Nice enough guy but struck me as someone with very little confidence in his abilities. He desperately seemed like he wanted to get me out of there and steer me to another doctor.. He gave me an estimate of close to nine thousand dollars to take some skin off my right eye which I can barely see out of. At that price you'd think I was getting implants or something. I'm not trying to knock the guy but he took $250 off me l for like a 5 minute talk where he was trying to play shrink more than surgeon. My advice is don't waste your time or money like I did. read more

I would unfortunately not recommend Dr. Phelps. I had a consult with him which he was 10 minutes late to. The call lasted about 20 minutes (consults are $250). He spent the majority of the call telling me that it gave him anxiety to operate on someone who doesn't live within driving distance of their office. That's a reasonable policy to have in place but I wish that information had been communicated to me before I waited 6 months for this appointment and paid $250. 19 minutes into talking with me, he said he had a patient who was "actually here in the office" and he needed to go. I found this incredibly rude. During our brief call, I got mixed signals from him about whether or not I would be allowed to have surgery since I don't live nearby. By the end of the consult, I asked the doctor point blank if he would recommend surgery and he said yes it would benefit me. He even sent an invoice for surgery! Then, the office informs me that he does not in fact recommend surgery. So Dr. Phelps lied to me during my consult. After a conversation with Jasmine the manager, it was clear that no one was at all sympathetic to my confusion and concerns. I was able to get my consult fee refunded which I was grateful for. However, I received no apologies and, overall, my experience at this facility seems fairly unacceptable for a doctor's office. read more

In my opinion, my experience with this physician's office gets a 0. I went in for a consultation, arrived about 25-30 minutes early for my appointment. There are two offices, one in Lincoln Park and one in Winnetka which is inside an optical which as somewhat odd. Not impressed with the optical but nevertheless figured I'd go ahead with the consult. I looked at the consent that was sent to me prior to the visit - I had many concerns so I opted to not sign until I was able to discuss with the office on the day of my appointment. It's interesting that a different patient down below commented on many to the same concerns I had about the consent - Page 1, Paragraph 5 - Some of the doctors/ providers who may treat you at COPS are not employed by COPS. So COPS is not responsible for anything that happens to you when being treated by them. And you may find out after they treat you, that they do not take your insurance which was accepted by COPS. It is the patient's responsibility to question all medical practitioners about their employment status. Really? This is a private practice surgical office that charges thousands of dollars for cosmetic procedures -the expectation is that the surgeon will perform the surgical procedure and it's the basic understanding. It's not my responsibility to question random medical providers - it's the basic understanding that Dr. Phelps will be doing the procedures, and with whom the consult is established. This is not inpatient medicine or the ICU with rotating physicians. Page 2, Paragraph 1 - COPS will take photos of you & may use and disclose them [aka - end up on the internet for their personal marketing purposes] for any reason afterwards without additional permission. This part was shocking and something I specifically asked the front desk staff woman that I was NOT in agreement with!!! I am a physician myself - I. have no interest in having any of my private images taken for medical purposes to be posted anywhere and I have a right to say *no.* Having had prior surgeries this has NEVER been an issue - I have ALWAYS been able to opt out of this section and state that I would under no circumstance would I want any of my images on a website or any type of third party marking site - the woman assistant tells me that "there is a separate consent for images posted." Really? You are giving me a medical consent which is a medicolegal document that states OTHERWISE - clearly if I agree to it, it's stating that I am ok with the terms. I am not! Why would I allow a physician's office to post my images and to not need additional permission afterwards to use my images without consent!? Telling that there is a "different form to sign" clearly is contradictory to this consent. Additionally the assistant tells me that "my appointment will have to be rescheduled" since I did not register (part of the reason I came in close to 30 minutes early) - I tell her clearly that shouldn't be an issue as there was about 20 minutes prior to my appointment. She also tells me that "I did not contact the office with my concerns about the consent" and tells me that I should have "emailed them" circling a supposed email on the paper consent - news flash - there is no email on the electronic format!!!! I have NEVER ever ever had a surgeon's office tell me that I cannot opt out of images being on their website or not needing my consent and refusing to have me opt out by marking it on the consent form and saying "do not consent." It is shocking that they couldn't just allow that. Another area on the consent says that "COPS may use your tissue or body fluid [that they take during surgery] for research purposes afterwards without letting you know." I express that I did not consent to this at all - I have never seen this as part of any consent form. Why on earth would this be part of the consent form? Any and all unused body/fluid tissues should be discharged - this is supposed to be a private practice surgical office. One of the staff members went to show the parts of my consent that were marked that I did not agree with - the assistant woman comes back and tells me that because I did not preregister (which as above, I still had about 20 minutes to do so and I even asked her why that couldn't be done then), and that I had not "emailed my concerns before" (even though there is no email on the electronic consent form - so that my appointment "would have to be re-scheduled." Yeah right. This is absolutely inappropriate. The consent form is rather concerning, and the fact that they won't allow patients to opt out, etc is shocking and worrisome. It would have been easy for me to register in the 20 or so minutes I had and for them to allow me to opt out of having to agree to let them post pictures without asking for permission, using bodily fluids without consent, and having random providers including trainees! If I could give Dr. Phelps and his team 10 stars I would! From the moment you book your appointment, the consult, surgery, and face treatments I was blown away at the level of care I have received. I had lower bleph, Morpheus, and a peel done all on the same day. I have wanted lower bleph for years and my only regret is not doing it sooner. I look way more fresh . Took years off my face. I no longer look tired all the time. The face treatments did a huge difference for texture and sun damage. I will be back for more of those. I also bought amazing skin care from there. EVERYONE in this office is absolutely amazing, kind, and refreshing! Dr. Phelps has a life long patient in me. Will def be using his amazing skills for more! I am sure a very competent doctor. However - I believe takes cases that are assured a perfect outcome. I paid a 250.00 consultation fee and he saw me for at most 5 minutes. Told me I had a malar bag and there was nothing he could do for me. Too risky. Did not explain what he meant by "too risky" so I had to write later to ask what he meant. Said I wouldn't be happy with the results. One year later I found an EXCELLENT surgeon who stated it was not a malar bag and my under eye looks phenomenal. I have since found out that not many surgeons are well versed in the type of actual surgery i had. read more

My visit to Dr. Phelp's office at Chicago Oculofacial Plastic Surgery ("COPS") was very concerning regarding their registration consent form & their attitude towards me about it. These are some of the items in their 4-page registration consent form: * Page 1, Paragraph 5 - Some of the doctors/ providers who may treat you at COPS are not employed by COPS. So COPS is not responsible for anything that happens to you when being treated by them. And you may find out after they treat you, that they do not take your insurance which was accepted by COPS. It is the patient's responsibility to question all medical practitioners about their employment status. o My comment: Ridiculous. Your patients are not your workers' professional human resources department. * Page 2, Paragraph 1 - COPS will take photos of you & may use and disclose them [aka - end up on the internet for their personal marketing purposes] for any reason afterwards without additional permission. o My comment: I have had recent cosmetic surgery done several times prior. Other normal offices' registration consent forms gave 2 choices: for either the patient to allow photos to be used for marketing & internet purposes, or for photos to only to be used internally to gauge progress. COPS is the first plastic surgery office I have ever been to, where they MANDATED patient photos to be released for internet & marketing purposes. So I told the receptionist that they can take photos to gauge my progress, but they may not release them to other patients/ internet/marketing. And at first she said I can decline it. * Page 3, Paragraph 2 - COPS will load all of your plastic surgery medical records, onto shared open network electronic medical records (EMR), & likewise they will access all of your EMR from any other medical office you have ever been to. And it says "If I prefer not to participate, I must notify COPS in writing at XXX address or xxx email." o My comment: I told the receptionist that I will be taking their opt out option to notify in writing. * Page 3, Paragraph 4 - COPS will sell their patient's personal contact information for marketing purposes, including "I understand & expressly consent to be contacted by auto-dialed or pre-recorded text messages or telephone calls or voicemails at the number I provided to COPS." * Page 3, Paragraph 6 - COPS may use your tissue or body fluid [that they take during surgery] for research purposes afterwards without letting you know. o My comment: No you may not secretly enroll me in an unknown clinical research study behind my back later, which could genotype me & make these results available to other entities, w/o letting me know of my participation or the results. The nerve! There should be a study coordinator person to review this clinical research study w/patients, & patients should sign a full Clinical Research Informed Consent Form which is IRB Approved. It is against federal regulations to involve a patient in a clinical research study without letting them sign a Clinical Research Informed Consent Form to be informed of the details of the study & to give their permission (Code of Federal Regulations 21 CFR 50.20). According to the federal regulations in 21 CFR 50.25a8, clinical study participation must be voluntary & there should be no punishment for declining. So for Dr. Phelps to tell his assistant to tell me that patients are REQUIRED to participate in his secret clinical research studies or else he will not treat them, is in violation of 21 CFR 50.25a8. My comments: After I lined through some of these inappropriate exculpatory phrases, they said they needed to show this to Dr. Phelps first. Then they came back & said that he does not want to see me because of this. This is despite that the receptionist told me I could decline the public photos if I wanted, & the form says patients can write to opt out of the EMR internet sharing (which was phony & was just for show) When I asked questions & lined through these items, I never raised my voice, said an unkind word, got excited, or went towards anyone. Even though I did not agree with some of the wording, I was very polite, quiet, soft spoken, & well-mannered about it. So after Dr. Phelps sent word that he didn't want to see me, & the office staff started getting huffy & nervous that I had asked these questions, the other male employee in the lobby went towards me to tower over me, invade my space, & stare at me at close range, to make me feel physically threatened and intimidated, like he was a wanna-be bar bouncer. He & the lady were both standing close & towering over me together, to act like it was two against one so I would feel outnumbered. That was so inappropriate & low class. I welcome any manager or doctor to check your lobby security footage to see this. However, I am content with the outcome, because this helped me dodge a bullet before I proceeded to have any work done in this questionable situation. read more

I had an orbital socket blowout fracture and was referred to Dr. Phelps. Dr. Phelps was kind, professional, and took great care of me. He answered my questions and performed my surgery. After surgery he was always available when I needed him. I did have to go to another doctor to get a repair revision due to nerve complications with the surgery Dr. Phelps performed 11 months later. I may need one more additional surgery due to fix the look of my eye. I'm just praying I can get back to some type of normal without constant pain. read more

I am so happy I did my eyelid lift with Dr. Phelps! I couldn't be more thrilled with the results. I was a little nervous about the procedure but Dr. Phelps made me feel more confident by explaining what to expect. The hospital experience was very streamlined and bing,bam boom I was done. Thanks to some great twilight drugs I didn't feel a thing. The best thing was that my insurance paid for it since my vision was being affected. I did have swelling, but Dr. Phelps assured me that the bruising would be minimized if I was religious about icing in the first 48 hours post the surgery. True to his word, I did not have much bruising. After a week I was still swollen but felt it wasn't that terrible. I could immediately see that my eyes looked more open. Today after 3 months my eyes look so much better. Wish I had done it earlier! Really want to get the bags under my eyes taken care of, but that insurance will not pay for so it will have to wait. I needed Upper Eyelid surgery and had few consultations with oculofacial plastic surgeons. Needless to say I was loosing hope. It took over two years of research and I finally found Dr. Phelps. I couldn't say enough good things about him. He is extremely professional and very knowledgeable. I had ptosis in my left eyelid. For a while I had avoided gatherings and refused to take pictures. Driving was difficult as my vision was shadowing. Each visit to Phelps' office has been professional and pleasant, but what I most appreciated was the treatment from the staff as well as the doctors at the time of surgery. Everyone was friendly and patiently answered all of my question and concerns. My partner was treated kindly and his questions also were treated with respect. Both Dr. Phelps and the anesthesiologist approached this anxious patient with humor and calm reassurance, taking more time than I have ever gotten before going into surgery.
