    My first visit was quick, but at my second I waited nearly an hour past my appointment time. I…read morereceived a standard skin check and had one wart frozen. No biopsy, no prescriptions, nothing else treated. Despite this, I was billed separately for a wart removal procedure and an office visit, totaling about one hundred seventy-four dollars. According to AMA CPT coding guidelines, the evaluation for a minor procedure already includes the basic exam and discussion. A separate office visit can only be billed if there is a "significant, separately identifiable" service documented beyond the procedure (AMA Modifier-25 guidance). CMS policy also states that the initial evaluation for minor surgical procedures is always included in the global package unless there is clear extra work (CMS NCCI Manual, Ch. 12). The AAPC further warns that billing both codes together without documentation is improper and considered "double billing." When I asked the billing office, they said they charge for an office visit every time and never code annual skin checks as preventive. That means patients are automatically billed for both, even when guidelines don't support it. Patients should be aware that this practice leads to higher out-of-pocket costs, especially on high-deductible plans.

I was travelling to Alabama (from Charlotte, NC) to celebrate my birthday weekend to bear witness to the civil rights movement, and especially to honor those whose lives were taken so brutally (lynchings). Since I was in need of an electrolysis session, I figured I'd take advantage. Zainab replied to my initial email inquiry 8 later. No issue there since I had plenty of time before arriving in Birmingham. We set my appointment for 30 Oct at 2pm. Arriving 15 minutes early, the receptionist indicated which way I should head. I knocked on the door; however, there was no answer, so I thought perhaps Z was treating another client. I went back towards the reception desk asking for the ladies room. After being done in the LR, I headed back to Z's office. (note: I was wearing my face mask & had my cell phone in my hand as was hoping I could charge it since the battery was very low). I knocked on the door again. Z opens the door (no face mask nor shield, no gloves), looking a bit frazzled made me think she had been napping. I said hello & her response to me was "Darlene?" No smile, no "hi"...nothing. She was abrupt and off-putting. Honestly, my initial reaction was to tell her how rude she came across, then to walk off, but I really wanted to have treatment for my chin hairs. I replied with "No, I'm Lola, I have an appt @ 2pm." Again she just looked at me with a puzzled expression, and repeats "are you Darlene, my 1 o'clock appointment? I repeated that my name is Lola, and I added, I don't know about Darlene, but I'm Lola. Then she tells me that her 1 appt never showed up and asks me "do you have a 1 o'clock appt?" Clearly, Z was all kinds of confused. I figured, she's an older woman and obviously, I woke her up from her mid-day nap...I'll cut her some slack and get this quick 15min session over with. I walk in, set my bag on a chair beside a small table and that's when I noticed the available outlet so asked her if I could charge my phone. She doesn't reply, so I said "I'll take that as a yes?" In a curt manner, Z replies "I guess so" I'm thinking, this chick has got the worst attitude !! Before taking my place on the bed/table to begin my treatment, I asked her if she would sanitize her hands before we got started. That immediately set her off. She sanitized them and grabbed for her plastic facial shield, then turning to face me, pointing to her face shield, says "if this is not ok with you, then you can leave"......again, here I wanted to walk out, but I didn't. The treatment starts and I take off my mask, showing her the hairs along my chin. She asked me if I'd had electrolysis before and though she was still less than friendly, at least I appreciated she was curious about my experience with the mode of hair removal. She also went about to make an assessment about my skincare regimen (sounding judgmental about my pigmentation, she must not have realized that I have freckles - ones I'm are quite fond of). At one point I mentioned that being from NYC, I've had all kinds of electrolysis and I was curious about her training. She defiantly mentioned she was schooled in London and said "if that's good enough". Dude, chill out!! End of the session came. Z dabbed a bit of witch hazel on my treated areas. I said "merci, merci" (because she told me she's Iranian - that's a whole other conversation). That was the ONLY light hearted moment she let out a small chuckle. When I get off the table, I commented to her about the "you can leave" remark. She retorted that since I had asked her about sanitizing her hands, she wanted me to know that she had a face shield and that I could leave if that wasn't acceptable. I told her that her remark was off putting. I also reminded her that she was less than cordial when she "greeted" me at the door. Her reasoning: "I was startled." To which I replied, "well, you didn't even say 'hello' and normally one would greet a customer by saying 'hi' or 'hello'.... Z replied: then I guess I'm not normal I paid with my credit card (ZERO tip) and walked out. Final note: I had sent to her a follow up email to inquire about methods of payment she accepts (cash, check, credit card, debit card, Venmo) because I wanted to be sure I was prepared and I usually do not carry much cash on hand. She did not respond to that email, so I sent a follow up request about 1/2 day later....still no response. I would NOT recommend her service. She did not thank me for my patronage and gave the impression she was doing me a favor. You couldn't pay me any amount of money to return. Z has zero affability and an absolutely awful bedside manner!!
