Everything!!! Communication is horrible the place looks ok Mondays through Friday weekends it stinks my moms hair is never brushed she looks like she has had the same clothes on for a couple of days. I wish I could find a different place but with Medicaid your choices are limited the DON is the most uncaring person I have ever met. A few nurses are sweet and work hard but I can count them on one hand. The staff only smile and act friendly when they are doing a tour then after that very few people even speak when you come in. The care is the minimum they have to do. If you have a choice go somewhere else. read more

WARNING: DO NOT SEND YOUR LOVED ONE TO THIS FACILITY!! My loved one was sent to this facility for physical therapy/rehabilitation purposes only. In the week and a half she was there, she was thrown in a diaper, kept in her room, sedated, and without any rehabilitation. My loved one fell from her bed on 3 separate occasions, and not once was I notified of the incidents. I only found out recently, that the medical assistants/nurses, were rude and condescending to her, and would roll their eyes regarding whatever she was asking. They would keep her in her soiled diaper for up to an hour at a time, multiple times a day. She/we never once spoke to a doctor during the week and a half there. I don't even know if a doctor was even at this facility. Even after repeated attempts were made daily to speak to the doctor. Her health was declining rapidly there. The last straw, a final fall from her bed! My loved one called for help and no one came, so she had her phone and called me and I immediately went down there and removed her from that hell hole, and placed her back at the hospital. When she arrived at the hospital, she was given a POOR PROGNOSIS and was referred to PALLIATIVE CARE, to finish dying!!! In a week in a half, my loved one went from only needing physical therapy to DYING!!! She would not have made it if she wouldn't have left that hell hole the day she did! This place is by far the worst care facility. My grandmother was left to lay in her urine and dried poop on her backside, all day long. Bed soaking wet from the back of her neck to her calves of her legs. Has happened several times and now she has an open wound bedsore. They don't get her out of bed and dress her ever. She doesn't even know when she had a bath last. Nurses aids here are lazy and charge nurses don't do their job making sure their aids do their jobs. Our loved one was at Park Bend for about ten years. She was cared for the best way they could care for her. There are no perfect places, but this place has staff who treated her with respect and dignity throughout her stay. Every time I asked about her stay there, her reply was, "I'm happy". They cared for her during Covid, loss of staff, and changes in management. I am grateful for the staff who consistently showed up and did their very best. read more

TERRIBLE EXPERIENCE. My step mom was here for about two weeks after a few months in the hospital and within those weeks she became completely immobile, incomprehensible, and fell out of her bed 3 separate times. After the first time she had fallen we'd asked if they could put rails on the bed and nobody gave us a clear answer. The second time they told us that they cannot put rails on the bed because it "makes the patients feel claustrophobic." She never saw her doctor after asking countless times to speak with her. The care taking was terrible. Before she went into the facility she was able to go to the bathroom on the toilet on her own; once she got to Park Bend it took her a few days to become bed bound and being put in a diaper! And don't even think about telling the nurses they're cleaning you too hard because they'll just roll their eyes at you and scoff. Unbelievably unprofessional with the lack of care they give to their patients. Even the physical therapist was awful! As soon as she would begin to feel dizzy after sitting up for a few minutes he told her to sit back down and that he would be back soon. He NEVER returned! He gave up until the next time they had their weekly appointments and would continue to do the same thing each time he saw her. Overall, we do not recommend this place. The quality of care patients receive there is COMPLETELY UNACCEPTABLE and I can only imagine what the people who reside there long term go through. read more

My mother has been at this facility a little over two years, I am extremely involved and pop in to visit very sporadically- I must say I have been pleasantly surprised to see that the majority of her care takers seem to generally care for her- my mother is no walk in the park and can be verbally abusive and just plain mean at times- yet staff seems to maintain a professional and friendly demeanor around her- especially Eli, he seems to have a positive outlook and is always willing to lend a helping hand. She was initially admitted for physical therapy following extensive surgery and several strokes- we thought she would be only there for a few weeks but unfortunately it turned in to a much longer stay. The PT director, Debbie was amazing, going out of her way to work with mom to make progress which would lead to her being able to be up and moving now, I do not think we would be as advanced as we are today without Debbie's help! The majority of the CNA's there are friendly and helpful. The food is good, my mom enjoys eating there and often times tells me the food "is really good", and the times I have been there to eat with her in the cafeteria, they did have very tasty meals. My mother developed a bedsore from a hospital stay- stage 4- so it was extremely bad, the wound care nurse there- Jennifer was amazing at caring for it. She would make sure to send me progress reports and track the size, depth and make sure all medications were administered when they should have been- the PA, Christina was phenomenal- she would make sure mom was always comfortable and would periodically review mothers medications and make informed suggestions regarding her care. Christina is a Godsend to this facility for sure!! My mother's assigned nurse, Kusum is invaluable to making sure moms care is successful, she is very attentive and helpful, my mother calls her, daughter-that alone speaks to the relationship she has fostered with her!! Why did I wait over TWO years to write a review.... its because consistency matters!! Any facility can treat your loved one good one or two days.. but for a solid two years my mother has received excellent care. When I have spoken to the facility director, Adrianna, I have always been greeted with a smile and a very responsive and positive attitude, I believe that when there is good leadership there is a positive environment, and that seems to be the case here. I am glad I did my research initially and choose this facility for my mothers care. read more

I have to point out it's easier to find decent reviews on restaurants then it is for important family care decisions. This one hit home and it's been a pretty horrendous experience. We just checked my grandmother out of this facility today for basic needs not being met. We're a family that doesn't take kindly to our family members being treated like an after thought. She was admitted after she had a brain bleed after a fall. The entire time she was there we were close by her side. Every single day we watched what was happening and it was rather alarming and disappointing that they allow our elders to be treated like garbage. She text me this morning begging me to leave so we did just that. She wasn't given any pain medications, any hope for physical therapy and they barely had water available for her. The day staff was somewhat available. But that was because we were there. After we left it was a much different story. The nights were a nightmare for her. I got there this morning and she had been waiting two hours to have someone help her put her pants on. After a night of having a night nurse demean her and leave her unattended in the bathroom. She was told her to hop into her wheelchair. She needed help and couldn't do that. It has all left a terrible taste in my mouth and we are thrilled to have her out of their care. Don't buy the brochure pictures my friends. Not everything is what it seems. Do your research and find a different place for your loved ones to recover. The obligatory one star was because their food was good. But what good does that do you when you see people walking around in soiled pants and hear of others with rashes from said soiled pants. Do better for your elderly. And have some basic respect. There is not a cover your a$$ clause when it comes to taking care of my grandmother. Trying to reach the front desk this evening and the first time I phoned, whoever answered hung up on me. I waited a few mins then tried again and it just rang (no answer) and the third time whoever answered the phone just sat there and didn't say anything and then hung up. You always here stories about unsavory goings on at old folks homes and the behavior of whoever is sitting at the front desk on Wednesday December 23rd at 6:25pm isn't inspiring any confidence. After having trouble getting them to contact BCBS and coordinate with the hospital, I thought things would go more smoothly. I thought wrong. Meeting the ambulance with my husband there to admit him into the facility was one of the most unorganized things I have ever seen. No one at the door to let us in. No one at the desk to guide us. Finally I found a nurse to guide me to the corridor he would be on. That nurse stated he had no paperwork and didn't know what room he would be in. This was a real red flag since my husband has specialty medication that needs to be given at specific intervals to sustain his life. After finding the paperwork and telling me the room number, I went back to tell the ambulance driver who was assisting delivering my husband. Again, no one to show us to the room and when we entered the room it wasn't a private room. The vibe wasn't good either, so with all the red flags, I asked the ambulance driver to assist me in getting my husband into my car so that I could take him home instead of risking his life there. I sincerely hope this was a one off, but I wasn't willing to risk it. read more

The staff at this facility are top notch! I was impressed with the cleanliness right from the beginning of my mother's rehab visit, and the staff couldn't have been more caring. A special thanks is in order to the Admissions staff, Jerrica. She worked very late on Friday evening to process my mother's intake, including taking steps to accommodate her special diet. The admitting doctor came in at 8:40 pm that evening (remember, this was Friday) to examine my mother and I received a personal call the next day with an update. The food was excellent and the long term patients that I spoke to all said they liked living there. I believe the staff makes this a five star facility! read more

I just ended a three week stay at Park Bend recovering from a ruptured Achilles tendon. Nursing and therapy are both top notch. The staff is caring, friendly, professional, and helpful. I was subjected to both physical and occupational therapy (Slavedrivers! The lot of them!) and I was noticeably stronger and more balanced after three weeks. I told everyone I wanted to go home and they helped me make it happen quickly and safely. There are some negatives - the food sucks, the facilities are bare bones, some of the equipment is pretty old - but these are quibbles. It's a nursing home, not a spa. After trekking around North Austin looking at nursing homes and not finding much room at the inn, unless it was a really crappy inn, this place stood out. In a good way. For one, it's clean, and for two, it's well staffed and well managed. The intake manager and business manager are both kind and professional, and made the experience of having a loved one in this situation just a little easier. It made visiting a fairly pleasant experience. Overall, the caregivers and care managers also were pleasant, patient and compassionate. During the year I regularly visited a family member here, I never saw a patient mistreated or miserable. Residents are allowed to bring their personal furniture and belongings and to decorate their rooms as they wish, so it has a homier feel. They encourage family member involvement. The atmosphere is one of caring and respect. I can't comment personally on the food, but several residents said it was good. I didn't see much staff turnover. These are all good signs, as far as I'm concerned. The only thing I would change is for the place to offer more interesting upbeat activities/entertainment, and to make a concerted effort to bring more residents to watch or participate. Some music over the intercom system would be nice, too. Especially in the dining and sitting rooms. read more

What a sad and depressing place.My God.They allowed our grandmother with dementia to sit unaccompanied in a wheelchair and she tried to get up and fell and hit her head. When we arrived no one was even with her.They had just cleaned the blood up.No ice pack.No concern.no monitoring. We later found out that they hadn't even feed her for days,it was another patient who told us in secret! My mother has been at Park Bend for about 5 years now. She has been treated well, and our overall feeling is positive. It is quite a challenge operating a healthy and positive environment for elderly people, many in very challenging mental and physical conditions. My wife and I credit the people and administration for working hard. No place like this is perfect, and there are better days, and not as good days. Park Bend is in a state of disarray. I agree with Elizabeth and Claire. Some of the staff care, but most do not. I have witnessed actual heckling of a dementia patient by the staff. Many things are promised, most are not delivered. The most recent disappointment is the Acivities Director ( one who cared) has left. No explanation to the residents and no schedule of activities. Giving this place even one star is being nice. Our loved one wasn't taken care of at all here. They didn't change them when we came at 12pm. They had been sitting in their urine for who knows how long. They didn't eat because nobody made the time to stop and help feed them. They would just drop off a tray. Then the worst of it was that they fell and hit their head! They had a enormous bruise on their head and they acted like it wasn't major. They had left them in a wheel chair unaccompanied! This place is really good, mainly due to the staff and nice grounds. The staff is very responsive and you can actually get a hold of them pretty much whenever you want - which is not the case at other rehabs in town. The staff also seems to really care and they are kind and accomodating. It always seemed clean to me. The only thing that would be better are the beds and linens - pretty thin. read more

My mom was at Park Bend for a year and half before she passed. A few of the staff members truly care about the patients that are there. then there are others who don't give a damn. I witnessed patient's telling CNA's that they need to go to the bathroom and the CNAnot help them and telling them that I am not assigned to you. It seems that they tend to run off all of their good employees. Don't let the pictures on their website fool you. All of the pictures are staged. The rooms do not look that nice. I will say that they do keep the place pretty clean. We did my mom's laundry but for some reason all of her clothes would go missing on a regular basis, even though they had her name and room number stamped in them. If your loved one is a private pay and they pass in the middle of the month, do not think that you will receive a refund anytime soon. My mom passed over a month ago and we are still waiting for them to refund us for the 1/2 of month that mom was not there. They keep telling it takes 30 days. Mom has been gone over 45 days. I wish I could give this place 0 stars. My father was here after a hospital stay for the flu. Never again! I would not even put an animal here. The nightmare starts with the admission nurse and we should have just left when we met her. She stated she was an LVN but she could not even read the documents she had to complete to admit us. The nurses Priscilla and John are so nasty and unprofessional I am shocked they even have licences. They sit at the desk and ignore everyone and then when they do arrive in the room they act like you are putting them out. The aids where generally very good but they are only as good as the people above them. This place is disgusting! The food is so bad it is not even identifiable and comes cold at all meals. The director of the facility yells and degrades her staff in public and even came close to making an aid cry she was so nasty and did this in front of us. She has a dog that walks around the building freely and uses the bathroom on the floor and nobody even cleans it up. There is no discharge planner and when I asked who could help I was told nobody the discharge planner left. The management nurses are so overwhelmed they can't even answer a question without getting interrupted. I am sad to say this is the worst facility I have ever seen and fear that it is only a matter of time before it is on the news. They should be ashamed of the lack of care provided. I was told that this was a 5 star facility by the representative at the hospital if this is true someone clearly paid off the state as I can't imagine anyone saying this dump is 5 stars. My dad was only there for a short time but there are halls full of patients who are there for life and it is just so sad to see how they are neglected. Please stay far away from this place! read more
