I was told by the staff that appointments needed to be made to visit a loved one staying in Midtown Oaks Post-Acute, and I was wondering why. Also, why is the visit limited to 30 minutes?

I needed PT really bad, my initial assessment from PT was several days after I moved in then I never saw anybody else for about two weeks, when I went down there to inquire as to why I was told their records show OT has seen me everyday. I filed a complaint with Medicare for fraud investigation. I was also shocked how little equipment they had, I was told their other facility had all the equipment, so we had to wait in line and there were some severely disabled people there. Absenteeism was a big problem which resulted in over an hour response time minimum, one of my CNAs spoke no English at all I would ask her to do something she would nod, leave and I would never see her again, it got to be where I would yell "don't act like you understand me when you don't!" Finally I went down to the nurse's desk to complain and the whole staff came out to gang up on me, how dare I complain about someone who speaks no English, the nurse finally told me that I am receiving good care and I should be happy to receive the care I'm getting. I complained to the administrator and got an apology, after that when I asked that CNA for something she'd bring in somebody else to interpret (a waste of staff don't you think?). My roommate contracted COVID and they wound up quarantine us both no medicine. I asked the doctor for something like paxlovid any kind of prescription, he turned around and walked out. Was it effective no they wound up quarantine the whole Wing. All the physical progress I made with my PT over the weeks went right out the window and I had to start over again. This used up the time to get covered by Medicare which I felt they should cover as they didn't properly treat me. Speaking of the doctor he played footloose with my medication (they just can't leave it alone), when I showed my PCP what he did, he was livid why go to the trouble of assigning and tweaking medicine only to have somebody change them willy nilly without consulting as to why? I consider that arrogant! I had a talk with the dietitian and was assured everyone was getting their nutritional needs considered only to find out all the patients got cheap sugary Kool aid to drink Food was horrible I lived on sandwiches and take out Good luck getting somebody to answer the phone. No I will not go back.

Their competence, knowledge, compassion and great listening skills is why I believe UC Davis has partnered with them as their affiliate Home Care Agency. Communication skills, patience and experience with homebound patients is different from health care providers in an outpatient or hospital setting. They definitely have that from years of experience in home care. Thank you to this team and the owner of American River Health Care for his insight and providing extra much needed services for free that are not provided by insurance. You have set the bar high! It's important for NEW home care patients to understand that home care professionals are unable to stick to a specific appointment time frame due to travel time and the importance of judging when the patient is physically and mentally ready before leaving for their next patient.

With each visit, Suzanne demonstrated outstanding listening skills, and happily answered all my questions, with clarity, making sure I understood her responses. Our sessions went beyond the traditional PT kind of exercises one expects. She explained why the given exercise was important to speed my recovery, and she modified and added to some of the exercises the hospital had sent me home to do. Her extensive knowledge base was also evident when I asked her questions such as, "How would I know that I am about to dislocate my new hip joint, when bending incorrectly?" Her explanation was clear, and assuring. It was a pleasure working with Suzanne every visit. I would recommend Suzanne strongly for anyone's home PT needs.

I had visited several SNFs earlier…read morebut when I took a tour of Advanced Health, I knew in a minute that this is where I wanted Jane to stay. I can't adequately express just how good this SNF is. If you want exceptional care from a team of people who go the extra step to insure the well-being, comfort, and progress of your loved one, this where they have to be. There are no exceptions to that. You must get them admitted here. The entire staff are kind, friendly, competent, professional, and, in my opinon, without peer in this area. If you have the need and they have the space, you take it. You don't think about it, you take it immediately... because that space will be gone in a Hollywood minute. The physical plant is modern, bright, airy, super-clean, and never once had that stale "SNF smell." The director of nursing, Fedilyn and the clinical nurse manager Karren will see to your loved one's every need... no exceptions. Yolanda will make admission and move-in totally stressless. When your loved one is ready for discharge, Bethany will handle everything for you. Sean and his PT staff are well-trained, patient, and totally supportive. The nursing staff is terrific as are the nurse-assistants. When you need something in the middle of the night, someone is there for you. If there is a need to be transported to a facility for a quick scan or x-ray or doctor visit, their vans are modern and their drivers are safe and personable (ask for Manny!) They have wonderful activities during the week with an ice cream social on Friday. But if your loved one wants to just stay in the room and read, that is fine too... there is no pressure to do anything... except recover. Every SNF should be as good as Advanced. None are. If you or your loved one needs rehab... this is the place.

At Sutter Roseville Medical Center I was not subjected to fluid restrictions nor at the current rehab facility I am at. That's the reason for the 1-star rating.

My 83 year old mom was in a car accident and as a result, suffered a fractured hip. After hip surgery she was sent to Greenhaven for rehab. When my mother arrived at Greenhaven she was not in the best shape mentally or physically. She was in a lot of pain from the accident and often talked about wanting to die. Maria and Angela from the physical therapy department actually saved my mom's life. The love and care that those two physical therapist demonstrated, you can't teach!! You either have it or you don't. EXCEPTIONAL!!!!! Jon in the physical therapy department was also, EXCEPTIONAL!!!! I had the opportunity to watch some of the other physical therapist in that department work on patients, and to be honest, they were all warm, friendly and extremely knowledgeable. They were, GREAT!!! The physical therapy department gets an A+ in my opinion. Angelina from administration was a dream, she made my mom feel cared for. She checked on my mom periodically to make sure she had everything that she needed. Friendly, professional and a hard worker. Patrick was also extremely caring and professional and checked on my mom daily. I know because I was at the facility everyday, by my mom's side, for close to two weeks with my sister. I stayed there with my mom from 9 am til sometimes 6 pm and then my sister would relieve me and takeover, and stay from 6 pm til closing, everyday. I'll simply say, my mom would be a challenge for most nurses, but, CeCe, Progress and James, definitely have a spot reserved in heaven, simply by how they cared for my mom. They are 3 of the kindest souls I've ever met. They were responsible for the general care of my mom, which included, changing her diaper, but, they would also comfort her, and assure me and my sister that my mom would be just fine and that she's getting better everyday! Some of the, other staff that changed my mom, I had a problem with, but we ultimately resolved our issues. Greenhaven would get 5 stars if not for the few staff members that did a subpar job. Lastly, I did notice that the patients were out and about and moving around more at Greenhaven, than at the other facilities that I toured, so that's a plus.

Avoid this place at all costs. If you care about your loved ones, please take them somewhere else…read more Based on what I witnessed, patients here appear to be heavily medicated to the point that they can barely stay awake or interact with their families. Instead of helping people recover, they often seem completely out of it. Watching this happen was extremely disturbing. Overmedicating patients to keep them quiet raises serious concerns and, in my opinion, borders on elder abuse. Patients deserve attentive care, dignity, and proper treatment while they recover. Families place a lot of trust in facilities like this, and unfortunately this place did not live up to that responsibility.
