I stayed here for a month following a hospitalization for a knee infection that became septic and required continued antibiotics. I thought my care was very good. The people were really nice and they worked very hard. You need to realize; however, that this is not a resort. They assist a lot of people, with different needs who require precise medical care. There is a degree of "regimentation" with med time, meal time, shower day, therapy time. They do try to have some fun with posted activities to try. I am now looking at additional surgery on my knee and I would not hesitate returning to The Center at Val Vista for my recovery. read more

Although we did not select this facility for my husband's care, it remains on our short list. It's a newer facility. Patient rooms are on upper floors. Rooms are nice, but don't offer refrigerators and microwaves. Family can provide them. They offer personal services as well as quality health care and therapy. He has a cut in his forehead and he was sitting at the nurses station at 7:30am a gown freezing. I was told he was there since the fall. They refuse to take responsibility for their lack of care. One person was stern with an elderly person when I walked in. Please do not go here. read more

They took AMAZING cate of me for the two or three weeks I was there. The facility looks like a five star hotel. It is beautiful. The staff is amazing, especially Nurse Melody who whose care was exceptional and so was her comforting manner. She is as good as it gets and I am so thankful for her. I am a nurse and a Director of a surgery center. My mother has been at the Center at Val Vista for 6 weeks post hospital stay for kidney stones, she was wheezing when we got there and I brought it up multiple times and they didn't even listen to her lungs until I requested a chest x-ray and they found she had pneumonia and a collapsed lung, they never gave her an incentive spirometer, so I brought one in myself to help her recover. She has been in physical therapy there every week day and still has not progressed over 2 steps, is bed bound 2 person max assist with hoyer lift and the CNA didn't read her chart and got her up in the shower by herself to stand and she fell (surprise) whole leg bruised knee swollen, again, I had to insist on x-ray. I was in changing her diaper one day and all I had to do is lift her one side to get the tabs out and the doctor walked in (Dr Veege I think, Asian descent) and I asked if he could help turn her towards me so I could finish getting the tabs out and he said "no" I asked why since she only weighs about 100 pounds, and he said " I can get an aide for you" Yesterday, they put her in a wheelchair and left her there, I get a call at work that the wound care nurse came in and there was blood on the floor from her foot from trying to put it on the foot pads and now she has her heal open and they will have to do wound care on that. She cannot feed herself and the chart has written bite size food and I also circle it everyday and they bring a full chicken breast not cut up and leave it in from of her for 4 hours from lunch and then document that she's not eating. I reminded the CNA Amanda that and the next day she did the same thing. She eats when I cut up her food but she cannot do that for herself and they are aware. We are moving her to another facility and I am filing a formal complaint with the State. Don't take your loved ones here. read more

My mother has had to stay twice. Her nurse Hannah has been so great for her and to her. My mother can be a crabby old lady sometimes and for her to like a place and a person means it's good. The treatment has been good and my mother says the food is good. The communication is good with the staff as well. They called me frequently when there were changes or for updates. Additionally, the facility is clean and looks new. The only 2 things we wish that could change is for patients to be able to eat in a dining room if they wanted to and not just in their rooms all the time and better activities. If my mother needed rehab and care again we would definitely recommend The Center at Val Vista. Would rate 4.5/5 stars. read more

My mom was in their care due to a hip & pelvis fracture. I did tour many different facilities and this was the nicest one. I was told they have the lowest patient/nurse ratio of 10-12 patients per nurse and that's not true. They were at 17 patients per nurse, and the nurses were very tired and run down. Some would get rude and make comments about not having the time, and how short staffed they are. Unfortunately, that's probably the case everywhere. We had an incident, and they had a very dramatic response to the incident. They talk very condescending to patients. The case manager told my mom that she sounds like a "child". They are missing compassion for their patients. My mom was given a note from Dr Monday morning that she's good to go home if she can walk 200 feet. They would not discharge her until Wednesday. The only reason they got any stars is because their physical therapy team is absolutely amazing. read more

I am currently in the long process of rehabbing for 2 broken tibia plateaus. After a short initial stay with another facility, I was transferred to The Center for more extensive care. I was praying they would have a bed available and they did. I had heard good things about the facility, and although no place is perfect, it did not disappoint. The rooms are nice and clean and the regular staff caring and compassionate. I want to give a shout out to some of my favorite nurses - Rosie, Jackson and Ty - and CNAS - Amanda, Paula, Mahko and Abby and Sarah. Our dad was there 1day. It was terrible. We moved him to another facility. He was so scared. They were not comforting. He was ignored, didn't receive his medication and was pretty much trapped in his room. He was vulnerable and was not cared for. They didn't answer the phone. Tried to call his room, nurses station and operator. We sent a message looking for contact never received a reply. If you care about your loved one, do not choose this center. read more

My father had the best care at this facility. 10/10 The therapists got him out of bed and moving the first day. The case manager was very helping organizing a safe discharge. The staff were attentive and gentle with my father, they checked on him frequently and he was very happy there. The nurses answered the call lights and the facility was very quiet at night. My Dad was a patient at The Center at Val Vista following a 2-week hospitalization at Mercy Gilbert. He had sepsis in January and needed 6 weeks of iv antibiotics to make 100% sure the infection was gone so he could have an aortic valve replacement. The iv antibiotics needed to be administered around the clock 6 times per day. The care my Dad received was hit and miss. There were certain nurses and CNAs who seemed to care about the patients, but with a ratio of 16-1, no one actually gets the care they need. I estimate that a patient actually gets maybe 15 minutes of nursing care per day. Some of the CNAs were completely disrespectful and clearly need to be in a different line of work. There were several things that happened during my Dad's stay that were seriously concerning. They ran out of one of his IV antibiotics the first week, the oxygen delivery system went down/failed twice, and the elevator went down. If we had not have been there every single day, we would not have known about these issues or been notified by the staff about these glitches in care. For someone who is oxygen dependent, not having oxygen is an absolute emergency! No one thought to temporarily put him on the oxygen concentrator while they fixed the problem. I think the worst part of his stay was the inattentive attending doctor that "cared" for him. He was not receptive to my Dad's concerns at all. My Dad told the doctor that he felt something was seriously wrong. And he was right! On the last day of his stay, my Dad had to be taken back to the ER at Mercy Gilbert where he was diagnosed as being in cariogenic shock. He was immediately transferred to the ICU at Chandler Regional and he passed away 2 days later. One week after my Dad passed away, someone from the Center at Val Vista called to check to see how my Dad was doing. I informed her that he had died a week prior and she didn't even change the tone of her voice to express sympathy. No matter what skilled nursing facility your loved one is at, you absolutely NEED to be there checking on them every single day, ESPECIALLY if your loved one is elderly. Call buttons are constantly going off and it will take staff 15 minutes to check on a patient. How is this acceptable? We saw staff sitting around talking or on their cell phones while calls went unanswered. What if it was your mother/father/husband/wife that needed help? How can you let them wait that long??? I will be reporting this facility to Medicare. There's no way that they deserve the rating that Medicare gave them. I think most skilled nursing facilities are under-staffed and I toured 6 of them before settling upon this one. I wish everyone had better options. If you need skilled nursing care for an elderly person, look at other options! I am not the first family that has lost their loved one shortly following their stay at The Center at Val Vista. Let that sink in before you make a decision about where to obtain care. read more

Let me start by sating if I could give this place a zero stars I would, I placed my dad at this facility to get therepy to assist him at walking better, they never got it up in the 3 weeks he was there and when he left he couldn't even walk. He had deteriated and never walked again. I went to visit him and always asked how he was doing they always told me great things, I walked into his room and there was urine all over the floor, I went and found the nurse she was peeved that I even had asked her to clean it up. We toured some other facilities in the Mesa/Gilbert/Chandler areas and no where else compared. Sadly, my mother had to go back to Val Vista for the second time in 12 months. So that's not such a great thing but the great news is that Val Vista is a wonderful facility. Super new and super clean. Wonderful staff! We really enjoyed the caregivers Kayden and Marcy. They did such a great job. My mom said the food was great. She stayed for three weeks and was finally ready to leave, but I think she got great care while they were there. The speech, physical and occupational Therapy departments were also amazing!! read more

I had no idea a place like this existed till I needed one. Thankfully, The Center had an available bed for my stay after the hospital. The entire staff is great. Kyle, the wound specialist, was just awesome. Gwen and Mario in PT worked with me daily and had me able to leave in just a couple weeks. Aspyn, was my favorite CNA, just a very nice person. My mother was transferred to this facility from Mercy Gilbert Medical Center. she was recovering from flu/ pneumonia complications. In the forty eight hours she spent here, she was only attended to by PT once and left mostly unattended to by the rest of the staff. I found her lying in a mess of vomit and I alone had to clean her up with absolutely NO help from staff. The aid said, " I already cleaned her up twice". There was no immediate concern for her condition and I insisted that she be transferred back to the hospital, where it turned out she had kidney stones, which was the cause for the severe vomiting. I don't blame the staff for my mothers kidney stones, obviously, but I do blame the poor attitude and lack of staff to properly care for patients that need more attention. I found the Case manager to be unprofessional and unhelpful. It is a newer facility and the online pics are nice, however the care is sub par to say the least. read more

Rate This... My 90 yr old Mother was sent here in a back brace on 24 Oct 2021 by Honor Health Osborn to recover and rehab from an L1 compression fracture. Staff at "The Center at Val Vista" were unfamiliar with the brace and how to adjust it. PT seemed to be going well, and then... On 5 Nov 2021, Mom was taken from "The Center at Val Vista" by ambulance to the Emergency Room (just down the street from "The Center"), at Mercy Gilbert Medical Center, where she was put in isolation, treated with 3 different antibiotics simultaneously, intravenously, as well as receiving a Blood Transfusion, and she was Bleeding Internally. The ER Physician said to me, 'Your mother's Life is in Danger, she's VERY ILL right now'. I watched aghast, as the E/R Staff work feverishly to Save my Mother's Life. She Looked DEAD. Tests showed and confirmed that Mom had Sepsis, and FOUR Hemorrhaging Ulcers. Mercy Gilbert Medical Center Saved Mom's Life and they took Great Care of her. She was released on 09 Nov 2021 to a Different rehab facility where she was treated with care, dignity and respect. What rating do YOU think I should leave? read more

I will always feel this facility is responsible for my mom's death. She was moved there from Mercy Gilbert. The next morning she would not wake up. The doctor claimed it was her medication. We asked for her to be taken back to the hospital. They did not get her there until 2am the next morning. She had MRSA. They were so irresponsible and time is so important when an infection sets in. This was preventable if they had only acted in a timely matter. Do not put your loved ones here. read more

This facility has the potential to be a good facility. they are severely under staffed and rely on a CNA registry, a few of them should not be in that field, they only do what they absolutely have to, the good ones are over worked. my husband sat in a wheelchair for 8 hours in severely soiled briefs with an open bedsore, while he was next to his assigned CNA playing on her phone. Physical therapy was assigned for 5 days a week, he rarely had that, but I'm sure insurance will be billed for 5. I was asked on several occasions to spend the nights, and stay with him all day to make sure he hadn't fallen out of bed. He sent sent to Chandler Regional twice for falling out of bed, making sure he didn't have a brain bleed. The employees they have are stressed and overworked, as many health care facilities are but a wage increase would help keep the good ones. read more

Reading these reviews, I am sure my recovery was mainly due to my own care. The PT and OT people were great and the Craft area went over and above their job. The place supplied the wheel chair, bed side cammode. That way I didn't need any personal care. People read the reviews. Decide if they can recover with little help. More intense care would not be supplied here. read more

Sadly we had to use the Center at Val Vista as my wife had a fall and couldn't walk. I remember the PT department was very good with her back in 2917! It looks like it has not worn since it began this month 6 yrs ago. So what has changed since then? Ths staffing appears to be somewhat less and CALL button has a longer response time, better than 20-30 minutes The current staff seems to be trying their best, however. The food which wasn't the greatest, has taken a nosedive!!! For me I would not go there, but my sweet wife has never been a big eater They serve very simple meals like hamburgers, lots of Mac in Cheese, tough chicken fingers, PJ sandwiches, deli sandwiches, ham sandwiches with most of the time, tepid hot foods and little diversity was noticed. Interestingly, the staff appears almost intact in their PT department. Do not get me wrong, the newer staff are very nice. The A.C. has been making me so miserably cold that I wear 2 tee shirts, first a short sleeve one, then a long-sleeved winter one, followed by a winter sweatshirt, and then a zippered ski jacket!!! I never take off my hat or my head would get cold. (unbelieveable!!!) I would give a 2 1/2 star rating but halves are not used so it will be a bit higher due to the pleasant people that work there, so it will be 3 stars. dropping it some since my visit in 17'. My wife has lost 9 pounds in the past 3 weeks!! read more

Our father was a patient there recently. Due to Covid no could visit so we had to rely on updates from the case manager. After one 1 week we were told he would need care 24/7 if released then. Week 3 we were told he would need care for a couple of hrs a few days a week. They released him on week 5 having my brother pick him up. He was so sick we had to call 911 to take him back to the hospital in order to get clearance for home care. He was and is totally bed ridden and needs care 24/7! How they can release someone who lives alone but now needs full time care without telling his family members is beyond me!!! Obviously they don't even communicate with each other for the case manager to think this was an acceptable way of releasing a patient! I really don't have the words for how bad this really was! Totally Unacceptable!!!!!! read more

"I know your sugar is at 250 but your insulin isn't scheduled for two hrs 3 hrs to respond to a need for band aid or I need water for these pills you gave me no diabetic diet for diabetic patient being monitored closely. no restrictions on food brought in. this is policy. counter to what the goal of monitoring sugar in the system horrible lackadaisical place. putting in time filling out charts read more

Zero Stars!!! My mother was sent here after a minor fall in late January. She was fully independent, driving every day to run errands and lived by herself in her own home. After three weeks she was released but had to go to a group home because she was a complete invalid, riddled with colitis, UTI, bedsores, and extreme muscle atrophy. She was left in bed for 20+ hrs per day and received no PT, which was the whole point. She is now back in the hospital, suffering complications from the above mentioned issues. She will now have to go back to another skilled nursing facility and I've been told she may never walk again. This facility needs to be shut down. read more

I heard so many good things about this place. I had to move a family friend for rehabilitation. The room is never swept up, food on the floor, they put his urinal on top of hIs bedside table where his food goes. So many other things that aren't great. Response time is super slow....Communication isn't the greatest, but I do understand they are short staffed. Still, a little courtesy would go a long way if the staff just acted like they care....This facility definitely needs some work on cleanliness, answering questions and better patient care. I definitely give 5 stars to the lady, Sherry at the front desk. She has been kind and helpful. That is why they are getting an extra star on facility!!! read more

Thanks Kira well as all the case manager's doctors and others who helped me while I was there. Because of the resolution of this problem to my satisfaction, I am changing my review to five stars. I would like to personally think the nursing staff who gave me the customer service while I was in the room for about two weeks. To the nurses, Ali M, Jeff M, Steve R, and Marsha K. You guys are the best. You always took time to see how I was feeling, and to provide the best of service.. For the CNA's Jackson E, Megan L, Mercy M and as he called himself the little guy, Abraham K you guys are the best always asking what you could do for me whenever you came in the room and never disappointing. For the therapists both PT and OT I'd like to mention Morcelia A, Amy M, Anissa, V, Robert R, Jeremy C, Amanda H, Maria S, Mary F, Steven R, Sierra M, Melissa B, Andrea O, and Megan B for giving me every opportunity to do therapy, and to make sure I was safe while I was doing it. For anyone that I forgot to mention by name the older you get the worse your memory is and as your memory goes, you'll never remember losing it.. thanks guys for the care and helping me get back home where my dog misses me but I'm not too sure about my wife sometimes. Best luck in the future for everybody there and if I ever need it again I hope that this is where I get to go. . I've also spoken about the service I received to a coffee buddy who where I recommend he should take his wife after a fall and I believe that he is seriously considering this facility and I believe that his wife will get the best service possible care. I've done business with The Center at Val Vista since it opened. The administrative, clinical, marketing and rehab staff have always been above and beyond...always accommodating and helpful. The facility itself is just lovely. Always bright and cheerful with a modern and up-to-date decor. Each patient room is private and beyond roomie in size and provides its patients their own private bath...amazing accommodations. It hurts me to give a facility this kind of rating, but it is warranted. My mom went here back in November of 2020 for rehab after her hip surgery. The facility was great and got her home very quickly. Fast forward to March 2021... mom once again found herself here after a stint in the hospital for another fall. This time she was on the second floor where the care was horrible. First there were issues with her meals. Mom has no teeth and needs a very specific menu, this was the first problem. My sister and I visited her on a Thursday. She seemed a little off, but was in good spirits and was thrilled we were able to visit. I tried calling her on Saturday, there was no answer. Tried again on Sunday, and again no answer. I called and talked to her nurse. She said everything was fine but mom was having trouble sleeping at night and was currently asleep. This happens at home so I'm not concerned. The next day I'm not able to call until 5pm, was told her nurse was was giving a report to another nurse. About 10 minutes later I get a call from the nurse saying mom had gotten out of bed, she had to use the bathroom but was ignored, and fell and lacerated her ear. They were sending her to the hospital. When I get there, mom is agitated and hallucinating. She also is horribly thin from malnutrition and dehydration. At this point I've already decided to not send her back. Then the kicker, two weeks later I get a sympathy card from them. Apparently my mom not going back means she has passed. How horrible is this facility??? On a scale of 1-10, a 20! read more

Please DO NOT select this skilled nursing facility for your aging parents. My Mom was there for 21 days & got progressively WORSE day by day. I called; emailed & even knocked on staff office doors to try to get them to be more proactive in her recovery. Instead of being able to go home after her stay here - she ended up back in the HOSPITAL. She needs oxygen 24/7 - one day I stopped by & she was having trouble catching her breath. I discovered her oxygen was OFF [for at least 2 hours]. The staff is busy - but their attention to these type of details is not good. She is now at Sante of Chandler & doing so much better. Lastly - I sent a letter to Dr Senkoff on 2/25/20 expressing my concerns & as of today, no feedback. This indicates to me that this facility is not interested in listening to my concerns and improving their patient care. read more

My dad died there. Staff was incredibly incompetent. They are not trained and had no compassion. I wouldn't let my dog stay there.

My father has been here twice previously. The first time, they were great. The second time they seemed to not care at all. They were always changing or cancelling PT sessions, and his room was constantly filthy and I had to repeatedly ask them to clean it. He went there for rehab and they won't even help him to the bathroom. It's been about a year, and my father had another incident in the hospital where they made him stay in bed for days and then he can't get up on his own. Skilled nursing facility was ordered and we regretfully said he could go back to the Center at Val Vista (it was our 3rd choice). The non caring or willing to actually do their jobs staff there called me and said they would only take him if we provided a 24 hour personal nurse at $25 an hour to be paid by us. If you want your loved one cared for 24 hours a day, go somewhere else! read more

If I could give them ZERO stars, I would!!! I would not recommend The Center at Val Vista. It is a beautiful facility and is approximately 3 years old. Of course, the initial tour is always impressive and everything looks and sounds great! My mom's husband spent a couple months at Friendship Village, but due to a change in Medicare in January, he was not able to return to their facility and I had less than 24 hours to find a place! So, we chose The Center. Very disappointing. On his first day, his dressing fell off his wound and was left uncovered for an hour and a half! Part of that time was spent sitting in the bathroom. This was a fresh wound from an amputation that had been performed 6 days prior. He heals VERY slowly due to Diabetes, severe Peripheral Arterial Disease, and other complications. So, this fresh wound was exposed to germs and bacteria in a bathroom setting and then left exposed further after he was returned to bed. The response time after a patient pushes their call button is ridiculously slow and the bedside manner of the CNA's leaves a LOT to be desired. His nurse, Cynthia, has been great - very friendly and efficient. However, the other staff on the floor have been rude and very abrupt and do not seem interested in the welfare of the patient at all. Chris and Marco (PT and OT) have been very good as well as Cynthia. Food variety is OK but usually arrives lukewarm at best. I was told they only provide transportation to appointments for the first one and then after that, the patient pays for it! They also do not provide laundry services. They make mega bucks off insurance and the care does not reflect the cost. They don't even come close to Friendship Village! And now they're discharging him because they don't want to deal with us!! read more
