Tooth #4 Extraction -- Updated from an enthusiastic 5-star review to 1 star: I am extremely concerned following my 12/11/25 appointment for a #4 extraction. Two days prior to that visit, I clearly observed teeth #13 and #16 in the mirror. While inspecting a different issue with a crown on 3/21/26, I realized teeth #13 and #16 were not there. I was only billed for #4 back on 12/11/25, and my clinical records from that day make no mention of the other two teeth. I strongly recommend any patient take "before" photos of their mouth in the parking lot to document exactly what is there before sitting in the chair. read more

We will do an update after service, but so far, we were left with the impression that Dr. Mantor is there for his patients and doesn't seem to be money hungry like other folks in the same field. Over Priced, money hungry, and a lack of knowledgeable staff. My wife had an infection which required a tooth to be removed and a small piece of bone implanted for a future crown. The extraction was done and the infection was cleaned out by a root canal dentist not at Periodontics limited. Over the next year year she had more problems with that area and were told she should see a periodontics for a deep cleaning. We went to Periodontics limited and saw Dr Swenson. He suggested she get a deep cleaning before he could do anything. It didn't help her gum or tooth problem so he suggested another deep cleaning with the use of a camera to get deeper into that area. Again, more money before dealing with the actually problem. Since that didn't help, he said the only way to know for sure what was going on was for him to do surgery and open up her gums. He would also prepare that area with a bone graft for a future implant since he said the old bone graft from a year before wasn't growing. We did that costly procedure figuring it would help her. She chose to be unconscious for the procedure so had no idea what was done. When we went back for a follow up a couple weeks later, he said it was healing well. She asked him if he had cleaned out the infection during the procedure and he said yep. Prior to that procedure he had been very open and went into detail when we talked with him about what was done and needed. This visit he gave 1 word answers and no details. That is a sign a person is hiding something and doesn't want to slip and say too much. A couple months later we called them and decided to cancel the appointment for what we thought would be placing the tooth implant. They didn't do crowns so we were going to have her regular dentist do the implant and crown. They surprised us by saying the implant had been done when he did the surgery. Normally when a bone graft is done and the person had an infection, they wait 3-6 months to do the implant to ensure the bone graft is growing and there is no infection. A couple months later later she developed another infection in that area and had puss coming out of her gum. We went to her regular dentist, who was going to do the implant and crown, to see why there was an infection. Her Dentist couldn't understand why the implant had been done without waiting to ensure there wasn't going to be any problems following the surgery. She said she would have to open up the gums to see what was going on and if the infection had affected the 2 implants she had in that area. We complained to Periodontist limited about how this had been done and Cathy called us back. She asked us to come back in so they could take a look and see what was going on. We did and saw Dr. Mantor. He took x-rays and looked at her mouth. He said everything looked ok and that the area of the gums she was having a problem with looked like Lichen Planus rather than an infection of her teeth. That disease does cause most of the symptoms had been having in her mouth which made all the work Dr. Swanson had done a waste of time and money. Why Dr. Swanson hadn't known this shows his lack of knowledge. What's even worse, is that after this visit they charged us for the visit. I would suggest everyone stay away from Dr. Swanson who now has his own separate practice, and Periodontics Limited who apparently is only out to see how much money they can squeeze from their patients. read more

If you are reading this, you found the right place! Let me set the stage for you... My last dental visit in the year 19 ought 92 or so, 30 years as it was not a pleasant experience. I was extremely hesitant to even set foot in a dental office. Well, not anymore! My initial consultation was with Dr. Doug Sandquist, which I highly recommend as well. Once I had the consult I was referred to Dr. Dane Swenson, just as amazing. From making the appointment, completing any initial paperwork (standard operating procedure), to submitting pre-authorization to my dental insurance, it was all a breeze. The entire team was informative, patient and professional. I was extremely nervous about the procedure (which was really only a cleaning, with LASERS). I initially requested that I be knocked out because there was no way I was going to let anyone near my mouth with out putting up a fight. Dr. Dane and his team were very accommodating, even to the point where I realized I was making more out of it than necessary. I rolled in there the day of my appointment like a Big Boy and went with some simply numbing. I even asked for a few extra shots, which I didn't feel a single one. Dr. Dane and the Hygienist walked me through each step and it was over before I even knew it. I literally didn't feel anything, healing was almost instant and Dr. Dane is the MAN! Dr. Dane Swenson at PERIODONTICS LTD - I absolutely love, love, love Dr. Swenson! I am so thankful to him regarding my tooth extraction (30 year old root canal). I had been to Periodontics Ltd for a second opinion, as I am a returning patient from two years ago sent by my retired biological dentist, Dr. James Heltzel. My experience two years ago and today were fantastic. Dr. Swenson is knowlegeable, empathetic, professional and skilled. I was literally on my deathbed with fear over this procedure, but he made me comfortable in every way. He and his staff explained everything from start to finish in a dignified fashion. I elected to have IV Sedation due to sensitive teeth and past bad dental experiences - I am happy to report I did not feel anything! Post op instructions were clearly written and highlighted for me to easily follow. The office is clean and there is a positive vibration in the office and those working there. Staff, from the front desk to the dental assistants, are positive, caring and helpful. As you can guess I was not excited when I found out I was going to lose a tooth, I had never needed an extraction before and was very nervous about the whole thing. Dr Mantor did an excellent job of explaining the procedure and suggested I have it done under sedation so I could be more relaxed. When I arrived for my appointment his assistant Arica did a wonderful job of making sure I was comfortable and that I knew how to take care of my mouth after the surgery. We were referred to this office by my daughter's orthodontist as she needed a complex procedure performed. My daughter was scheduled early this morning. Insurance coverage was limited of course for this particular procedure so I was grateful when I found out they would give me a discount for paying cash. The office is not super easy to find as they are within a large complex on the corner of W. Charleston and Valley View Blvd, but my best advice is to enter from W. Charleston Blvd, and look up. The doctors names are listed on the side of the building. Once inside, the office is clean and the front desk crew is ok. Not overly friendly or welcoming but not rude. I worked with Diana who assisted me while completing my financial obligations. She was also helpful in taking my phone calls and answering my extensive questions. The office does a nice job preparing a portfolio of information regarding how to prepare for the procedure, providing prescriptions if any are needed and giving you post op directions too. Very efficient! My big appreciation goes to Elizabeth and Dr. Gifford. Elizabeth, one of the assistants was extremely kind and so helpful with my daughter. I don't think anyone enjoys dental procedures and Elizabeth seems to have chosen the perfect profession as she is very empathetic and genuine. I felt very comfortable leaving my daughter in her care. Dr. Gifford is a gifted periodontist. He explained everything he was going to do regarding the procedure and reassured me and my daughter. Before I knew it, he came out to let me know how successful the procedure was and that my daughter would be ready to go home shortly. Several years ago I went to Dr. Gifford for gum grafts over multiple teeth during two separate procedures. Dr. Gifford is extremely professional and articulate making me understand everything that he explains. The gum grafts are in good shape except for over one particular tooth. I noticed this one failed a few months after it was done. The informed consent did state that it was possible that a gum graft procedure might fail and could leave the recession the same as before. I am not sure as to reason for this particular failure. I have just decided to live with it and make sure to keep the area clean. I didn't want to waste money and risk an unsuccessfsul graft again, especially when I have no idea what caused it not to work on the first try. It was strange because all the other grafts were effective. read more

In the past several weeks I was experiencing acute pain in my gum. After consulting with my dentist and an endodontist, I was referred to Periodontics Limited. I met with Dr. Mantor who met with me and took a 3-D X-ray scan of my jaw. He wondered why I have so many teeth missing. The reason is that for many years I had a terror of going to the dentist unless I was already in a lot of pain until only an extraction was possible. I've overcome that in the past dozen years because I finally have a great dentist now. However, due to years of neglect, I had to see a periodontist. Not only did Dr. Mantor decide on the proper treatment of my gum issue, but he also discovered a molar on the other side of my jaw was cracked in half. Not only that but the gum and bone above is deteriorating. I am so grateful he discovered this and in 2 weeks I will have surgery with Dr. Mantor to solve these issues. After consulting with him I am confident I am in the best of hands and grateful he explained everything in a clear and concise way. His frank manner and empathetic attitude are deeply appreciated. The office staff (Yvonne and others) could not be more professional and caring. They explained the pre and post-operative instructions and financing. Just had a crown lengthening procedure performed today by Dr. Goaslind, and I couldn't be happier with the results. My teeth look beautiful even though my gums will take several weeks to heal so I can't imagine how great they will look when fully healed and when my braces are removed! I had IV "twilight" sedation and I don't remember the procedure, no pain, no swelling, no nothing negative! So if you're squeamish or the slightest bit nervous about having surgery, never fear and opt for IV sedation. I was recommended this place by both my general dentist and oral surgeon and it's easy to see why they think Periodontics Limited is "the best [periodontists] place in town." The issue with this doctor is not his technical expertise, but his very poor skills with his patient. My wife was very apprehensive about her procedure, and instead of reassuring and calming her, she was pressured to decide on treatment very quickly. My wife is not a native English speaker, so instead of taking the time to slowly and simply explain the procedures, they were impatient. The assistants abruptly positioned her, not taking the time to ask her to move where she was needed. She said she felt manhandled. They spoke to her in a derisive manner. When she needed an x-ray, the protective vest was thrown on her and not gently placed. At the end of a treatment, they did not remove her bib, but required that she remove it herself. The procedures she did have were successful, but all she remembers is being poorly treated, not the eventual outcome. This was no way treat a patient. We will never use this practice again. read more

I was referred to this office from my dentist who I trust and I can't speak on behalf of the other doctors here, but the one I dealt with was rude, arrogant and incredibly disrespectful. Not only was his tone sales pitchy but he also seemed annoyed at my questions and concerns. Also made judgemental remarks about the current condition of my gums (something I'm obviously seeking treatment for). I even noticed the hygienist giving me a sad look to make sure I was ok when he left the room. I was referred to Dr. Trylovich for a grafting procedure. I came for an exam and was told after my exam that my insurance won't cover a dime of my exam or procedure. I gave them my insurance cards when i filled out my new patient information and they never cared to mention to me that I should find a periodontist that will take my insurance (even partially), not knowing that my insurance won't cover any of this $2200 procedure that I need. I paid $165 for the exam and have to do it all over again. This staff needs to let patients know if they do or don't take their insurance. read more

I never felt the shots to numb my mouth and his staff made sure I had proper instructions post surgery. I even left with the ice pack and Advil. I would like to commend Doctor Brian Mantor and his staff at Periodontics Limited. The Veterans Administration Dental Clinic referred me to Dr. Mantor for surgical removal of an erupted tooth and ridge preservation. It was extremely easy and quick to schedule the appointment and when I arrived, I was greeted with a warm, friendly smile from the receptionist. Within a few minutes I was sitting in the room and Dr. Mantor was prepping for the procedure. He was professional in every way and gave me a reassuring smile and yes, I was somewhat stressed...who wouldn't be? After sedation, I was out and didn't wake up until the procedure was over. I didn't even know when it started! Absolutely incredible. The VA was willing to do implants, but the wait was way, way too long. I discussed implants with Dr. Mantor and he assured me I was a candidate. Rather than wait for the VA, I decided to have Dr. Mantor do the implants and a wise decision it was. I spent a lot of time researching periodontists in the Las Vegas area. I chose Periodontics Limited, despite the 2-3 hour roundtrip travel time for each appointment, and have been extremely satisfied. I needed to have my front tooth extracted after some previously failed root canals. Dr. Mantor quickly impressed me during his consultation. He is intelligent, patient, empathetic, able to explain all options and procedures clearly. You have his full attention, and it's obvious he cares very much about your well-being. I was able to schedule my procedure the same week, and also decided to have my four wisdom teeth out at the same time. Five extractions, cleaning of the infected site, bone and soft tissue grafts, placing of the implant screw, all in an appointment. I am now on day four after the procedure, and I am off all pain medication and back at work. Dr. Mantor is excellent at what he does, but it's the patient care that really makes this practice special. He personally called the night before the procedure and the following afternoon, and has continued to check up since. I also had an issue with my temporary prosthetic not quite fitting, and he made himself available the next working day to solve the issue. I was so pleased I didn't even mind battling the traffic going home. the entire office staff was professional, polite, caring, and I felt like I was right at home. Highly recommended. read more

I can't speak highly enough of Dr. Mantor or the staff at the Valley View/W Charleston location. I needed a dental implant on one of my incisors - right in front - and my general dentist, Dr. Denton, recommended Dr. Mantor. Given the visibility of the gum line at the location of the implant, I was quite anxious about the procedure and the risk of gum deterioration or of a visible post. Turns out there was absolutely no reason to stress. Doctor Mantor and his aides did an amazing job, not just cosmetically, but also with the comfort factor. For comparison's sake - I've had a root canal that took longer and was more painful than this procedure was. I should note that the brief hammering, for lack of a better word, was a bit disconcerting, but it was over in a flash, and not even painful. Best of all, you wouldn't even know there is now an implant behind the gum. If you're sweating it like I was, don't even stress. This doc and his staff are accommodating of the urgency, put you at ease, and do phenomenal work. With regard to the crown work, if you're looking for a recommendation - Dr. Denton and his staff also did excellent work and made a perfect match to my teeth. read more

These people are money hungry. After the extraction they will not perform the second procedure until I paid off the bill. I was told that both my insurance would cover the bills but they lied and both insurance did not even cover one half of the bill. I keep getting calls from these people and I will be so happy after I payoff my bills. Beware. read more

Do NOT go here. They rip you off. They are rude. They are NOT upfront about pricing either. I had a consultation here a month or so ago. They told me before coming to my appointment it would be $180 so i made sure to have that on me. While I was there they stated I needed a scan, but did not state that this is extra or how much more the cost would be. Upon checking out, my total came to $350 ish dollars. I was not expecting that as we had only gone over the $180 price but I paid in cash with 4 $100 dollar bills. The older heavy set woman with a wrist brace informed me they didn't have change but they would put it towards my account. I ended up not going with this place due to clear lack of communication and some of their other reviews. I had called about my refund for the overpayment but no one answered so I left a message. No call back. Then after waiting another week, I called again. Spoke with Kathy whom is quite snarky. She scoffed and said "it's only $10" and states that I paid $360 and gave $20's that day which is a bold face lie since I had nothing but $100 bills in my wallet AND why they couldn't give me change. They refuse to give me my hard earned CHANGE back. These people are sketchy as hell. Do not trust them. Doctor was nice but if his office is ran this way I can only imagine he allows it. When people are having such tough times coming to you all for help and you rip them off and are rude as hell to them. Do yourselves a favor and find a better, more compassionate place that won't STEAL from you. read more

Dr.Trylovich did my gum procedure and all I can say is wow! I am amazed at the work he did! I was never confident with my smile as my gums came down rather low and were uneven, so one side came down even lower on the one side. A procedure like this can be nerve wracking because at the end of the day it IS you face and your smile, the first thing people see and remember about you. I couldn't have woke up happier with my smile! The day of was so calm and smooth, and the days following, and check ups as well were all pleasant.
