My dad went to this facility after his hospital stay. At first I was apprehensive due to the negative Yelp reviews. [[HIGHLIGHT]]I'm happy to say we had a very positive experience over the 20 days my dad was there.[[ENDHIGHLIGHT]] First off it's super clean, [[HIGHLIGHT]]the staff is so friendly and helpful and they were very attentive to my dad.[[ENDHIGHLIGHT]] PT was wonderful! My dad has lower extremity Parkinson's which means he has struggles with balance and walking but Bobby and his team were so patient and compassionate with him. They really helped him achieve all his goals. [[HIGHLIGHT]]Most CNA's and nurses were very nice and helpful. The case manager, Jeanette was very nice and helpful[[ENDHIGHLIGHT]]. This facility is understaffed and needs to be remodeled. When my mom needed help sometimes, she had to wait, once for 1 1/2 hours just to use the restroom. When a CNA is on break or with another patient there is no coverage even when other CNA's are doing nothing. [[HIGHLIGHT]]Upon leaving, my mom said she needed to use the restroom and the CNA said I just took her. I told them it was a 45 minute ride back to Thousand Oaks but they did not care. I drove with my mom upset the entire way[[ENDHIGHLIGHT]] Luckily, when we arrived at the assisted living they found a wheelchair and 2 of their people took my mom immediately to the bathroom. The food at Northridge looked disgusting although my mom ate it. Sometimes I brought her food. This facility is understaffed and needs remodeling before I would ever recommend it!! read more

First, the staff does the best they can with what they can do. Because they are so woefully understaffed, everyone is running around and things fall through the cracks. [[HIGHLIGHT]]There is a pretty bad language barrier on the 3rd shift. I was in there for knee replacement surgery and a couple of the CNAs during the night shift didn't under the phrase ICE PACK.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]Communication is pretty bad with two of the nurse supervisors. I won't mention them by name, but I think it's time for them to find other work.[[ENDHIGHLIGHT]] One of them almost made me miss a very necessary surgery and didn't bother to let me know that they were having problems faxing information over to the surgery center. [[HIGHLIGHT]]I was having problems with a very loud neighbor across the hall, and the weekend night nurse supervisor said that if I didn't like it that I could go elsewhere. Apparently, she got written up for that.[[ENDHIGHLIGHT]] We did not speak after that. On the whole, the staff tries to do the best they can under the understaffing circumstances. Some of the CNAs are golden. The nurses are unbelievably giving with the brief time they have to offer. The housekeepers do a terrific job. The kitchen staff was very helpful. The monthly "candlelight dinners" were pretty cringe and while they're trying to do something special, I found myself wanting to leave every ten minutes. I think they went with the most mobile people. Pro tip: lose the Karaoke. Sweet gal who tries her best, but no. Just. No. I shared the bathroom with the patients next door. Both staff and the patients next door walked in on me doing my business more times that I care to remember. If you're going to see the show, buy a damn ticket! LOL. Or... learn to KNOCK. It's not that hard to raise your fist to the door. Unless you're missing a hand or something, then kicking is entirely appropriate. The Physical Therapy team was terrific. They went above and beyond. They recently had their yearly inspection from the City of LA. They ranked a 3. According to the people I spoke to who were in charge of the inspection, Northridge is one of the better Nursing Homes in the area, and that scares me. [[HIGHLIGHT]]I did my best to treat everyone there with kindness and respect. I saw Alzheimer's up close with some of the patients. A stroke patient screamed like a siren. The patient across the hall from me is verbally abusive to the staff and they have to just shut up and take it. During the final days of my stay there I was this close to letting the bully have it[[ENDHIGHLIGHT]]. Now that I have had a chance to decompress, They did the best they could do with the resources they were given. I would not go there, however, if you have been injured or are recovering from surgery. I had knee replacement surgery and there was no structured plan in place to help me to get from point A to point B except to go to Physical Therapy. So I was on my own. read more

My good friend has been since January, and having a hard time getting out. As he says, "once they got you in here, you will never get out " [[HIGHLIGHT]]The staff is constantly changing which doesnt help. The caring people don't last and the mean ones stay.[[ENDHIGHLIGHT]] He has lost so much weight after he got admitted heer, big reason is the poor quality of food, it looks and taste horrible. There's no true physical rehabilitation or therapy to get their patients to be mobil again. He gets "walked" one circle around the rooms, that's it. No heating or cold therapy no stretching no actual program to target healing or recovery. Now, he is doing with some infection, Nobody knows what it is , yet, he is not taken to the hospital. My dad was sent there after his stroke, the day after he was dropped him off there, I went to visit him, [[HIGHLIGHT]]the helper person told me your dad really liked the yogurt and apple sauce, I screamed and said no, he can't swallow even water![[ENDHIGHLIGHT]] I ran into his room, he was choking, had to call 911. My dad was admitted in this care center and passed away after a week here. my dad had underlying conditions however we knew his illnesses and when my dad got transferred here it took them 5 days to get all his medication which is unacceptable. They shouldn't accept patients if they don't have the medications they need for them. My advice to anyone bringing their loved one here is I would make sure they have the medication and needs before they get transferred here. They accepted my dad but really they shouldn't have. The facility couldn't meet his medical needs. This is a place for someone that needs an occasional check in and some physical therapy maybe once in a while. IT IS NOT somewhere you want a loved one with a lot of medical needs to be at. I was in terrible pain in my lungs. I begged to go to a hospital e.r. The pain was so bad That it hurt to move. I screamed and cried the other patients and staff mimicked my cries for help. Finally after hours sitting in a wheel chair all alone. I called 911 myself and begged the firemen to help me. The paramedics came to help me get to a hospital. My copd had turned to double pneumonia and 101'fever. I was hospitalized for 2 weeks. Do not trust the owner or staff, I was very sick and ignored by every person there including nurses and office staff who also mimicked my cries for help. Do not trust them. read more

No! Keep looking ! But, If your loved one is lucid and can manage themselves then maybe it's a less than fair experience. My dad was very sharp mentally but his body got progressively weaker in that facility. I think the stress of lack of compassion and constant errors added to his inability to heal. We were there because Northridge hospital insisted on that center. Let me try to give broad overview without the details. These were situations I personally witnessed and have photos to document. 1. The intake process was not properly completed for 3 weeks 2. The night nurse or CNA unplugged his call light from the wall 3. Nurse wrapped the wrong ankle& foot and in house medical care was average & sporadic 4. Someone stole his BRAND New body lift for dialysis center from his personal wheelchair 5. I would have to constantly CALL the center while Access transportation was outside for a pre scheduled pick up for the dialysis center ( I would sometimes have to leave work and take him to the dialysis center because Access had to leave) 6. The workers seemed nice, loved to smoke at the end of the building 7.Roommate was a young guy who was very loud and rude to my dad for various reasons. TV too loud !... Not a good fit for roommates. ( a patient next room said he was cussing at my dad; the administration never allowed us to change rooms) 8. The administration was distant and didn't know any patients really. 9. There's more.. but this should be enough. I've been in many facilities. This is the worst because so many issues in every department. God bless you if you choose this place. I'm so disappointed in this facility. My father was a patient for 21 days. Their nurses and physical therapists are nice enough, but the cons far outweigh the pros. It smells like urine, everywhere They don't have enough staff, so patients call out repeatedly, sometimes for 30 minutes unanswered to go to the bathroom or to get into bed. I called for the social worker no less than 12 times. I only ever got her on the phone twice. They ruined the discharge and my father not only never received a wheelchair, he also never received a referral for home health. I called four times after discharge and never once was called back. I ended up buying my father a wheelchair because Northridge didn't close the loop on the referral. I also had to pay out of pocket for an aid because Northridge never closed the loop on that either. Disposable. If I could give zero stars I would. Beyond terrible. Do not let your loved one go there. They'll end up worse than when they got there. Maybe now someone will call me back??? Don't judge a book by it's cover! That's exactly what came to mind when I went to visit my uncle at Northridge Care Center. From the outside or at first glance, I wasn't that impressed. But once I got inside, it was cleaner and much bigger than I expected. The staff was attentive, fast and very caring, which made my uncle extremely happy. I would highly recommend this place if you are looking for a Skilled Nursing Home for your loved one. read more

Had a family member here in the fall. This facility has a constant smell of urine. The beds are truly uncomfortable and the food is vile. How are people especially seniors supposed to get well when the food they are being served is absolutely horrid? There is not enough staff for the amount of patients they have. One CNA at night for about 16 patients. So if you need help with the bathroom best of luck to you. There was one shining star and that was the wound nurse. She's the reason this review gets one star. Nurses and staff have done a wonderful job caring for myself and my roommate. The staff always goes above and beyond to meet my needs and I always get my medicine at the correct time! The activities staff makes me feel like I'm at home. Gemma the RN always pops in to say hi and answers all my questions. This place is so wonderful and has made my stay excellent. I would highly recommend anybody who needs round the clock care to come to here. read more

My mother entered Northridge care center to rehab from a bad fall. The rooms were clean and the staff was attentive. There was a problem with my mom's insurance. Marie was awesome. She jumped right in immediately and solved the Insurance problem. My mom received wonderful physical therapy while she was in Northridge Care Center I believe my mom was well taken care of. read more

My father died one year ago (not at Northridge Care Center) and every month, Northridge CC sends him a bill. It is addressed to "Father's Name - Expired". When I called the billing office, they told me they will continue to send a bill to a dead person in perpetuity. My father was destitute and had no medical insurance. Because he's not in a conventional cemetery, I don't have a forwarding address for dead people. Northridge doesn't care. For clarification: I am not now, nor was I ever, financially responsible for my father's medical expenses. read more

The staff at Northridge care Center are very dedicated people. David is a good Administrator and listen to the concerns of his patients in the facility. I was in rehab for 100 days and I received very good care and excellent therapy. I am grateful for All the nurses and CNA's. They love Their job. I had a good recovery from my illness. My father in-law needed a place to go for recovery after a hip replacement and Northridge Hospital recommended Northridge Care Center. At first glance, it's not the fanciest place, but it was clean and the staff was very caring. This was my father in-laws first time in a nursing home and it was better than he expected. The physical therapy department was very hands on and motivated my father in-law to recuperate faster than we had anticipated. He even got a cute welcome bag with goodies, which was totally unexpected but very much appreciated. Thank you to all the staff for helping my father in-law get through the rehab and get back home! Would definitely recommend this place to anyone in need of rehabilitation after a surgery. read more

I'm only putting 1 star because I have to. If I could put negative stars on Yelp, I would for this review. Nearly every time I visited my father in the nursing home, I would see call lights going off up and down the halls. There did not seem to me to be a sense of urgency amongst the CNA's and nurses. One time in particular my father needed assistance and I literally had to walk up and down every hall until I found one nurse on the other side of the building. I asked her where is everyone? Why can't I find anyone to help my father? She said they were all on break. For a facility with patients who mostly cannot help themselves and are mostly disabled, breaking the majority of your staff at the same time does not make sense! My father complained of long wait times in response to the call bell constantly. We were in constant communication with the administrator. She and the staff knew who we were because we went there each week. However when it came down to it, it was my impression that we were just told what we wanted to hear, but did not observe any follow through. I felt like she was picking and choosing what she wanted to tell us and not what we actually needed to know. On more than one occasion, I personally witnessed staff members sleeping on the job and texting on their cell phones while call lights were going off up and down the halls. One night I witnessed my father's roommate being neglected. He was a very elderly man who was blind and disabled. He kept saying out loud "I am hungry! I am hungry!" The CNA didn't want to feed him until I insisted that he be fed. I would not recommend Northridge Care Center for anyone's family or loved ones. I know we sometimes have to make the extraordinarily difficult decision of how to care for our loved ones. You must be an advocate for your loved ones - especially those that are too old or sick to be an advocate for themselves! read more

A little House of Horrors! If I could give a 1000 negative stars I would. This place nearly killed my sister this week. They disregarded EVERY discharge order provided by the hospital upon her release and transfer to the facility. When my sister entered, she was walking, was continent, and talking. They put a diaper on her, put her in bed and essentially walked away. They NEVER asked her if she'd like to get up and use the bathroom, they NEVER offered to get her out of bed, they NEVER provided her with a television they never did a damn thing. I noticed that her feed tube sight was becoming infected on Monday-- the doctor REFUSED to prescribe anti-biotics! Tuesday, the head nurse was able to get her anti-biotics, Wednesday, when I arrived the tube sight was pussy, bloody, had a BAD odor and was leaking feeding fluid! Additionally, the feeding tube port was FILTHY. I demanded that she be transported to hospital. The night nurse provided false information to the doctor on call -- who was not going to transport my sister back to hospital. Luckily, the on-call doctor asked to speak with me. When I provided her with the accurate information, thankfully she agreed to transport my sister back to the safety of the hospital. Once in hospital, the infection was treated with "big gun antibiotics" before the infection could enter blood stream. Sadly, my sister's seizure medication levels were DANGEROUSLY low-- Gee, I wonder how that happened?? My sister,while in their care, stopped talking and began to have a psychological and emotional breakdown. The Northridge Care Center is negligent, sub-standard, and abusive. DO NOT EVER send a loved one there. read more

My mother in law was admitted for physical therapy after being released from the north ridge hospital. During her stay for one month she regained much of her strength and mobility, and was released. So good so far... She had good relationship with her care givers and was bid farewell by those we encountered as we waited for the release. 5 stars right here because of mom's recovery. Here is where the story goes south. Her medication history and care requirements were not clearly available and there was need for multiple employees/supervisors to confer regarding those details. Just about two hours to discharge. Down to 4 stars. Eventually we were released with the details and the promise of the required medical equipment (hospital bed, wheelchair, oxygen) delivery later that day. Didn't arrive... No communication... Down to 3 stars. Well... It has been two days and my mom is still without a bed... LOTS of miscommunication and unavailability on the part of the facility. The rating is dropping because of this... Down to 2 stars. The ordeal is not over yet, so the rating may drop further. The people we encountered were NICE enough... Just seems to be problems in keeping records and administrating discharge details and equipment requirements. Interestingly enough... If mom were still staying at the facility and receiving the physical therapy and routine care she was receiving, the rating would be very high... It's just been the last two day's debacles which are driving the numbers down. My wife is furious... So I'm providing as unbiased an account as I can. read more

One star is too much for this place. It's not even worthy of being in business, let alone receiving a single star rating. I won't go into detail of what my grandmother went through; however, will share that this place is disgustingly unsanitary. Many of the CNAs I encountered, including Rosalinda & Jasdeep (spelling might be wrong), were EXTREMELY unprofessional, lacked compassion, patience and regard for the patients' dignity & rights. They treated my grandmother worse than a street animal. The act different when they know they are being watched, FYI. THESE PEOPLE SHOULD NOT BE WORKING IN THE HEALTH CARE FIELD. The administrator, David, did not follow through with the corrective actions he promised. He, too, should not be working in the health care field if he's not going to do what is ethically & morally appropriate to protect these residents and ensure the highest quality of care. I wouldn't recommend this place for absolutely any reason to anyone. Horrible experience. I'm glad my grandmother is mentally sharp & aware enough to advocate for herself and tell me everything she personally experienced (could be considered elderly abuse) and what she witnessed other residents/patients being treated like. Hello my name is Kathleen Niez and yes I have been here before. My recent stay was for amputation of two fingers. This place is Awesome this was my 4th time here. I think there are some Awesome CNA"S here. The time I was here was very good, so while I was not doing the theraphy. They have an activity room run by two wonderful women. I made many friends especially 1 friend very close. This is a nice facility. I wish I could name all the staff that were good to me. The activity room was outstanding:) The Admin David was very nice . But the cna's all of you do work very hard. I had skilled nursing also the nurses were great:) The Care at Northridge care center is excellent. I must mention 1 person his name is Michael shall I say he goes above and beyond to help residents. I represent Dr Arthur Fass' office in northridge, he is a surgeon for Northridge Hospital who performed surgery on a patient in the hospital. The patient is required to come in for a follow up after surgery a week after being released from the hospital. This facility waited almost 2 weeks to call and make an appointment. When I asked them why they waited so long, they just said they were "busy". Then when the patients appointment presented they had a transportation service drop the patient off in our waiting room with no further instruction or even who to call when the patient was done with his appointment. The patient is wheel chair bound and needs help during ambulation. He was new to our office so we did need to collect insurance cards including filled out forms which the patient was unable to do on his own. Upon calling the facility I spoke with a very very rude man who said he was also the supervisor for the facility named RYAN. I explained that the patient had been dropped off unattended and that we needed someone to help him while here. The patient could be thirsty or have to go to the restroom and I am not able to help him because I am not certified or trained to do neither. All patients when coming to a doctors office (especially for the first time) should bring insurance cards and know that they need to fill out form in order to be seen. I explained this to the facility when they made the appointment. In any case, this review is specially directed to the staff and the unprofessionalism displayed by the person they have in charge of this facility. RYAN is not helpful and very unprofessional. Did not take responsiblity for his patient and infact told me that he was too busy to send anyone that he would reschedule the patients appointment. The patient is post op from hospital surgery and could not wait longer for another appointment because he would run the risk of infection. In any case I would take caution when using this facility because if I would have allowed them to reschedule the patient the patient could have suffered from complications in the end. If you have a choice DO NOT CHOOSE this facility. Very unprofessional, not helpful at all. Extremely UNPLEASANT experience. read more

My mother is patient at the Northridge Care Center. It is awful place when it comes to care for patients; they are short staffed. If you ring the bell no body shows up ever. Most of the food comes from cans or boxes. Most of the nurses mess up with patients medication. Terrible place! read more

My grandmother stayed there for 5 months and I was impressed with the personal touch the staff gave her and how they warmly treated me every time I came to visit. As my grandmother caretaker, I had many interactions with every level of the staff at The Northridge Care Center, I felt like they truly cared about my grandmother as a person and not just as a "resident". The social workers there go beyond their job description and attempt to help you out with anything they can and offer advice that really is based on their professional experience. I would like to give a personal thank you to Nancy, Sada, and Gaby for their support. Even the director of the facility, gets to know you and is aware of the issues involving each resident. Lastly, I would like to thank the staff of nurses form the RN's to the CNA's and LVN's, so many whose names I can't all remember, for always being light-hearted with my grandmother especially when she was a challenge. My mother was at this facility after having have been at a 5 star facility that nearly killed her due to their negligence. Don't be fooled by the outside appearance of the facility. It is very clean and orderly. The staff showed they truly cared about my mother as a person. I have to send a special thank you to Nancy and Ruthie as they really went above and beyond their job description. I also have to state that the nurses (I don't remember all the names) but Socorro, Christian, Melita,Victor, Maryssa,etc. truly monitored my mom's conditions and as result they were able recover and go home. They also truly work with your doctor and do not hesitate to call your doctor or 911 should they see it is medically necessary and that is very important. this facility was the best i could have found for my mom she just had a pacemaker install and this facility provided all the care that she needed . they really care about the patients which is a plus for me, they always call me if there is something wrong with mom and they always call the doctor rigthaway to take care of things also the facility is always clean and also the have a nice activity room which i am happy cause mom is always doing different things all the time painting ,playing dominos,reading ect ..well i am very happy my mom is here. read more
