On the way to the nursing home, the driver aggressively slammed on the brakes, and caused by mom to go flying forward while still strapped into her wheelchair. (This should not have happened, because the driver neglected to fasten the wheelchair to the floor ramp.). The driver NEVER REPORTED the accident, and the staff ignored it. There is much more to this story, obviously, but not enough room here to write it all. Bottom line, I have tried tirelessly to get a call back from the current director (there have been 4 since my mom was there), because I would like a copy of her medical records. She has recurring ailments now as a result of the accident, and Prestige refuses to acknowledge anything. This was in December, 2023, and we are still trying to resolve the poor treatment she received. BEWARE!

All things considered she enjoyed her stay…read more Sometimes it took a bit for a CNA or other staff to respond but she said that she really liked every one of them. She had a few that went out of their way to make sure she was comfortable. I believe one was named Nye and the other's nickname was Kai. Both very kind young men. Mom liked her occupational therapist and her physical therapist named Kyle. She often shared that Kyle was very encouraging. Skip, Sunny, and the other male nurse were all very kind and helpful. Mom said she thought the food was good. She didn't care too much for sharing a room but luckily her roommate was not too bad. The facility is definitely older and shows wear, but overall not too bad. A deep cleaning, repaint, and new window treatments, and more comfy seats in the rooms for guests would definitely help the overall comfort and ambiance. Overall our family's experience was really good and we are thankful for the good care and help she received from all of the staff at Avamere. :)

Three therapists she worked with were so compassionate- they had concerns that were expressed to me, so I ended up taking her back into emergency (after three visits to her primary where nothing was happening) and finally got her the help she needed. In another instance, he pulled my post op recovery arm back so far it brought me to tears. Instead of easing up or checking in on how I was doing, he just held the position and waited for me to stop crying. I found his behavior extremely unprofessional and lacking in basic compassion. If you're trying to heal and looking for a place that truly supports recovery, I would not recommend coming here.

Then the wind off to put that person somewhere else still ignoring the fact my wife was out in the cold needing a wheelchair. Once they finally did get my wife in her bed it took him another 3 hours and 5 minutes before she officially got admitted to the facility. The oxygen cylinder I brought with me for my wife ran out completely because it took three different people coming in and looking at the giant oxygen cylinder before they got it to start releasing oxygen. There was no back up oxygen nearby and all were oxygen concentrators were already in use my other patients according to the staff. Sometimes the staff came back from lunch or break reeking of cigarette smoke with no concerned that my wife has COPD and then smoke on their breath and clothes bothered her.

Kindness Abounds…read more After a loved one suffered a minor stroke, they went to the Fort Vancouver post-acute care center for their rehabilitation. After visiting many times in the past two weeks I've consistently observed nothing but professionalism and kindness from the staff. I've watched them respond to numerous requests from patients, which were all met with cheerful and mostly immediate responses. Check-in was a breeze with no waiting around or delays. A team of people all took turns doing their jobs to get the patient situated and comfortable. The Physical Therapy and Occupational Therapy staff I've met are also top-notch. The facility itself is very busy and sometimes noisy, but is always welcoming to visitors. They immediately gave me the door code for after-hours visits and a password to the guest wi-fi which works really good. Rest-rooms are in every hall. You are welcome to join the patients in their rooms, the dining room and there is even a family room area for longer visits. Yes, parking on 8th street is tight and their lot is perpetually full, but I'll put up with that for the kindness and quality of care we've received. It's also only five minutes away from the Camas freeway which means just a couple of stop lights and you are there. If there is one suggestion I could make it would be to put "pillow speakers" on the television sets so the patient can hear the program without blasting the rest of the facility, and especially the poor roommate. The TV sounds are the principle cause of noise pollution I observed while there. I don't know where they find all these really sweet people, but I'm really glad they were there for us in this time of need.

The RNs are in touch with the patient's needs and contact the proper medical providers when needed. Unfortunately there is significant delay in initiating proper treatment, such as antibiotics, etc. In addition, treatment managers to not contact family members routinely nor do they give details of care, planning, testing, etc. prior to the group sit down. In some cases this can be a real challenge for the patient, who may need better communication due to hearing, visual limits etc. After release, we inquired about some dates of care via telephone x 3 to the care specialist, with No response. While at Village Rehab, my husband complained about the food, did not eat, and lost weight. So I tasted the "soup"....he was right. After returning home he has regained some weight and pursues regular meals, so it was not his illness.

The problem was that the communication from hire up was awful. To be specific to get information from Social Services or the Case Manager (who knew nothing about her case) was like pulling teeth. When I would call to get updates, it could take up to 2 days to get a call back, and when I got an update it was only to find out that they were not telling my mother-in-law what was going on or we got 2 different stories. When we were ready for discharge, it was rushed, supplies were not ordered as we were told they would be and I had to constantly be in communication with the Social Services department to make sure things got done. I actually had to have my daughter go by the next day to pick up supplies that we were going to run out of before Home Health could come to our home. As a first time caregiver dealing with this situation, it was traumatizing.

My dad is there recovering from a heart attack. No one there communicates from one shift to the…read morenext, or from one day to the next. They would not give him his pain meds as prescribed. When he complained about not being able to sleep, and how much pain he was in, the nursed would ask him the same questions that another nurse had asked him the day before. Then we got a lot of "I'll leave a note for your doctor". It seems that the note never got left because we were told that several times about my Dads pain. He would occasionally go up to 11 hours without it. It was prescribed for every 6. So I asked for a list of his medications and the times that he is given them. I have medical power of attorney, so it seems easy enough. Nope! Not here. First I asked a woman at the front desk area, who refused to give me her name, and she told me I had to find his nurse.... Well, he was nowhere around my Dad's room, so I asked his CNA, Moreno. She told me that I had to ask his nurse. Like Im supposed to wander around the care facitly looking for his nurse. So I hit his call button. 25 minutes later, I timed it, Moren, his CNA came in. "I don't need you, I need his nurse", I said. He finally came in and asked the same questions and said he'd leave the doctor a note. Unbelievable. So I tried to contact 2 people in charge of his care, Debbie and Joyce. No one answered the number I was given by his nurse Jordan, and Debbie hasn't replied to my email. It's been over a week and still no response. I am beyond pissed off about this. I would tell anyone looking for a care facility, full-time or for recovery, to pass on this one. They obviously don't care much about their patients. And, sorry this is so long. If anyone from the facility would have called me back, I would have told them all of this in such less public way. Please! Pick the next one on the list...

I was in this faciity from mid December, 2024 until January 8, 2025. It was horrible, the food was…read moreawful, and the kitchen ignored requests. I stated several times that I am allergic to cucumbers, and yet they were still in every salad (I ate a lot of salad because so much of the food was inedible.) They were short staffed, so I frequently waited up to 20 minutes for a CNA, not good when the meds give you diarrhea. Most of the staff gave it their best, but not all. One night an older woman (I presume a CNA) came in, bent over my bed and shook me angrily. I was stunned, but later figured out that she thought I was the woman crying, "Help me, someone please help me," on and off all night. Still no excuse for what she did, and she should be fired for patient mistreatment. I would NEVER allow a relative of mine to be here; they don't pay enough attention to what patients need, and at least one of the actually abused me.
