    If I could give zero stars I would. My father was placed here to recover from a broken leg and…read morepneumonia. During his stay, his leg was never elevated and he was always laying flat on his back. They took him in a wheel chair naked to weigh him in the activity room where other residents were and he had no privacy. He said people would scream at night "get me out of here!". He was doing okay the first few days but after that he was a heavily sedated on pain meds. They were giving him pain meds every 4 hours plus a sleeping pill at night. He was on so much pain medication that he forgot how to use his phone. He wasn't making any sense and stopped eating. He even lost his dentures. The nurses will Leave you sitting in a bed full of urine all day before they change the sheets. They don't clean you up well and tear the skin when wiping you. My father ended up being discharged back to the hospital because he developed double pneumonia plus and intestinal disorder called ileus (which was most likely caused by the opioids). My father passed away a few days later at Palomar Hospital and I believe this rehab contributed to his death. Please do not send you loved ones here. They are short staffed, the place is a dump and your loved one deserves better.

My first pick, for sure. A family member has been here for nearly one month due to an illness requiring the level of care provided at a skilled nursing facility (SNF). I've visited at least 5 to 6 days weekly. Every visit I am more and more confident/pleased with the level of care and cleanliness here. You enter the bright/light building and are warmly welcomed/greeted; currently there's fun, festive decor through out. The holiday decor is a great addition, nice touch to bring some holiday cheer. There's a beautiful library/room for residents/guests, not quite complete, but nearly ready/renovated. The entire property is slowly being updated and renovated and it shows. It is evident that there are a lot of changes taking place here (literally and figuratively). The property has been maintained pretty well, considering. All of the staff from the LVNs, nurses, therapists (OT, ST, PT) and so on have been extremely helpful and provide the type/quality of care to match. My family member is always treated with respect and dignity, no complaints expressed. There are daily activities (often enriching), a gym, and the food menu is quite extensive. The meals have been described as both "nutritious and tasty." The dinner menu offers a month of varied meals; without repeating a meal! They even asked if there are items my family member doesn't particularly like. Some additional things I've observed, the staff is able to communicate clearly and collaborate effectively. The staff has been attentive and responsive; exceptional round-the-clock care, truly. There's a large, private parking lot, so parking isn't an issue here. Additionally, there are policies and procedures to protect residents' possessions. They take a thorough inventory of personal belongings and provide plenty of storage space for the belongings. Things haven't disappeared here. A BIG shout out to Tine and Wendy, so kind and caring. They truly are exceptionally hard working, so empathetic, and patient. Escondido Post Acute Rehab offers a high-quality skilled nursing program. One of the best SNFs around! The administrator, Daniel D. is empathic and eager/open to feedback. Thank you for being open, ensuring quality care, managing staff, complying with regulations, all things! I am happy my family member is here during all of these great changes, wish we had discovered this rehab five facilities sooner!

    Our family is devastated by the care--or lack of care--my 75-year-old mother received while she was…read moreat this rehabilitation facility. My mom was elderly, immunocompromised, and recovering from surgery. During her stay, she developed an infection and was clearly becoming increasingly ill. Despite her worsening condition, we felt that her decline was not taken seriously enough and that there was a lack of urgency in getting her the medical attention she needed. When her surgeon identified an infection at her post-op appointment, my mom was sent back to rehab rather than immediately being transferred to the ER. She then had to wait more than a week for a CT scan to determine what was causing the infection. During that time, we watched her health deteriorate drastically. We repeatedly witnessed how sick and weak she was becoming, and as her family, we felt helpless. She needed someone to recognize how serious her condition was and advocate for her before she became even more critically ill. My mom ultimately passed away. There are no words to describe how heartbreaking it is to know that someone you love was so sick and vulnerable, yet you felt like the urgency of her condition wasn't recognized or acted upon quickly enough. We trusted this facility to care for my mom when she was at one of the most vulnerable points in her life, and we feel that she deserved a much higher level of attention, monitoring, communication, and advocacy. She was placed in a wheel chair to receive a CT scan the morning I came to pick her up. Couldn't open her eyes, barely communicate, and the nurse said nothing to me. Just went about things like nothing was wrong with my mom. Our family will live with this loss forever. We only wish that our concerns had been taken more seriously and that more had been done when there was still time.

At this time Jenny told me Records Manager was in meeting all day and couldn't return my call. This is ridiculous and so unprofessional. The family is desperately requesting records for their loved one. This is requarding a precious like that was lost and facility is displaying NO concern or consideration for the family. Before I came to this facility, the nurse liaison, Sarah, sold me on this place. What a phenomenal salesman. She knew I was eager to get Good physical therapy. I've been diagnosed as a partial quadriplegic. I've held the deep belief that I could improve if I could get the right physical therapy. She convinced me that would be the case here; I would receive the best physical therapy, with the best facilities, with the best trainers. I'm on my seventh week without physical therapy. Why? The same Sarah is also somehow the one responsible for getting my letter of agreement to the insurance company. For seven weeks she simply hasn't done this. Why would she? I'm not a person after all, I am an insurance payment. If you happen to run into Sarah, she is a great spinner of stories; none of the stories are true. I was getting RNA until they head-trainer Frank, canceled the program. In a phone conversation with him I tried to explain that I thought there was a miscommunication in regards to the cancellation. He repeatedly talked over me. Not letting me get a word in He repeatedly said, "I don't care what you have to say, I canceled the program." Frank's favorite things to do are moping around and watching TV (not training patients). Recently after coming back from PT I had a new roommate. The roommate I recognized from a previous facility. He assaulted numerous employees: kicking, punching, even throwing his own feces. He is not sane. Imagine the shocking horror I experienced when walking into my own room and finding him feet from my bed separated by a small curtain. It took less than an hour before he threatened me telling me he would **** me up. k*ll me. These aren't empty threats. He has further assaulted at least two employees here. A nurse heard this and he was moved to the room next to mine. We now share a bathroom and a door that links our rooms together. So he can now walk into my room and carry out his threats whenever he's so decides. Imagine the fear and anxiety that are my constant companions in this godforsaken facility. This man can walk, i am partial quadriplegic. I am not safe here. It's hot on my side of the facility because they keep the windows open to outside. Why? Because the stench of urine and defecation is so strong and overpowering coming from this patient Room that the windows have to be open to mitigate the smell. He urinates and defecates in his bed. He refuses to be washed or cleaned -and with the threat of assault who would want to clean him anyways. The administration regularly has 5 CNA's on the floor for 74 beds. That is 14.8 patients per CNA. That is budgeting patient-care out of the building. "They got it", says Jay the administrator when made aware of this predicament. No Jay, they don't. Jay is the third administrator in the year-plus I've been here. Patient- care (or a lack there of) is at an all-time low. The collective morale contained within this facility is at an all-time low. Experienced employees are being fired or leaving and being replaced by 22-year-olds with zero experience (why pay an experienced CNA when you can hire a one with no experience for much cheaper). One of these was put on the floor without finishing orientation in a section he had never had. The incompetence and carelessness of this administration is concerning, it's frightening. My quality of life in this facility is one of constant anxiety, sleepless nights, and complete the dehumanization. I can go on and on with the truth about this place. About how, for instance, I was pushed out of bed by a CNA who said I fell out of bed because of my "bad attitude" (does that mean she did it on purpose?). A new administrator who didn't introduce himself (Jay)came in my room and told me he wanted me to "feel safe" (not to be safe). The fact is: I'm not safe. Like a street preacher proselytizing, here I am trying to keep one person out of hell. Will I be further punished and denied physical therapy because I write this. I very well may be. So be it. If you read this, believe me: to lie in a bed here is to forgo your humanity. Don't come here.

I wouldn't recommend this facility for a patient that requires significant help or has mild cognitive impairment. Post stroke she was wheel chair bound and required assistance for everything. In summary: Dr. B was excellent. Thoughtful, problem solving mindset and treated patient and family with respect, and some humor when appropriate. Rehab Team: Competent and caring. Speech. OT AND PT good therapists. Management: Poor. All patients receive 2 consecutive days with no therapy. That is 2 days isolated in your hospital room with no activities. This is truly detrimental to the healing process. In my experience this exasperated anxiety and feelings of isolation, reversing progress made during the week, disturbing sleep cycles and recovery. It's very important if you are considering this location for a loved one that you reflect on this. Your loved one will be sitting in a hospital bed for two consecutive days with no interaction except a nurse popping in to check on them briefly. Of course family can visit and we did, however it is not a substitute for professional engagement. This workaround that administrators have finagled to ensure patients hit the certain hours required while limiting weekend staffing is absolutely horrible and very transparent in their motives. Nursing Staff: Very inconsistent and overall not good. Didn't appear in alignment with goals of therapy. I didn't feel safe most of the time with the quality of care my mom was receiving. Not all the time, there were a few standouts that were excellent, but overall very lacking. I share this info. so if you're considering this facility you know what to expect. And with hopes that administrators will make changes for future patients. When searching for a rehab facility I admit I was influenced by the the gold star ratings from outside organizations, but after my loved one's experience, I realize this is more a reflection of savvy administrators working to get accolades from governing bodies vs. true gold star care.
