There's a very long line near the entrance that snakes around, but if things are that busy they open a secondary line to the right, which seems to reward the locals in the know. You place your order and they don't scoop the ice cream immediately, but later on they start calling your name out at the counter. We ordered in a group of 4, and I was surprised to see that 4 different people scooped out our orders so they were calling out the name our order was under left and right. I tried the spumoni (chocolate, pistachio, and black cherry) and the dirty banana and they both were very true to their descriptions. There was a decent amount of seating but it was pretty packed so I think sometimes people were waiting around for a group to leave for the next table, but it wasn't too bad.
