Don't read too much into the lower reviews here. For elderly care, the Right place to look is Medicare and/or Medicaid websites, which where I did my heavy research. After 3 long pages provided from a hospitals case manager, I narrowed it down to 5 that I had the patience to tour and interview in person. That's a lot to go to!! From there, I boiled it down to 2 places, West Valley and another on Devonshire was my backup. I am VERY pleased that after all that due diligence, I found out a friend that recommended this West Valley Post Acute center, without even knowing I had already chosen it as the winner. My detailed research and personal referral from someone else who had experience here made me feel even more at ease to place my Dad here. My Dad was released from this place last year and has been in our family's care at home. But after a recent hospital incident, a year later, my family had to put him back in rehab, per the doctors recommendation. Of coarse we had to have him back here and only here! Luckily, there was room for him. I was even able to request an amazing CNA named Sofia to be assigned to him again. All the nurses, doctors and entire staff are great, but we felt really comfortable with Sofia and grateful to have my dad assigned to her station. Overall, it's not easy to put your loved one in a care center.... even for short-term. But if you have to, then trust me this is the right place. If it wasn't then I wouldn't consider him back in the same place.... And I feel at ease to have him back in the same care that we trusted. read more

I wasn't happy at all my dad was there he got sick, he fell, they don't have enough stuff to take care of all the patients that they have. He ended up at the hospital twice while being under their care not because of the condition he went in the first place it's because of the lack of care that they had over there. My wife had Shingles - We took her to West Valley Post Acute after 3 weeks that she spent in Tarzana Hospital, and she was there for 3 weeks. [[HIGHLIGHT]]Having had to spend 7 weeks rehabbing from a pelvic fracture there, my experience was a nightmare.[[ENDHIGHLIGHT]] Very uncomfortable rubber bed and pillow, loud reverberating noise throughout the hallways constantly, I was attempted to be given a wrong rx, 3am room checks for noisy ice water refills, dirty bathroom, 30 to 50 minute wait for pain med request, food was beyond gross, no doctor check ins, rn overdosed a rx. not understanding the difference in the percentage, But the physical therapists were great. read more

And depending on the cna responsible that day for him he may stay in a dirty diaper for hours . [[HIGHLIGHT]]Whenever Daunte is on duty and my dad hits the call button his 1st concern is coming to the room turning off the call button and just saying the cna is not available now but they will come later[[ENDHIGHLIGHT]]. This really frustrates me because if the call button is turned off, how will anyone know that my dad needs assistance? If your biggest concern is shutting off the light and not my father, that's a huge red flag for me. Probably not the best place - if you, or someone you love is partially immobile...my visits usually occur around 6-7pm. Definitely changes your mindset of..."one day, this could be me" and end up getting stuck with a caregivers/nurses with a horrible attitude. Scary. At location, except for the receptionist at the front desk (who was extremely nice and very pleasant ) and MAYBE 1 or 2 nurses but other than that, very welcoming at all. The way they treat some of the patients who are immobile...is just heartbreaking. I cannot speak for everyone else so I will just speak about my visit and the nice lady who shares the room with my friend. My friend used her call-button to let the staff know she needed to be changed. A few minutes go by and then I saw a short nurse walk by the door - glances, but didn't come in and then immediately turns around and walks off. 13-15 minutes later...a nurse, or MA walks in to say that everyone is out on break. My friend sat in her feces for over 48 minutes. I cannot fathom every nurse, and every medical assistant working that evening, taking breaks and or lunches all at the same time. The poor lady next to my friend in the other bed, no one checked in on her the entire 2 plus hours while i was there. Even with her constant coughing...no one. Until they finally came in to change my friend's diaper. One of my friends nurses / MA had the nerve to be upset. She walked in with this super epic attitude. Rolling her eyes. Just horrible. It's like, come on. Really??? This is what you do. Trained to do...this is the profession YOU chose - helping and caring for others that aren't able. So to put them down and kick them at their lowest point. Heartless! I don't want to take away from the bubbly, very polite and professional receptionist nor the 1 or 2 nurses that were tending to other rooms but when I went looking for help - were helpful. read more

I was so thankful that west valley took such great care of my mother! I must be honest and say at first the evening shift did not cater to my mothers needs but as soon as it was brought up to the right parties, workers were fired and new hires were absolutely wonderful. Nursing homes have a bad rap so I avoided them until today. I visited my grandpa here after he recently moved in and I'm pleasantly surprised. It's super clean first of all, which I really like having OCD myself. All the staff we passed by and saw were really friendly and helpful. Everyone seemed happy to be there and to help all the elderly living there. The lounge in the front is really nice and the people come and talk with each other. They even come around with a beverage cart and serve coffee and water. Never thought I'd say this, but I want to live here when and if I get to live this long. Since I value restrooms, I should mention the guest restroom in the lobby is clean and cute. read more

I took my mom to West Valley Post Acute last year upon her discharge from the hospital. I did a tour around the facility and it did not feel like a skilled nursing facility but more like a rehab facility. They keep it very clean at all times. We were very skeptical about taking my mom anywhere as I have been her main caregiver and of course, no one can take care of your parents better than you right?! Well, I was completely wrong! Every single person in this facility is so caring and skilled. My mom actually enjoyed her stay there. She loved the food, the way the nurses and staff took care of her. They were able to nurse her back to health and help her out with physical therapy 5x a week. My mom was able to come home stronger than before. Unfortunately, she got sick months later and ended up in the hospital, while we were discussing discharge my mom asked to come back to West Valley Post Acute. We had many issues with her insurance but the admissions office worked diligently to get her back in. When she was admitted all of the previous staff who worked with her came into her room to greet her, let's just say that lifted her spirits immediately. They were able to coordinate her travels to the nearby dialysis center and arrange PT/OT right away. They accommodate her dietary restrictions fo and her physical disabilites. I have toured many skilled nursing facilities and this one does not compare to any!!! read more

We checked out a number of Skilled Nursing Facilities in the area, and they mostly seemed to have a smell of urine and were quite depressing. Until we walked into West Valley Post Acute. Upon walking in we immediately noticed the clean facility with a nice clean smell. We were greeted by a warm and friendly staff, including residents that seemed truly happy to be there. I ran into what looked like the family member of a resident and stopped to ask her opinion of the facility. She couldn't stop raving about the care and PT team. She said they have had multiple family members there over different periods of time which really made me feel at ease with leaving my mom there. The staff has truly been great with my mom and always willing to give an update on how her rehab is going. I would absolutely recommend this place for any family member. read more

A positive, happy, lively atmosphere is evident at this facility. The corridors and patient rooms are clean and well maintained. every staff member we encountered was patient and knowledgeable-it wasn't just a "job". Vanessa at the reception desk and then Maria Jimenez at the nursing station were so kind to everyone, amazing and able to navigate the specific paperwork requirements. They don't care about their patients, my father was send here from the hospital, and he was only there for 2 days and that was enough for me to go get him and took him back home, they don't feed them and don't give them their medicines and if they have COVID they just let them die there. Contacted West Valley and they worked with the hospital and insurance to complete the transfer within a day. Everybody at West Valley was helpful and would certainly go here again if needed. read more

My gramma stayed is this facility a month ago and went to visit her many times, my review is not against the helpers of my gramma they were amazing but when I had a concerns about her nursing management and asked to speak to the Director of Nursing (Flor) she completely ignored my request.... literally I was able to get out of my gramma's room and see her in her office just watching her phone but she was too busy to sit with me for a couple of minutes, I was always direct to other people... We had to asked to a discharged to take my loved one away from a place that the main supervisor doesn't care for the patients and families, she is giving a bad reputation to your place, even that others are working hard the Director is not a true leader, management should be looking into this seriously. read more

If I could give this place a negative 10 I would. ***DO NOT SEND YOUR LOVED ONES HERE**** this place is the worst of the worst. My mom was brought here from the hospital. She has severe dementia. This place did not care for my mom properly. Put her on medication that is not FDA approved, is BLACK BOX WARNING, and states everywhere " DO NOT GIVE TO ELDERLY PEOPLE WITH DEMENTIA IT WILL CAUSE DEATH" when I said something to them about it, I was told this " a lot of people here are on medication that is not fda approved. Then they moved her 4 days later to another facility who 2 days later sent her back to the hospital saying they are not set up to handle dementia patients. Hi there. My brother David has been a part-time resident of West Valley Post Acute on and off for about three years maybe less. So we have some history here. I will be honest and biased in my opinion for all positive reasons. We as a family have been so blessed with the Location, Administration, Staff, Kitchen Chefs, Nurses and amazing RA's!!! From Medicine/Pain Relief Department to Physical Therapy...and shout to Social/Recreations staff as well. You, ALL, Collectively as a Village give us a peace of mind. Oh my gosh!!!!! if you want to kill your family member go ahead and let them stay here at this facility. There's so much neglect it's ridiculous. It's the most mismanaged substandard facility I've ever been in in my whole life. They killed my Parent. My Parent died of a cubitus sore so large that it was not possible to even heal at all. There are so many other places .. none of them are great but this one in particular is horrible! Watch out for this one doctor his last name Dr.Shaaraway and Dr. Hirsch...Two of the worst medical providers I've ever come across totally substandard medical care please watch out! I only wish that what happened to my Parent would happen to both those doctor's parents that's my wish. Then they can see how much pain we had to go through to watch our parents die under their care. West Hills Hospital is the worst hospital ever as well they hide things that they do there they don't put stuff in the reports !!! This is a rehab facility not geared toward geriatric patients. The administrative staff was the most helpful as we were checking my mom out. They seem very short handed and nurses are hit and miss. If your loved one requires a lot of assistance, this is not a good choice. read more

This is the worst care facility I have ever been to. I have worked in the nursing field for 23 years and I have never encountered such incompetence. The staff there does not meet the basic needs of the patients. While my dad was there, he developed a really bad rash, because he was not changed in a timely matter. He was laying in stool and I asked three people to change him. Nobody showed up and when I went to find somebody, the entire staff was in the dining room, eating. I would not recommend this place to anybody. You do not want your family member here. The nurses were fine, but some of the CNA's were awful. They didn't seem to care at all about the patients and a few of them had really bad attitudes. I had to report the facility to the state and I have never done that in my life. I work as a nurse and care for my patients as if they were my own family members. Please do not send your loved ones here. they do not care and any complaints you have will just fall on deaf ears. There is also a lack of communication regarding the plan of care. I feel bad for the patients that do not have anyone to advocate for them. It's a shame what our healthcare system has become. read more

Worst post acute center my grandpa was there only a short time and his experience was terrible staff does not tend to patients accordingly for three days they did not clean his incision, per Drs orders he was not able to get up unless someone helped him he called for a nurse for two hours until he finally decided to go on his own he wasnt taken do his dialysis until we personally had to go and take him, also the nurses threatened us to call the cops because we were taking him to his dialysis. read more

The facility just went through a renovation & the place is so nice & clean. The ambiance is great & very inviting, the staff were friendly & very helpful. As far as a facility of this nature I really couldn't ask for anything better to help my family members. I truly have all the trust in their staff & procedures. 25 "Deficiencies" in the past 3 years. My mother-in-law was in for two days and we got her out...Stay Away!!!!

zero stars is a better rating. Do not use this place! My husband was sent here from the hospital because they were the only place that would accept a covid patient. The staff don't give a damn about what they should be doing. my husband is on paxlovid and they never gave him the medication so now the five day cycle has been broken. When I explained how horrible that is Carmela told me it's not her fault. really? Who then? My husband arrived there by ambulance at 6 pm and I sprung him from that place the next day at 10 am. 'Against medical advice', since there is no doctor there to assess patients until FIVE DAYS LATER! He came in on Thursday and the doctor would see him on Tuesday, what an abomination - STAY AWAY!!!! read more

Recently, my grandparents got unhealthy so I decided to send them both here. The staff is very kind and nice and they make sure to keep my grandparents happy :) the food is excellent and the place is also very sanitary. They wash my grandparents clothes and take them showers every few days. They are always nice to visitors and bring a very good vibe whenever I visit this place. If you or a loved one is considering to send them to a nursing home, I really recommend this place! read more

I am fed up with this place , they are the most unorganized facility, I just can't take it anymore! I am tired of calling, texting & going there to ask for simple things! They can't follow simple instructions We have repeatedly asked the dr to take care of simple situation, the nurses are clueless , all just walk around looking busy and celebrating each other's birthdays One more incident and I will report them A simple incident hasn't been solved after a month of back and forth , talking to every single nurse for each shift and talking to management The worst !!!!!! read more

I've been to West Valley Post Acute several times and have always appreciated my stays. The administration cares about the patients and the staff do a wonderful caring job. Think twice before bringing your loved one to West Valley Post Acute. Our loved one was 89 years old, suffered from Dementia so she was confused & didn't want to stay put, was an extreme fall risk, & they put her in a wheelchair with only one working wheel brake. In the dining rooms/activity rooms, when a resident needed something or wanted to use the bathroom, many times the residents were ignored, much like when the residents were in their bedrooms & needed something or wanted to use the bathroom. They had our loved one in a diaper for bladder leakage, however she was able to verbalize when she needed to use the bathroom. She would push the call button to alert the nurse's station, but many times the button didn't work, & if it did, it was ignored. Daily when I would be there, I would have to go into the hall to try to alert someone that my loved one needed to use the bathroom. I would be told she has a diaper on, & to let her go in her diaper. Again, she was completely able to verbalize when she had to go to the bathroom, so how humiliating is that to be told you have to go to the bathroom in your diaper? I would also be told her nurse is on break & she would need to wait to use the bathroom. One day our loved one's nurse told us she was passing out the lunch trays so she would assist our loved one to the bathroom this one time, but normally she can't assist patients during breakfast, lunch or dinner time. This nurse then proceeded to pull our loved one to the edge of the bed to have her walk to the bathroom...Our loved one couldn't bare much weight & was more or less wheelchair bound, so I'm not sure why the nurse would have her stand & attempt to walk to the bathroom? When I told the nurse our loved one needed to be wheeled in the wheelchair to the bathroom, she told me our loved one would have to walk to the bathroom or go in her diaper because the wheelchair would not fit in the bathroom, yet the following day when I had to hunt someone down once again to take our loved one to the bathroom, they wheeled her into the bathroom IN the wheelchair. There was another resident down the hall who the nurses would taunt & mimic. This resident was older & clearly had some mental handicaps, perhaps Dementia and/or Schizophrenia, & she would call out "Momma", "Momma where are you?", "Momma I need to use the bathroom.", & the nurses would stand outside her bedroom door & mimic & laugh at her, & then go in & tell her if she wants to use the bathroom she has to get up & walk over there. Mind you, this resident was wheelchair bound. Every day & night they would call us that our loved one wouldn't eat, was acting out towards staff members, yelling & didn't want to stay in bed or sleep, & we need to have a family member come sit with her all day & night, or pay someone to come in & sit with her. Mind you, this was not typical behavior for her, & although Dementia & change of scenery can totally throw a patient off, it seems to me that it was up to them to stay in contact with the doctor to figure out what was going on, & work on a solution & medicine regimen to help with it all. Instead of the doctor intervening & evaluating the situation, the facility just continued to call us daily stating we needed to come sit with her or pay someone to come sit with her. It wasn't until the facility director for the new place we were going to move our loved one to, & the Hospice Nurse who would be caring for her at the new facility came & met our loved one at West Valley Post Acute, that they confirmed our suspicions that our loved one was not being properly cared for & that her needs were not being met & she needed to get out of there immediately. Whenever West Valley Post Acute would touch or even try to shower our loved one, she would scream out in pain & lash out at them as a result. Our loved one was admitted with some head bruising from falling, but each day following when we would go visit, she would have new bruising in a new area of her face...That was worrisome. When we would follow up with them or ask how our loved one did, they would tell us they don't know; it was someone else that was on that shift. I'm not sure if no one ever took the time to document notes & put them in the computer system for the following shifts to pull up, or if at change of shift, notes were not exchanged, or maybe the staff was just too lazy to pull notes up in the system each time we would ask, but the facility was very poorly run & managed. The Case Worker assigned to our loved one never seemed to be available, whether it be through email, phone or in person, so we were basically never able to get answers from anyone unless we were there to witness it ourselves because otherwise it was I don't know or he said, she said. Needless to say, I would never recommend this facility, & perhaps question if they should even be in business based on their level of care, or better yet, lack of care. read more

This place has a beautiful and clean design. I was happy to bring my father here after his hip surgery!! The staff was warm and inviting during his stay.
