CNA students from Divine. Dec. 2024
    Anna L.

I am pleased to share a positive review of our experience at Linden Grove. Linden Grove provided a valuable learning environment for my CNA students. Divine CNA Institute is always grateful to be able to learn from the welcoming, knowledgeable and helpful staff here. There always seems to be an activity that the residents enjoy. The holiday decorations are well done, it feels very festive. Thank you. Anna

Staff asleep or avoiding eye contact/work.

One star is too generous. Avoid at all costs. Horribly understaffed, those that are working avoid eye contact, ignore family and patients, or are sleeping on the job. Patient charts left out in open-complete disregard for privacy. There was no proper intake, on arrival parent was deemed mentally fine (according to their test), but within two days showed signs of dementia. When we talked to staff their response was "that's not normal?". If the intake had been mentally stable and no sign of delirium on arrival, this should have been a red flag. Patient sat in soiled briefs for 2+ hours, even being told by staff around meal times they were too busy and to wait until after meals. Disgusting. No bed rails and ultimately my parent fell overnight and was brought to hospital. This ended up being a god send. Positive for UTI and infection so bad required over a week stay in hospital. This is the worst facility I've ever seen and should not be running. Absolute negligence. It's appalling the state allows facility to run.

Bed was horrible and narrow and no side rails.

This place is absolutely horrid. My mom was there for less than 2 hours as that is as long as it took to get her out. It was so run down and the furniture and beds were old and broken. I will never forget the smell. This is the place to send those you don't love but if you care for your family do not leave them here.

This facility has improved so much this past year. The female maintenance director is such a great asset to the building and their patients.

I'm disabled and have lived with my parents most of my life. We were welcomed to bring my 2 pets, and my mom's cat with us. That was a big deal for me. The meals there are very good. Not too big, or too small. Maintenance is quick and it's done right. The management staff is wonderful! They did more than expected for my mom when she was at her end stage lung cancer and helped to make her comfortable before she went into Hospice. They really went above and beyond, and I will be forever grateful for that. I made many friends and acquaintances there and will go back often just to say hello, to take part in activities or have dinner with the people I know.

Called for help and she hung up on me, when I called back I told her that kindly she hung up on me and she was rude and said she put me on hold. She doesn't know how to transfer you anywhere probably doesn't know how to use the phone. Hell, even answering the phone she had to repeat herself a few times. She probably just knows to how to pick and hang up the phone. You need to retire SUE cause you suck at what you do. This company has bad management, HR is never there when I call, been calling this whole week trying to get ahold of someone but SUE is always answering and won't transfer me anywhere but to the wrong people outside of the company. Had to call upper management from another state and report HR, management & SUE.

My mom has been living here for about a year. The thing that impressed me most when I took my tour…read morewas how clean it was. I have been in many retirement homes and they usually have a smell. This place is always clean, no elderly smell and no chemical smell covering up any sort of smell. The staff is kind and always friendly. The grounds are kept well. They provide 3 meals a day in the dining room and it always nice. My mom has made friends and calls bingo. She is active and no longer living alone waiting for someone to stop by and break up the monatoney. She has a social life for the first time in a long time. Yet she has her own apartment and privacy. This place has been a terrific option for my mom since living alone is not an option anymore but neither is giving up her independence. This is the best of both worlds.

I urge you not to consider this facility. On the surface, Willow Gardens presents as a friendly,…read morecaring, and well-run institution, truly interested in your well-being. However, these values are paper-thin and are demonstrated by the following observations: * FOOD: Portion sizes are extremely small and are often more on the child-size. It is highly unlikely that the daily calorie requirements are satisfied by the sizes provided by the kitchen. Although residents can ask for "seconds", they must wait for the completion of the entire food-service only to discover nothing is left. Making things worse, management is often observed taking "double-sized" portions while residents eat undersized portions. Food is heavily based on carbohydrates with little attention to fruits, vegetables, protein, etc. And deserts are heavily vetted on cookies! * HOUSEKEEPING: WG claims to be guided by an incredibly detailed checklist, but their small housekeeping staff only permits 20-minute limit to complete a superficial job. And, they have limited service to once every two weeks during the Covid crisis, with further limitations on an already reduced service! * MAINTENANCE: Service takes a long time. A simple light bulb will take 3 weeks to complete. However, I have to give them credit for acting quickly on urgent things like heat. But, management hampers the ability of maintenance to act quickly. For instance, sprinklers cannot be repaired because management claims to be "bidding" the project. Consequently, we have flooded sidewalks, at times. * MANAGEMENT: Managers are new and inexperienced at large hospitality facility management. They readily pacify residents with "we're doing the best we can" and "we're working on it" excuses. While they hold resident meetings, they do not readily seek out resident perspectives on their needs or satisfaction. * APPEARANCE: As I walk around the facility, little things pop-out that signal problem. Pictures are not hung at the same height. Dust is everywhere. Chairs are askew, and clutter hallways. Managers are sitting at their desks, not walking around interacting with residents. Smells from the rooms can be pretty obnoxious, at times. Some residents have reported obnoxious smells from resident rooms. So, why am I here? Well, it's for an entirely different reason. Other things overcome these obstacles. It has to do with location and the offerings that would give away my identity, but these problems are still overwhelming and deserve mentioning. I suspect, though, that I'll be shopping after the crisis.

First off this place has bias and implicit discrimination against women of color. I have received…read moresub par care from this place. They basically keep you stuck here with little to no resources for leaving unless you get placed in an adult family home. Their pharmaceutical provider doesn't listen to patients and will not provide any information when trying to get ssi or ssdi. If you already have ssi they will take all but $70 of your check. The new director "Candace" has turned it more so into a prison like environment and has no knowledge of trauma informed care, so if you're suffering from ptsd like me this is not the place for you. They do not treat ADHD rather the symptoms of it. They took me off of the only medication that helped my panic attacks within days of being here, while a different person who looked different than me was kept on it for multiple weeks if not months. I have seen an actual doctor all but three times since being here almost a year. Being homeless and having no options for housing aside from here makes them highly capable of taking advantage of some of the most vulnerable individuals. Do not seek care here they are not a good residential facility, they treat individuals like criminals rather than human beings who suffer mental health illnesses. I want to express my current state of distress and concern regarding my safety and well-being in this environment. Unfortunately, I no longer feel safe here and have been experiencing a sense of being threatened and singled out. It has come to my attention that there are extreme double standards at play, which have further contributed to my unease. I feel compelled to raise the issue of Candace's lack of trauma-informed care and knowledge about mental health conditions, particularly concerning my own struggles. Her actions have resulted in me feeling harassed and bullied, specifically due to her disregard for my anxiety and trauma response around food and large crowds or groups of people. As a result, my panic attacks have significantly increased in frequency and intensity since she implemented her rigid and harsh approach towards me. I kindly request that my concerns be taken seriously, as they greatly impact my overall well-being and ability to thrive in this environment. It is crucial that steps are taken to create a safer and more supportive space for individuals dealing with mental health challenges. I hope for a resolution that ensures my housing stability and promotes a more compassionate and understanding approach for everyone involved.

Most of the staff were very accommodating. However, because the doctor and nurse practitioner were unwilling to adjust his medication when he needed it, my husband has been hospitalized for a week. He developed huge water blisters and sores on his legs as a result of their negligence. At one time, he was left on the toilet for so long and the nurse did not respond to the call light, so he had to get up himself. There was constant urine on the bathroom floor because the equipment was old and inadequate. Many times there was noone in the halls or at the nurses station through the day. Wait times for a response to a call button many times exceeded an hour. It is only pure luck that he did not fall again undertheir poor care. The facility is old and outdated. The phone didn't work properly due to being old and dropped so many times. I had to bring in many things from home because they provided a bed and old nightstand period. We chose this place because we were told it was a 5 star facility with a doctor and nurse practitioner 7days a week. There was no doctor or nurse practitioner on site on the weekends. Be very careful if you place a loved one here. The staff is friendly and seem caring, but when you need real medical help, the doctor and nurse practitioner make the decisions. In my husband's case, their negligence caused him a week in the hospital.
