After a slightly rocky start (which we kinda expected), we figured out how to schedule within bigger windows and expect minor glitches. The service is much appreciated. When a scheduled ride is truly coming we are given the name of the driver; if it is still only pending, you will get a message that it was canceled, then another Uber, even an Uber (WAV) as we require, is dispatched. I was discharged from the hospital yesterday at 1:36pm. The case manager at the hospital called for medical transport to get me home, which is not even 5 miles away from the hospital. ModivCare told us that I would have a ride somewhere between 4:45 and 7:45 and they would call back before 4:30 to confirm. They never called the hospital back. When the case manager called at 5pm for an update, they wouldn't give her one and said there were 4 patients ahead of me. Not too bad, so i was content to wait. When we still hadn't received an update at 8pm, the PCA called and was told there were 3 patients still ahead of me, so ModivCare still could not give a time. It took them 3 hours to transport just 1 person?? By 10pm, we were told there were still 2 patients ahead of me and again could not get any sort of time frame. I gave up and called a couple of friends of mine, who were wonderful and gracious enough to come get me. Otherwise, I would have to spend another night in the hospital due to ModivCare not being able to fulfill their obligation in any form of timely manner. I have spoken to many people about this company and also read the reviews here and I see my complaint is not unique. Do not use this company if you can avoid it. There are plenty of other transport companies that serve the area and I will be using any of them before I ever call this place back. read more

I had some very serious incident in reference to this company several times I had to cancel my doctors appointment due to them not showing up or either late for pick up. When they send me a ride, the ride is very dirty inside you would not want to sit on the chairs or get in it. I cannot understand when they're getting top dollars from your health carriers and that we have to go through this this makes no sense is disrespect to others and myself. This company is incredibly irresponsible, and, if I could avoid them, I absolutely would. Today I had an appointment at 8:30, and my driver didn't show up and didn't show up, so I called and was told that he was coming at 7:40, but he still didn't show up, and it was getting later and later, so I called again and was told that he wasn't coming because he had gotten into a car accident. Is this the caliber of driver that they hire? Besides, I think he was lying about the accident because he said that he had tried to contact me, but he never did. I was with my phone from 5 AM on, and nobody called, and there were no incoming calls in my recents list. So I missed my appointment. I was told that they were looking for another driver, but by then it was too late. I was told that someone would contact me either way, whether they found a driver or not, and of course nobody did. I had been waiting for a month to get this appointment, and now I have to reschedule it and just hope that someone actually shows up this time. If you can use any other form of transportation, do. As a wheelchair user, I have repeatedly informed this company about my transportation needs, yet they continue to schedule rides that are completely unsuitable. They often book me with Uber or Lyft vehicles that are too small to accommodate my wheelchair, making it impossible for me to use the service they arranged. Even more frustrating, they frequently schedule rides through a transportation company that has wheelchair accessible vans, but that company has repeatedly failed to pick me up. I have reported this issue multiple times and explained that I cannot rely on a provider that consistently leaves me stranded. Despite these complaints, they continue booking rides with the same company!!!!! Reliable transportation is not a luxury, it's a necessity, especially for individuals with disabilities. To Whom It May Concern, Your company, Modivcare, was scheduled to transport my 92-year-old mother from a Jersey Shore Medical Center hospital in New Jersey to a rehabilitation facility at 1:45 PM today. As of 8:30 PM, the transportation still had not arrived. My mother had already been discharged from the hospital and was left sitting there for hours, waiting for a transporter that never came. This treatment of an elderly patient is completely unacceptable, inhumane, and deeply disturbing. Unfortunately, this is not an isolated incident. During a previous transport arranged through your company, a driver scheduled for 1:00 PM did not arrive until 11:45 PM. I consider this pattern of conduct to be negligence and emotional mistreatment of a vulnerable elderly person. Please be advised that I am formally opening complaints with: the Better Business Bureau, the New Jersey Division of Consumer Affairs, and the Office of the Attorney General of New Jersey. I am also preparing legal action regarding these repeated incidents and the unacceptable treatment my mother has endured. So two days ago, I had two drivers canceled on me, and the third driver never showed up. Hence, I had to call them and cancel the third driver because it was literally 15 minutes until my appointment. I could not wait any longer so I just had to pay for a lyft. I'm on a fixed income, which is why I use them to begin with, and now today another driver canceled. Modivcare is the worst transportation company. The employees are racist, abusive and harrassing and I hope that Modivcare goes out of business because they are cheating the government and State agencies by receiving money for services not rendered. I was personally told racist things and sexual comments. Refused OHP Plus mileage reimbursements and told in writing by the insurance company that I qualify for the mileage reimbursements that was refused to me. At first I went through WVP Health Authority and Modivcare transportation didn't send my mileage reimbursement checks so WVP Health Authority had me drive to their office every two weeks to pick up my mileage reimbursement check. Then WVP Health Authority paid for a P.O Box to make sure that I received my mileage reimbursement money Then PacificSource took over and I had to fight for my .25 cents a mile for 2 years until I had an advocate who was the Vice President of Modivcare and taught me Modivcares rules. He was let go from the company on January 21st 2026 so he stated after almost 6 years of being my advocate then I was abused again by Modivacare. Modivcare sent me e-mails containing private information from all their patients. Names, addresses, diagnosis of the patients and their insurance information that violates patients HIPAA Act Privacy Rights. These violating e-mails happened at least 5 times. I gave a couple of the e-mails to PacificSource who pays them and nothing happened. I currently am being harrassed, told I cannot make trip numbers, and being harrassed and made fun of by Modivacare employees that refuse to do their jobs. I finally made a report to the Arizona Attorney General. I pray Modivcare goes out of business for causing me and other patients harm, pain and suffering. Karma is real and I believe that Modivcare will go out of business. IronicallyI helped Oregon Constituents to receive .49 cents a mile mileagereimbursements and being refusedmy mileagereimbursements. This company is trash. They cancel rides for stupid reasons and don't even notify you that they canceled. It's nearly impossible to speak to an actual agent to book so you have to use the flashbook thing then wait and see if they approve the address. It uses Lyft drivers, but I am almost deaf and there is no process to talk on an incoming call. (incoming has no captions) so you cannot communicate with driver. I wish I could leave a zero rating but this forum only allows a one. This company was already bad but had only gotten worse. Currently unless you are disabled you cannot book a ride online. They have blocked all customers. You have to call in and hope the person answering gets the information right. They only offer but tickets to get to appointments and those have to applied for all least a week prior to the date. There is no way to check the information because you can no longer log in. If you are having surgery they may out may not pick you up after. So go with God. No company deserves a chapter 11 more than this one. It is deplorable. Terrible organization. If there was an option for zero stars, Modivcare would earn it -- and then some. I run a skilled nursing facility, and Modivcare is supposed to handle our medical transportation for dialysis patients through Alameda Alliance. In the past week alone, they've failed twice -- completely no-showed for dialysis pickups, stranded vulnerable patients, and ignored repeated calls from both our facility and the dialysis center. The latest incident (confirmation #57200) was an absolute disgrace. The contracted provider, A to Z Logistics LLC, never showed up, and no one from Modivcare answered the phones or provided updates. The patient's sister finally had to call a Lyft just to get him home safely -- out of her own pocket. Dialysis transport isn't optional. These are people's lives we're talking about. And yet Modivcare operates like a call center that's asleep at the wheel -- no communication, no urgency, no accountability. I've had to personally stop what I was doing on a Friday night, Halloween night, and drive to Oakland to pick up one of our residents myself because Modivcare failed so badly. That should never happen in healthcare, ever. If you are a patient, family member, or healthcare provider -- do not rely on Modivcare for anything critical. They are unreliable, unresponsive, and dangerous to anyone who depends on them for care. read more

I have used Modivcare for 3 years through my insurance. They are easily the worse company to make reservations for. The voice recording, MARA, is not helpful most of the time. If I need to speak with a live agent concerning something other than a reservation, I have to ask at least 4 times! I tried the app this year and it is so confusing I have yet to successfully make a reservation. Unlike many others, I haven't had problems with drivers or late rides. read more

My last ride made me file a complaint because she lied about everything from her arrival to dropping us back off home. She already had an attitude when she pulled up and began driving before I could properly secure my children. Modivcare completely disregarded the complaint, claiming it's unsubstantiated. That's fine. I'll wait until someone sues them into bankruptcy. Unfortunately, since they hire anyone and don't really investigate someone is going to be terribly hurt during one of these rides. ModivCare enforces a shameful and unsafe policy that prohibits passengers from being accompanied on rides unless a very specific medical necessity form is sent to them and "reviewed." It doesn't matter if you're a small, young woman being sent on a four-hour trip with a total stranger -- you are not allowed the basic safety of having a trusted person ride with you. When I called to ask if my doctor's signed note would suffice, I was told "absolutely not." It must be their paperwork, with no exceptions -- even for urgent or one-time trips. This rigid red tape forced me to cancel, leaving me with no choice but to pay $400 for a taxi so that I could be accompanied and not put in a dangerous position. To make matters worse, ModivCare doesn't even provide a photo of your assigned driver beforehand. You are expected to climb into a car with a stranger, completely alone, for hours. In 2025, it is outrageous and offensive that women's safety is treated as an afterthought. This policy is not only sexist -- it places vulnerable passengers directly in harm's way. ModivCare should be ashamed. Until they change this policy, I strongly warn women and families: do not trust them with your safety. read more

I'm located in Logan, WV. If I could give 0 stars I would. I am in an outpatient program for addiction. I depend on them to get me to the clinic four days a week. They CONSISTENTLY mess up on actually getting me a ride. I'm lucky if I make it to the clinic 2 days out of 4. (To stay in the program you're supposed to be there every day you're scheduled). Which means cutting my meds in half, but Modivcare doesn't care about that. I will look on the app and see who's driving me and one minute I have a ride, the next they list me as "mileage reimbursement" (meaning they'll reimburse my gas $ for driving myself). I DON'T HAVE A CAR PEOPLE! I end up arguing with them at least twice a week. My mental health can't take much more of this bulls**t. read more

Everything about this company is broken, especially the way people are treated. The phone app and the computer booking don't work, I was able to get a live person when calling just by repeating "agent" multiple times til a live person came on. I booked a ride for my wheelchair dependent husband 3 days before his doctor appointment. Long story short - they were at least an hour late, was told they were on the way, then told they had no wheelchair van drivers but were looking for one and finally that he had been picked up. Got to the doctor's office too late to make the appointment. Then he sat and waited for for at least 90 minutes before being picked up to return to assisted living. Not a single text from Modivcare about any of this. I must have been on the phone with various operators trying to find out what was happening for a total of 3 hours. This is not the way to treat people in wheelchairs or otherwise disabled. The absolute disregard of this company for people's welfare is reprehensible. I will never use them again or refer them. When I receive transportation rides I am satisfied with the drivers and trips. What I remain frustrated with is when I take myself and try to get mileage reimbursement I am on the phone with numerous people transferring me or giving me multiple numbers. I am on the phone for up to 3 hrs or more trying to get my mileage reimbursement straightened out. They are rude and often hang up on me. Some of us in society are poor and disabled and are just trying to get help financially by taking ourselves to the doctor and these are funds that we are entitled to and if you are a Modivcare employee then it's ur job to help us. If you don't like your job then find another one. Please be nicer to us on the phone . But I will say that the drivers who give the transportation rides are nice and do a good job. When scheduling more than one trip a day (4 legs) the App gets confused on the second trip. Have to spend 30 minutes trying to get to someone to correct it. Very slow to input new authorized rides from insurance - took me over 3 weeks to get Modivcare to enter that 10 more rides were approved by insurance. They have the wrong fax number? Don't work the faxes? Customer service is terrible and AI interface doesn't understand any problem except scheduling rides. App needs a lot of help as it freezes up all the time and you have to reload the app. I have let my insurance know to find another company. read more

I have notified Modivcare multiple times about Gray Wolf transportation here in Ohio. Everytime I complained I'm told that I need to contact my healthcare provider. When I ask to never again have said transportation that they will pass the information on to their dispute team. When asked if I could be transferred I was told that they don't have a phone number. Modivcare's reviews are just as bad as Gray Wolf's and it's obvious that no one cares about customer service. If I could give them a zero rating, trust me I would. In fact it's so bad that they say that my type of insurance doesn't cover reimbursement if I have someone else drive me. So now I'm stuck paying $120/month so I can use my local transparent transportation to get me to my appointments and pick me up on time. read more
