They try to charge you upfront prior to submitting to your insurance, lie to your face that insurance denied the claim (it was denied because the "claim" was integral to the procedure and no separate payment was applicable), and try to charge you beyond insurance negotiated rates by calling it an "upgrade". Take a printout of your insurance EOB and compare with their itemized invoice prior to paying anything!
