My dad was worsened at this facility. He was admitted in January 2026 from a seizure and remained till beginning of March 2026. Because we could not go every day due to 3-4 hours distance, we had to call. Communications was terrible especially with the social worker. We asked the social worker to provide us with hospice options in our town. It took a long time to get a document signed after my dad was released. We saw the physical therapist force my dad to sit up seeing fully well that he was in pain. When my dad was sent home, we did not have much support. The social worker just requested a nurse to visit instead of requesting hospice which anyone could clearly see he needed hospice instead. It was the nurse who immediately saw that my dad needed hospice and he told me which hospice to call. Something the social worker along with the facility nurses, physical therapist and doctors could have done in January 2026 when my dad was first admitted at their facility. After my dad was released from their facility, the social worker would not answer my messages and phone. After 2 weeks, I reached out directly to her manager to request the appropriate doctor document to be sent to the local hospice to help my dad. Such a waste of time waiting to get her to provide something simple while my dad was suffering more and more. My dad passed away end of March 2026, and I really feel this facility made him worse. read more

ZERO STARS First time experience with Midtown Oaks. I am appalled by the care that was given to my family member. I got the notion that my family member was put in a room and left alone for hours and days. Food stuck in his mouth, he seemed to be choking, and never was his mouth cleaned, or moistened with water, he was so critical, he was suffering, and no one paid any attention. His bed sores were so bad and painful, he was not repositioned every few hrs. My Goodness, what kind of care are you pretending to give? Thank God, he was removed from you to Sutter Health, and was immediately taken care of, with comfort and cleanliness and kindness and compassion. Stay away from Midtown! Shame on them! read more

ZERO STARS!!! This place is trash. THE NURSES ARE MEAN AND HARSH . This is a place for people who are recovering and are vulnerable. YOU SHOULD HAVE COMPASSION TO WORK HERE . Also, These photos look nothing like the actual place. My uncle is going through a hard time and is bed ridden- he needed to go to the bathroom but he needs help getting out of bed. He rang the bell for assistance but no one came. They ignored my uncle for 2 hours. He was unable to hold it and was left to sit in it for 2 HOURS! UNACCEPTABLE- this place should be shut down. After he was thoroughly embarrassed over the accident a house keeper helped clean him up- not even a medical assistant. My uncle called a family member to come pick him up and the facility didn't notice he was missing for 3 hours! If you care about your loved ones don't let them stay here! SHUT THIS PLACE DOWN! read more

Please Do Not Leave your Love ones in there hands my Mom is in ICU right now fighting for her life they don't pick up when calling they make so many excuses I can go on and on about them and it's not good so I will stop right here but if you have to leave them there check on them go see them everyday make sure they are being taking care of !!!! This was an awful facility. My mom came here after a major stroke. I visited every day and several times I found she had been laying in feces and/or urine for hours which would create sores on her private areas and backside. On warm days I'd walk in and she'd be laying in an awkward position with just a sheet and a malfunctioning air conditioning system. One nurse was so rude and neglectful, I ask that she never be assigned to my mother again. The food was absolutely terrible. My mother lost 26 lbs during her stay (3 1/2 months) I complained to the ombudsman and the state, twice, due to the deplorable care she received. Each time they responded just enough so they wouldn't get fined. I will never recommend this facility to anyone. It was a terrible and traumatic experience for my family. read more

My experience at Midtown Oaks was HORRIBLE! I asked several times to speak with the Dir of Nursing and even the Admin. Neither called or came to speak with me about my concerns. My CNA was wonderful but the nursing services were poor to non existent. I was not offered a sponge bath or shower in the 5 days I was there (my wife took me home due to poor care at this facility). I was not given the medication I needed and worst of all there were cockroaches in my room. No one would address these concerns. I highly do not recommend this facility for anyone. The Ombudsman was notified on my behalf by my family. The Administration clearly does not care about their patients. read more

I made a delivery there today and I didn't even get in the front door before witnessing a nurse or whatever she was abuse a patient. When I walked up an elderly woman in a wheel chair was trying to get out the front door. Nurse came up and tried to pull her back in the door. The patient splashed her cup of water back onto the nurse and got the nurses shirt a little wet. This nurse HIT the patient and whined like a child about how she had an appointment to go to and now her shirt is wet. She did this at the front door in front of me and some other person trying to get inside. If she is hitting patients out in the open like that, what is she doing when no one is looking. Please don't send your loved ones here. They refused to give me her name to file a complaint against her. I filed a complaint with the ombudsman. Should have called police then and there. What a garbage human being to hit an elderly person. read more

My sister very ill! Multiple needs requiring extra skilled nursing care! From IVs to wound VAC to just you name it she had it. She's also very depressed because she's been through so much she had a bad fall as a result of loss of her knee and things were not looking good for her and then she got a bad infection and she was so sad. She is a retired nurse and so am I and so we know how hard it is to take care of all these issues. Everybody at midtown oaks with few exceptions has been so kind and so competent in their care that she's doing well it's been a little rocky but it's not because of them it's because of her health can't tell you how much we appreciate this! I would recommend this place to anyone it does not smell the staff is absolutely lovely from the admissions person to the Don do all the other people therapy no complaints whatsoever and if I did they would just be minor she's happy she's doing well! Poor Management and poot coordination between staff members. But make no mistake nurses take their full breaks exactly on time even if a patient is of fire. This place is terrible and got worse each day. Very understaffed. Stop admitting new patients if you cant handle your current workload. No one ever seems to know what is going on or what to do. A total mess and danger to anyone in their care. The screms at night are frightening. read more

Major misinformation and huge communication gaps; nurse gave false information to justify ignoring my directive as medical power of attorney and purposely transported my father, that was not Code 3 criticality, to the wrong yet closer low rated hospital; nurse gave false information to supervisors to justify the issue claiming it was higher criticality; Unbelievably still trying to justify it wasn't their call on what hospital he was taken to! Medics called it in to the hospital as Code 2- yet everyone was too lazy to make sure he went to the hospital directed to go to!! Staff was totally unknowledgeable about his treatment plan. Don't go here is all I can say. I'm filling official complaints with Sac County. People make mistakes, but not owning them and giving false misleading info is NEVER ok and it seemed too routine. read more

ZERO STARS this place is ridiculous my mom went here after her stroke and it was one problem after another. Don't get me wrong some of the staff was great but unfortunately there were others not so great that overshadow the good individuals. Not to mention management was also never there to help with issues that would arise they were always "off" or on vacation. My biggest concern now is that my mom has been out there for 2 months and they have yet to update her status with medical. They are showing her at the facility still trying to charge as if my mom was there. We need in-home support services, and because they have not updated her status, we cannot provide the support. It's 10:30 and the nurse came by to tell me I needed a second TB test and what federal law is being illegal they want to stick me with something they want to test my blood alcohol level I do not understand the plan of the business plan of this place but I don't understand why do they want to go public a second TB test after I've been here a week you tell me why? read more

Relative left care here a few days ago after ~8 weeks here for broken hip. Summary- we picked Midtown Oaks based on CA Dept. of Health/ Medicare ratings, location and "atmosphere" when compared with other local facilities. PT/OT staff were very positive/ supportive; nursing staff was nice but seemed overworked; some administration (transport) was good, some was mediocre at best, and the discharge process was an unbelievable nightmare. ++ PT/ OT staff- John, Ken, Renee, Jessica, Megan, a couple of others whose names have blurred- they all worked hard to effectively deal with the non- compliant patient, eventually resulting in a return to mobility. + location- midtown just off of freeway/ 2 blocks from Sutter General made for quick/ easy transport. ( +/-) facility is older and feels "closed in", not helped by covid- related closed areas; Front desk staff was generally pleasant and very helpful but also juggling covid restrictions on number of visitors and masking/ shielding/ gowning requirements. (+/-) Nursing/ medical care- most services were provided by CNA's with oversight by RN, various MD's seemed assigned infrequent rotational checks; there were often significant delays in receiving help after the call button was pushed- up to 30 minutes on a couple of occasions. Nursing care often felt understaffed, although they were usually pleasant and help resolved any remaining issue when they did show up. ( + /-) Other staff (admin)- transport (Ruth) was very helpful arranging transport to/ from appointments and was very active with confirmation calls, only negative was that her voicemail filled up, sometimes interfering with callbacks. ( - ) Food- typical Sysco institutional. Relative wouldn't eat it so we supplied outside food as we could. Staff dietician tried to help, but there's only so much you can do with Sysco + low salt diet. ( - - ) Discharge- Discharge was a nightmare. After being told that discharge would take 1-2 days after new facility conducted an assessment, it took ~10 days after this assessment to get final discharge papers signed- and even then we only received the paperwork after we had contacted the California Ombudsman to investigate the situation and had already loaded the patient in the car and delivered him to new facility. Delay was related to medical record/ care plan and 602 report inaccurately reflecting patient's condition, including the status of a heel sore and hospice status. Ombudsman said it was the only time he could remember working with patient to get the patient discharged- normally a patient is fighting to stay in care longer and the facility is moving them out. read more

I am still in midtown post acute and my experience has been great! From the janitors to the doctors have been so professional and attentive to my every need. Estrella Montano here- I was admitted to this place for rehab PT/OT. This place is a rockstar- rehab is wonderful, persistent, they are good what they do. Assist me every step of the way to my recovery. Ken, Amanda, Jaya & Megan.. Nursing department- wonderful nurses. Kind, they never complain and help me all the time. Never complained of anything I asked for. My best nurse is Jas- always listen to me and what I had to say about my meds & gets it right. Jas- thank thank you for all you do. CNA- my CNAs - Pricilla, Breana and Adriana are the BEST! Really good people. DC planner- he is okay. Assisted me for the DC. Activities- The best activities department in town. Love their activities and assist as much as they could to get the boredom while away from home. Dietary- need room to improve, Elena is great talking to me and offer other alternative food. But the food needs big giant improvements. Administration: the administrator- kind and friendly man. Get things done! Social Worker-She is the bomb! She follows through as what I had asked for, really kind and there for each patients. Things can improve: Food and increase in shower. Over all, I want to thank Mid Town Oaks Post Acute staff and management for doing an awesome work.. I appreciate my stay.. read more

My father in law was placed at mid town oaks and it was a horrible experience. They didn't get his anxiety or agitation under control. We spoke with the nurse and let her know he needed the anxiety medication (why he was so agitated) in took 24 hours to be addressed. He has terminal cancer and was supposed to be on comfort care. He was everything but comfortable. My wife was his power of attorney and they didn't call her when he called 911 to bring himself back to the hospital as his pain wasn't being properly managed. I understand that sometimes one nurse has a lot of patients and things happen however, waiting over 25 min for someone to come when he pushed is nurse button is unacceptable. read more

My mom is a patient in midtown oak. She likes it there and so do I. I have met a great person who works there, we are now really good friends. Her name is Naomi and she is wonderful. Also Peggy in the business office is also another great person I have met there as well. It is nice to know that my mom is well taken care of. There have been many problems with my mother's care and it has not even been 24hrs since her arrival. The following are complaints we as a family have at the moment. 1. On 3/15/20 by 9:26PM PST the bed side commode was not emptied out and when it was used repeatedly by the patient the patient got wet from the commode not being properly cleaned before it begins to overflow. Note that the patient just arrived on this day in the afternoon. 2. The patient asked to have the window closed but the care provider at the moment informed the patient that the other occupant is older and would like to have the window opened. There was not regard for my mother's wellbeing although she did inform he care provider that she is cold and in pain. 3. The patient asked to be straightened out from her legs at least since the legs are at the edge hanging down almost and it is causing pain- especially since this patient has a sciatic and the legs are atrophy to an extent due to the severe pain with walking, sitting and standing. 4. We as a relative did call in to the facility asking for the patient to be warmed and to help the patient sleeping posture be corrected to a more comfortable position due to the pain and cold and on the next day we were informed by the patient that not only the posture was not fixed but that the night was cold for the patient and it still is. 5. On 3/16/20 the patient asked to be assisted to the restroom and the care provided gave the impression of not liking to do their job in assisting the patient. Afterwards there was laughter while glancing at the patient once she was taken back to her resting location. 6. The patient asked to be allowed to wash their hands for breakfast and after the patient was taken to the restroom the patient was left without breakfast for 3 hours after all of the other residence had been fed and until another family member called to complain that the patient was not being fed. 7. On 3/16/20 it is now 10:23am pst and the patient is still cold, in pain and nauseous. UPDATE!!! I HAVE SPOKEN TO MY MOTHER ONCE AGAIN AT 12:11PM PST, and the nurse reprimanded my mother for informing us about the issues and had threatened to take my mothers phone away if she continues informing us of the discrepancies done to her. read more

It's under unfortunate circumstances that I came to know the people at Midtown Oaks, but they've made the situation as comfortable as possible. My mother needed to recover there from a fall and Juliet (coordinator), Chris (physical therapy) and Bless (physical therapy) all showed a genuine concern for my Mom's health and recovery. my sister went there after back surgery at sutter and was infection free. 3 weeks later she had to return to sutter with cdif, mrssa and a huge 12" square bedsore. the mrssa had infiltrated into her spinal column and she could not withstand another surgery. she had to go home on hospice and died shortly thereafter. they killed her. beware! read more
