My brother has been here for two weeks, post a major heart attack, and seizure, which almost took his life. He has recovered so well and has truly enjoyed all of the nurses he has had. He is a vegan and the food accommodations have been quite good. They make sure he takes all of his meds before leaving and they take time to talk to him (he is very friendly and chatty) The Only issue was the room phone not working) they addressed that within two days. The thing I would like to see is comfy patio chairs and make it more Inviting, as it is a nice area for those who are able to walk around. I will update this post with some of the names of the caregivers that have been so patient and kind. Also, PT and OT have been very good. read more

My uncle suffered severe neglect on a septic open wound t-11 t-12... absolutely avoidable if it had been treated earlier. Needs way more staffing and attitude change. He was a veteran. They wouldnt even cut his hair and relatives living far away could not travel to help so they neglect patients this way. Not recommended facility read more

Who after Woodland care was amazing making sure that we found the right care facility for extended rehab just amazing these two women. ***BEWARE*** This is the absolute WORST place ever!!! My aunt was here and it was horrible. She's currently bed ridden and they would leave her in A soiled diaper. She would press the button for someone to come to change her and they wouldn't show up. They ignore the request of the patients. Whenever I called, it took a good 20 minutes if not longer to be able to get anyone to answer the phone and get me my aunt to speak with because she's currently bedridden. She could not reach the phone. At times when I would call they would not say anything and simply throw the phone on hold They did that over and over again. Never anyone IN CHARGE to speak with. I would never allow my worst enemy to stay here. This place is dirty, smells, they don't give you your meds, the staff doesn't speak English and the food is disgusting! DO NOT ALLOW YOUR LOVED ONE TO STAY HERE!!! read more

This faciity is a place without ac in patients room and my friend who is 6.2 tall was given a small bed and not ac he wanted to leave the same day he was admited, and the caregiver was mad because he my friend had a bowel movement on his bed, this caregiver called him "pendejo" in spanish, meaning mother fucker in english then the caregiver touch very hurtful his testicles, it really make me legally act to the point I wont stop until this "facility" is close for ever Not even to animals you treat them in this inhumane ways. My friend has cancer, parkinson and is in pain, this morning they didnt have pain meds and their excuse its the Dr didnt send the order for it. So my friend called 911 from his room and the ambulance picked him up to take him to the hospital This is a Horror place Its a killer place read more

Horrible place and should not be open! Bed did not support my broken neck, nursing staff was more interested in talking on their cell phones to boyfriends, refused to give me the meds required by my doctors for treat and went 24 hours with no treatment! I checked myself out and they called protective services to have me picked up and brought back in! Their doctors and nursing staff should not be allowed to work with any patients at any level! It's criminal they are open! Also tried to force a TB test on me regardless that west hills emergency where I stayed for three weeks said I was clear of TB and they had the records! Their only interest is to bill your insurance! They should go to jail! read more

Woodland Care Center Skilled Nursing Facility was a godsend for my 93 year old Mom and my family. The administrative staff was kind and conscientious and took such good care of my family's questions and concerns. The entire nursing staff, CNAs, wound care specialists were gentle and very caring to my Mom. The professionalism was outstanding and made my Mom feel safe and free to heal without worry. Gloria, Criselda and all the staff greeted us with a smile and encouragement. With their help we were able to get my Mom home and the arrangements were made for her continuing healing. My brother was there about a week. At first, I thought this was a good place but when my brother went home we discovered he was not walked & his bed sores were not taken care of properly. Also, a care craft agent called & made me think she was from the facility! I do not appreciate the deception! My brother is in worse shape because of their neglect. My father has been staying here for a few months now. He likes it here. He is being well taken care of. The staff is very friendly, especially Sharon at the front desk. Please be aware, Ramen in the business office is horrible! Based on her, I want to give this facility 0 stars! The nurses are kind and may deserve a slightly higher rating (some more than others for sure) but my father is ignored a lot. He is left in a soaking wet diaper, quite often, and as a result, has had 4 UTIs in the year he has been there. Most of the nurses are so sweet to him and are lovely, but the nurses at the nurses station are cold and impersonal. Woodland Care Center is a typical nursing facility. From my experience, no nursing facilities are great, but that in mind this facility is on the better end in respect to its nurses. HOWEVER, this review is not of the nurses, but rather the business office, which is a whole other story. Anna called me to collect our monthly payment for our father, but I did not have the money at the moment. My father's social security check was coming in a few days, so I asked them to allow me to pay once his social security payment was received. This had been something the business office had done for me in the past. To my surprise Anna put me on hold saying that she needed to speak to her supervisor. Her supervisor, Ramin/Ramen picked up the line and the first words out of her mouth were... and I am not joking...."where is the money?!". Not hello, not I'm sorry, but we can't allow more time. Just immediate nastiness. When I tried to explain to her that I was waiting for my father's social security check, she just began to talk over me and threatened to report me to social security. This was clearly an implied accusation that I was stealing money from my father. How dare she?! I was my father's caregiver for 12 years before he had a life threatening fall, which put him permanently in Woodland Care Center. If I was going to steal his money, I would have cleaned him out years ago! What a bitter, miserable, witch. Needless to say, her threats and accusations did not encourage me to pay her any quicker than I am going to. This woman is horrible and I will refuse to deal with her in the future. So while most of the nurses are extremely kind and take decent care of my dad, the admin just doesn't make it worth it. Good job Ramen! read more

Three years ago, I was sent here to recover from major surgery for cancer. This place was worse than the cancer! Meals and medication were not delivered when they were supposed to and nurses rarely responded to calls. The bed did not function properly and I had no ability to turn on the light above my head. When I needed to arrange for the free ride service provided by my insurance company to take me to a pair of doctor's appointments, the case worker conned me out of $200! Finally, the place always sounded like an insane asylum because at least one patient was screaming incoherently. I recovered from the cancer but I'll never recover from the horrific experience of being forced to stay here! In my opinion, these people are criminally unprofessional! read more

This facility is toxic. Elder abuse and $$$$$$$ abuse. Never Never place your loved one in this facility. It's the absolute worse. Complaints have been filed with several State Agencies including Office of Attorney Generak; CA Dept of Public Health and LA County Ombudsman. What a terrible experience I have gone through regarding my husband's stay there for only 14 days they kicked him out saying he didn't have insurance coverage but they lied. When I first arrived here at Woodland Care Center, I was already coming with a very negative view about everything about this place. I had read very negative reviews and it didn't help that I was kind of far from home. I was very defensive about everything, I was very critical and kept looking for fault. However, little by little people here continued to treat me with care and respect and with time they broke my walls and my negative mentality. I was so ready to leave to another facility. However, I thought about it and told myself, you can do this. You're a people person, no challenge is too big. I'm glad I followed my heart. Everyone here from Sharon, Matthew, Charlene, the entire administration department. The medication nurses like Dulce, Funkie, Daisy, Adriana. A special shout out to my favorite CNA's Yanira, Francisco and a few others that I just can't remember their names. The staff here have gone out of their way to accommodate me. Thank you to the kitchen for putting up with my continuous food changes. When I leave I will truly miss you. I know I have made some friendships here that I would not have if I had gone somewhere else. I'm truly grateful for everything you've done for me. I almost forgot to mention Annie the night charge nurse. Thank you from the bottom of my heart for everything you've done for me. Sharon the receptionist has shown me so much love. Overall care provided by the staff exceeded expectations. The caring attitude of CNAs Esmi, Yu-Be, Petra, Eunice, Veronica will always be 5***** in our book. The unwavering and heartfelt support these caring personnel provided my partner were so genuine in the quality of care you' usually find only from family members. The helpful nurses Onie, Arnold, Robert and specially Geanie made our stay and personal challenges manageable. To Sharon in the front desk for her caring and thoughtfulness not only to us but to everyone who enters the premises. For my partner who is also going thru memory issues to remember the new friends and support he made and provided it was something. He didn't even want to leave the day we were discharged. To the PT staff for helping my partner walk during our 6 weeks of rehab please accept our thanks for a job well done. Do not send your family members here. my mother who has dementia spent two nights here, and these were our experiences: constant screaming from many rooms where most patients were crying out for help, not answering any phone calls, patients waiting to be changed for over an hour, upon arrival no nursing services provided for over an hour, and then only because we wandered and found an administrator, who Ordered it immediately - only over an hour late. Similarly the rooms do not look anything like the pictures online. The Woodland care center is clearly extremely short staffed. If you bring your loved one here, please expect them to be waiting. I visit a friend in this facility often and want to express how nice and kind the staff is. From the Valet parking attendant to the check in desk and all the nurses and aides. The facility is clean though dated. It could use a refresh but overall a very good place for rehab and care for those who cannot care for themselves. I would like to mention Bobby....she is a delightful, caring person. While I was visiting my friend there was a meeting going on that Bobby led. She was so nice to the residents asking for their input on how the facility could improve. She was very kind and listened to and wrote down the suggestions and or complaints. There were some interruptions, which she handled very respectfully. I was impressed! My friend is treated well and is making progress due to the help she is receiving. Thank You! read more

This is my dad's second time being admitted to this facility, and he has been home since last week. I can't pinpoint exactly what changed, but the staff seems more attentive, nicer, and less annoyed. If I could give zero stars I would. I was a patient there last week for physical therapy after a total knee replacement. One of the 1st requests I made was to talk to the dietitian due to food allergies. She finally came 2 days before my discharge and she took notes. I continued to receive milk (lactose intolerant) on every tray. The food was inedible most of the time. The only shower I had was given to me by a male CNA and I felt like I was in a prison shower. He got soap in my eyes and ears. He used a towel to dry my hair 1st, then used the same towel with hair all over it to dry me. All of the CNA's were terrible. They put a diaper on you and change you when they feel like it. One night I waited almost 2 hours for someone to come. Then there was the nurse who assumed I knew nothing about the meds I take on a daily basis. I asked her for a pain pill and she brought me a medication that I take for Fibromyalgia and argued with me that it was a pain pill. She then brought me the medication I asked for. Did I say the food was bad. Yes, it was TERRIBLE! Last, but not least was the internist they gave me. He has a 1 star rating on all the searches I did. Lev Gelman, MD. He talked to me like I was a child. He put me back on a med that my doctor took me off of over 6 months ago. He also kept trying to give me a medication that I was told not to take for 1 month post surgery. All in all the WORST experience of my life. read more

A one-star rating is more than generous for this dump. My godfather was sent here for skilled nursing after being released from the hospital. His "care" was practically non-existent. The first time I went to visit him, I was shocked. Once past the deceivingly tidy reception area, one immediately notices how filthy and unkempt the facility really is. As I rounded the corner to his room, I could hear him constantly calling, "Nurse, nurse, nurse" from his room. Once inside his room he told me he had been calling for at least 20 minutes. He needed to use the restroom, but was unable to get out of bed without assistance. When i finally located someone, I was told they didn't have enough staff to help him up so he should just go in his "diaper". He was utterly humiliated not being allowed to get to the restroom. His "PT" consisted of two attendants getting him out of bed and plopping him in a chair to sit for hours. During his short stay, he was transported via ambulance to the ER due to complications. A few days later, I went to the facility to pick up his belongings. His watch (a Breitling) has been stolen, as was a new scarf, sweater, and blanket. I was told by the staff as they handed me the few remaining items that were left that those were the only belongings he had. Do not subject your loved ones to this place. It's terrible. read more

If you love your loved one, PLEASE DO NOT SEND THEM HERE!!!! From day one there have been issues, starting with: 1. The facility accepting my husband as a patient while they were in the middle of a COVID outbreak and not allowing visitors. They did not tell us or the hospital case manager that this was happening, until AFTER my husband was transferred. 2. My husband was on morphine in the hospital for bone pain. Please understand, my husband has never been on a narcotic before. This was medically necessary. My husband was transferred to Woodland Care at 1:30 PM. I called my husband at 8PM. He was in severe pain, stating he only received Tylenol for pain that evening. I called my husband's nurse and asked where his prescribed pain medication was. She said she had medicated him for pain with Tylenol. She stated in a juvenile tone, "I guess I'll fax it now". She insisted the medication would arrive the same evening. I was horrified for my husband, being in so much pain and having to be treated by an RN with no critical thinking skills! 3. (con't from #2) I called my husband the next morning, early. I asked how his pain was. He said, "severe". He reported only receiving half a tablet of medication. I called the nurse and asked why. She stated my husband only reported moderate pain. I found that interesting because I've been on the phone with my husband several times when the, "Medication Nurse" comes in, NOT ONCE was my husband asked what his pain level was. I contacted the Director of Nursing and told her I found this highly suspicious. Under reporting and documenting that in an effort to under medicate a patient. Who took the other half of the tablet? 4. Expect your loved one to remain soiled for 30 + minutes before anyone comes to assist. I happened to be on the telephone with my husband when someone answered his call light. I was on speaker, so I heard the "nurse" say, "You need to wait for YOUR nurse to comes back, she just left for lunch". Then she directed my husband to eat his lunch. Who does that?? Would YOU want to eat while soiled? I called the nursing supervisor and asked if this was their policy to make their patients sit soiled for over an hour before they are changed, yet told to eat their lunch in the meantime? How is that okay?? She said she would send a CNA to change him. I called my husband back and we set a stopwatch until someone came in to change him. It took 30 minutes to get someone there AFTER talking to the nursing supervisor! There are other issues, these are the worst. As I write this, I've been on hold for 40 minutes waiting for the nurse, or someone to either answer a question or take a message, and no, it is not change of shift. I have been an RN more than 20 years, this facility is truly the worst I have ever dealt with. I am working diligently to have my husband transferred. read more

I would give -10000000 if I could. I have a Boarding Care and one of my residents was transferred this care or whatever center you call this place. I visited her today, she is non verbal old woman, She was hungry, she was thirsty naked in the bed with one sheet on her VERY VERY POOR CONDITION. No one knows what treatment are being done to patients, no one cares. People here work only on computers and doing some paperwork. Nobody cares about people here. I am shore this is a place to go and die from hunger. VERY VERY POOR QUALITY OF EVERYTHING, the place the service EVERYTHING. Even the homeless people on the street are treated better then in this center. read more

So disappointed in a place like this. If I could leave 0 stars I would. Just lost my Grandfather here the other day. Apparently he passed away the morning of when my Mom was called. When she had been calling him non-stop trying to reach him the day before yet was not able to speak with him. She talked to him every day he was there. He was either over-medicated and wasn't making any sense or in severe pain from the stage 4 cancer that he had. I'm sad my Grandpa died alone with no one at his bedside. read more

When we were forced to find a rehab facility for my 78 year old mom with less than 24 hour advance notice we visited Woodland Care Center and at first glance it seemed like the best facility (we had visited others and read plenty of reviews) Please note that the low reviews on facilities like this are there for a reason. My mom can not swallow (she is on a stomach tube and has to be fed and given meds that way) and upon arrival one of the CNs offered her WATER! Luckily my brother was there and stopped it otherwise my mom would have drowned as the water would have gone to her lungs. They also pulled on her G-Tube which is a huge no no. My mom also has to wear a Life Vest and they claim to be super knowledgeable about it yet 2 nurses had no idea where the battery even was on it. My brother and I were trained once and knew more about it then they did. My mom stayed one night at this facility and said that people were coming into her room all night to use her bathroom and people screaming like they were partying in the hallways. She was in tears when we went the following morning and after all these events decided to pull her out and care for her at home - her cardiologist recommended bringing her into the ER after they pulled on her g-tube and the facility fought with us to take her there. I do NOT recommend this facility at all. We had a horrible experience and it was only one night - one of the other patients there who saw us arguing with the staff about pulling my mom out commented that basically we are doing the right thing and that she begs to go home. Broke my heart. Stay away from this facility!! read more

Such low expectations but had hope for safe rehab, only to prove very low results. I hoped at a minimum my father would receive unbiased care and an opportunity to recover from a hospital stay. This is a fast food establishment with fast food quality. Beyond disappointed, everyone points the finger on insurance/Medicare. The "on-site" doctor didn't speak to us, never returned calls. I was very disappointed with the care my mother received there for a month. She went in alert and she left in a coma and passed 2 days later in hospice. The only reason she was there was that she had leg wounds that had been surgically repaired and she was there for PT. Basically they starved her to death. Then they locked down the hospital so that I couldn't come and see her because of Covid. Just a really awful place to bring your loved ones. read more

The rooms were clean, no odors in the hallways, didn't see nor hear any patients in distress, the posted schedule of activities seemed varied, the outdoor patios were in use by the patients, dining room was closed due to COVID protocol, rehab facility looked clean with various machines. clean and sparkling through out. Be careful with this facility. Thus far, they accepted a new patient knowing they had Covid-19 cases in the facility and have now experienced an outbreak causing a lock down. They also refuse to comply with the reasonable wishes of the patient for treatment and the establishment of a surrogate in violation of the patient's bill of rights. The administrators here thus far have been condescending and rude in addressing concerns on behalf of the patient to the extent that legal action will need to be taken in civil court. read more

If I could give them no stars I would. They never listened to my grandma and her wishes. I went from being the only one on her emergency contact to not being on there at all. A daughter who was given up had no legal rights but they allowed her to control everything and not even come to CA. My grandma told them I do not want my daughter involved and I want my granddaughter helping. They did everything to stop me voicing my opinion an just take more thann$10,000 from her. The moment I got cut off from communication my grandma declined fast. Then we hear she had Covid but I think she died of a broken heart being disconnected from all whom she knew an loved. Woodland care killed my grandma and let her daughter run the show. A daughter who she didn't want involved at all. I couldn't say goodbye to the woman I have cared for and I was in contact with her every few days. I came by and brought food, blankets, slippers, everything. They knew they had Covid cases that were not contained and took her in when she had a kidney infection, uti and dehydration. She shouldn't have been transferred so shame on Sherman Oaks Hospital too. I'm disgusted and they kept her from those she loved. I know I did all I could but there will always be this anger and disgust. To say I'm a mess is an understatement. read more

My grandpa was brought here and I would NOT recommend! If I could rate it a 0 I would! They are late with their patients meals. Every time he pushed the nurse button they wouldn't respond. They day I come to visit he pushes the nurse button and hour later nothing, and they just don't come it took for me to go the desk and ask for a nurse twice for them to finally show up. He had knee surgery and the physical therapist here put his brace on wrong, I had to take it off and put it properly for him. They ran out of diapers, it smells, it's dirty. Patients are screaming, one ran out, they don't warm up meals if they get cold or if you bring one from home. Please if you're thinking about bringing someone or family in here this is NOT the place! read more
