They admitted her to the ER, then placed her in their Oncology dept. because she was six years post cancer diagnosis with no new cancer. Ten days later, she died from pneumonia and "malnutrition ". Do not take your loved one to this facility. The nurses were good. The attending MD's had zero bedside manner. I was told we had three days to move my sister to hospice or they would remove her!And after two days, while we frantically called every hospice facility on the hospital's recommended list,Dr. Fortuna and their hospice coordinator, Noreen, became rude, borderline aggressive, w/lharassing me about moving to hospice. The last week of my sister's life had little dignity. It was unnecessarily stressful.A minor observation: their bed linens were ripped and disgusting. Who had been sitting there the entire time! Calling it Poor care quality is being polite. I was there almost two hours and we left because they would not say if it would be 10 minutes or 3 hours. But No one was being seen except the 96 year old who threatened to leave. Went home called an ambulance. They gave meds and IV bag. They took her back to this hole and they set her in the hall . We left. Don't bring a dog to this place! Special shout out to the emergency room receptionist, she certainly wasn't a nurse (no nurse would ignore two people in distress, who are writhing in pain!) . FU. And the nurse who FINALLY GOT OFF HER ASS to help the 96 year old man , hope your parents end up with a similar care giver. How do two sleep at night?

Dr. Kim, was, absolutely, WITHOUT a doubt a lifesaver! I literally can not thank the Banner University Surgical Team, Dr. Kim and Anesthesiologist, Jack and RN Amber- enough!! This was an amazing Team of PROFESSIONALS and I was in at 0530, and out by noon! Allow me to specifically recognize a few people here, the building and process. The Anesthesiologist: Jack was from the VA, and came in to greet me and speak to me about the entire process. His calmness, astute professional and caring demeanor was top-notch! You CAN NOT get a better Anesthesiologist IMO. When I came out of surgery, I literally felt like I took a quick nap, and was energized, non-drugged and felt the best I've ever had in the 14 surgeries I've endured in my life! Thank you Jack!; you really did an incredible job! The attending RN: Amber. What a sweetheart! Absolute golden personality, caring, graceful and funny! thank you Amber for your generous approach and humorous start to my day! Surgeon: Dr. Sunchin-Kim. Here, it has to be said, I CAN NOT recommend Dr. Kim, ENOUGH. He's straight to the point, extremely well versed in the HoLEP procedure and I remain, 100% approved of his care-taking and surgical process. I walked out of the hospital in less than 6 hours, with major surgery accomplished and felt amazing. I applaud you Dr. Kim for booking my procedure, and taking care of my issue prior to a cancerous outcome! The Building: Impeccably maintained, extremely clean and easy to find my surgical section. The surgical room is STATE OF THE ART, and not a single thing in short supply. The surgical room looked like something out of Star Trek in high-tech components and monitors! Thank you in the deepest sense to Banner University for Dr. Kim being on your docket for surgeons. 2 weeks post surgery, I stand firm in this assessment, Banner Surgery was a perfect experience for me! I absolutely, without worry highly recommend Banner for surgery in the Urology department! Much respect to the people that make this happen each and every day!

I was advised, my next pregnancy would be high-risk and need to be seen asap. April 2026, I had a second miscarriage waiting for a response from the staff. They offered contraception knowing my goal was to conceive. The bedside manner was absent. June 2026, I had an ER visit outside of Banner for severe abdominal pain and suspected etopic pregnancy, advised to be seen within 2 weeks with my OBGYN provider. I call to make an appointment, scheduled for the following week! What a great experience!!! NOT EVEN 30 minutes later I get a call from the office stating the appointment needs to be rescheduled because that provider does not specialize in miscarriages and the appointment needs to be rescheduled. I was offered an appointment with the "resident clinic" instead of my established provider. Why would I want to see a different provider every single visit instead of seeing the provider that knows my history?? Where is the continuity? AND IF THAT WASN'T ENOUGH THE "hospital follow up appointment" was scheduled 2 months out while I'm still in constant pain.

I want to focus here on the little bonuses of emotional support. When you realize this is where you need to be for two weeks or longer, there is some mindset and emotional issues that are struggles. There are really special little details that this care center does to support you emotionally. There is ice cream available every day at an ice cream bar 1:30pm. There are some really great activities listed monthly on the calendar but not always a large amount of participation. Bingo happens almost everyday, there are all different levels mobility of people who need support to those who don't need support and an amazing community church group comes and support this work. There is a behavioral health counselor the CNAs can refer patients to who need emotional support and Ginelle- the bx counselor- has wonderful skills to offer and a kind caring way of connecting quickly. The CNAs are so very kind and considerate. On the rehab side they work very hard around the clock with most of time 2 CNAs to manage the rehab floor. They do it so well and make you feel validated and welcome. The food was as struggle it times for me and my husbands love language is cooking. The CNAs would help me heat food or get extra salad when they could. I saw them doing this for my roommate also. I needed ice packs round the clocks especially at night that I brought from home and they were always helpful with this. Linda the overnight CNA goes above and beyond at night to help my roommate and me with what I needed. They have gratitude cards you can write for an employee who does stellar work. I was sharing with the Dof O, Mara all my positives. She gave me a pile of cards. I filled out 28 cards and had to ask for 5 more. The peoples' level of care made the things that they struggle with from my Google reviews the reason why I would go back there, if I ever had to and I would recommend it to a loved one.

Then transferred to La Canada Care Center. The second day there , one of the nurses aids noticed in the room's bathroom had an electrical outlet was really singed, and the box of gloves, above the outlet was too! They reported it to the nursing station. When I asked later about it, the nurse said the outlet was tripped, so it was not a problem. I later told management I did not feel safe with my husband in the room, he could not even get out of bed by himself. Also, concerned about other patients & staff. About 6 hours later he was moved. He was in isolation for 10 days. Once it ended, it took a contract wound nurse, who came 3x a week, to talk to someone 5 days after he was cleared of the infection. The room had not being cleaned or sanitized. She told them he had wounds & would not be safe in an uncleaned area. Within a few hours the room was cleaned. Food was not very good, I would not eat it. The OP & PT were great Most of the nurses & nursing aids staff were very good, but a few should not be in that field. It was very chaotic with patients yelling for meds or get me out of here, etc. My husband tried to get out of bed & skinned up his knees, & fell out of bed once, when I was not there. I was there usually 7:30 - 5 to make sure he had care, they changed him, refilled his water, get anything he needed, You need an advocate! At the NW Medical Center I was given a list of facilities, either the ones I picked were full, did not take our insurance, or too far awayl. So I had my husband transferred La Canada Care. I do not know how Medicare gives them the top 4 Star rating, they need to check out the facility.again!

With every office visit, I was required to leave a urine sample. This is the first time they didn't want one. I thought it was standard practice. A staff member told me later that they had no supplies, so unless I had a UTI, they wouldn't take a urine sample. Fun fact, you "can" have a UTI without knowing but a urine sample would show. Could I call this a Catch-22? They even rescheduled my appointment twice. Just one hour before my appointment they called me to say said she had car issues. Had I done that to them, they'd charge me $60. Also, although they are in my insurance's network, I still got a bill even after my co-pay. It was a simple quick office visit. It's always been covered by my insurance. I think their billing department may have coded it improperly, or maybe... double-billed it? Guess which department is impossible to get..... Billing! I even wrote a letter to Dr. Lokale explaining why we are having to leave his care, but they refuse to deliver it to him. Had shingles, called for an appointment , go is 6 weeks later. Dr. Lokale asked why I didn't get in sooner. Every appointment takes months to get in to see anyone. My privacy was of no concern when I was lying down naked on the exam bed with my legs in stirrups. The door was left ajar, though I have no idea who, if anyone passed by, but if they did, they got a great view of me. A total of three females were present during my procedure. No introductions were made, so I had no idea what their roles were or why there were so many. Eventually, I learned one was a Urolift representative who was advising Dr. Berry during the procedure. So, this immediately tells me that this procedure is relatively new for Dr. Berry, though this was never communicated to me prior. I opted for nitrous oxide as an anesthetic. Bad choice. I wasn't given instructions other than to stick the tube in my mouth. It took the external Urolift rep to observe that I wasn't exhaling into the tube and to correct me. The two Southwest Urology females in the room must have been preoccupied with god knows what. Overall, the procedure was pretty brutal, and painful. Despite my pain, Dr. Berry continued violently moving the tools around that went through my urethra, into my bladder. No warning either of the intense pain I was about to experience with each Urolift placement. In the end, he used two additional Urolift wires because he obviously made mistakes, as evidenced by the Urolift rep correcting him verbally. In the end, I'm not confident the procedure even worked as I'm still experiencing similar issues with frequent urination, and low stream of urine. But, I would never return here for a follow up. Would I return? No. Would I recommend? No. Southwest Urology, not to be confused with this place, is right across the street. Only by confusing the similar names did I ever end up making an appointment with Southwest Premier. Even that name is misleading.
