The quality of care of the Physician as well as the physical therapy at this location is horrendous! The doctor simply does not care, it is just a money billing factory, you cannot get answers from The Physician nor does he give you perspective and insight on progress. It took me 3 days to get a phone call and some sort of report back on the physical and occupational therapy for our mother. We ended up removing her from the facility abruptly after seeing what happens there after 7:00 p.m. I would highly recommend if you have a loved one that is placed in this facility visit it several times unannounced you will know what we mean. Our mother was admitted to Well Springs SNF following a hospital stay and was supposed to receive physical and occupational therapy to help her regain strength and independence. The evening she arrived, she was still able to walk. Unfortunately, the therapy she was supposed to receive--at minimum 3-4 times per week--rarely happened. Sessions were short, inconsistent, and often skipped entirely. Instead of helping her improve, staff frequently left her lying in bed or sitting in a wheelchair for most of the day. Over time, her muscles deteriorated significantly, and walking became nearly impossible. Eventually, she could no longer take steps without the assistance of a walking device. Basic care was severely neglected. She was not properly bathed or given adequate peri-care. Staff tried forcing her into Depends, despite the fact that she had never needed them in her life. As time went on, it became obvious this was simply because staff did not want to assist residents to the restroom. There were many times she would call for help and wait 30-45 minutes before anyone responded, often resulting in humiliating accidents. Her medications were mixed up on several occasions. We also witnessed feces- and urine-soaked bedding and clothing being left in her room and even thrown onto the floor. Our family met with management, including Heath, multiple times, but despite plenty of promises, nothing ever changed. We ultimately removed her from the facility, and within 12 hours she had to be hospitalized with a severe blood infection caused by unsanitary conditions and neglect along with bed sores on her back side. The night staff were by far the worst. My mother began recording the amount of time it took for someone to answer her call button because the delays were so excessive. She also suffered falls there--despite never having fallen before in her life. On multiple occasions, night staff left her lying on the floor with only her own pillow placed under her head while waiting for another employee to help lift her, sometimes for 20 minutes or longer. There were also several times our family arrived and found all the doors locked, forcing us to wait extended periods before anyone answered. Each visit, our mother appeared more neglected and distressed because of the poor care she was receiving. Late at night, children--presumably employees' kids--would run through the halls loudly, disturbing residents. Her water containers were frequently left empty and dried out. This place was an absolute nightmare. We personally witnessed verbal abuse from staff toward other patients in common areas. Based on our experience, I strongly urge others to stay far away from this facility. Their negligence nearly cost our mother her life. read more

I am a CNA of 20+ yrs that picked up a PRN shift Christmas Day 2024. The nurse I worked with was so awful & unprofessional I returned to Wellsprings the following morning waiting for them to open to report this nurse to HR. The nurse I worked with was VERY unprofessional in his remarks, attitude, & tone toward me. This nurse did not help me with fulfilling patient needs when I specifically ask him. This nurse made a point to tell me 2 times that "ya know, I am a good nurse". Never asked him if he was, this came as responses to asking him for help by grabbing more bleach wipes when I was in full PPE in a room assisting a resident. He never helped me once. This nurse created issues between a patient & myself that did not exist. He would not let me speak. He believed his conclusion. The resident corrected the nurse in his wrongful accusations toward me. Another incident while making rounds, I knocked on a residents door to check on her again. This nurse was angry with me because he was in the room I "disrupted him while talking to the resident". This nurse was very egotistical. I didn't understand why he was so upset with me. I had not done anything wrong. This nurse scolded me again & told me to stay out of this particular patients room that the issue was with. That I'm never to go back in there. I didn't understand why or what happened. A couple minutes later this nurse completely flips out & starts talking VERY loud in the common area where all the residents & their families could hear & see him and loudly said that "he doesn't trust me with any of these residents!" Then this nurse, not a manager, not HR, but this LPN nurse tells me to just leave now & go home that he doesn't trust me, to stay away from all the patients!! Of course it's nearing the end of the 12 hour shift &another CNA had showed up. But everything was ok until he knew he had to cover his lies & get rid of me so the truth wldnt be exposed. He wldnt let me speak or listen to me. He would talk over me or cut me off. I was absolutely shell shocked. This was my1st time working at this facility & I did a stellar job, especially having the limitations of it being a busy holiday shift & many families were visiting. I did my job & I ALWAYS assisted & cared for the residents as if they were my own family. The issue was a nurse that needed to cover himself & the mess he alone created & wasn't going to hear about anything else. He could have simply said he was wrong about what he thought had happened & I would have said it's ok, no big deal, water under the bridge. Now, I am happy to write this truth on the staff & the lies they will create to shift the blame &create scenarios that never happened. The Nursa app has evolved & now we can see the reviews left on our performances. Wellsprings of Gilbert has even left me a negative review about my quality of work. I was very angry at how I was talked to & treated by this nurse during my shift & I expressed that to Wellspring HR. I told them to watch their cameras! The resident did correct the nurse of what he thought happened or said but he didn't want me to speak to that resident again. That's when he flipped out. I am absolutely shocked that their management believe lies over truth. There were so many other incidents that happened with this nurse during my shift I would just shake my head in disbelief & continue doing my job. One that I will note is one resident was new to Wellsprings from the hospital. This resident had a couple incidents that pointed toward having C-Diff. I made the nurse aware & suggested to put a contact precaution on the patient's door so if in-fact she was positive for C-Diff, anyone who entered would know to take the proper precaution & prevent spread as it is highly contagious. I ask the nurse 2 times about doing this. He was angry at my suggestion &said he didn't like me telling him what to do. I told HR about this also. I explained to the nurse and HR I'm only reporting from previous experiences I have had with patients & hoping for protection by taking an extra precaution. This nurse never took a test to send off to see if the resident was positive nor did he put up a contact precaution sign at least until a test could be done to be determined. The resident did have several episodes & symptoms that pointed to C-Diff. I just report what I see. I was met with being scolded with egotistical remarks. Shame on you Wellsprings for turning on me. I brought this to your attention! I am a good, honest, hard working CNA. Yes, I was very angry & said colorful words as adjectives of behaviors I experienced, never direct name calling. I shared my experience with other healthcare management & they all agreed that I should have reported Wellsprings & that nurse to the AZ Board Of Nursing. I regret not doing that. I'm not perfect but I know God saw ALL of it and I can stand before Him blameless in your accusations. read more

I will preface this with the fact that I am an 82-year-old woman who also suffers from Parkinson's disease.When I was admitted I could barely walk. I went there for rehabilitation. I had just spent four days in the hospital for a severe infection. The rooms at the facility were dismal looking and appeared not to be clean. since I just had an infection, I was very leery.They did not have sanitation wipes when I asked for Them. At one point, I was also suffering from dehydration so I had no alternative but to call 911 And get treated in ER. When I was let back into the facility, I was accosted by someone that worked there. He was yelling at me-and threatening .to throw me out. I left the facility the next day and reported the incident to adult protective services. the facility denied all knowledge Even though there was a witness. Most of the CNA's and people that worked in therapy were very kind, but the upper management was nether caring nor honest. Unless your loved one has an advocate, I would steer clear of this facility. read more

The location and facility are desirable; however, that is where the positive qualities end. The treatment of patients, professionalism, and quality of care are all inadequate. Staff members consistently lack professionalism, often ignoring patients and family members' presence, whether they are entering the building, walking by, or sitting in common areas. My parent repeatedly described the staff as unresponsive. We were denied access to outside medical care, our repeated requests for a care plan were ignored, and we received no updates on my parent's progress. The case manager would not return emails or phone calls. Furthermore, the catheter bag was left dirty and leaked for five days before it was finally replaced. (The PT and OT medical team were great and are not included in this negative assessment.) Unfortunately, we could list many more issues. The stay was very disappointing, and our parent's health worsened while they were in their care. We moved to a different facility and are grateful to have found a place that cares for its patients. read more

3 days ago New If I could give this place negative stars I would. My father went there for a few weeks until he passed there alone and in the restroom alone! Tge treatment of him was abhorrent! His sheets were never changed a d had feces and blood on them, his bathroom was not cleaned one time. This place was a pig sty! One of the first nights he was there I stayed with him. I had to sit him up, lay him down, pull him up to the of my ability alone. I pressed the call button multiple times and was ignored. I went out looking for a nurse or CNA and the place was a complete ghost town. I did eventually twice find a nurse who said she would be right there and never came in to help. I changed my dad's adult diapers and wiped his butt! One time the nurse came in and he told her that he'd had a bowel movement and she looked and said, "it's a shart" and refused to change him. I changed him. No one wants to sit in that! When no one stayed with him he would press the call light and be ignored, the light would be left on all night. My dad would call out for help. The nurse that coincidentally thought that it doubled as a night time daycare and brought her 2 kids who were noisy at night and bothered the residents, told my dad if he kept calling out there would be repercussions. The next night she refused to give him his dinner tray. Thank god my brother was there and questioned her. She lied and said it wasn't his but it was literally the only tray she had t passed out. My brother took it and fed my dad anyway. The morning my father passed, they had him crying because they didn't even bother to cook the eggs he had ordered for for breakfast. My dad was having a hard time breathing and he was on oxygen but he wasn't even being given his oxygen. When he did pass, they lied about doing life saving measures on him. I have pictures to prove all of what has been said in this review. When we were finally called after someone decided to do their job and check on him, they didn't clean the bloody toilet paper strewn in the bathroom floor nor the bike in the toilet. My fathers head looked like it had been bashed in from a fall on one side. We walked in to visit his body and the nurses 2 kids were there staring up at us. How very inappropriate! Then later I stepped out of the room and the place was a ghost town, no nurse in sight! They were out front with male visitors talking and goofing off. Once the mortuary arrived. The nurses tried rushing us out of the room. Very unprofessional place. No one that works there treats you or your loved ones with any dignity or respect! It's disgusting! My father was admitted to Wellsprings at 8 pm from a local hospital. He is a brittle diabetic and takes quite a few medications. We were told that they wouldn't be able to administer his medication until the next day so I was asked to bring in a dose of his evening meds. Which I did in a baggie (no bottles) and no questions asked. The next morning, breakfast was served and his blood sugar was not checked nor insulin given. He also wasn't given any of his scheduled meds because they were having "issues" to get them from a pharmacy. Lunch is served no blood sugars checked, no insulin given and no scheduled meds given. I spoke to the director of nursing and was given a million excuses of why his meds were not given. Again asked to bring in his full bottles of his home meds. I did just that and still meds not given. The director came to speak to us with a million excuses to make it right. 3 pm no meds, no blood sugar check and no insulin given. Our only option was to sign out AMA. When asked is they could check my fathers blood sugar I was told no. Right back to the ER we went and his blood sugars were 680. ABSOLUTELY NEGLIGENT CARE!! We have filed some additional reports to the state. Also the front desk young lady is extremely rude and disrespectful!! Please do not send a loved one to this facility. She came in unable to stand on her own and will be released tomorrow, walking and moving on her own. She actually looks and moves better than she did in the last year. I have to give the therapists and one of the nurses a 5 star but the rest of the staff are lucky to get 1 star. I will not go into details of all I witnessed while visiting an elderly family member and all the things I bought to the attention of CNA's, RN's, case managers, Director of nursing, and CEO that needed attention and answers. I have never been around so many unprofessional, uncaring, abrasive, snarky, rude, poor attitudes, avoidance of issues, poor communication, lacking general people skills, condescending, deflection, and confrontational. The DON even went as far as deflecting a concern I had on a missing medical document and proceeded with LIES that I threaten her and accused me of going behind the nurses desk. The way all my concerns and question were handled, was the DON banned me from visiting my family member. I don't need to defend myself for advocating for my loved one. I really wish I took pictures with the first concern I had. I'm only speaking my truth. I was transferred to WellSprings after having aTIA and a fractured shoulder. I spent 7 weeks in there over this past holiday season. I can't seem to understand these bad reviews on here. I had the most wonderful therapists working with me .Three of them and some of their co workers on their days off. I had a large private room and sometimes I had 8 guests at a time in there making noise. Not ONE person complained or chased them away. The food was very good and when I asked for something else they obliged. I plan on visiting the staff very soon to thank them again for a pleasant experience. read more

There is a long list of reasons why we transferred my mom to another facility as soon as possible this week. The weekend staff was rude, and obviously did not want to be there. My mom was not given water from Friday evening to noon the next day and I got it for her. The ice and water machine was filthy. The room was filthy as well, sticky floor, rubber on the baseboards were laying on the floor, the AC unit has mold in the corners, and the bathroom was disgusting. The TV didn't work, and the nurse stated maintenance would handle but "they are never here". There was no one at the front desk all weekend, we just walked in several times. She has a wound on her hand and the dressing was never changed. It's a very scary situation, people depend on a place like this to take care of their loved one. We transferred to Center of Val Vista and it's night and day difference in the care, cleanliness and professionalism of their staff. read more

My loved one stayed here for a week for cdiff. No meds for the first three days. Facility recently sold and was informed they were waiting for medical records from the previous company. A sign was placed on door indicating transmission precautions and ppe was placed outside the door but staff did not put on required gown to avoid spread to other patients. To the patients that may have contracted cdiff by staff failing to do use ppe, sorry. I reported them to the state health department. read more

He has to ask more then once for his pain meds. He ask for it, they tell him ok then go and sit on their ass and play on their phone. This man is Hospice and is in pain. My father was here for rehab after a fall. He was in fine health when he checked in - just needed rehab for a broken hip so that he could return to independent living. He died about two weeks after arriving. They never took him to a hospital (which is practically next door to the facility) for proper care despite our request to transfer him for proper care, as he contracted Covid while in their care. Because he contracted Covid at the facility, we weren't able to see him the week that he passed. When I called to get details of why my father wasn't moved to a hospital if he was having issues, I dealt with extremely rude and condescending staff. There is a lot more that I could put here, but suffice it to say that I cannot recommend this facility and would give it negative stars if I could. read more

The place looks nice in pictures but furniture is old and scraped up The doctor wasn't on-site and when I picked up our loved one no Doctor or nurse discussed anything Run fast DONT have your dear family members stay at this place it's traumatic I heard only bathing on Tuesday and Friday and they lie if asked about having anything to even warm up a patient. I spent the last month at Wellsprings for a broken hip and shoulder and found it be be an outstanding faciity. The entire staff of Doctors, Nurses and CNA's were amazing, always smiling and always positive reminding me I was special to them and encouraging me each day. THe Physical Therapists and the Occupational Therapists taught me to walk again and gave me hope for the future. My father had a stroke. This place came highly recommended. I have to say, I'm sorry my family didn't keep my dad here. This is a skilled nursing facility and since it's a smaller place you get one on one care, BETTER care. We moved my dad to an acute care and he got BETTER care at Wellssprings then the acute care. The nurses, the staff, the family atmosphere, this place has it. The food is even eatable!!! They kept my dad motivated, socialized and most importantly they kept him moving. He regressed upon arrival at the acute care. I would recommend this place HIGHLY!! read more

My grandma stayed at this location for over a month and on the last day she found a large roach. When she called for help to kill the roach, they apparently couldn't find it and left her room. Later on in the night, she found it next to the heater. She again called for help and the staff member said she couldn't kill it because she was scared. She left the roach in my grandmas room, a women who isn't stable enough to walk. If you want your grandma staying with roaches, Wellsprings of Gilbert is your place! read more

What a pleasant visit. I was able to tour this facility today and I left saying this is where I wanna go should I need therapy and/or rehab!!! The grounds were clean, the staff was smiley and I got to see the super cool Aquatic therapy pool. I so wanted to try out the under water treadmill. I have serious concerns about the safety, hygiene, and overall well-being of the residents here. My grandma stayed here for a short period of time. We discharged her early for many reasons, one of which was that she was told to stop calling for help to the bathroom because they "couldn't keep coming to help" and that she should "just pee herself in bed." We found her saturated in her own urine as a result many hours later. The durable medical equipment was also broken. Her wheelchair did not have legs on it; when this was pointed out, they found two mismatched legs, one of which was broken but tied together with a rope. Her bed was duct taped. My grandma was also instructed by the nurse aid to not watch TV at night (she was having trouble sleeping) because the nurse aid was "trying to sleep." There seems to be a lot of staff turnover, which does not surprise me. No human being should ever be treated the way my grandma was. I would never recommend this place to anyone. read more

We had to move our Dad here during a stay that was not going well with another skilled nursing facility that is nearby. Lara was extremely helpful and accommodating and did everything she could to get him moved quickly. He is still there now and has been there for about 3 weeks. He really likes being there, and I have to say it just "feels better" when we walk in. While he said the food may not be as good as the other place, he is definitely happier with the therapy and staff which is what matters the most. We have started to work through finding his next place which will be a long term care facility, and case manager Laurie has been super helpful with recommendations and making sure we are finding a place that will be a good fit so he doesn't have to move again. I know there are a lot of options out there, but I would not hesitate to recommend Wellsprings to anyone I know. My dad has been at this facility twice. On his first stay he was very weak and not in the best of health when he came in. The nurses were great with him. It was nice seeing the same nurses each day who got to know my dad and formed a great bond with him. They provided him aquatic physical therapy which helped improve his strength and daily movements. By the time my dad left he was able to walk with a walker to the car and get hisself in. The nurses and staff were friendly, knowledgeable and caring. That is why we chose Well Springs again. read more

I recently spent 3 1/2 weeks at Wellspring of Gilbert and I was very happy with the experience overall. While nobody wants to have to go to a rehab center, I would strongly recommend this one if the need should arise! The facility was beautiful, very clean, open and spacious. The rooms are large, private and comfortable. The staff was awesome! The CNA/nurse to patient ratio is very good. The CNA's were amazing, always smiling and happy to help with whatever needs came up. I felt they were attentive and responded to the call button as quickly as they were able to, keeping in mind that I wasn't their only responsibility. The nursing staff was also very good and knowledgeable. They were responsive to my needs and worked well with the doctors to make sure I was getting the best care possible. My mother spent several weeks at Wellsprings after being hospitalized for chronic pulmonary and cardiac issues. It not the place for someone with these conditions to recover and strengthen themselves before returning home and Wellsprings should not have accepted my mother as a patient. The administration was in flux at the time - I believe that my mother's case manager and perhaps someone else were let go while she was there - and it showed in staffing and level of attention my mother received. The therapists, nurses and aides themselves were warm and competent but there were not nearly enough of them to go around. For weeks, with the exception of an hour or two of therapy each day, my mother sat alone in her room, and was not taken on walks or outside, which was important to her recovery and to slow the onset of edema. She regularly waited for 40 minutes and longer for someone to help her to the bathroom. I understand that Covid required facilities to make adjustments, but even accounting for that, I feel as though Wellsprings simply took the insurance reimbursement money and did very little in exchange for it. She would have been better off at home with therapists brought in. It is shameful. read more

My mom had been at this facility a few times. The last time she was there, she got an Urinary Tract Infection. She was in such bad condition that she could not ambulate...she could not transfer from bed to wheelchair with even the help of 3 people. I told the nurse there that I suspected she had an UTI because of her symptoms and I wanted her to be sent to the ER and two nurses refused! I was adamant about her getting tested for an UTI (even though the nurse kept telling me she was sure my mom didn't have a UTI). The following day they finally did the test themselves after my prodding which took 24 hours to get back the results. The test came back positive for an UTI. She had an UTI for a few days (or maybe longer) and they wouldnt let her go to the ER! The day after they discovered she had an UTI, with me pressing them to test her, she was being discharged home and they were sending her home in a condition where she couldn't stand or ambulate!!! I told them I wanted her to be sent directly to the hospital because I couldnt bring her home in that condition. They said, absolutely NOT...you have to take her home. Three of them tried to put her in the car and mom my fell on the ground outside their facility!!! Previous to that they wouldnt allow me to call the ambulance to take her to the ER and get treated right away as she was being discharged! Once she fell on the ground and three of them couldn't get her up, I took matters into my own hands and called the ambulance and had her taken to the ER. It blew my mind that they were allowing my mom to go home in that condition. In the hospital they said that my mom had an advanced UTI and she was close to septic which could've killed her!! This place is disgusting...I would not trust my loved ones life in their hands. The nurses think they know it all and are control freaks...they do not listen to family members who know their loved ones better than they do. I would never ever trust them again to take care of anyone I know. read more
