The facility itself is immaculate--spotlessly clean, well-maintained, peaceful, and welcoming. It truly feels like a place focused on healing and dignity. What impressed me the most was the incredible staff. From the nurses, CNAs, therapists, therapy assistants, physicians, case managers, reception staff, dietary team, housekeeping, and every other employee we encountered, each person went above and beyond. It wasn't just professionalism--it was genuine care, empathy, patience, and love for the people they serve. I intentionally won't mention any individual names because that would be unfair. This is his FOURTH stay here at this location. He has vascular dementia, is blind, and has Alzheimer's as well as a few other medical issues. Each time he ends up in the hospital and then we request for him to come here to this facility until he is strong enough once again to come back home so my husband and myself can take care of him. My FIL has been gradually declining in health, mainly in his mind and getting more difficult to care for and a lot of times makes it hard to want to care for him but everyone here at the facility is very good with him and have so much patience with him. He is not always easy to take care of and regardless if Don is having a good moment or bad someone is always here to lend a hand. I know some people would say it's "their job and what they get paid to do" however we can see these people care about their patient's needs. The center has always been clean and nurses have always been available anytime we have questions to ask whether it be lab results, doctor appts and getting the transport assigned for them to take Don to these appointments outside the facility. We have recommended this facility to many of our family and friends and will continue to do so. My MIL was also here at one point while my FIL was here as well. We have been a few places, and this by far has been the one place where we can leave from visiting our dad and feel safe knowing when we aren't here, he is getting cared for and we need not worry. I myself have been in the medical field for years and see these cna's ,nurses, PT/OT, doctors concerned about everyone and treat them as people and not just numbers. Even down to admissions, kitchen staff, and janitorial workers, everyone has been caring and helpful! The food here has been pretty good too. Also they have daily/weekly activities for everyone here as well and celebrate holidays for those that are here and can't be home with their loved ones. There are patios to sit outside and get fresh air as well as a large room in front of the facility where if we have a few visitors at once we will take our friends and family into for more private visits. This is also the room that is used for celebrations and pot lucks. The rehabilitation room is also decent size and has everything to assist someone to build up strength etc. (See pictures included with this post) Don will be coming back home soon. However, we know he will need care like this again at some point (unfortunately these visits are getting closer together as his disease progresses) and both Jason and I find extreme comfort in knowing we can call Lodi Skilled Nursing & Rehab again for our father's needs when the time comes.

The physical and occupational therapy was on point, they help me build my strength and taught me how to navigate stairs so I could go back home. The Director of Nursing, Hansen Khan, RN has a great team of RN's, LVN and CNA's that work at station 1. They took pride in the care they given to their patients. Mr. Haneff made sure he let the patients know they could come to him if they have any concerns with the nursing staff. I want to mention the womb care staff, the kitchen staff also was on point. I really liked how the patients can get their hair,and nails done, i love the daily activities they have. I thought it was cool how they included all the patients in celebration by walking through the halls and in patients rooms like Halloween for example they dressed up.My issues was the food was not great and certain times of the day it gets cold. When her care was less critical I would say it was bordering on neglect . A cna said she was in charge of 15 patents and almost half had to be spoon fed including my mother. Hence, myself and Family made sure to be there so she could eat. My observations were patients who didn't have family step in for them might of not ate well. I heard one rn supervisor talk extremely rough to like a dementia patient to be still and stop moving. It appeared as work burn out. There were many times mom was soiled and staff would not come to help. I had to change mom myself and do best I can to get her to the restroom, changed and brought back to bed. They had no sense of urgency in helping despite telling me to not take mom alone. I waited but when it started approaching two hours I had to do it . I also Had to help cna put mom To bed as she had no help as mom was too weak and a two person job. They got mom's diet order wrong as moved up to solids and kept on doing a blender diet for days. We had to Go out and buy food so she can eat as she hated blended diets and I don't blame her. I was told this happened cause dieticians use a different system and it don't data share with the facility but nobody could make a phone call. There was like a comittee but I don't think they were proactive (except supervisor of nursing) as made excuses For some of my concerns . I am not going to say this facility is the worse but definitely not the best. I was told this place was the best in town. I am Just going to say it's like any other nursing facility from what I hear. When I thought mom was well enough to go home I demanded she leave immediately as they wanted to keep mom longer. I overheard admin mentioning mom was a "good" patient as had dual insurance. I told them at my house mom is not neglected and will get better care as I had all her bags packed. I don't recommend this facility for your loved one and it's definitely not the best in town but can't say the worse cause I haven't seen many . It missed my high expectations and them trying to say get better care cause they are small is garbage.

She interacts with the patients so beautifully and it's obvious that she enjoys her job. There are many staff members, and they seem to be uplifting toward one another. It feels like a family. Dr. Garcha was excellent and I had a very thorough eye exam. He's down-to-earth, friendly and he took time to explain everything very clearly to me.
