I had a urinary tract infection which they didn't get around to treating, wrong antibiotic, needed both a culture and sensitivity to find what works, until just before I developed urosepsis. They missed the signs but my friends noticed immediately that I had become confused, convinced them to call an ambulance to take me to the hospital. If my friends hadn't shown up, I would have gone into septic shock. Plus the hospital found benzodiazepines in the toxicology screen. They weren't prescribed. My daughter was very upset as she almost lost me.

When I was discharged from the hospital, I did not have the "PROMISED" option to select my rehab facility. I have to say, that the best thing about Citadel Post Acute, was the morning oatmeal!! As a newly diagnosed diabetic, I received NO guidance on diet, nor were there any menu choices suitable for a patient with diabetes!! Pasta & bread at every meal?? One whole week with NO salad options?? PT, OT workers were caring & encouraging & they did get me up, standing & walking again. There were two night nurses who actually seemed to give a darn, but the rest of the "caregiving" staff did not seem to give much of a darn about patient care. And don't even get me started on the "medical care" - I contracted an eye infection that the doctor (?) did not look at until after they had been treating it with saline drops (no improvement) for over a week!! And in the 6 weeks that I was there, they stuck me with 4 different room-mates, & apparently because I was not a "complainer", each one was worse than the one before! One even tried to smoke in the bathroom while I was 6 feet away on OXYGEN!! Stay away!!!

We don't believe she was ever under the care of a medical doctor any of the days she was at this facility, which is medical negligence and fraud! Well in a few days there's bedbugs everywhere, they attempt to take care of it moving people washing clothes, shower, not that easy to get rid of bedbugs and she has bites all over her. She asked to have her sheets changed and noticed the bed bugs in the clean sheets. Then a nurse tried to remove her pic line for no reason at all, she called 911 because the nurse said she didn't need it. She's in this place to get her meds and the IV antibiotics, which are to be given 8am and 8pm, they're given usually about 4 hours late each time. Her blood pressure was very high and they didn't seem to think a systolic number of 212 was important so they take their sweet time getting her anything, with high blood pressure comes headaches, well she got Tylenol 4 hours later. Now her pic line which was moved the next day is backed up bleeding and not secure. And on top of it she now has a bad infection on her elbow which is red, inflamed and warm to the touch, probably from bed bug bite. They won't help her. She's told she has to wait because the nurse does everything in order and can't be bothered. The saga continues with horrible care. Please note to all that you can file a complaint with the AZ attorney general's office!

One nurse needed help putting a large man back in bed and they had to wait for the cna to finish trash b4 getting her help. I was floored. I'm just grateful I wasn't bed bound this 2nd stay here. I was in 2019 during my first stay here. That time was 6 weeks I believe. I did have a nurse who made me cry and then laughed about it. No idea why he did not like me. Even my roommate agreed he had some problem with me.

I'd heard so many horror stories about SNF and reading the reviews online made it even scarier. I'm so happy that I chose Good Samaritan! I can't say enough about the teams that assisted me in my rehab from a broken hip. Two RN's, JoAnne and her team as well as Julie and her team made my recovery easier. The PT team, after one correction, were top notch! Here's something you don't hear too often about SNF's -- the food was awesome, that is, when Tammy was working in the kitchen. Otherwise it was dismal. The dietician was wonderful to order special food for my diet. Rosie, the Social Director, made me feel like I was with family; she is the best! All in all, the experience was good and I highly recommend Good Samaritan Society-Mesa. The only reason I couldn't go five stars is because of two incidents: Upon arrival from hospital my intake nurse was great and assured me that they would get my meds right. She was able to provide a pain med four hours after arrival according to dr's instructions. She left instructions for the night nurse to do the same every four four hours. Instead the night nurse totally ignored the dr's instructions and the ones left for her by the intake nurse. She wouldn't even come in to my room but sent word with a LPN that they didn't have the meds the doctor prescribed. (Which wasn't true). I was not offered any substitute, not even Tylenol! So for eight hours, I lay in pain, crying. The second reason I can't give five stars is because the executive team tried to dismiss me without pain meds, which I understand is not lawful in AZ. They had a temporary doctor on duty who could not or would not prescribe them. What? Yes, it's true. It was midday on Friday and I was told I would have to see my primary care physician to have him prescribe the proper pain meds. On a Friday??? (The last time I tried to get an appt with my primary care physician I had to book four weeks out.) I had remaining pain meds at this SNF that its' regular doctor prescribed and after a meeting with the nursing supervisor, we finally worked it out that those pain meds were mine and could go home with me. (My dr's office says that is the AZ State Law) It was enough to get me through the weekend. The pain meds were not strong Opiods, in fact, they just took the edge off the pain. I am allergic to Codeine and its derivatives so that eliminates all the oxy-contins, etc. Would these two events have not happened I would give Good Samaritan another star.

As I entered I was warmly greeted and directed to my cousin's room. The front entry way was clean, organized and well lit. His room was close to the front. It's a 2 person room- the room was small with a bathroom and a curtain as a divider. Both sides have a bed, tv, phone, night stand, chair and a wardrobe closet. The room was clean when I went. While there, I observed the nurse and 2 students come in to administer antibiotics intravenously. They introduced themselves, explained what they were doing and asked if we had any questions. They were friendly and communicative. Before I left, I used the public restroom. It was very clean. Now here is what the patient (my cousin) has shared with me- " all of the staff- CNA's, nurses, nurse practitioner, etc are very competent and nice. I have a wound that needs attention daily and at 7am every morning they attend to it. They use gloves and maintain a clean environment for me. All of the staff have checked on me, asked if I need anything and offered assistance. The social worker is easy to reach and I've left messages for the provider and she gets back to me in a timely manner. I have nothing bad to say. It's an awesome facility. A place I will return to if the need arises. The food is good- serving portions are smaller than what I'm used to but I'm sure I'm used to overeating. Overall I am quite pleased with the way I've been treated"

Employee concerns about safety did not appear to be addressed consistently, leaving many feeling that both staff and patient well-being were not a priority. The lack of management support made these challenges even more difficult. Employees were frequently expected to handle complex and potentially dangerous situations with little guidance, communication, or backup from leadership. Overall, the work environment suffered from poor oversight, low morale, and a lack of accountability. I hope the organization takes meaningful steps to improve security, strengthen leadership, and create a safer workplace for both employees and patients.

After my mom's stroke and brain bleed she was brought to this facility for memory care and rehab. I…read moreseriously wish I had taken photos to show what was taking place. The first evening my mom was there I walked in and she was laying on a plastic mattress with no sheets on the FLOOR!!! I demanded they raise the bed, put some rails on it and put sheets on it. They said they couldn't find the sheets to fit it! I told them to bring in a different bed then that had sheets that fit. The following day the in charge nurse told my father, in front of my mother that this was the best it was going to get. She would not be getting any better. At this point my mom couldn't speak, couldn't get up and walk, but knew what you were saying. It was the first time since her stroke and brain surgery that she cried. She knew what this nurse was saying. I was furious. I got my mom up, had her stand while I held on to her and we would just hug and sway back and forth to get weight on her legs and get her muscles working again. (Isn't this what a rehab hospital should be doing???). Within 3 days, my mom was able to walk to the restroom and use it. Now she didn't have to lay in her waste for HOURS waiting for someone to come change her. Within a week, they had a hard time keeping her down. She began talking again. Although still very confused with memories, she was improving greatly. That nurse tried to make me feel stupid like I had no idea and because my mom had had a seizure, she was now going to live an infantile existence for the rest of her life. If it was up to them, she would. They were doing absolutely ZERO memory care with her and occasional pt. I was horrified at what was going on. I had to throw fits to get her bathed. I was constantly being told they were understaffed or the 'shower girl' didn't show up. Today, almost a year later, my mom has some memory issues and dementia, but she is home and functioning pretty darn well! Especially for someone who was never going to get better! The picture is of my mom the other day, riding her bike. I've thought of taking it into the nurse and telling her don't you dare EVER tell a patient or their loved one this is the best it gets and she won't get any better.
