I broke my hip and needed both operation and extensive rehabilitation...I had to re- learn how to walk. I chose Petaluma Post-Acute Rehabilitation because it was close to my home in Rohnert Park. I have been in Rehab before for other medical reasons, but have never experienced the quality of care that I received at PPAR. From the admin staff to the nursing staff to the physical and occupational therapy teams to all their associate staff, they treated each patient with a personal interest in their well-being. Whst's more, they were always available 24/7 to meet your every need. I have never experienced the quality of care as I received at Petaluma Post-Acute Rehabilitation. Obviously, I highly recommend both the establishment and the totality of its staff should you ever require the many types of post-acute services they provide. read more

My experience at Post Avute Care was great. The staff was was very caring and professional. Management was very involved with my care and made sure I was recieving all the Rehab I needed to return home as soon as I could. It takes very special people to do this work. They have to work with people at their worse and it is very stressful, so I was impressed with their attitude day in and day out with their work ethic and compassion. (Addition to original review which disappeared.) I don't know how I did that, I meant to say it was a Kaiser nurse assigned to the Petaluma rehab facility, not "KP" for Kaiser patients. I am sure that nurse will say she was just doing her job. Maybe she could include some empathy towards patients and family members and not straight-arm family off their loved ones. She was never cordial to me the two weeks Tom was there, and I suspect some level of hostile disapproval. I wanted the best facility for him and thought it was nice, but of course he would have preferred being at home. Sadly Tom did not make it and we were both left with an unfortunate memory of the place. I miss him so much. read more

My mom is currently staying in this facility following a complex femur surgery. The staff is INCREDIBLE. Everyone is so kind (on top of being competent!). Nick was incredibly attentive to her needs. I have visited other facilities when other relatives have needed post-surgery care, and this one is by far and away the best. read more

My mother spent a couple weeks at this facility recovering from a broken femur injury. The facility is excellent (clean and always smells nice), everyone is very friendly and smiles a lot and, most importantly, my mom got good care and the staff was attentive to her needs. If you need to be in a "nursing home" for rehab purposes (broken knee) this is a very decent place. The CNA's, Nurses and staff were wonderful. The only negative was the food....it really should be better....canned fruit cocktail just doesn't make the grade! The PT and OT professionals were very good. I loved my very hard working CNA's who took such excellent care of me! Humor was my savior and the CNA's didn't disappoint! Special shout out to Nindia, Margarito and Rubi who handled me with humor and kindness from taking showers to making my bed. Also, a thank you to PT's Jessica and Nino. Your valuable instruction to improve walking and going up and down stairs helped me immensely. Thanks to all who made my stay better than expected! read more

PPAR provided exceptional care to my 87 year old mother in law who spent over a month at the facility. I cannot say enough about the exceptional care I received here at Petaluma Post Acute Rehab!! I would definitely give them 10 stars. The care here is exceptional. Every person I dealt with, from the people who clean the rooms to the CNA's to the Nurses and to the Administration treated me with the utmost respect and were dedicated to my well being and to my recovery from a serious emergency operation to my L2-S1 lumbar spine that came close to causing permanent damage. Their attitudes were always cheerful and they all went out of their way to make my stay comfortable and pain free and always with a smile on their face. They were always there- day or night to help me. My training by the Occupational Therapists really prepared me to be able to transfer to my home and be safe. I live alone, and they helped figure out how to do my laundry, how to cook, and how to shower on my own. Spinal cord surgery has its challenges when healing and they dealt with them exceptionally well. I am not allowed to Bend, Lift (more than 5 lbs( or Twist (BLT) They taught me how to function at home alone without doing any of those. It has been a safe and easy transition thanks to them. The incredible training and support of the Physical Therapy team and especially Caleb was top notch and extremely professional. When I arrived at Petaluma Post Acute Rehab, I could barely walk. I was unsteady and weak. With their extraordinary care and attention I was able to leave walking over 2 miles a day with certainty and safely. I really had to learn to walk again and through their knowledge and support I was able to make progress quickly. The food was also good and healthy. I was able to get snacks between meals and when I started to be more active and walking more I was able to get some extra when I needed it. I was amazed that the food was that good in a hospital like setting. The Nurses were of the very highest quality. They took exceptional care of me. They went out of their way to explain all the medications I was. on and made sure that my pain level was always managed. They were always there for me 24 hours a day with their warm and caring presence. They supported my when I wanted to get off the heavy Narcotics I was on and made that a painless transition. After such a major operation on my spinal cord, I felt very vulnerable and the care I was given totally made me feel so safe and so cared for. The Administrator Haley McLaughlin is one of the most professional and sweetest hard working, caring people I have ever met in the HealthCare field. She is a total professional and her energy and smile and caring lift you up every time you see her! She made sure I had everything I needed and encouraged me all the time! The Head Of Admissions- Nick was equally caring and attentive and even made sure I got home when my ride bailed on me. He is also helping me get a friend transferred there. I am now safely at home, living on my own and healing beautifully. I will never forget my stay there and all the incredible people who took the very best care of me. If you need to go to a facility like this I can tell you that there is absolutely none better! I thank them all with all of my heart!! My grandmother was sent here after she was in the hospital after her stroke. I was happy with this facility and the care she was given. The staff are friendly and caring. They were attentive to her needs. The physical therapist gave her great therapy, where she made great improvement. The only minus is the facility itself. It is on the old side, though the PT room is fine. read more

My 93 year old Grandma was a resident at Petaluma Post Acute Rehab from November 2012 until her death in February 2013. Not only did the staff take great care of my beloved Grandma, but they went above expectations in supporting me. The nurses and CNAs really made a difference in my Grandma's last months. One nurse in particular was her absoute favorite. Just by being on duty in Grandma's wing calmed her. When he wasn't on duty one day, Grandma seemed sad. When I asked what was wrong, she said "Oh, I guess it's because Doug isn't here." The other nurses were also helpful, compassionate and caring. Everyone took the time to listen to the patient and family. The CNAs made sure Grandma was comfortable and clean, and did so gently. The food was healthy and flavorful, not that watered down no-seasoning stuff you get at other places. The nutritionalists came by often to make sure that the food was good, and that she received the food she wanted and that was best for her. Social services made sure that even though Grandma was unable to get out of bed that she had activities that she liked. They arranged for a volunteer to come by to visit and read with her, who ended up being the daughter of a friend - that's what you get in a small town! The admittance people were so accommodating. Every little request was met with a smile and done promptly. Housekeeping was thorough and also made sure that everything was always clean. This is all possible because of the leadership provided by the Administrator, Christian. He's an amazing guy, not just there for the paycheck, but because he really wants to make the residents stay a positive one. By the way, I'm not one who writes only positive reviews. We recently had my mother-in-law at this facility. I would give 5 stars, 10 if I could to Maggie, a nurse, who is an angel from heaven. She connected with my mom-in-law in a way I have never seen anyone in my life connect with a patient. The other nurses were fine, but Maggie was a star, and my mom-in-law loved her. I am giving 3 stars because there are some areas that the facility can improve upon. Internal communication is one. We found that staff at some levels did not know what decisions, requests were being made at other levels, so we had to fill that role. Communication is a toughy, but solvable by instituting processes that ensure common knowledge where it counts. Staffing. There seemed to be adequate levels of staff in some areas, like CNAs and aides, but it seemed like nursing staff were at time short handed. It was good that we had mainly two regular nurses, usually, tending our mom, but sometimes that was not true, which was stressful for our mom. Food. Room for improvement. Billed as "5 Star Food"...oops, not true. Not horrible, but could be so much better. Work on that. Good food may not matter to some patients with no appetite, but a life saver for those who want to work at rehab and get better and look forward to normal life. Rehab: 5 stars for the staff in rehab! They went above and beyond in trying to get our mom up and moving and were very encouraging, professional, communicative when I asked for status, and very kind. My mother, at the age of 79, had major back surgery. I rescued her from Golden Living Center, which was a complete nightmare, and placed her in Petaluma Post Acute Rehab. The care, treatment and rehabilitation my mother received there was outstanding. They did everything we needed and more. I made a point of getting to know the staff by name and they checked in on my mom frequently. They were very attentive and patient with her. The facility is attractive and nicely decorated. The lobby and hallways with nice lighting and wall sconces give off a nice hotel feel. It does not look like a hospital, which was important to me in selecting where my mom was going to be. She lived there for 3 weeks and was cared for tenderly and respectfully. If I ever have to place my mom in another rehab center, it will be Petaluma Post-Acute Rehab. I'm sorry the other reviews are so poor. That was not my mother's or my experience. I highly recommend this Rehab facility. read more

If I could post a zero, I would . Never bring a family member you care about here. Several years ago, my mom was admitted for Parkinson's rehab. Within a few days, we received a phone call saying she fell and broke her hip. After a brutal hip surgery, we found out she had bone cancer and died shortly after. The problem was we could have had a good few last weeks with her instead of having her in great pain from a major surgery . My mom was supposed to have a fall alarm attached . She was unable to remove the alarm. The facility told us she fell about 30 ft from her bed. The problem was the facility never turned the alarm back on at one point and never owned it or they were too late responding to the fall alarm (which I witnessed many times with pts). My dad had a stroke last year and why I returned here I don't know. His whole life had been turned upside down. He lost the ability to communicate so you can imagine how frustrated he was. He was angry, depressed and difficult. We hired sitters for times we couldn't be there in order to help the staff. Our family was there helping each day. With time and our family interaction, my dad was able to walk out the door but still had a long road ahead of him. His recovery was due to our family support and certainly not the facility. We constantly had to check on them, follow up on errors and incompetence. My dad was difficult but it is a rehab center. Isn't that what they are supposed to do? There were patients worse off and definitely more disruptive. My dad had a second stroke and needed a little rehab and he was refused here. Out of curiosity I asked why and was told it was because of past issues and he was too disruptive. Brett and Shirley the nursing director told me this. The next day I spoke with Shirley and told her what I thought and she said the reason was due to the fact they were unable to have someone that needed one on one care. I asked the hospital if they told the center that but the case manager from the hospital said no and he hasn't had one on one care when he was here. The admin staff completely lied or changed their story and that isn't a good sign. It is ok. Things happen for a reason, my dad was placed in a much, much better center and is being cared for. He is clean and shaven everyday , constantly checked on. The staff seems happy and more interactive and attentive . There is definitely less chaos. At petaluma care and rehab, unless my dad was in therapy , he was left in his room. This new place brings him to all the activities during the day and engages with him. I don't care for incompetent staff, unavailable and unhelpful admin. I also work in this industry so I see what is good and bad care, good and bad customer service . This place spells bad across the board. read more

This place is a living nightmare for your loved ones with any medical needs. Might be OK for general assisted care but they failed horribly to meet my father's wound recovery needs and he needed to be removed with 60/40 BP due to dehydration via 911. Ask questions about what your needs are and make sure they can handle them. No doctor on sight. read more

My mom was in this facility for a few months after suffering multiple strokes. The care was good for the most part but it was a lonely place. Four people to a room and the curtains were always closed so everyone was alone, with a tv, the majority of the day. We had so many issues the day I came to move my mom closer to me that I would love to give one star but that was just a day so I won't be a dick. I'd like to say to PPAR that I feel your staff need to communicate better so A knows what B is doing. This would have made our transition so much smoother. Also, to the social worker who will remain unamed, you get paid to do your job so I'd suggest you do it or take your miserable self into a new profession. You caused more misery than I can ever explain during a very stressful time in my family's life and there was no excuse. The silver lining is that my mom was moved closer to us and I can be part of her care from here on out. read more

My dear mother broke her hip in 2010 and was sent to Petaluma Post-Acute Rehab. She was in very good health, but caught a "super" bacteria bug the first week she was there. It's called C-Diff, a life-threatening disease. C-Diff involves a ridiculous amount of diarrhea that leaves the victim dehydrated and very ill. My mother has always been in excellent health and simply entered the rehab for physical therapy because of her broken hip. As the first week went on, my mother became very sick, but we were unaware of just how sick she was and how threatening this bacteria is. After several days of severe diarrhea and asking every nurse and CNA why my mother would have the diarrhea and be so ill when she simply came in for rehab for a broken hip, they said it was probably from pain meds she was taking for the broken hip. Looking back, I think that was very inaccurate and misleading. Pain meds are usually constipating, not the opposite. C-Diff was never mentioned as a possibility even though now I have come to know that it is a prevalent problem in health care facilities. It wasn't until I INSISTED that my mother be given a blood test to find out what was going on and INSISTING that she be put on an IV, that something was done and she began to turn the corner from her dowwnard spiral. It was at that point that the blood test showed the C-Diff for which she was put on Flagyl and Vancomycin, a couple of the world's strongest and last-line-of-defense antibiotics. Up until that time, I felt my mother was going downhill fast. It was scary. I don't know if they would have EVER tested my mom if we hadn't insisted. Neglecting the cause of her severe illness went WAY TOO LONG! I think she should have been tested right away at the first sign of the diarrhea. Now I know that it's an easy diagnosis by health care workers EVEN WITHOUT THE BLOOD TESTt: multiple bouts of diarrhea per day and VERY foul smelling bowel movements, listlessness, stomach and abdominal pain, loss of appetite, difficulty eating because of the stomach pain, fever. It's horrible...and VERY contagious! I don't think there are enough precautions in place at the rehab to stop this contagious super bug. That's another long story and I can't possibly write about that here. This was our first experience in a rehab. We never saw a doctor once even though my mother was so ill. I'm not sure whether there is even a doctor on-site. My mother recovered well from the hip break, but she is still suffering the after effects of the C-Diff months later and is struggling to regain her health. I believe the staff suspected C-Diff and did not do a thing about it until I INSISTED they do something. Now, in looking back, I believe they were hiding this diagnosis and why, I don't know. It was at the expense of my mother's recovery, that's for sure. I will NEVER put a family member in a rehab facility again. I will make sure they are treated with physical therapy, etc., in their own home or bring them into my own home. If my mother hadn't had me and my other family members as a STRONG ADVOCATE for her, I don't believe she would be with us today. And I still worry that she will never fully recover from the lasting effects of this nasty bacteria. read more

My grandmother has alztimers and is in this facility.... i absolutely love the staff and the care she is getting here! She has made huge strides in her health and her physical therapy! The food looks great and they have activities going all the time. They have a large cort yard that the residents can enjoy and some beautiful siting areas. My only complaint is that in my grandmothers room it has writing on the wall from another resident i did ask to have it removed due to the fact of what is written is vary unsettling... im hopeing this will be removed on my next visit... read more

My grandma has had to be admitted into this facility many times and the staff is amazing. From housekeeping to the Director of Medicine, every single person seems to be there for all the right reasons. To care for the people that are struggling with to improve their condition. My grandma has had some tough situations brought her way and I can honestly say that the staff has always made the right decisions for her care. Also, with they have helped me be able to coordinate her arrival home, to be able to make sure I can set her up to eliminate struggles and be on a road of success. I feel that the tradition home will be easier because of how Social Services walked me through every step. Setting my grandma and myself up only to succeed. DO NOT GO TO THIS "REHAB". I had a serious concussion, and did not receive adequate care other than "warehousing", - just putting a lot of beds in one room and feeding people. They did not offer what they billed for. They claim that they do "rehabilitation" but other than two short visits & two teeny walks, I did NOT receive any rehabilitation. ( Is rehabilitation walking a total of ten steps?) I immediately complained to the California licensing board when I got home. They investigated, but unfortunately they did not find proof of their lack of care. I believe they lied about it or falsified the records - but of course, I can't prove this. According to well-recognized standards for post-concussive care, the "physical therapist" should have offered at least two hours a day! They did tell me tell me I needed to work on my speech, but didn't offer me any exercises to help. (This from a "speech therapist") Did I get anything other than that? No. Oh, someone offered two teeny walks, one outside, the other inside. I hold a doctorate and researched what should have been offered. Just as I suspected, I should have gone home instead, I could have hired someone to feed me. The other rooms I peeked in when I was able to walk down the hall were filled to the max with older people. I rarely saw an attendant in my small room that had three beds. Yes, they had tvs over the beds but hey - nobody showed me how to work it until the night I left. On top of that, I overheard an orderly laughing at a diagnosis that was in my chart!!!!!!! By law, he wasn't even allowed to read it. Do not let yourself be swayed by their attractive setting, a converted house with some fancy name plates to impress you. Your loved one is worth investigating a better place or hiding a camera to expose their lack or care if you feel you have no choice but to warehouse them read more

My mom was sent there to help rehab her broken femur. The first thing I noticed when I went there was they were not happy to see visitors. Like anybody else I had questions and they either could not or did not want to answer them. On December 24 at 3:00 PM they kicked my mom out because of her insurance ran out. I told them it was just paperwork that got mixed up and I could clear it up right away, and they said come get her. Now this was Christmas Eve mind you. My mom left the rehab without the complete program she needed to fix her leg. Since then I have taken it upon myself to help her at home with her rehab and she is much better. A very nice facility (after our nightmare experience at Golden Living in Sonoma a previous time, I insisted we receive our follow-up rehab here). I'm so sorry to hear others having poor experiences. Possibly management has changed as of this review, I don't know. My friend receives plenty of attention and is never rushed. The therapists, nurses, and CNAs were upbeat, patient and compassionate and appear to love their work. They looked in on him frequently. Physical Therapists Cheryl and one whose name I cannot recall were patient and mindful of what can and cannot be accomplished (one was not). The facilities are clean and well-maintained. The food is not bad for "institutional" and my friend was never rushed to have his tray removed. Every instance of care is recorded. Every patient has their own TV, walker, and wheelchair. (Not the case at Sonoma Golden Living.) Update: Bring clothing and take home to wash. Several items of clothing were lost, even though marked with name, nowhere to be found. Some CNAs although efficient, are not so patient, but that's the workplace for you. Some have language issues. CNA Patty Lopez was exceptional, unfortunately, you can't request favorites. I hope she considers becoming an RN. My issue, is that nobody monitors whether patients are drinking enough water. A fresh pitcher is set in the room every few hours, but there's not guarantee seniors will drink. We all know how serious dehydration issues that will cause. read more

My 98 year old father fell and broke his hip in January 2018. After surgery he came her for rehabilitation. He came home after three weeks and contracted pneumonia and had to come back. Both times he was there he had excellent care. The staff was friendly, patient and caring. The facility always smelled and looked clean. I want to thank everyone there for all your help in getting my father home. I told my wife, "If something happens to me and I need rehab I want to come here". I can't think of a higher compliment to pay. read more

I hate having to give this place three stars, but as the saying goes, one bad apple can spoil the whole bunch. I called last week to tour the facility to find placement for my father. The person who answered the phone, "Vinny," was just awful. He was not friendly or personable at all, and couldn't answer my very basic questions about services. I heard him as he was hanging up the phone mutter under his breath, "I don't have time for this sh%#." I was so appalled. I decided to go for the tour anyway, and Vinny didn't even come out of the office to give me the tour saying again he "didn't have time." He didn't look very busy, but whatever. Instead, Cory stepped forward to give me the tour. Cory was super nice, funny, charming, and very knowledgeable about the care they provide. The facility itself is beautiful and seems very well managed. It has a very peaceful atmosphere and everything looks brand new. If Cory was the only person I interacted with, I definitely would have chosen this facility and it would be a 5 star rating, but Vinny left a bad taste in my mouth. read more

Was very pleased with this place. My mother was referred here by Kaiser after having a rod placed in her broken hip. She stayed here two weeks. They rehabbed her to where she could walk well and use the toilet and shower. She was in a room with one other lady and they had their own bathroom. The patient aides were EXTREMELY attentive and helped my mother with whatever she needed from dressing, to transferring out of the bed, to washing, etc. The food was adequate, not haute cuisine. They also have a lady who comes in on Monday's and does ladies hair which was great for my mom. There ARE people who are here for the long term and on their last legs. Don't be shy. If you need something, ASK For it.. They will accommodate your needs. I would not hesitate using this place again. read more

When someone you love is hurting or sick, sometimes it's hard to leave them alone, but I have to say that at Petaluma Post Acute Rehab, I NEVER felt like my mom was left alone. She was always clean, her needs were met and when she finally left, she left to hugs and cheering and feeling of support and caring that was priceless. I am grateful beyond words for this facility. The minute I walked in, I knew this place was different. It is warm and inviting and as close to home as one could feel in a place like this. And the staff is attentive, focused and very strong, from the CNA to the RN to physical therapy, there was nothing but positive experiences for my mom. Petaluma Post-Acute Rehab is a wonderful place. The facility is top-notch. Always clean and bright, but still has a hotel/home feeling instead of the sterile hospital feel. The staff is extremely helpful and friendly. Everyone always greets us with a smile and is willing to help with whatever we need. My husband's grandmother was admitted over six months ago and has another 6-8 months in her stay due to two knee/ankle surgeries. She has been treated with kindness, patience, and care the entire time. We all have! We are a very large family, who visit often, and who I'm sure can be a little demanding and overwhelming at times, and the entire staff from the CNA's and RN's to the admissions and business department have always been great. I highly recommend this facility and if anyone else in my family should need in-patient rehab again we will definitely be going to Petaluma Post-Acute Rehab. read more
