I have regularly visited the parent of a close friend at Marin Post Acute. The patient suffers from dementia and cannot report on their care. I have found them numerous times only partially dressed. Their dirty clothing is routinely soiled with excrement. Over a period of several months, I took the patient by wheelchair into an outside patio area. There was a portable potty there during the entire summer as if that space was additional storage. The dumpsters overflow, creating the appearance of neglect. When alerted about the patient's condition, the staff is attentive and promptly addresses any issues. They seem sincere and caring. I can only guess that there are management or patient load issues that prevent the type of attention required and paid for. If you have the ability to compare staffing ratios and patient outcomes, I suggest that you carefully consider that before admitting a patient to this facility. read more

I'm in the home care/healthcare industry, and as much as we try to help our clients stay at home, sometimes events occur that result in admissions to the hospital, and then step down/rehab care before returning home. Marin Post Acute is my favorite SNF to work with when it comes to the people whose care I'm overseeing. The team here for managing patient care is AMAZING! Definitely my go to when I'm approached for recommendations for my clients, or their friends and loved ones. read more

Top notch quality of service. We brought in our mom for rehabilitation after hip surgery via Keiser and feel blessed that she is being taken care of in this facility. The most important services we wanted for her was patient care combined with quality of rehab services. They met it and exceeded! Starting with the entire staff, day and evening shift, whom look after her in various roles - always caring, always attentive, and very knowledgeable. They respond quickly, they follow-up, and they make time for her. They all at Marin Acute Care take full responsibility of what they do and team up to succeed at it. The Kaiser team overseeing her care topped off the services - they have are onsite services team here at the facilities whom are extending her treatment care here. The entire group of services has been comforting from a family perspective, knowing she is cared with the maximum care and goal to help recovery and prepare her to come back home. Regards food services, they must have a top notch chef hidden in the kitchen!, the quality and quantity of food is the best - well balanced and with great variation. The facilities is going through renovations so it is nice to see that this will enhance living comfort and safety services as well. read more

This facility should be closed. The communication between staff was non existent. My Dad was at Marin General. He arrived at MPA and did not have the most important medication he needed and no one followed through to make sure he got it. It took 4 days. Doctors aren't available on the weekend. I had to hire two private caregivers 24/7 in order to be sure he was changed and did not get out of bed and fall again. The food was inedible. I bought my own food and brought it to MPA. and left the food in their refrig. The food tray would remain in his room for 5-6 hours after he had finished his meal. PT and OT were not able to help him. There was no schedule for his PT and OT sessions. So I ne Er knew when he would be going to his sessions. The therapists tried to help him but they gave up very quickly. After 3 weeks, his overall health declined more and more everyday, It's hard to believe this fact...He was not given a shower in 3 weeks. Both the private caregivers and I would ask the nurse EVERYDAY when he would be given a shower. The answers ranged from, "He only gets showers on certain days, I think he had a shower yesterday, He will get a shower tomorrow or I'm not allowed to give him a shower"...When I asked the nurse why he hadn't had a shower in 3 weeks (on the day I was taking him out of MPA, the nurse said "He must have refused to take a shower, but we can give him one now". I said, No thank you! I am taking him out of here. The care was abominable. The private caregivers would push the call button several times all night long (as we were told that the private caregivers could not do anything except supervise my Dad and had to call MPA staff to move him or feed him or move him from the bed to a wheelchair). The staff at MPA would not respond to the call button for hours. His dirty laundry was bunched up and thrown in the bottom of his closet. I had two extra sets of eyes from experienced private caregivers with my Dad 24/7 so I know for a fact these things happened as they kept a journal hour by hour, and I witnessed these things myself as I visited everyday. The administration at MPA were aware of the lack of care he was receiving and promised he would receive VIP treatment. The lack of attention and care was appalling and I demanded that he be discharged immediately. I would have given my review negative stars if I could. No one should be placed in this facility as the extreme lack of care due to the facility being understaffed and poor communication from the incompetent staff will negatively affect anyone trying to recover in a rehab center. Unfortunately my Dad never recovered from his fall once I brought him home. The three weeks he spent at MPA were a death sentence. read more

Marin Post Acute is a very nice facility, it is very clean and tidy. The staff from the Clinical Director Judy, to all the Caregivers like Asher, Manly, Imelda, Ricky, Magdaleno, and Danalyn were very nice. Nurses and the PT are on top of patient's progress. All of the staff here works with their heart and have a great goal of giving their patients the best care that they deserve.Highly recomended for our loved ones. read more

My dad had fallen down, went to the emergency room then was transferred to Marin Post Acute for physical therapy. When we asked my dad about physical therapy, he said he hardly ever saw anybody and when they did come in they had him raise his arms up and down.They never got him up out of bed, never put him in a wheelchair to go outside, they left him wearing soiled diapers. He had a bloody Band-Aid on his arm for 6 days! This went on for 2 weeks! He is now forced into an assisted living facility unable to walk, unable to stand, now incontinent. Prior to admission, he was mostly independent still living at home with caregivers. He was not like this a week prior to going to Marine Post Acute. Hopefully now in the Assisted living facility he will get the physical therapy he needs and hope it's not too late for him to regain some independence. The discharge planner, was awesome! Facility pleasant. Great Physical Therapy. Very understaffed. For example I had to wait 5 days for a bath having to wear the same clothes and underwear. Over an hour to get water. Over 24 hours to get ice for serious bruising. Doctor ordered physical therapy 5 days a week but the management only allowed 2 days a week. Medication patch ordered by doctor to be changed once a week which they only changed after 2 weeks. Unable to eat without help but no one helps. I finally had to ask for a supplement to maintain my nutritional intake. The list goes on and on. read more

The staff was very caring and encouraging towards my mother who was there for rehabilitation after major back surgery. She received excellent physical therapy and occupational therapy. Since she has back precautions of no bending , lifting and twisting, they worked with her on adjusting her way of getting dressed and all her activities of daily living. She would definitely go back to the facility if she needs to in the future. read more

Do not be fooled by the beautiful, inviting newly renovated entrance. Go upstairs to the second floor and notice the drastic difference. I believe the long-term patients are upstairs and the short term rehab patients are downstairs. Downstairs looks, sounds and smells significantly better than upstairs. Take a close look, not at the decor, but at the patients. Do they look well cared for? Content? Also take a look at the staff. Are they rushing? Are they taking the time to talk to patients? Are they in front of a screen or in front of a patient? Upstairs has a distinct odor and a noticeably gloomy atmosphere. This is where the MediCal patients reside. CNAs appear to spend more time entering information onto a hallway screen than attending to patients. Patients are parked in their wheelchairs in the hallways or wander around asking for help. The one general meeting room is opposite the rear entrance and serves as the dining hall, TV room and activities room. Therefore only one activity can occur at a time and the patients are constantly distracted as people enter and exit the building often letting in the cold air. The patients are poorly dressed and look unkempt . Marin Post Acute is severely understaffed and it shows.   Day-time CNAs are responsible for 13 patients each and Night-time staff have had 26 patients each. The quality of care and employee moral have deteriorated since the facility was sold  a couple of years ago. The company seems to be more interested in facility appearance than the quality of care. I hope they intend to renovate upstairs as beautifully as they decorated downstairs. And I hope they will make the same investment in their patients as they have in their facility. Take a very hard look at the staff and the patients. Ask about CNA to patient ratios, ask how often the residents are bathed, ask if your family member will have a CNA assigned to them so that a relationship can develop with the patient as well as with the family. Ask if the family is informed of staff or administrative changes. And keep asking and keep visiting. Maybe Marin Post Acute will listen. Be sure to read the posts below that were not recommended. Interesting how only the positive posts are readily visible. Even Yelp disappoints. read more

My dad fell at home and other than cuts and skin wounds his mind was fine he ended up going to Kaiser as he had elevated heart issues, while at Kaiser we believe he had a minor stroke and he was confused and always trying to get out of bed, they had someone with him 24 hours a day to make sure he didn't get out of bed as he was a fall risk. Kaiser had him moved to this facility. The first day I visited him, everyone was so attentive and nice to me and my dad. I went in the 3rd day and walked into him choking and covered all over in food, he was suppose to be hand feed. I ran to nursing station saying my dad is choking the nurse said he is not in my care you need to find his CNA, I started screaming he is choking so she finally called someone and the other person came in so angry that she had to go into a patient that wasn't hers either. She was not kind our nice to any of us just mad she had to help and clean him up. Finally after about 30 minutes the CNA in charge came in and apologized that he had a new person and he didn't realize he had to be hand feed. They were very sorry and finished the cleaning. I tried to find the head person at Kaiser in that office or the head of the facility they were in meetings. At that point the CNA assured me this would not happen again, I decided not to complain as I didn't want my dad getting treated poorly. A few days later without issues at least while we were there we got a call that my dad tried to get out of bed and broke his hip. Mind you he was on 24 hours a day watch at Kaiser and here they didn't keep an eye on him. My brother was there before he fell and they had him sitting up in a wheelchair while they changed the bed and my brother noticed there was food on the floor. My father was still in the wheelchair when he left and they were still in the room. Shortly after that is when we got the call he fell. So did the CNA leave him alone in the wheelchair or did they forget to put the rail up on the bed?? My dad was in know shape to move and get over a bed rail and if not buckled in wheelchair he could have fallen out. We will never know but I would not let my parent go to this place if they needed a lot of care. My father died a week later as they could not operate on a 96 year old. read more

At Marin Post Acute the foundation is Kyle Whimpy and Judith Rooks, who were most helpful in my care and the development of my continuing health needs from a fractured spine. The facility is top notch in all other aspects as well, especially the nursing care, the occupational trainers and the physical trainers. They were all spot on and timely with my rehabilitation and restoring my health issues. I have given them Five Stars because they deserve it. I suggest you research all of the facilities and exercise your due diligence in choosing the right facility for your needs. You will find Marin Post Acute right there st the top. read more

My experience with post-surgery rehabs centers has not been great. The two I'm familiar with both suffered from a "dingy" feel, loud patients and a staff that didn't seem to care much. So, my hopes for Marin Post Acute were not great. I was so wrong. When my older Japanese friend was injured in a fall, she was taken there for six weeks of rehab. They were such caring and lovely people. Even with all the COVID restrictions, they were very good about letting us visit at the window, and there was virtually no wait when we came to visit. We particularly loved Sanji at the front desk. He actually remembered my name every time I came in. We also thought Debra in Social Services did an excellent job of coordinating everything. When I went to pick Sumie up to leave, she was crying that she had to leave and everyone came out to say good-bye! I was blown away! Anyway,on behalf of my little friend and our family, we send a heartfelt "thank-you" to MPA! Heroes really do work there! read more

I had my love one in Rafael Convalescent Hospital acquired by Marin Post Acute on April 2017 the facility has experienced a serious decline. Many employees have left, and the new ones are not good enough, I try to check her at least 4 times /week and feed her so I was aware of the change and deterioration of quality of service there, the quality of patient care and the cleanliness of the facility has deteriorated. she was left soiled and dirty, she did not received any more daily shower, and the place has an terrible smell, I read the review who was happy to left the love one there may be you never go to check and see how is she? read more

My dad has been at Marin Post Acute since January of 2018. The facility is clean with beautiful surroundings. The staff is friendly, helpful and accommodating. read more
