My mother has dementia and she was admitted to this facility a few weeks ago for physical rehab after an illness. Her cognitive ability has noticeably declined since she was admitted. Outside of physical therapy and possibly an occasional art class, she is being given nothing to do at all. They roll all the patients into one area and set them around tables so it's easy for the staff to watch them. There's a single TV, but if they're seated facing away from it, there's nothing to stare at but an empty table. Imagine doing that for hours on end. She is lactose intolerant and the staff does not care. She had a lot of stomach distress when I visited her. Later, when the staff was handing out ice cream, my family pointed out that she was not supposed to have dairy. The staff member passing out the snacks said there are too many patients and he couldn't be expected to keep track of what each person needs. We took her outside for a walk and there was no procedure for checking her in and out. We tried to flag down someone at the nurses' station to let them know she was going out with us, but they ignored us. If someone had come looking for her while we were taking her out, the staff would not have known where she was. We're arranging to have her transferred to another facility soon. I feel terrible that she has to stay there even a few days more. I hope the other patients have family that can help them get out. read more

The staff is friendly, warm to my mother, and excellent caretakers. This is an extremely stressful time for me and they are doing all they can to help me and my mother adjust. I wanted to write a review and highlight the entire team at Villa Rancho Bernardo! Everyone from Karlene from Admissions to Nurse May, Faith -Asst. Dir of Nursing, the desk nurse May Andrea' Frank, to the Unit Nurse Moshi, all of the hardworking CNA's, Rico from PT the list goes on and on... My Dad was discharged from a long 53 day stay at the hospital and the team here have been great his first 3 days (of 100. My Dad is a taller/larger man and they accommodated into getting him into a larger, more comfortable bed very quickly. Thank you Matt (Director of Rehab) for putting the ball in motion and Elbert (and team) for delivering the bed & getting my Dad situated. All of the nurses have been very attentive to my Father and I feel very confident that my Dad is in great hands. I was admitted last evening after they promised that my antibiotics would be ready for IV in the morning. They lied. It's now 10 am & they hope to get them by noon. I'm on a 8 week infection protocol that has been going 4 weeks. My antibiotics both injection & IV weren't even ordered until this morning. This place provides untrained staff & no integrity. My grandma stayed at this skilled nursing facility for a month of rehab after her coronary artery bypass graft surgery while also living with Parkinson's disease. The care she received was outstanding. The facility is very clean and well-kept, and the nurses and staff were attentive, compassionate, and truly dedicated to her recovery. They not only managed her medical needs with professionalism but also showed her kindness and patience every day. After a month of excellent rehab, she was discharged home stronger and more comfortable. Our family is so grateful for the care she received, and I highly recommend this facility to anyone looking for a place where their loved one will be treated with dignity and compassion. Sadly more than disappointed in Villa Rancho Bernardo. My elderly mom was transferred here after broken femur surgery from a local hospital. She was at the facility for approximately 8 days. During that time she was dismissed and disrespected frequently by nursing staff and labeled as the most difficult patient on the Fairmont ward by the nursing staff as she had a low appetite and was not able to swallow her numerous oral medications. VRB is unable or I was told they were unable to put my mom and other patients on IV fluids or IV medication, not even for critical patients or those who are unable to swallow numerous oral medications or for pain relief. My 87 y/o mom contracted pneumonia while at the facility, our family was not notified. I only found out after speaking to the case manager several days later, prior to transferring my mom back to the hospital as her health was dramatically declining while at Villa Rancho Bernardo. We also found out after she returned to the hospital after comprehensive blood tests that my mom had, a heart attack, most likely during that week she was at VRB and she had a dangerous blood clot on her heart. NOT caught or detected while she was at VRB. Only by getting her back to the hospital where they routinely run comprehensive blood and urine tests on elderly patients to detect heart and other critical health issues. Moving her back to the hospital likely saved her life. I will never have a loved one or anyone I care for be treated at Villa Rancho Bernardo Skilled Nursing Facility in the Future, FYI! read more

My father received excellent care here for almost 2 years. Both the social work staff and the nursing supervisor of the ward were very kind and compassionate, and had exceptional communication with us. My father had numerous medical issues and many er visits. Great medical care was provided. The facility is immaculate and there are many activities available to patients. The food is nutritious and looks appetizing. The grounds are well kept and clean. Most of the support staff provide good care. The facility also smelled and was very clean. One issue that was difficult was communication with many of the staff, especially on the phone but also in person, due to a different native language. This issue is frustrating , especially concerning critical medical issues. Also clothing and some other personal items went missing. Most of the clothing was eventually found. read more

My friend is there. Her care is atrocious, sub-par and needs to be looked into. The staff are gruff and uncaring. They do not listen to the patients' needs, and tell them they know what they are doing, even though they don't. It takes hours or days for a patient request to be met. read more

My mother is 92 years old and often falls, so my siblings and I had to put her in the Villa Rancho Bernado nursing home and receive care from all the staff and nurses. However, because my mother is Asian, she feels lost among the locals. We have to take turns visiting her every day so that my mother does not feel lonely (it is very difficult because we still have to go to work). After a while, I was very lucky to meet Ms. Marian, the director, who came to visit and receive the opinions and wishes of the patients. I presented my mother's case to her and hoped that she could help my mother stay in the same room with a fellow countryman so that they could talk and make friends to help each other. After listening, she promised to arrange as we wished and furthermore, to help my mother's spirit better. She did as she promised. Less than one star to be honest. They neglected my dad and seems like it's pretty common for them. Every time we came in to see him, he was in the worse spirits. He looked so depressed, his feet and legs were SO SWOLLEN. They lost his shoes so he was barefoot all the time. They kept him and other patients in wheelchairs in one room for the entire day. So long that they had to sleep with their heads on the tables. My dad also had diaper rashes all the time whenever my mom checked. Eventually, they grew tired of caring for my dad as his dementia got worse and kicked him out. They said we needed to pick him up ASAP in the pouring rain or else they were gonna send him to a PSYCH WARD. When we got there to pick him up, their attitudes were so nasty as if we did something wrong to them. I do not recommend this place at all. My sisters friend said she used to work here and she told us some horror stories. If you care about your loved ones, keep them away from this place. read more

I was transferred from Palomar Hospital and spent 20 agonizing days there. They left laying in a bed 22 hours a day and I lost 22 lbs for the food is not fit for the pet shelter! When complaining about the food they conten acknowledged and said they knew. Don't EVER send a loved one here. The care is inhumane and beyond subpar. We have tried to address the poor quality of care with the staff and the issues persist. Administrators told use to contact them to discuss issues but they don't return any of our phone calls. Patients sit in dirty diapers, have bed sores, are left in the rooms for hours, laundry not washed and the only 'clothing' is a towel thrown over their groin area to cover them. Great, friendly people, very kind, and they are working so hard to make sure the residents are being taking care of. The CNAs are great. The nurses are awesome. The maintenance department staff are so helpful, Elbert M. is a great friendly gentleman. The front desk ladies are always greeting people with a sweet welcoming. Ava, my dog loves the Rehab, and got to know almost everyone since we are there to visit my mom twice a day. My mother is going home next week, and we will miss seeing everyone at VRB. This place is awful. My father has dementia and stays here, and the way the staff treats him is horrifying. The call him the wrong name, dress him in dirty clothes, nicked his scalp while cutting his hair (and didn't tell me), and are completely disorganized when scheduling visits. The last time I saw my dad, I noticed he lost a front tooth. No one bothered to inform me. That is so negligent! The worst part is how many staff members tested positive this year for COVID-19. Why aren't workers in a setting such as a nursing home required to be vaccinated? Again, completely negligent! I stayed in this Facility for over a month. It is in a beautiful area with trees, flowers, etc. The female CNA's there are very hard-working & were kind to me. My mother has been in various types of nursing and assisted living facilities over the last decade and this is by far the worst. She was placed at VRB about one year ago after contracting COVID at another facility. VRB receives thousands of dollars per month to provide "care" and appears to only be interested in cashing checks. As I write this my mother is in the hospital for an infection that was left untreated for months, despite her regular requests to be evaluated by a doctor - something more than one employee assured our family would happen. Do not place your loved one here if you expect them to get routine care. My mother has a specific diet that needs to be followed in order for her to remain healthy. Something this facility assured us they could do. Instead, she is fed meals that not only go against her diet, but are unpalatable. The food makes her ill (vomiting and such), so, the facility takes action, not by preparing food that she should and could eat, but by handing her a chocolate Boost for many of her meals. Do not place your loved one here if you expect them to receive a healthy and nutrient rich diet. I can go on about what the family has shared with the facility about what my mother needs and how it has either been neglected or ignored. Calls go unreturned. During my most recent attempt at getting someone on the phone last Saturday, Maria assured me that the MOD (Chrissy/Christi?) would call me. It is now Monday afternoon without a call. That same Saturday I waited on the phone with my mother for over five minutes after she pushed the call button to ask for help. Five minutes. Five minutes where she could have choked. Do not place your loved one here if you expect them to be safe. If you speak with employees, what you will hear from them, aside from endless apologies, is that they are "the only one here", or that they "don't have enough people", or it's "so-so busy here". My message to VBR: if it's too busy, and you can't properly care for the residents/patients you have, then you should stop admitting people and taking thousands of dollars a month from families and government programs until you can handle what you've got. read more

My aunt is a current patient in this facility. I am not happy about this facility. We went to see her yesterday- and my daughter and I both had to go to use the restroom- a lady used a keypad and opened the door- she mentioned that was the patient restroom- in the restroom there was a large trash bags of recyclables, the trash can almost overflowing...so I stepped out and asked where is there a restroom we can use, she led us to a different restroom marked "employee restroom" she entered the code on the key pad and let us in. Restroom was clean! We did our business and walked out- shortly after a nurse walked in demanding I tell her who gave me access to the restroom. Extreme rude. While another male nurse waited in the hallway. I wouldn't tell her who let me into the restroom. She walked out of the room, my aunt said that was her night nurse the previous night and was extremely rude to her - left her in dirty bedding because the day nurse had not changed it out. Told her let's see how much longer she can take her dirty wet sheets. I DO NOT recommend this place. Sunmar- I would be happy to chat with you... especially after checking in on my aunt - she called for a nurse for help to be able to use the restroom and it took someone over an hour to help her despite her yelling for someone. Interestingly, new flooring was put in throughout. Better wear slip-proof shoes in the entrance and lobby area because it's a highly polished and slick material. Second time I had to go to a rehab center. Had a choice and picked this one. They have outstanding nursing staff and look to you needs promptly. They schedule you appointments and transportation to doctors as needed. Physical and occupational therapy staff works on your schedule and is cordial as wells as professional. One of the worst skilled nursing facility that you can send your loved ones to. No medical attention, medications aren't given as instructed , very unprofessional and untrustworthy nurses and staff. My mother was neglected , wasn't getting her medicine which worsen her condition, they refused to transfer her to a hospital although she was in Coma and they kept denying it until I went there , confronted them and finally the nurse called 911, after 12 hours... My mother passed away at the hospital after a week... It was a horrible experience and I will never recommend this place to anyone. read more

My mother in law was transferred from the hospital to this location. The first two days they were great; after that it was HORRIBLE! She would call the nurse for assistance they would come in look at her and walk away. Her roommate tried calling for help in her behalf because she was screaming in pain and they never came. I was there for dinner time and they never brought her food. I walked to the nurses station and I found a one young girl texting. After requesting food for my mother in law, I walked back to her room; I find out that her food was given to her roommate; the roommate never got her tray. I had to go back to the nurses station to get the nurse to help the roommate with her bed sores. It was sad when I was walking out with my mother in law property, an old man clapped as I walked away. So sad, the seniors at that place look miserable. read more
