After making ageist comments about our father (he was a very young and active 80 year-old) and reminding us several times, as an excuse for why they weren't invested in continuing life-lengthening care, that he had been in and out of hospitals over a handful of months, the nephrologists and a cardiologist got together with a representative of the hospital's ethics committee and decided to allow him to die. Indeed, our father had been to hospitals several times. He was on a journey to discover why his legs were swollen. The doctors treated him with kidney-damaging diuretics (Lasix), which acutely (temporarily) damaged our father's kidneys, prednisone (which damaged his skin), and gave him a medication for destroying his immune system, as they incorrectly assumed he had an autoimmune issue without first confirming this diagnosis. Destroying your immune system apparently prevents it from attacking the body in an autoimmune issue, but it allows you to be more predisposed to getting infections or not fighting them. Our father did finally gain an accurate diagnosis: a very manageable form of leukemia that doesn't shorten life, if managed, which, after his correct leukemia diagnosis, he was doing. The side effects from the medications he was incorrectly prescribed during his hospital visits caused additional hospital visits, as the side effects were worse than the leukemia and leukemia treatment (which he was tolerating beautifully). While hospitalized for the side effects from the incorrect medications the hospitals gave him, he obtained infections such as RSV, C Diff, and a bladder infection. Due to my father's physical issues from the side effects of the medications, the doctors at Providence decided to not provide life-saving treatment and instead wanted to give him pain medication to lessen the effects of the death sentence they decided for him; otherwise known as "comfort care." We argued that dialysis could be an option as his biggest issue was fluid accumulation in his body. Providence's nephrologist said no, but when we asked for a second opinion, the second nephrologist said she would give him two weeks on dialysis only. We worried about this as it sounded like they would "pull the plug" after two weeks no matter how our father was doing. The dialysis was very effective. My father began recovering. His blood work revealed steady progress, which we were monitoring. He was chipper and not in any pain but also very worried, like we were, that the hospital would "pull the plug" on him as they had threatened by saying they would only give him two weeks of dialysis. We searched for how we could get him needed continued care after the two weeks and discovered that the state of Oregon shut down all long-term care facilities in the state. It would appear Oregon doesn't value the life of anyone who might need long-term care. The closest long-term care facility was in Seattle, Washington. We were ready to have our father moved to Seattle, Washington, if needed. While at Providence hospital, our father's kidneys showed signs of improvement, as he urinated with the appropriate color. On the last day of his two weeks allotted for dialysis, the hospital gave him a pill form of a medication that helped to stabilize his blood pressure so he could tolerate dialysis, which lowers your blood pressure while being treated, instead of the correct IV form of the same medication. The hospital admitted that the pill wasn't as effective or helpful as the IV version of the same medication, which our father was previously getting. Thus, on the last day of the two weeks of dialysis, his blood pressure dropped too low to safely continue. The nephrologist known as "Dr. K" let us know that she was going to let our father die. She refused to allow our father to again be on the IV medication that had worked for him which would allow him to continue dialysis. We argued with Dr. K on behalf of my father that if his blood pressure improved, would she allow our father to continue dialysis? She said if his blood pressure improved, she would okay it. We asked our father if he had seen a cardiologist while at Providence, and he said he hadn't. We got a cardiologist to review our father's chart, and she confirmed he could indeed continue on the IV form of the medication that effectively stabilized his blood pressure so he could continue dialysis. Even though our father's blood pressure was stabilized by the IV medication that was cleared by the cardiologist, Dr. K, would not allow him to continue the needed dialysis. According to the dialysis technicians, it takes a few weeks up to approximately three months for an acute kidney issue to heal. We were invested in giving our father that time. Providence Hospital was not. Upon learning that Dr. K would not continu

My dad is there recovering from a heart attack. No one there communicates from one shift to the…read morenext, or from one day to the next. They would not give him his pain meds as prescribed. When he complained about not being able to sleep, and how much pain he was in, the nursed would ask him the same questions that another nurse had asked him the day before. Then we got a lot of "I'll leave a note for your doctor". It seems that the note never got left because we were told that several times about my Dads pain. He would occasionally go up to 11 hours without it. It was prescribed for every 6. So I asked for a list of his medications and the times that he is given them. I have medical power of attorney, so it seems easy enough. Nope! Not here. First I asked a woman at the front desk area, who refused to give me her name, and she told me I had to find his nurse.... Well, he was nowhere around my Dad's room, so I asked his CNA, Moreno. She told me that I had to ask his nurse. Like Im supposed to wander around the care facitly looking for his nurse. So I hit his call button. 25 minutes later, I timed it, Moren, his CNA came in. "I don't need you, I need his nurse", I said. He finally came in and asked the same questions and said he'd leave the doctor a note. Unbelievable. So I tried to contact 2 people in charge of his care, Debbie and Joyce. No one answered the number I was given by his nurse Jordan, and Debbie hasn't replied to my email. It's been over a week and still no response. I am beyond pissed off about this. I would tell anyone looking for a care facility, full-time or for recovery, to pass on this one. They obviously don't care much about their patients. And, sorry this is so long. If anyone from the facility would have called me back, I would have told them all of this in such less public way. Please! Pick the next one on the list...

I was in this faciity from mid December, 2024 until January 8, 2025. It was horrible, the food was…read moreawful, and the kitchen ignored requests. I stated several times that I am allergic to cucumbers, and yet they were still in every salad (I ate a lot of salad because so much of the food was inedible.) They were short staffed, so I frequently waited up to 20 minutes for a CNA, not good when the meds give you diarrhea. Most of the staff gave it their best, but not all. One night an older woman (I presume a CNA) came in, bent over my bed and shook me angrily. I was stunned, but later figured out that she thought I was the woman crying, "Help me, someone please help me," on and off all night. Still no excuse for what she did, and she should be fired for patient mistreatment. I would NEVER allow a relative of mine to be here; they don't pay enough attention to what patients need, and at least one of the actually abused me. When I first drove up, patients were coming and going as they wish…read moreand smoking in the parking lot, leaving their cigarette butts on the ground. i was going to meet our father there, but arrived earlier. When I went in, worker after worker just walked right on be me. I ended up going to the nurse's station and they just said he hadn't arrived yet and I could wait in the lobby. No welcome or offer to show me around. After over an hour he hadn't arrived so I went back in with his stuff. They just told me his room number without showing me where I could put his stuff, so had to go back and ask. Care was terrible. Our dad said it would take them over 30 minutes to come in for help to the bathroom or anything else and would just watch him struggle to get out of bed without helping. They basically left him in bed, including to eat. He finally asked if they could help him get out of bed in the morning so he just wasn't laying there all day. Only one option for meals, which he mostly did not like. He lost over 15 pounds in 2 weeks, and left very weak, which lasted several days after discharge. He was in the hospital for one week prior, but was eating regular meals. I brought him a toothbrush, but he never once brushed his teeth. Hygiene needs were not tended to. Therapy was supposed to be everyday, 5 days a week, but lucky to get one session every other day. Their answer for his bathroom needs was to put depends on him. When he was discharged he was to have a home health nurse to go over 3 times a week. It turns out the home health agency Sapphire sent the orders was at capacity so they let Sapphire know. It's now been 1 week, with no home nurse. Nobody from Sapphire has notified us and do not return our phone calls. They seem to be there to provide the bare minimum of care.

I wish I could give West Hills 10 stars, because they went above and beyond not only for my beloved…read moregrandmother Barbara D., but our entire family. Following a stroke, my grandmother was transferred from OHSU to West Hills, where she stayed for two months. Last week, when she was facing death, rather than going home, she chose to stay with her incredible team at West Hills. We got to know nurses, CNAs, case workers, therapists and more on a first-name basis. They all were so kind and gave such skilled and compassionate care not just to her body, but also her mind and her spirit -- she discussed her Zen Buddhism with one of her favorite nurses, talked endlessly with her physical therapists and wanted one of her CNAs to move in with her. Throughout a painful process, West Hills was a true partner and comfort to my grandmother and the entire family. We would have care conferences that lasted hours and involved upwards of six people, everyone working hard together to find the best choice. When we were down to the final hours, they brought food and water and reminded us all to eat. So many caregivers throughout the building came to say goodbye to her, came to give us hugs and cry with us. We could not ask for more from them. They came on good recommendation from a family friend, and she was so right. We are so grateful to all they have done for us (and tolerating when we all piled in her room for pizza parties). I've been in rehabilitation for a small stroke here for almost two weeks. The staff here are warm,…read morefriendly and professional. I came here with a lot of medical needs including a feeding tube and pump, 24x7 oxygen concentrator and a lung suction device. Not a small feat to support me all day and night but The Pearl was up to the task. I've made friends with many of the staff and I look forward to returning to do some volunteer work if possible. I totally recommend The Pearl if you are in need of rehabilitation services like I was.

The Pearl provided excellent care for my husband after an extreme spinal surgery. He ended up with…read morean infection at the hospital requiring emergency abdominal surgeries. After all of the surgeries my husband spent a month in the hospital and he was kind of at square one when discharged. When he started at the Pearl he was unable to stand. After his month long stay, he was in good enough shape to slide transfer from a wheelchair to his bed or shower/commode. He was also able to stand for brief periods of time. He progressed enough to join the intensive Rio Rehabilitation Program at Legacy Health. During his stay he received top notch care from Rio staff. They were very caring and met him where he was physically. The therapists pushed him to get where he needed for additional rehabilitation. Thank you Colin and Debra! The nursing staff were caring and as timely as they could be and several nurses were standouts (like Carol and Sherry). The weekends seemed to have less staff so things could take longer. That said, they were upfront about other work they needed to do before being able to help. And when they were helping, they took all the time and care that was needed. The food was very good with several alternatives if the main menu didn't sound good. Bambi was great in helping set up wheelchair transport for appointments, Sondra helped coordinate lots of appointments and helped make Rio happen. Tawnya was amazing at coordinating transition to Rio and backup plans. Penny was the nicest therapy pooch. She lifts everyone's spirits. I highly recommend the Pearl as a skilled nursing facility. Complete professional staff of OHSU doctors, therapists for PT and PT were top notch and experienced. Staff was responsive, food was good. If was able I could have gone to their sit down restaurant. I was even able to get a haircut in their Salon before I left. the only negative is it was impossible to connect on the phone to try to get a room reserved, way understaffed there but that is the only place that was an issue. Finally, my son went to the facility and was able to meet with them and reserve a room. I would go back in a heartbeat if I had the need.

This lady was grumpy and it looked like she already put on her overtime. Maybe she did! But that was strike 1. The ladies in my team (1st meeting with them all) were cordial and polite, so nothing much to write about them, but I did met a new friend, or should I say an old friend . . . . so I have someone to sit & converse with. Here comes the manager of the food & beverage department, serving us. She smiled, when talking with us, but she wasn't smiling when she walked away. I don't expect someone to smile all day, like 100%, I don't. . . but the kind of reaction I was reading was . . . . her staff aren't there because of cut-backs. That kind of smile, so she's putting more hours. Probably another reason why the reception was 'grumpy? The manager, did gave me the drink I asked for, so that's a bonus. Now comes the food. There are many selections there to choose from. The oil & vinegar dressing was very, very thick. What makes oil & vinegar thick? Beats me! Cornstarch? Flour? Glue? LOL If I wanted thick dressing on my salad, I would have chosen Ranch or Thousand Island, maybe Blue Cheese? I was hungry, so I have no choice but eat all that I picked, plus a small slice of chocolate cake. Yum! One lady had very little on her plate, my rooster could eat that, and she's got good control if she was trying to lose weight. Or it was just not appealing to her. But why would anyone pay $9 for a buffet, if you're gonna eat like a rooster. My new friend tried their soup, she said it was VERY hot, but good. OK, now it's raining so hard. So I stayed a bit longer, but eventually I have to walk out back where I parked my car, but I don't like getting wet, not even for a spit. I asked the lady @ the front desk, if there was a back exit, and again, she was grumpy @ me, no smile, and the tone of her voice just shows she's now getting mad @ me. I also met up with the food & beverage manager, again . . . . and she pointed me where the bathroom was, but still no smile, like she had a lot in her mind. Was her job hanging by a thread, and overworked like all the rest of us? What ever the reasons, THEIR EMPLOYEES just doesn't have the hospitality one gets when they walk inside a room. UPDATE: I did an event here, and I can tell you how the people who lives here behaved, including their employees. SOMEONE must have read my 1st paragraph, because they now have a very friendly GREETER working behind the reception desk! Thanks for that! They are like a large family, they are helpful, friendly, and they have many people who are residents volunteer - getting more involved than other retirement facilities I've visited. They have activities to get them more involved, and as volunteers, you are part the 'community'. It's not like, you go out to have a meal, then go back to your room. Their gift shop are run by volunteers. Because of my observations, I will add another star!

There are currently 10 therapists on staff and they represent an impressive diversity of specialties: orthopedic, neurological, pelvic health, pediatrics and sports. They also have sophisticated equipment to utilize diagnostic ultrasound and biofeedback to determine the etiology of your condition. For treatment, they have all traditional exercise equipment as well as shockwave and laser therapies. If you're looking for exceptional care from a professional group of highly trained providers, this is your place for superior musculoskeletal treatment. I had an experience at West Portland Physical therapy with one of the PTs, Rhi, that breached the…read moreboundaries of what is appropriate behavior for a medical provider. I went here for more than a year. I loved working with Rhi for a while, But then something changed. When I would come in and talk about the pain I was in (I have an old spinal cord injury from an extensive spinal fusion with rods and bolts), they started to push the idea that it was my fault that I was in pain because I wasn't being mindful enough of how I was moving- she told me I was sitting down wrong, and that if I couldn't apply my PT exercises to my everyday life, that they were meaningless. This was really discouraging. She decided I needed to create a schedule for my week and bring it back to show her, because this would "fix" my "mindfulness" issue. I have been so busy that it was hard for me to find time to sit down to create a schedule. As a result, I rescheduled my next appointment because I wasn't able to get it done in time, and I was nervous because she seemed to think most of my problems were coming from a failure of personal responsibility. But I still got this homework completed and brought it to my next appointment. Unfortunately for me, however, I had put the wrong time down in my calendar (appt was at 11:40 and I put 11:50) so I got there late. I was very nervous about this, considering I had to create a schedule to show her to prove I was putting in the effort she expected of me, and I had ironically arrived late. What followed was a dressing-down the likes of which I never anticipated to get from somebody who was supposed to be helping me. She told me I was late/rescheduled more than once and it was a problem for her. She told me that they'd made "certain accommodations" for me like doing appointment reminder calls, but that it was a burden for their office staff to do that. They were the ones that offered this in the first place. She then said "I don't want to be just another person you have to worry about disappointing" which was really hurtful because I hadn't thought of her being disappointed in me because she was my medical provider who was supposed to be there to support me, especially when things are hard. She asked if I thought physical therapy was too much for me to handle right now. I told her no, I need to prioritize physical therapy. She told me that was her job (????) and then told me she was going to cancel the next 2 weeks of my appointments (including appts made with a different provider for pelvic floor PT that she was not involved with) so I could "get things together at home" before continuing physical therapy. I was visibly panicking at this point and buried my face in my hands. She then said I could "change her mind," putting me in the position that I would have to beg and justify why I needed to continue physical therapy in order for her to not unilaterally make the decision to cancel appointments, half of which weren't even with her. I asked her not to cancel my appointments because I was afraid I wouldn't be able to gather the courage to come back. She said fine, but that I should consider whether or not she was the right PT for me, and that I *have* to be on time, or she'd cancel my appointments. She said I'd come a long way but I was still late too often. As an example, she said I used to no longer walk with a cane and that was food. She spent such little time on MY needs as her patient that she didn't see the new cane I brought to get her opinion on. She finished the appt by telling me she'd have an assistant walk me through some breathing exercises because by the end of her tirade, I had dissolved into tears. The schedule I made to show her didn't even matter. I've been in physical therapy for more than half my life and I have never experienced anything like this. I cried in my car for an hour. Anna and Megan are great, but Rhi hurt me in ways that will take a long time for me to heal from, and will likely set my progress back by months as I try to gather the courage to try PT somewhere else.

While a few individuals on the care team truly made a difference--Damalesh, Shantay, Louie, and a handful of others were kind, attentive, and absolutely wonderful--the overall experience left me shocked and disappointed. The social services director, Sam, is the main reason this facility gets a 1-star review. He was dismissive, condescending, and ultimately responsible for the stressful, manipulative mess that surrounded my attempt to leave--saying that I was "denying care" when in reality I was taking my "care" elsewhere... Had to fight to get basic discharge paperwork (that I asked a half dozen times to get for hours on end), was pressured to sign documents without being given full information, and felt completely stonewalled when I asked for reasonable medical equipment and referrals for in-home rehab. Sam made what should have been a smooth and supportive process incredibly frustrating. As for the food? I'm vegan, and while they made an attempt to accommodate that, I can't live off bread and tofu forever. Toward the end, I could barely eat anything at all--it got that bad I understand care facilities aren't luxury resorts, but respect, communication, and basic follow-through shouldn't be optional. If it weren't for the handful of standout staff members I mentioned earlier, I would've lost my mind here. If you have a choice--keep looking. You deserve better.

What can I say about this place? The fact that my concerns were not met when I felt completely…read moreviolated, disrespected, and shamed while having to take a shower, while a male staff member sat just inches away from where I had to take a shower. I was told that if I didn't take a shower at that moment, I would not be able to take one the next day. There were enough female staff on duty that one of them could have come to the shower instead. But yet, being humiliated. I still have trauma from that night. Only one staff member approached me about what happened. I was not apologized to by Hussain nor by anyone else. I felt absolutely outraged and degraded. Yet, no one seemed to care. The only reason I was here was to rehab from cancer treatment It was much more banal at best. A complete lack of communication between staff members. Often told "oh, I forgot, or no one told me," which is absurd when dealing with people you are there to care for. On more than one occasion, I was brought the wrong tray of food while the staff jokingly said, "oh, it happens all the time", like that's ok. Another issue.. having to put a sign on my door asking people to knock before just walking into my room. It is a common courtesy to knock on someone's door before walking in... it does not matter where you are. Yet, even with the sign, some just walked in without knocking. As far as rehab.. I did better PT in my room. A couple of old bikes and a few weights. It was a waste of my time and dignity. I stayed there two weeks too long. However, that being said.. some of the nicest people you ever wanna meet. All seem pretty stressed most of the time, definitely overworked, probably underpaid. From my personal observation along with asking the R.N's CNA's, LPN's, the rehab staff and others. Each caring for patients with different needs and specifics. The length of time spent that particular resident varies, leaving others under their care basically screwed until they can get to you personally. That's why the one star, because they do try. But ,things definitely need changing as far as respect, patient care, effective communication, the fundamentals and most valuable not treating your patient as you did your last. Each patient are different in their own right, they should be respected as so. As an individual, which we are all and not just another bed filled.

So caring, kind, attentive and take great care even in unpleasant circumstances. I would give them 10 stars if I could. The facility isn't fancy but if your loved requires care you will not find a better staff. My husband was a patient for 3 weeks.
