This facility has improved so much this past year. The female maintenance director is such a great asset to the building and their patients.

My father in law was a resident at Linden Grove and was treated well. He enjoyed the staff who were caring and friendly. He was able to discharge to an assisted living facility after receiving the rehab he needed. Thank you all. read more

The building has been up kept immensely, the new light fixtures and fencing has made a huge impact on the external shell of the building. I would not recommend Linden Grove for post-surgery rehabilitation. This facility operates as a nursing home and was not appropriate for the level of care my father required after major surgery. Overall, our experience at Linden Grove was deeply disappointing. If you are seeking attentive care, proper rehabilitation support, and a clean, well-maintained environment after surgery, I strongly recommend looking elsewhere. I realize this review is lengthy and may seem extreme, but it reflects our experience. I strongly encourage anyone considering Linden Grove to tour the facility in person before making a decision. Seeing the environment, cleanliness, and level of care firsthand is essential. All you need to do is walk one of the hallways, and you will understand. Staffing was clearly inadequate, and patient responsiveness was poor. When the call button was pressed, it consistently took 15 minutes or more for staff to respond. For a post-surgical patient, this delay is unsafe and unacceptable. My father was on a stool medication which causes diarrhea, and he had two accidents in his bed because no one came to help him to the bathroom. Then they tried to humiliate him with rude and insensitive comments, but I shut that down real fast!!! Post-surgical instructions were NOT followed. Every day, I had to ask the nurses to read the instructions. One of the things my father was required to do was walk the hallway at least 4 times per day for 5-10 minutes to help build stamina in his lungs and also prevent pneumonia. I was told this was not a feasible request, since he was on oxygen and a feeding tube, yet when he was in the hospital, they managed this just fine. Instead, he only got out of his room for 45 minutes/day for PT (except weekends). Hygiene was a serious concern. My father was told he could shower every other day, yet he often went 3-4 days without a shower or even a basic wipe-down. I had to beg the nurses to help him. At one point, I finally told them, if they don't help him with showers, I will do it myself! This lack of basic hygiene is alarming and shows a lack of attention to patient dignity and care. The room itself was lacking in quality and basic furnishings. There was a standard hospital bed but no chair or recliner to sit in--residents were expected to sit in their wheelchairs as their only seating option. For the type of surgery my father had, he was required to be in a reclining chair for at least six hours a day, which this facility could not provide. We were also told he would receive bed rails to help him sit up independently. Despite multiple requests and repeated promises from nursing staff, this never happened. We escalated our concerns to both the nursing director and the physician assistant, yet the issue was never resolved. The facility is very run-down and poorly maintained. The smell throughout the building--mentioned in other reviews--is absolutely accurate. There is also no ice machine available to patients or families; instead, you must ask a nurse to scoop ice from a hallway cooler, which feels outdated and unhygienic. Although my father was on a feeding tube, we saw the meals provided to his roommate, and the quality was extremely poor. Examples included a very small hamburger with about ten potato chips, a cold cheese sandwich with half a canned pear slice, PB&J sandwiches, and Campbell's chicken noodle soup. Overall, the food was subpar compared to other facilities we have experienced. Another concerning issue was linen handling. Used towels, bedding, and rags were placed on the floor under the bathroom sink (there was no linen hamper) and left there until housekeeping collected them the next day. I do not understand why a basic linen hamper was not provided. I will be submitting a report to DSHS, and encourage others to do the same if they have had similar experiences. This place needs updating and new management! read more

My family has been through the wringer trying to find a facility for my aunt that we felt comfortable enough with. My Aunt is now a patient here and I must say; I really appreciate the staff at Linden Grove. With all of the horror stories that I've seen online, my family and I were very skeptical in choosing a healthcare center to care for my Aunt. However, after carefully observing for about a month or two, this facility comforted my worries and reassured me that my Aunt was in knowledgeable, positive, and caring hands. Every single time I visit my aunt, I'm greeted with a smile. Not only by the person at the front desk, but by everyone I encounter. There is an undeniably positive atmosphere and an overwhelming feeling of care that I experience every time I come to visit. This facility has eased the anxiety that my family and I had; making the transition way easier for all of us to deal with. Thank you Linden Grove Center, and all of the people there who create such an environment that soothes the uncomfortable reality of our situation. You all truly are a blessing. read more

WORST POSSIBLE SERVICE EVER!!!! I was a patient here and one of the nurses who is leaving in the 24th was very rude to me and treated like a piece of trash. She should be fire before the 24th and she is also saying that I'll be billed for an ama when I'm on state insurance and I will not be sent a bill I was reassured that today by other office staff about how she was saying that and even had the audacity to say the same thing to my emergency contact. read more

One star is too generous. Avoid at all costs. Horribly understaffed, those that are working avoid eye contact, ignore family and patients, or are sleeping on the job. Patient charts left out in open-complete disregard for privacy. There was no proper intake, on arrival parent was deemed mentally fine (according to their test), but within two days showed signs of dementia. When we talked to staff their response was "that's not normal?". If the intake had been mentally stable and no sign of delirium on arrival, this should have been a red flag. Patient sat in soiled briefs for 2+ hours, even being told by staff around meal times they were too busy and to wait until after meals. Disgusting. No bed rails and ultimately my parent fell overnight and was brought to hospital. This ended up being a god send. Positive for UTI and infection so bad required over a week stay in hospital. This is the worst facility I've ever seen and should not be running. Absolute negligence. It's appalling the state allows facility to run. read more

One star is too generous! They nearly killed my son. Foley catheter was improperly cause a severe infection. And the stench of pee and poop combine with whatever slop is be cooked was so nauseating. The don't answer their phones. Need to get rid of the entire staff and start over. Do not put your loved one here. I am looking into legal action. read more

This place is absolutely horrid. My mom was there for less than 2 hours as that is as long as it took to get her out. It was so run down and the furniture and beds were old and broken. This place was is hell on earth. It took hours for them to answer a call light, my father ended up with 2 bed sores. He had a nurse that ground her knuckles into his ribs almost nightly because he did not move fast enough. I did report this to them and got a phone call but they were supposed to call me back but never did so I called them and they were like we already talked to you.The staff just do NOT listen to you and will argue with you. Do not send your loved ones here. Very dirty and not well maintained. My dad was in here for 5 weeks after having foot surgery. He was 100% nonweight baring he would push the call button on average it took them 45 mins to come help him up to go to the restroom. Toilet was covered in stool to the point that my father and his roommate refused to use the restroom in their room. Dad was there for 5 weeks and not once was he given a shower. He was discharged home and was back in the hospital for infection on the foot he had surgery on 3 days later. They lost his insulin and his foot ointment I brought in both from home until they could get more in. I'm very aware that they were short staffed. This was my first experience with a rehab/nursing home it sad to think that someday it could be me and I have told my kids and husband to NEVER send me to this place I would rather die than go here. Activity guy was the nicest person there and I could tell he really cared about the patients. read more

One word...HORRIBLE! My best friend was in there for a broken leg. She was unable to walk, so she was bedridden. They left her in Her urine and feces for 4 hrs, she didn't have a remote for the tv, her roommate was a bully left light on all nite and constantly screaming, food was horrible. When she left the aid was trying to get her in her husbands truck and lost her grip and they dropped her on the wet pavement causing a fractured rib. This place should be shut down. I wouldn't take my dog there, not even fit for an animal! read more

Worst place EVER..Dirty..my mom arrived to a bed with no sheets and was left like that(after a serious femur surgery) The complete staff has awful attitues and thier faviort answer for any question you ask is"I DONT KNOW" I had to call an ambulance after 3days of neglect n abuse from this awful place. Once back at GOODSAM hosp..my mom had baterial infection in her blood,all her stitches were split out,she had ecoli bladder infection. I repetatively asked: can you please check to see if my mom has bladder infection..The answer was no..theres no reason for random bladder infection checks. Ill tell you as her daughter whose taken care of her for 13years,I know if my mom has bladder just by the way she acts..Still they refused to check. Hmm and I was right. This facility had me and my brother afraid to stay. Upon arrival we were escorted to a small room with two beds, one of which was occupied. The air was heavy with the odor of clove oil. Not that it's a bad thing, but the air was stagnant. My brother used the bathroom and reported to me there was no fan nor window that could open. Thats when I realized that germs and diseases have a breeding ground right there in our room, and any room without ventilation. With new strains of Covid and close quarters, I didn't want to take a chance I might catch domething. I had not been fully admitted when I made the decision that I would not stay there. There was some objection on the staffs part but I made myself clear and they obliged. On our way out I saw many individuals in beds and wheelchairs that didn't look like they'd smiled or seen the sun in decades. There were many who appeared to have only a day left on earth. Upon checking into this facility for post operative physical rehabilitation from the hospital, my mother-in-law was unfortunately assigned a young staff member who had no knowledge of total hip replacement protocol, and the inherent physical restrictions associated with this medical procedure. When trying to assist her into the bed, she physically bent her in a position which was not only extremely painful causing her to cry out that she was being hurt, but was physically harmful to her as a result of her procedure. This staff member totally ignored her objections that she was in conflict with her hip protocols as prescribed by the hospital, and continued to physically force her into the bed in a manner that caused her great unnecessary pain and discomfort. My poor mother in law was crying in tears because of the lack of training and disregard the staff member had for her operation. My mother-in-law immediately notified us of the situation and we contacted the staff liaison for Social Services, and explained the situation. As family members, we requested that signage be placed in the room that she was under Hip Protocols. We insisted that all staff listen to her concerns, and follow her directions regarding physical restrictions due to the physical limitations of her recent surgical procedure. The food was absolutely horrible, and as such, when possible brought my mother-in-law food from Jack-In-The-Box rather than subject her to the gruel they passed as meals. We soon realized that this was more of a rest home for the aged and disabled, than it is a rehab facility for the rehabilitation due to surgical procedures. Because her Medicare insurance did not cover the physical rehabilitation to be done at the hospital, we were forced to go into a facility like Linden. read more

After breaking my back and having 2 extensive 8 hour surgeries with a complete rebuild spinal fusion from pelvis to neck, 22 inch scar. Immense pain and needing some good rehab and medical management. Choosing Linden Grove was the best for me. I am a Critical Care RN And have an extensive history working long-term care. I am familiar with the good and the bad and I can tell you Linden has no more issues than any other place around, that being mostly staffing issues. Not having enough nurses and CNAs, and retention rates are low, why? Because pay is low and the work load is hard and grueling. Imagine taking complete.care of 8 adults, getting them up, and down, ready for baths, passing trays and feeding, toileting, charting vital signs, etc. Its exhausting and its one of the reasons I ended up having my 6th & 7th back surgeries this Summer, after, 44 years in healthcare, I trashed my back, lifting far too many patients. So I have a little insight to my review. First off. I was greeted at the door (from an ambulance) by Justin, I'm not kidding when I say this young man is a superstar, attentive, extremely knowledgable and caring. He is going to go far in his career and I can only hope L.G. knows that he is an enormous asset to them and that he is highly overworked and has to juggle a lot of different hats and does it extremely well. He made sure I was in a nice room, since I was one of the younger patients there at 57, (lol) he knew that privacy and being near PT was important. He ensured my things were taken care of, thay I had a locked cabinet, my TV worked, that I was given a tour and introduction to the entire place. My room was clean and faced a beautiful garden and gazebo. Then imagine my surprise when one of the best institutional meals ever was delivered to me, offers of your beverages of your choice and after working as a mock surveyor for long term care for years, I was surprised that only 1 meal out of about 6 wasn't too great (please never serve that "chicken pot pie" to anyone ever again)! But offers of food substitutions were there. So yes, hurray for the meals. The nurses are a bit harried and of course one reason is being understaffed, but not their fault, they worked their tails off. I was highly impressed with Andrew, who was so attentive and concerned about my condition and my pain. Really all of the nurses were kind, but overworked. The CNAs, again overworked and quite a few young ones, many working in hopes of becoming nurses. Yes many call lights stayed on longer than they should, sometimes at night it would take several minutes to answer. Those kids worked hard and it's tough covering an entire wing by yourself, answering lights, changing beds, doing vitals, toileting people, etc. So the owners and administrators need to step it up and try to budget more employees and decrease the work load, you are burning your staff out at an incredible rate! Retention is everything, throw in a few invisible handcuffs to them, how about an appreciation board, employee of the month, give them rewards, like pizza and ice cream, believe me, little things like that, keep them there. The rehab department, wow what a great crew, especially Susan (OT) and Matt (PT), they all were especially helpful in teaching tricks of recovery, strengthening,It would be a huge asset to LG, to open after Covid to the public for outpatient therapy. Every new patient has to have 2 negative covid tests (ouch) and then placed in a 14 day hold in the restricted wing. The staff is very diligent in ensuring EVERYONE uses the required precautions, they get a 10/10 there. All in all, it's a pretty decent place, a few changes from my perspective, better HS snacks & offer some beverages in the afternoon, perhaps the activity dept could do that. Yes, they just need more staff, honestly one evening, I was out doing my walk and this poor man had his light on forever. I finally stopped after my 4th time by his room and asked him if he was ok. He said he just needed his light turned off. With covid I won't never go in, but I wanted to, as a matter of fact, I could have answered lights all night and then those poor kids could have really had enough time to give some great care! Things like the bath aide always getting pulled to the floor, so when the patients were okayed to go to the spa to bath, they couldn't because theres no bath aide. And even the housekeepers are overworked! Probably missed my room every 3 days or so. Last, I would have a good thorough deep clean, it's nothing bad, no smells, no dirty floors, walls or windows. Just a lot of clutter, at the nurses station, on their carts, equipment cluttering the hallways, just everywhere, it may be me, but clutter means dirt, germs and an unappealing rehab. Overall, I think LG passes. Cause they have some amazing nurses there and I had a great rehab there and was sent home, able to have a great plan of action. read more

My mother recently stayed at this facility and we decided to move her to another facility due to the poor care that she was receiving. The staff response time to a patient using the call button was appalling and it clearly puts patients at risk. Unfortunately, the staff does not even follow their own stated policies and procedures. We were scheduled for a care conference and the management did not show. But this was only one of many examples that we and other patients observed. We've been to other facilities and this one stood as being uncaring and unprofessional. It's almost like no one knows whom is charge of the facility and the systemic failures were occurring daily. Lastly, don't believe their slick marketing and advertising as their promises don't seem to be rooted in reality? Best advice is to stay away. read more

I was appaled at how bad the care was: rampant theft staff would not let residents report....., less and less food,..... no virus measures until it was already around th home. Residents who came in negative were sent home positive..... No one took responsibility for anything. ....When something went wrong and was reported staff would go immediately into why it was not their fault, not help....All staff could take requests but all got to decide if they got acted on also.... A large number of my clothes disappeared after a single laundry. They seemed to really like that families can't be around now.....Getting the right medictions to continue was an epic struggle......fixing medication errors was almost impossible........There were no discharge instructions or precautions shared...no adaptive equipment shared or arranged.....no notes sent to your GP........no treatment review or shared plan...no one shared meaningful info among staff....staff depended on residents to tell them what other staff said and did......little bathroom help at night...big push to get us all in adult diapers not in our treatment plan... read more

My brother was admitted to Linden Grove Health Center yesterday for rehabilitation following eight weeks in the hospital. During those two months he underwent two open heart surgeries to replace 3 heart valves and multiple surgeries on a serious leg wound, all of which resulted from an undiagnosed systemic staph infection. He was transferred to Linden Grove via ambulance where he was to receive skilled nursing care and physical therapy to allow him to transition to home. The morning after his transfer, his wife and nephew came to the Center and found him lying in feces; he had pushed the call button but no one ever responded. They had to find someone to help him. Additionally, they discovered that between the time he was admitted and the time they arrived the next morning his cell phone and wallet were stolen. They were told that the cell phone and wallet were not listed on his personal item inventory at admission. When they asked to see the inventory they found very few things listed. Many personal items that were transported with him from the hospital were not listed, including his laptop. I do not know if the failure to accurately account for his personal items was an intentional act or incompetence, but the theft and failure to respond to a call button, resulting in him lying in his own waste, is enough for me to ask that he be transferred immediately. When I called and asked for the name and phone numbers of the medical director and director of nursing the nurse I spoke with gave me their first names. She told me she didn't know their last names and had no idea how I could contact either of them. read more
