I am extremely pleased with this facility. My grandpa was in Carmel mountain before this and it was absolutely horrible. Here there is great communication and all of the staff really seem to care. They have great PT (Dr. Katie) and OT (Dorsa). The facility is also very cool. Some of the amenities are not that new, but they have such a nice patio and it is nice to hang out there. I would give the a 10/10! read more

Dad spent 3 months there. Overall, it was a pretty good experience especially with his therapy, but not so much on the nursing care side... the staff during the day versus evening and weekends were not the same experience. (Days were good) Dad needed more rehab than nursing care and OT/PT was awesome. Melissa and Theresa stood out, they were very engaged with my dad challenging and encouraging him nonstop and they stayed consistent with him. He always talked positively about them. Marlene I believe her name was as a RNA that was good. Pawan was very attentive as well even though I don't think he was really assigned to his room. Appreciated him. Amy in the dining room was so attentive my dad didn't care for her in the beginning but he quickly learned she was just persistent and was looking out for him. Jamil I think his name I was also good. Brandi comes to mind too. Areas they could improve in was not just response time to red button sometimes taking 30-45 minutes but also many times they would come and ask what was needed but didn't return with what my dad would ask. Some (guys) were also rough with my dad and he would call them out. I think they thought he was out of it? Many times my dad would need to use the restroom and they would tell him to just go in his diaper. This happened A LOT. I had to speak up multiple times saying he needed to practice transfers and get on the toilet. It was obvious they just didn't want to do it. Also he was to get up for each meal and I'd catch him in bed many times and they would say "oh he didn't want to" and my dad would say "I was never asked". I should and would expect patients CNAs to just do it every time making the most of patients time there. Seeing or talking to a doctor was a lot harder than it should have been. Maybe they had staffing shortages but this place has a lot of potential if they were just more consistent with care. read more

Dingy, dirty, dark, depressing. PT staff I encountered seemed to be students with no clue on hip replacement offering care contrary to protocol. Encountered insects on the bathroom floor along with dirt and debris in bathroom corners, walls and doors dirty and damaged, base of rolling bedside stand caked with dried liquid and food spills. Most support staff were helpful! Their "5 Star" claim is totally misleading, and IMO a "2.5"would be a stretch. Don't go in blind on this one! I xferred to another facility after 3 days. read more

My mother went there for rehabilitation for two weeks because she fell at home. My mom was admitted on Monday afternoon, and we didn't get an orientation welcome or a proper introduction to our CNA, MA nurse for daytime/evening shifts, who is our nutritionist. On the second day, I was unsure about whom to ask for assistance. The nursing care aspect is terrible. I did not receive the correct introduction regarding who our CNA, Would Care Nurse, and Medicine Nurse are. Press the nurse call button and no one arrives regardless of the time of day. My unfortunate mom has to walk to the restroom by herself because she couldn't wait any longer. Food - I am astonished by the kind of food they are offering. It's not a nutritious meal. We completed the substitute meal form, yet we don't receive our requested items and frequently ask for additional gravy and a fruit cup for both lunch and dinner. I sense we are an inconvenience to the chef by making our request. Doctor visits occur only once a week, but I believe it should be twice a week. The doctors never examined my mom. Activities Department: Marietta, show great kindness and a loving spirit to all. Even patients/residents experiencing a genuinely difficult period. Their sincerity is palpable. Physical Therapy Department: PT teams are wonderful, supportive, compassionate, and patient with my mom. read more

What kind of rehab facility would release you with a prescription for insulin without the needles/sharps to inject them? Well, Poway Healthcare would and then not be available to provide them because it's a Saturday! This begins a story of 3 weeks of Medicare covered care that includes, a rehab facility that doesn't do rehab on weekends, medication provided which is not consistently provided, Miralax provided even when you specifically ask them to not provide it, failure to answer call requests especially for an emergency. Or when you discover that the MiraLAX works! Patients allowed to wander halls screaming (literally) for help and left alone. And finally, being left without functioning oxygen for 14 hours when 24/7 is required. After all of this it literally took 5 days to actually exit the facility after requesting to do so. This is not the end of this review. I'm pausing to provide more information if they actually call back! It's now Monday, and they didn't call back, so let me finish this by saying that the hard working staff I encountered are working well beyond capacity as charge nurse assistant Rocio worked 17 straight hours and those that do PT, PA, Ivan and others, even the young lady who's every other word was "perfect" did their best work for patients! They will deny that they are understaffed but can't explain my Rocio example. While it should be obvious, I do not recommend Poway Healthcare for any type of rehabilitation. When management's response by several is "do you wish to file a complaint" is their primary tactic, it's time to seek better treatment! read more

Although I experienced some issues at PHC I didn't want to mix them with my review of the Activities Dept. Marietta, Tessa, and Tiffany really go above and beyond. They, especially Marietta, bring such kindness and a loving nature to everyone. Even patients/ residents having a truly rough time. You can feel their sincerity. My dad went there for rehab and had such a horrible experience there. The facility is old and not very clean. The rooms were too small to be shared between 2 residents. The pictures on yelps do not truly reflect what these room really look like. It was hard to move around. Imagine how hard it would be for older residents with limited mobility. On top of that, the food was horrible. The worst part was they didn't give my dad a lot of PT time. PT time was very minimal, maybe 5 minutes a day, day on and day off. How can recovering patients have a speedy recovery if they don't have enough PT OT time? When my dad first arrived, they were very very very quick to admit him. However, when he was stable and back on his feet again, I called, emailed, texted MULTIPLE people from admin for discharge plan, they did not respond at all. Michael whom I met when my dad first arrived there promised that their team will meet up for discharge plan but never followed up with me. This went on for almost a week without any updates or resolutions. The staff did not know who would help patients with discharge plan. I went around in circles without any results. My dad felt like he was trapped there. My dad ended up catching pneumonia there and was rushed from this SNF back to the hospital. The moment my dad was back in the hospital, admin people from this facility called and texted me immediately telling me that they will keep the room for him to come back. Ughhhh no thanks. Never again. I truly feel that this facility's goal is to keep the residents there and provide this low grade care as long as they can to make profit. It's quick for the patients to go in but it will be hard for them to get out. My dad luckily pulled him self through the pneumonia and went straight home. I do not Recommend this place to anyone even my worst enemy. read more

My wife just spent 9 days at PHC after being discharged from the hospital. Pros: - the caregivers at PHC were amazing, every single one - doctors, nurses, CNAs, therapists. Shoutouts to Jennifer, Mirari, Kayla, Trix, Manny, Kurt and many others whose names I've forgotten - the facility, although it's a bit old and tired, is well equipped, well kept up and exceptionally clean - everything happens like clockwork, and everyone on the team knows and embraces their role - did I mention how great the caregivers are? Cons: - the case management staff is capable but seems to be understaffed, which means that communication can be spotty. I believe management is aware of this issue, hopefully it gets resolved. In the meantime, don't be afraid to raise your hand if you have questions or concerns. All in all, a positive experience at a stressful time. read more

Transferred over from another facility that we had a horrible experience with. Overall the staff and stay was great. However don't expect your belongings to go home because no matter how many times you update inventory and mark clothes, chargers etc they will not be going home with you. It's been over a month since discharge and still waiting on an update from social worker in regards to replacing items or being reimbursed. We had a great experience and communication up until discharge. Hopefully we have answers going into month 2 of being discharged. read more

What is up with this place in answering the phone or getting to a room?! Called 4 times today (1-3pm) pressing the buttons to get to the Operator and never getting the call answered. Just called (7pm) and got somebody but when I asked them to connect me to a person "they asked what room?" and of course I didn't know because this was the first time I actually spoke to person there. She told me "please hold" and phone started ringing again and again with no answer and then I was disconnected. Wow!!!! So literally call #5 did not get me to the person I've been trying to reach all day and 5th disconnected call today. 0-5. Administrations needs to get somebody to work the phone lines! Sad I can't literally get through the "front door" of the facility. read more

My 72 yr old sister was transferred to Poway Healthcare Center after discharge from the hospital. From day 1 she has received the greatest care. From the doctors, nurses, CNA's to the cleaning and maintenance staff, she has received the best care. They pride themselves with cleanliness and it shows. It is clean and smells fresh. We, as a family, have been very pleased with the facility and staff and so has my sister. read more

From our family's experience, we had both of our elderly parents at this facility at the same time. The whole staff from CNA's (ex: Pawan), to Admin (Michael L. & Derek), Med Nurses, and Director of Nursing who also ran our Care Conference (Izzy), PT & OT were all amazing! Transferred my dad from a horrible facility 5 min away to a breath of fresh air and relief here! Everyone we ever met here cares and enjoys their job creating a happy & welcoming environment. My mom was unresponsive at this facility for "12" hours, so they say????? They didn't even have the audacity to call me and let me know she was unresponsive nor at her final walong moments!!! If I wouldnt have called up there asking to speak to my mom, I would've got a call saying she was dead!!!!! I hate this facility and if I could do it all over again? I'd take my mom home and keep her there! Who knows just how long she was unresponsive for?!?!!! They said 12 hours but I beg to differ! This facility and there nursing staff are by far the worst I've ever encountered! Please take my advise and take the time to find a better one! If not then I HIGHLY SUGGEST TO PUT IN A NANNY CAM!!! I didn't want to post this picture of my mom but I feel I could be helping someone by showing just how horrible this place is. If you look at my mom's face, you can see that her head had been at a side slant to the right for so long, that her nose started to stay as if it was molded that way... The other photo of her is just a week or so before she past. This place killed her. The staff is not there because they care. They're only there to do the minimum and get paid. The cuts above my mom's right eye are unexplained also. I turned her head and she had scars and bruising on the whole right side of her face and ear. Please take my word for it, don't put your loved one in this facility for nothing. Two stars are only for the 10 star pt and ot staff. 20 days in this facility was 19 too long. Less than zero stars for communication about what is going to occur, what can be done to help your loved one recoup, what needs to happen before your loved one can be released. It took 10 days and 8 requests and the threat of putting my father in a wheel chair and rolling him out of the facility for a care conference, and then they had me talking to someone who wasn't his social worker. Then, when I did get to speak with his social worker, I feel like I was treated like one of the patients, rather than a care taker readying in home care for a patient. I was told steps but not WHY... they will need to do thing. Drs appt with their general practitioner within 7 days before we can release him. Then she asked about what his pharmacy would be. When I asked about when she would call in the meds so I could set up pill trays. Oh that's what the Drs appointment is for. WHY would she bother asking about the pharmacy????? If these folks would create a sheet of paper that says "welcome, your social worker is X, the best way to reach them is Y, and they are here to help you with Z. You are scheduled for pt/ot 5 days a week while you're here. Breakfast lunch and dinner are served (with a brief run down). Laundry is handled in this fashion, or your loved ones can arrange to take your dirty clothes and bring you clean ones. (And yet his clothes still disappear to never be returned, put names in EVERYTHING) If you require assistance, please use your call button and a nurse will be in to assist you. There should be a remote for your television and activities are listed on the calendar hung on the wall at the foot of your bed. When it's time to plan for your discharge you/they will need to arrange for a variety of in home changes and equipment. We will review these items with you in the days leading up to discharge. You/they will also need to arrange for a visit to your general practitioner to review (if any) your meds and have them reviewed, approved and sent to your pharmacy. You will be sent home with whatever meds we have available for you, with instructions on how and when to take them. " Remember this is what i feel it should look like. My experience was quite Different. No care conference, until threatening to remove patient. Laundry missing, canceled pt on the final day, and got snippy with me when I try to arrange 5 days in advance when we wanted him to come home. "Our dr will have to have 72 hours notice to release him" last time I looked 5 days is over 72 hours. Over Memorial Day weekend I discovered whole groups of pills left unattended on his tray untaken twice. One was from the previous night. The nurse said oh that's last nights, I'm getting ready to bring pills go ahead and give them to him now. Then hurried in a few seconds behind me and says not the pink ones he doesn't need those his bp is high enough. I couldn't get him out of this place fast enough. But they do allow you to bring in their pets for visits so that's nice. read more

Know your rights. As a patient or guardian, you are entitled to a Care Plan Conference. Caseworkers here act as if they don't know this and wouldn't share a health update and evaluation until repeatedly pressed. It's the law. In addition, the staff is overwhelmed and slow to respond to needs. The facility is old and dark - dark hallways, dark colors and therefore depressing. Staff slow to answer the phone if they answer at all. It's a sad and scary place. read more

My father has been in here about 6 weeks. He's almost 99 and making a remarkable recovery. The administrator, physician, and business office are friendly and helpful. The nursing and rehab staff are truly exceptional. Their great communication with me and care and concern for my Dad are extraordinary. They are very concerned about his health and comfort. They contact me with any health or mood change, and explain issues to us in detail. They are right on any medical or mood change. The rehab staff are exceptionally encouraging. They go out of the way to provide him with wonderful care. read more

Recently I spent four months at the Poway Healthcare Center the reason for my being there was a broken vertebrae in my back and the staff there was remarkable starting from the nursing staff to the CNA's even the house cleaning staff everyone there just had my best interest at heart . I would like to give a special thank you to the nursing staff which consisted of Dan and Sean and Hugo and many others whose names I can't remember right now. But most of all my recovery at home now is due to the Physical therapists who took care of me every day for four months no matter whether I was too tired or not, we still did my physical therapy and my occupational therapy. A special thanks goes to Maria G ,Maria H, Caro, Teresa and Sandra. They were all just incredible women who took such good care of me that I am home now and recovering. read more

I'm so upset to even write this, my mom had hip surgery and was transferred there for rehab. Let's say when she was transferred she was full of life!! When I came by to visit her on Friday she didn't look too good, she was complaining about her arm hurting as she had a line fracture. Apparently she had a rough nite that nite so they gave her some meds for the pain, Saturday morning when I came by she looked horrible but they just said that she was sleepy because of the meds pain... feeling like you trust the people that work there I left and my family stayed but by 7 pm they all left to let her rest. Next thing you know I received a call at around 5 am letting me know that my mom had passed away!! Horrible news that nobody wants to hear!! My family decided not to have a biopsy as she was 87 but the more I think about it the more upset I get! She was perfect after surgery, telling me that this is normal for over the age is hard to believe that she just passed away just like that. Why wasn't she connected to a monitor if there was a chance that she was gonna stop breathing and that the last time they had checked on her was at 1 am and when they came in by 4:4:30 she was dead.... Walking the hallways that morning was horrible now that I think about it, there were some elderly that were just by their doors left like they were nothing... to even think that my mom might of suffered breaks my heart!! I hope that before leaving your loved ones you search and check on reviews!!! I wouldn't of never left my mom there after checking these reviews!! We let my mom process with what comes next and my mom taught me never to hold a grudge but I hope this place gets checked by whoever gives them their license, it's smells horrible there, the little old people look sad and nothing will bring my mom back however this review hopefully makes someone check that place out to see if everything is in place! read more

Horrible treatment by the staff here. My father has lost an extreme amount of weight since entering the facility. Sometimes they don't bring dinner to the patients after 8PM. They have also "lost" a huge amount of personal belongings of my fathers since entering the facility that no one claims they know anything about. I've seen nurses mocking patients that arnt able to care for themselves while walking down the halls of the facility. Extremely disgusting behavior. Today I was yelled at by the lead nurse in charge while having a visit on the back patio with my father. He proceeded to demand answers from me about matters I know nothing about. And all in front of other patients and their visiting families. What he failed to know is that i myself work in healthcare and know the legalities of his actions. Discussing legal and confidential matters in front of other patients is a violation of HIPAA. Extremely unprofessional. read more

I had so much anxiety and worry about having to go to this place (only because I don't like the hospital vibes). This place was so clean and everyone was nice. And not fake either. I've been to lots of places where the staff is just rude and or annoyed. My grandma told me that lol she had a pleasant visit.
