The facility was very clean and well maintained. When I had an issue with the AC the maintenance team promptly addressed it. The food was very good, I enjoyed it mostly. I felt well attended to, although on a few occasions it did take a bit of time for the staff to come to my room. My only real complaint, is that I was there to regain my strength after an illness. Unfortunately, being confined to a wheelchair 90% of the time, weakened me and caused edema in my legs. I was only allowed to use the walker during PT. I lost strength and agility not being allowed to use it. This should be addressed. All in all, I would recommend Palm Garden to those needing rehab.

He was on the phone the entire time until I asked him not to talk in the phone when he switched to speaker phone. He threatened me. Told me to be quiet when I asked him to stop taunting me. I don't let people drive me while they are on the phone. I would think you teach them better and to not take personal phone calls. I would have made him let me off and call the police and an ambulance to get back to my facility but I will make certain you never are allowed by Medicare to transport anybody else. I don't know what kind of dog and pony show you run but breathing a rider because he asks to be safely driven is of utmost importance. I will probably fike a complaint for elder abuse. I was so fed up that if he did as he threatened get out and teach me a lesson that nobody should be allowed to do that. Now as a Merchant mariner, for over 45 years, I've done this procedure, successfuly, ever since the USCG instituted it. Also, I was an registered EMT and am still a certificated Medical Officer, aboard ships. So I actually perform these type of exams. myself. aboard ships. Thus I was appalled, as I watched them, firstly screw up the DOT papers I gave them, and then proceed to fumble the Chain-of-Custody procedure. Well, sure enough, the Medical Officer called me and advised that there were "irregularitys", in the received samples, meaning the test was rejected. So now I am under suspicion, for their incompetency. If that was not enough, when I submitted the Physical exam papers, again, I was informed by the USCG, that there were "irregularitys" there, too. So now I can't get a USCG Medical Certificate, which is required to sail--I'm out of work, because of their "irregularities". Also, they charged me almost double what I would have payed, had I just gone to Concentra. Well, so much for going back to work!

She was in the memory care unit and never smelled when I saw her. She was gaining her weight back and was beginning to interact again after a considerable downturn at a horrible place in Jacksonville Florida landed her in the hospital. My younger brother transferred to assisted living in The Lynmoore At Lawnwood, Fort Pierce,…read moreFlorida, more than a year after he suffered a massive stroke. He had been in and out of intensive care while being bedridden in a different rehab and skilled nursing facility. After many months in that facility he had recovered enough to eat and swallow soft food but he was still unable to walk. Then he came to Lynmoore, just before Christmas in 2023. My wife and I live in California, so we can't visit him in Florida that often, but we did visit for a few days in June, 2024. What a difference! We walked in one day just after his physical therapy session ended; he was walking back to his room with the therapist by his side, not in his wheelchair! That never happened before. This alone would rate five stars, all gold, but there is more. During our visit we interacted with many people: the nurses, the therapists, the community engagement director, the records office, the business office, even the dining hall staff. Everyone was friendly and professional. Residents coming and going were greeted by name. The buildings were clean and well maintained. The food was good; I ordered the same thing that my brother had when we ate together in the dining room.  The affiliated home healthcare and pharmacy services are excellent; I continue to interact with them. However, there is even more to the story. My brother was in such bad shape when he first came to Lynmoore that, during our June visit, a couple of staff members admitted to me that they didn't think he would last in assisted living; he was that bad off. I truly believe he was admitted and flourished because the Wellness Director believed in him, motivated him from the very beginning, and everyone took good care of him around the clock.
