THIS IS NOT THE PLACE TO SEND A LOVED ONE IF THEY NEED WOUND CARE OR OSTOMY CARE! I toured the facility before moving my grandma there, and they assured me that they had adequate staffing, great wound care, and were knowledgeable about ileostomies. This turned out to be completely false! I had to come and care for her many times due to their incompetence when it came to adhering the ostomy bag. I also had to purchase several medical supplies needed for it because they said that they could not get them. There is no excuse for this! It is all covered and supplied through her insurance. They would not regularly empty the bag as required, and it would constantly leak. Because they were doing it incorrectly, they were changing it up to 5 times a day, which is unheard of! An ostomy bag holds, on average, for 3 to 5 days when it is done correctly and emptied on schedule. Her wound stayed infected the entire time she was there and grew in size. It became so bad that when we left the facility, her doctor had to make it larger and start a wound vac. One staff member even swatted at my grandma's hand when they were changing her ostomy, and my grandma wanted to help. They were accusing my grandma of "fiddling with it" and causing it to leak. I really should have reported this facility to the state, but I did not have the time or energy. I hope our experience will help others find a better place for their loved one, and that this facility will take a good, long look at what is going on with the care they provide and make changes. I hope they will be honest with themselves and others when they consider accepting patients who need this type of care. I will commend the therapy staff. The only thing this place has going for them is great physical, occupational, and speech therapy. So if that is all you need, it might be an okay fit for you, but be aware of the nursing staff.

This facility was completely unprepared to release my 91-year-old mother an entire three (3) days…read morefollowing medicare notice of non-coverage; which was issued 3 weeks after her hip replacement surgery. Because they stated the doctor "left for vacation to Mexico," they could not provide me her medications, a list of her medications, and none of her prescriptions had been called into the pharmacy. Further, they did not have a cheap aluminum walker to send her home with. Because the head nurse had expressed she was confident she would be kept on after appeal, she left me scrambling to fly from out of state the following day after rejection. This all took place on a FRIDAY evening, with no options but to go the weekend without meds or a walker. I managed to arrive the following day, but the couldn't handle a basic discharge that is part of what they do every single day as a business. They SHOULD have kept my mother over the weekend at no charge to her to make up for their MAJOR blunder (instead they estimated about $500 per night out of pocket). When I found my mother waiting for me, they had a quarantine sign on her door but my mother wasn't sick. The nurse told me the sign had to do with her roommate who had left a couple days prior, so my other said she was not even being attended to. I would NOT recommend this facility if you're looking for a place your loved one will be treated like an actual human being, worthy of respect, and who will help with any sort of transition after discharge.

Subsequently, we found our she had fairly advanced terminal cancer. The level of care and kindness shown by the entire staff (thanks, Kelly, Nedra, Michael) really touched our hearts. It's not the fanciest place I've seen, but the kindness, love and level of care mean that your loved one will be well taken care of. Our mom ended up in hospice, and eventually passed away in January, 2020. Throughout her suffering and our grief, the staff was kind, supportive and always helpful. They took most excellent care of her, both physically and emotionally. Thanks to all of you.

Bit of a turnstile in terms of therapists. I was here for an injury that was not getting better and at some point threw in the towel. The people working here are nice enough, but there seems to be a weird vibe going on, which is, of course, not a positive when you need to recover. I did not feel they were equal or better to other therapists I've seen, and had some concerns over getting different feedback/diagnosis each time I saw a different person at this practice.
