I found the place to be fairly pleasant that the rooms had a lovely view the food was good the staff worked extremely hard they were kind and helpful. Slow response to phone calls in attempting to reach the case workers and physicians on duty. A relative has been a patient for the past 2 weeks. Physical therapy needed however has not been consistent. Have left several messages with no return calls from various levels of staff to update me on the status od my relative. With COVID lockdown, not the best place to visit. Patients in crowded rooms of 3 and no response to patient needs upon using the "call" button at patient bedsides. Although the food there could use use an upgrade!overall it was a very good Experience. 3 other different times times I was having to come is having to convalesce in a percin a facility and I assure you my experience At these other places was not a good one I only spent 1 night before calling my husband and begging him to come and take me home. It's poorly maintained e.g. holes in bedsheets and mattresses, the staff is nonexistent e.g. taking a nap at the front desk, playing on their phones, 3 workers standing in the pantry eating, etc. the list goes on. All I can say about this place is great things. My husband's aunt was admitted here and the staff was just so good to her she wouldn't stop bragging. They were Very kind and attentive. They were on top of everything Meds Meals. They made her feel comfortable and welcomed. I went with my kids once and they enjoyed the new courtyard they have out. My sister in law, just spent 2 weeks there, doing rehab, after surgery. Her first night there, she fell asleep, while on her tablet. When she woke up it was nowhere to be found. The only people who could have taken it were staff. When she asked them, the told her she "she was confused and didn't bring one". Only problem, she had come from the hospital and it was in her inventory. Shame on them! Her rehabilitation staff was wonderful, medical, not so much. I have just completed my stay at this wonderful facility! I was welcomed by an absolutely fabulous admissions coordinator Sherwin. The room was clean and I love the new curtains. My roommate was a very sweet lady. I would love to thank you the best cnas Asmeralda, Alexis, Mayra, and Stephanie they were very attentive my call light was answered promptly and in fact they anticipated my needs. The nurses were wonderful Leah and Julie were very sweet to me, Mark was always checking on me. I have to say the food was delicious and when I didn't like something on the menu my choices were honored. The Administrator even came to see me to check on me and my progress he is so nice!. The rehab team was wonderful they saw me daily and help me in my progress so much thank you team. I can count on you to make sure I receive the best care! read more

I would give zero stars if possible. My 88 year old father went to the ER due to a UTI which was making him weak and he couldn't walk well on his own, so he was transferred to LJ Nursing & Rehab. He had been there once before about 2 years ago and we weren't happy with the care he was given but this was the only place covered under his insurance that had a bed available. You see a lot of staff walking around but not many in the actual rooms and forget ever seeing or talking to an actual doctor. Some of the staff did seem to care but all in all the level of care is really poor. My elderly mother received a call on a Sunday saying my Dad was going to be released Monday morning. My mom, who was recovering from pneumonia and had also been hospitalized asked if he was able to walk and use the restroom on his own. The answer was "YES". None of us believed it because a family member went every day to see my Dad and we were told by staff that he can't walk on his own just the day before. Well, we had no choice but to bring him home Monday morning. We were all in shock when he arrived, looking in worse shape than when he arrived there. They told us he had a UTI and sent him home with antibiotics. A nurse from Scripps Health came Tuesday and was also in shock when she saw him. He had open bedsores, a rash and was too weak to walk. The bottom line is, they didn't rehab him. He came home in worse shape than when he arrived there and they lied about his condition. The administration is no help at all. Avoid this place if possible. read more

My mother was cared for by La Jolla Nursing house for 5 weeks. I was very fearful after reading the reviews as they were on the negative side. I was very happy with the care my mother received. She was given a shower on Wed and Sat without fail. The on-staff Doctor was wonderful very informative and always gave me feedback on my mom's needs. I am saddened that I write this review. A friend recently passed at this facility and I was notified at 3:30 a.m. Sleeping, I did not get messages until 6:00 a.m. When I called, the gent who took my call informed me that I had until 7:30 a.m. to remove the deceased because they "are a rehab unit and not a morgue". I became very upset with his curt, unprofessional attitude. A couple calls later an attitude change and he was more kind. I managed to claim but later. Based on this and one other incident during his three day stay, I do not recommend. I ask the VA to reconsider this option. Thank you. read more

To be truthful, before my husband was transferred to the La Jolla Nursing and Rehab Center, I read the reviews and was so concerned I didn't want him to go. I inquired, and the Center told me they have new owners and are taking care of the previous problems. Once he was there, I encountered the very same problems, and in my inability to get proper communication on my husband's care, I wrote a very emotional review, understandably, but I am writing a new one to bring up what is most important to know. Communication with the health providers is disheartening! I have been unable to obtain a name for my husband's doctor or his contact information. I've asked several times. I have power of attorney and need to be informed. It has been impossible to connect with anyone a the Center for help or information. When calling the nurses, they NEVER - EVER! - answer their phones, and don't answer when paged so I can inquire how my husband is doing or get someone to help him pick up his personal phone so I can talk to him. It's hard for him to reach it sometimes. He needs to know "I"m there" in heart on the days I cannot go. I talked to the nurse supervisor and social service people, who promise to help, but nothing has resulted! I asked the social service person if she would be kind enough to walk down the hall and find out about my husband, and she hung up on me. In dismay, I contacted a Senior Lawyer Service (waiting for a return call) the Ombudsman service (very compassionate but overworked.. I'm waiting for a call back). The only people I've been able to talk to, and they are often not available, are the office people...never the nurses for my husband, or doctor. A couple times, no one answered my husband's call button when he needed water, the nurses told me they were too busy with other patients. I can understand that, but five minutes later I walked out and found two nurses doing paper work, understandably, one on a computer who just looked at me when I asked a question and said nothing, and two nurses sitting on the back counter engrossed in something on their cell phones. That lack of interest in a patient made me concerned that my husband is not getting the care I wish for him in his last days, making it harder for him to deal with his situation. These things need to be addressed, and I am hoping my requests will be taken seriously. Since my first review, perhaps because of it, this evening i finally got a call from a nurse. LVN Julie called me and told me all I wanted to know about my husband's meds, diet changes,his day and reactions, and said she would let the doctor know I need to talk with him and give him my number. She was lovely to talk to! Michael and Mona are the only others who have cared enough to help,but only when I've been there. The rest have left me feeling they don't care about their patients. Everyone deserves to live their last days in dignity and love.That's all I've ever wanted. If you have no other option than this center for your loved one, I hope this information helps you start out on the right foot when they are admitted. read more

One of the worst experience ever!!! I left my father here because I went on vacation,I will never leave my father here ever again. He was there for 7nights when I went to pick him up he was very very dirty found him sitting on his wheelchair left alone hunched over, on the table there was a piece of paper and some crayons and an old dirty cup filled with cold coffee. I looked at him and said hi! he looked very confused like if he was out of it. I was heart broken I had never in my life seen my father like this it was about 4:30 in the afternoon and he was still in pjs his hair was greasy and dirty he had not been showered or shaved, his shirt was dirty and stained looked like food from earlier or the night before his shoes had poop on them and his diaper was full of urine he had a bad smell to him like they did not even bathe him. Do Not!! Do Not! Recommend this place at all!! The staff do Not care at all for the patients they are all on there phone they could care less what the patient is doing. They loose things my dads clothes got lost even thought they told me to put his name on his clothes. They said they would reimburse till this day we have not heard from them I could care less about the reimbursement. They also asked me did you call and say you were going to pick him up and I said yes and they were quick to say who did you talk to? they wanted to put the blame on each other rather than taking full responsibility as a team and they call themselves nurses !! What a joke! This makes Me only assume that they keep there patients this way up until the family member picks them up. Don't be fooled ,Theses people need leadership, accountability,they need to take a caregiving class all over again. I am so disgusted by this place my dad was is such bad shape I left crying. I want to give ZERO STARS. I am a patient here. This place is in chaos. Poor leadership ,call lights unanswered except for an elite few. Long term care here is low priority people. We wait and wait for help. They discourage getting out of bed which makes it easier to neglect us held hostage in beds. Staff leaves for better jobs and pay. The good ones are fleeing rapidly. Sad because it could be better. No one visits or checks up now. Covid keeps outsiders away so no one sees or hears the misery here. read more

In the three days since my mother has been admitted to this facility, she has fallen twice. The staff, after the first fall, did not do anything to help prevent her rolling out of bed is second time. They did not move the bed up against the wall, so if that she rolled, she would roll into the wall and not the floor. They did not get her a scoop mattress, that one has higher size than the middle, so she would not roll out of bed I would rate this facility, which is supposed to be one of the best facilities in San Diego, the worst in my opinion. This place is kind of scary, especially if you are there with Kaiser. My dad got transferred here 24 hours after hip surgery. The staff left him in a puddle of fluid from his surgical wounds all night. There is a marked lack of supplies and staff to manage the patient load. The equipment in the rooms is old and run down, and there are no bed rails. My dad had to leave and go back to the hospital for an infection, I think caused by leaving his wounds open to the air and the the wet bedding. On the door to the storeroom there is a sign from management encouraging the CNAs to get their co workers to show up. After our 3 day nightmare I talked to 2 employees and they told me several concerning things. They would put your picture on a "wall of shame" if you called in sick. There were not enough sanitary supplies to make it through a month. Current managers are more concerned with keeping costs down than proper patient care, and get performance bonuses based on cutting costs. I would never let another friend or loved one stay here again with the current management. If I could give "negative stars, it would be a negative 2. read more

Taking them there is a death sentence. Someone that was close to our family died there because of COMPLETE NEGLIGENCE. They received multiple stage 4 pressure ulcers (bed sores) staying there due to the lack of attention. I can't even say "a lack" because she didn't even get ANY care. They did things behind the families back, lied about what's going on and refused to keep the family updated. She eventually died from the bed ulcers. My mother-in-law was at this facility for approximately six weeks. I have nothing but good things to say about her stay there. She really enjoyed her physical therapy and stated they always made it fun even though it was a workout. The overall facility and courtyard is beautiful with two gazebos. The food was surprisingly good from what my mother-in-law said. They offered different options in case she did not like what they were serving. The Cna's were always very attentive to my mother-in-law, and she stated that she received great care from them. From speaking with other families seems there has been new management that took over and the care really shows. The only downside that we found was about a handful of times nurses sometimes lacked a sense of urgency in bringing her her pain pills. We did have to ask multiple times. They were always given, but just not necessarily in a timely manner. I would not hesitate to take another family member to this facility if ever needed. Thank you all so much for her care! read more

Well what can I say about this place...words cannot express the disappointment and sadness this place has become. I've been coming to the facility for over 2 years and I have seen the staff dwindled down to people who obviously don't know what they are doing. The few that are wonderful unhappy and leaving. The ED I'm not sure how he become one. He is uncaring and it's obvious he only looks at dollar signs not quality of care. Its quantity over quality! The DSD I'm not sure how she got where she is. She refuses to isolated patients. There was. Notorious breakout and it took her for multiple patients to finally quarantine them. The DON Changed like every 2 months or so. I was unhappy with the old. DON/ADon/ED BUT ID TAKE Them back any day!! They need to revamp before someone is killed with the lack of care! read more

No stars for this facility. They do not have time to answer your calls to give you a daily update on your loved ones condition. They don't provide the care they are ordered to provide and they order hospice for your family member and representatives come in to have them sign themselves out to go home to be cared for by family members who are incapable to provide the care needed and unaware of what is going on. I'm very frustrated and speechless on the unprofessionalism on display at this facility. They failed to provide the special mattress ordered for wound care and healing and then they did PT on legs that haven't walked in over 30 years. Now they say after completely botching his care that he is ready for hospice. But doesn't communicate it with the family but to a confused patient asking him to sign himself out to go home to die. They are not in business to help but to make money. They need to be shut down. They now allow you to visit if fully vaccinated for 30 Minutes - bottom line is - DO NOT place your family member here you will be full of regret and they will be worse off then when they arrive upon their departure.
