My wife had a serious stroke in March. After a few weeks in the hospital, they recommended she be transferred to a skilled nursing facility. I researched the facilities that were within my medicare network. All of them received mostly negative reviews. My daughter knew a former of Mi Casa employee and she had mostly good things to say about the place. Thus, we selected Mi Casa for my wife's care. The facility is old and somewhat dated; typical Arizona decor, beige and brown. However, the facility is clean and the cleaning lady did her room daily. The nurses that cared for my wife were very good and caring. The majority of the CNAs were also good. Weekends usually had fill-in staff who were mostly OK. The physical, occupational and speech therapists were exceptional and caring. My wife made a lot of progress while there. Criticism of the food is warranted. Most of the entrees were excessively salty, as was the daily serving of instant mashed potatoes. The vegetables were what my wife ate mostly. My wife is lactose intolerant, but we sometimes received a carton of milk with the meal. The kitchen staff does not follow instructions very well. After two months of care, some desk jockey at Humana Medicare Advantage decided she did not need any more SNF care. Too bad, as she will require considerable physical therapy for the next several months. read more

My mother has been here for a month now. She suffered a massive stroke and has lost mobility on her left side, plus has a feeding tube. We have been very happy with everyone at MiCasa. She has an amazing PT/OT team and we love the speech therapist. Everyone has been very caring and attentive to my mom. There is a wound care Dr and wound care team that conducts regular checks. There is a doctor that visits daily. The food is a bit too salty for my mom's liking but she's eating it. The facility is clean and feels very safe to us. They have a lot of activities every day for the residents. My father spends 12 hrs a day with my mom and I have visited everyday. I love being greeted by the same lady at the front desk when I visit. Our only complaint would be the weekends. Sometimes it's not the regular nurses/cnas and they take a little longer to respond or we don't see them as often. I think they might be visiting or filling in for the day. The regular nurses and CNAs and staff are all wonderful. Finding a place after her hospital stay was so stressful and we're glad we chose MiCasa. It really does have a family atmosphere. read more

If I could leave negative stars, I absolutely would. Where do I even start? The level of care in this facility is beyond unacceptable. The nurses and CNAs are not only unprofessional but also careless in how they treat patients. The residents are severely neglected, left waiting for help that never comes, and their basic needs are brushed aside as if they don't matter. Being understaffed is not an excuse for NEGLECT. Regardless of how many people are working, quality care should never drop below the bare minimum, yet that is exactly what happens here every single day. Patients are treated like an afterthought rather than human beings deserving of compassion, dignity, and proper medical attention. The environment is filthy, disorganized, and shows no sign of genuine concern for patient wellbeing. Horrible medication administering, is one of the many forms of neglect presented here. The care of friends and family is highly disappointing. Staff are disrespectful amongst each other. Families who entrust their loved ones to this facility deserve so much better. This place should be ashamed of the conditions they allow and the lack of accountability they show. I would never recommend this facility to anyone and sincerely hope changes are made before more people are subjected to such poor treatment. read more

While the staff was friendly and attentive, they are very understaffed. I was the there for rehab/physical therapy, but most days that consisted of putting me on one of those machines where you pedal (presumably to strengthen my legs) for 15 minutes a day. That's it! The shower situation is atrocious. I was scheduled for 1 or 2 showers a week, but when they wheel me to the "shower," it is this huge concrete warehouse jam packed with all sorts of discarded large equipment. In a corner a shower head juts out of the concrete - worse than a prison shower! The food is OK for the most part, although one night they ran out of almost everything, even iced tea. Breakfast is actually almost always great! I don't know why they can't keep it up for the other meals. read more

My experience with Mi Casa Nursing Center and their case management team (Laren Colella Emery) has been incredible! Quality of care and billing issues. My father was cared for by Mi Casa in the past and the only real complaint was the food. It was disgusting. This last visit was more disappointing. He was left in a wheel chair facing the wall in his room away from the tv that was on and worse away from the tray of food he was not close to. I had to help him get to the bathroom and change him after since he was left in a soiled adult brief. I took him home as soon as it was safe for him to come back without being a fall risk. Looks like we will need to research and find a better post acute facility for him in the future. read more

They were very unprofessional especially Susan Bernardi who is supposed to be a case manager she talks down to the patients and lacks compassion for people in her care. She speaks rudely to patient but when family calls she plays nice. When you request simple things like water they make an issue out of it, yet they walk the hall talking and see your light on, look directly at you and keep right on walking and talking. The food was basically mystery food. My family even refused to try it even a basic hamburger didn't taste right. An aide would come in at 4am and just flush the toilet and walk back out no vitals or anything just flushing a toilet to wake you up. When asked for help getting undergarments on the nurse said" your ambulatory you can figure it out." Mind you I couldn't even stay without assistance from a walker or wheelchair yet helping wasn't something she was going to do. They wanted to have me take my shower at 9pm. After asking to have it taken in the AM hours. I got more out of the PT/OT people at home than I ever got there. The only kind employees were the medication ladies who listened and took the time to explain questions regarding the meds since I was receiving ones I had never taken before. Stay clear of this facility they are not patient friendly or respectful. read more

Some nurses do not I repeat do not administer medication correctly. They put my mom in the hospital TWICE! They almost killed her.

While the nurses, CNAs, and physical therapy staff at Mi Casa are compassionate and hardworking, the facility's leadership and systemic issues are deeply troubling. My elderly mother has been here for nearly a month after an accident. Despite efforts to help her regain strength for transfers, she's been given inadequate resources. A bedside commode was only provided three days ago, broke on first use, and has yet to be replaced. Minimal staffing means she's had just two showers in a month. During isolation for COVID-19, which developed into pneumonia, physical therapy consisted only of being asked if she wanted to move to her wheelchair. When she declined due to feeling unwell, it was documented as refusing therapy and reported to Medicare as a lack of progress. The facility then recommended ending her Medicare coverage and pushed for an ALTCS (Arizona Long Term Care System) application, raising concerns about prioritizing financial gain over patient care. Their reports to Medicare were vague and failed to reflect the lack of resources and support provided for her recovery. This has been a tough learning process--know your loved one's rights and stay vigilant. While the caregiving staff is excellent, leadership's questionable practices make it difficult to recommend this facility without serious reservations. read more

I don't even want to give this 1 star but am forced to. This place almost killed my loved one the last few weeks. The care is less than compassionate and my loved one was actually humiliated during their stay. The food is not edible and the nurses could care less about any patient. They're suppose to do 2 hour checks/bathroom breaks on them and they don't they drug these patients up to sleep all day so they don't have to deal with them. The hallways reek of rechedness every day. You don't want your love one to die within a week of being at this place which seems to be the norm. This facility is much more than I expected. I recently visited a resident / patient at Mi Casa. The location is hidden just north, off of Broadway. It's well situated close to Banner Heart Hospital, as well as Banner Gateway Hospital, so that serious care is very nearby. That said, the facility and staff at Mi Casa constitute a true care and rehab center. There are several nurse stations throughout the facility. Just like a hospital, one per section of a wing. I observed lots of care staff during the morning I was there. Additionally, there are regular cleaning staff and the facility is kept nice and clean and not as cluttered as you often see in hospital wards. The patient rooms are reasonable and spacious, according the area. There is a nice rehad gym and equipment and it is likewise staffed with rehab professionals. For the patient I was visiting, who was in long term care, they are monitored regularly and the nursing staff was kind and gentle. The rooms are well lit and have windows in each and have access to walking areas outside when the whether is amiable. Overall, I was quite impressed with the care, staff and facility I saw on my visit to Mi Casa. read more

If I could rate them a zero I would. My mother was in this facility for 3 weeks and she called it the hell house. The first night there nurses walked off at night and for hours patients were unattended. Some managed to escape the facility. My mother was sent for PT to get her strength back up for be able to do Chemo for her cancer but she is now completely bedridden due to lack of care and activity. They barely worked with her. In the 3 weeks she was there she was only bathed once even after asking to be taken to the shower area many times. I called the Facility Director and he stated they would get my mom a bath and somebody came in and partially wiped her down but still five days have passed since that call and she still was not bathed. I picked her up yesterday on a Sunday and she was wearing the same clothes from 7 days prior. The food is horrible and mom was frequently awaken at night from people crying out for more food. Many of her items packed are missing including clothes, a blanket, and hygiene items. She told me that she overheard one of the nurses stating a patients' family member gave them cash for taking care of their mom and my mom was thinking she too should give cash to get better care. WOW!!! I also told the director this and he basically told me my mom must have misheard but he would look into it. My mom has stage 4 gastric cancer and she will most likely need one of these facilities in the future , however due to this experience she said she will NEVER allow herself to be admitted to another facility willingly again. The guilt I feel now seeing my mother in a more diminished physical capacity due to the lack of care than before is heart breaking. DO NOT CHOOSE THIS PLACE! MI CASA YOU NEED TO DO BETTER THESE ARE PEOPLE WITH DIGNITY AND SHOULD BE TREATED AS SUCH. type of care... good and bad. my mom had a stroke and is only checked on a few times a day. we had to "speak to " the staff as she was receiving no oral care and was choking on the build up in her throat, she can only move her left arm and can't talk... the head nurse donna needs to find a new job .. she has no people skills. They have no way to secure my mom in her bed and she has fallen out twice now and once when they left her alone in a chair. I tried to reach out to the director 2 times now, the 1st time no response the second time they asked me to call back in 10 min... poof they were "not available". Alisa is a great nurse and they have some great staff. that said they are UNDER staffed when it comes to the aids.. 2 aids for 35 rooms. Will be seeking information on a new facility if we have another issue. My mom has only been there since 04/03/2024.... read more

I appreciated a few caregivers at Mi Casa in Mesa, AZ. Dahlia was so kind to my Mom and really showed extra care! Emerita was also a compassionate RN that helped me to navigate my Mom's care for the long month and a half of rehabilitation. However, most of the workers are overworked. One of the best caregivers walked off the job one day while my Mom had a visitor! When I advocated for my Mom, the Director of Nursing, Donna, told me "STOP!" As in "Stop talking." That's when Emerita helped and quietly informed me she was calling the doctor to help respond to my Mom's health concerns. The Director of Nursing attempted to sooth my Mom by telling her, "You can leave anytime!" Sadly, this want true. Insurance and other factors prevent elderly from just leaving anytime. I hope Donna Majercin, Dir. of Nursing at Mi Casa will feel more compassion for herself. God knows these patients under her care really need compassion, as well. Over all, it was a difficult experience to see patients in need & not having their needs met at Mi Casa Nursing Facility. Most of the days I visited, there was a woman calling out for help and the very busy staff members ignored her. I brought it to a nurse's attention and she waved her hand and reacted, "She does cry for help a lot, she just does that." Then, I cried, too. I have a picture of food they served & decided it wasn't worth attaching. My Mom couldn't eat much & the kitchen tried to help, but we brought my Mom food a lot to supplement her diet. Kindest regards to anyone reading this & hoping for a good place for your friend, or family to have rehabilitation. It was better than "Mission Palms" but just know that workers are so overwhelmed that you need to watch closely what's going on with the healthcare of anyone you care about right now. read more

This place is disgusting. From their website and publicity you'd think it's a wonderful, cheerful, happy place. Complete lie. Walk through it and take a whiff and your nose is overwhelmed with the odor of urine and feces. It's awful. The cafeteria is gloomy, dark and dingy. I would never want to eat there. The patient rooms are horribly dated and worn out. The whole place needs to be gutted. Carpet replaced, walls painted and lighting improved. I would not allow a family member to go there. My grandmother stayed in this facility for one month and during that time had an AWFUL bed sore the covering her whole bottom, she must have not been changed or showered for a week or more. In addition she had UTI go untreated for weeks and had a fall out of her bed due to lack of safety and care. It's hard to find a good nursing home. This is a bad nursing home. Most Life Care facilities are only about profits and numbers. They only want Medicare and will overcharge your Medicare for every cent. Most staff here are foreign and just want to make American money. It's understaffed and very sad how human beings are treated here, to die. No one cares if you are in pain or even die here, they just fill out the right paperwork stating they did everything properly. They get good ratings because the management are like snakes and know how to cover things up. I'm hoping in the near future nursing homes are exposed or reformed. If I could give no stars, I would. This place is horrible. My sister was admitted with a broken back (and lung cancer). She can't sit up or walk but nobody helps her to the bathroom or shower. When she does get to the bathroom, she can't get help up for 45 minutes. It takes her 3 hours to get her pain meds. And the food is horrible and ice cold. It's inedible. Rehab for her broken back is a choke. With her cancer the doctor said she must be Admitted to the hospital if her fever is above 100.4. Last night it was 102.7 and they did nothing! I would bring my dog to this place! read more

Upon receiving out mom's personal property from Mi Casa we discovered her manual wheel was not returned to us more then half her clothes were not returned. Seems like all the new clothes we bought during COVID that they kept. I found paperwork where they ordered over $1,500.00 of personal hygiene and incongroence supplies in November and early December on her insurance account. Those items were kept as well. This is a terrible place . The food is not made for diabetics diets . There was no physical therapy for over a year. This should be known as a hospice type facility. You go here to die. read more

This is not a place you want a loved one. My father has very early signs of dementia but is mostly sharp and easy going. In a matter of months he has gone down hill with depression and I see why, I have posted the disgusting slop I've seen him red and the nurses are brutal as well as negligent. When he complained when it took them hours to help him into his toilet the told him just to go in his pants "they will clean it up" now since one of thier aids dropped a resident they have a rule that anyone who needs help going to the bathroom is to wear a diaper and go in that. I spent 5 days as a rehab patient in Mi Casa. All I can say is BE YOUR OWN ADVOCATE! In the 5 days there the following took place: call light went unanswered for 45 minutes, I finally got myself to the bathroom. After that I never bothered to use the call light, just got there myself. Then the bathroom door was locked. I used the call light for help. No response. I had to use the potty stand for 3 days. I told everyone about the door and the potty stand. The potty stand was not emptied for 3 days. The weekend staff was mostly invisible. One aide in particular was always telling me they were short staff. I finally had an admin person who was helping out and told her about the bathroom and how NO ONE knew how to unlock the door. She left, came back with housekeeping who UNLOCKED the door. Just like that. The food trays were left in my room for hours on end. I was supposed to sit up twice a day and walk twice a day. No aide EVER did that. On a weekend night I used the call light and an aide came. I requested to see the nurse. Three hours later still no nurse. I called again, same aide same excuse When the nurse came at 10:30 I asked if she got the requests. No. The morning nurse came in with meds. I told her everything that had happened. She was appalled. Said she'd tell appropriate persons. The aide that came with her took my breakfast tray away and had no problem dumping the potty stand tray. Later the OT came in and again I told her what was going on. She said this wasn't the first time she'd had complaints against staff. She too went up the chain. Later the Director of Nursing came to visit. Told her everything. She said they were NOT short staff on the weekend. She was going to take care of everything. I was discharged the next day, which I wanted. There were several nurses and aides that were caring, proficient and kind. Several that were below par. Therefore I repeat to get the care that is needed please BE YOUR OWN ADVOCATE or HAVE A LOVED ONE BE AN ADVOCATE FOR YOU. read more

This place is horrible! My dad stayed here for 3-4weeks earlier this year. He was there to recover from his hip surgery. My dad has advanced dementia/ Alzheimer's and he is non verbal.He has to be spoon feed and needs help with giving him his beverages. We noticed how skinny he started to look. My mom stayed the night for the first 1wk and everything was fine, but after that is when we started seeing his weight loss. We also noticed when we would go to feed him or bring him lunch, he would look at the food and devour it, like he hadn't eaten in weeks. My mom went to visit couple of minutes after his lunch and she saw the food container in the trash and she opened it to see how much he ate and the entire meal was tossed with only 2 bites of his quesadilla. Good thing she brought him food from home because he swallowed it and drank all the juice she brought him. I (mom does not speak English) brought it to the managers attention and her pathetic response was "maybe he wasn't that hungry and we can't force him to eat or drink". Mom also walked in on my dad what she thought was chocolate around his mouth and turned out to be feces. She called the nurse and they cleaned him up real quick and apologized to mom.My dad was starving.. I had dad taken out of this facility after the non feeding issue. Mom told me about the "feces" incident after he was home. She was afraid what I was going to do if I knew sooner.. .The staff and ESPECIALLY the nurses put on this friendly, caring, don't worry about anything face, but as soon as those doors close it's hell.. This place needs to be shut down! read more

This is a horrible place for anyone person needing help. A lady was repeadly yelling out "help me" finally a nurse came in her room told her to stop and closed her door. I also heard a different person complaining for ten minutes. I could smell dirty diaper. All this on the first night my mother came in . I am definitely going to get her out of this horrible place. Her medication was scheduled at 9pm and finally after asking several times she received her medication at 10pm. All this happened at Mi Casa nursing home in Mesa on December 26,2019. read more

This institution should not be in the business of providing services to anyone. There is no leadership. Many of the staff seem to want to do the right thing but they have no idea of how to accomplish their mission. Their plan for my brothers progress was a joke. After eight days of agonizing pain they finally began to treat him. There was no cooperation between medical staff and the right hand could not even find the left hand. He was ignored and lay in his secretion from his bed sores. After three and half weeks my brother went back in the hospital because he had so much infection and pain. This is a horrible facility. Staff not caring about patients, all patients being feed the same meals even if placed on special diets from Drs, staff not knowing their patients ie when they are sick or even knowing they are sick not reporting it to the In house dr for evaluation and treatment. If you have to put a loved on in a home I would do my research and definitely bypass this facility! Ask any resident they will tell you how the party are really treated behind close doors. read more
