We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. Their whole demeanor and attitude is that of angels, compassion, empathy, love..When he was in the hospital, he was not receiving the proper pain management, they kept delaying the hospitals hospice program while my dad was dying in pain. I was desperate to get him home. Alena, the administrator, moved mountains to make that happen. They got my dad home in less than 18 hours, they were by my father's bedside within two hours of me calling desperate for help, which felt like a miracle during such a difficult time. Daniel, the nurse who came to the hospital, was so attentive and caring such a such a genuine person. And once we were home, nurse Julie and nurse Grace took over and truly went above and beyond. They made sure the bed oxygen tanks supplies and meds was delivered before my dad even got home.They made sure my dad was comfortable, out of pain, and surrounded by dignity and respect--something he was not receiving in the hospital. there was also a Sweet CNA that came to help as well, Because of them, my dad was able to pass peacefully at home, just as he deserved. That means everything to me my father and our family. All of my father's friends and loyal family were able to come visit with him before he passed the next morning.. I highly recommend this hospice agency to anyone going through end-of-life care for a loved one--or even for themselves. Don't wait until the last minute. Have this hospice team set up ahead of time--you will not regret it. They are truly angels doing God's work. Shannon Johnson-parson

    My mom lives in the Northwest Suburbs and we finally decided that she could no longer "do it…read morealone". We had gone through a lot with my dad before he passed away and so we knew what we wanted and what we didn't. Unfortunately, the informal network we used for my dad didn't have women caregivers so we were back to to square one. After doing a lot of research with the local senior center and online we called For Papa's Sake, which is based out of the town next to where my mom lives. From the very first phone call I suspected we had gotten lucky. After a couple of calls and some group discussion between me, my mom and a For Papa's Sake senior person we decided we'd try them out. And am I glad we did. They listened to my mom's and my requests, likes, dislikes, deal-breakers etc. and they never batted an eye. They did say it might take a bit longer than usual (given our needs) to find the right care giver, but they called back in less than 5 days and we had a care giver within 10 days to 2 weeks of that 1st main discussion. That care giver has worked out so well. It has really improved our lives and I can go days without worrying about my mom...what a relief. That's what For Papa's Sake provides: relief! If you are looking for an alternative to a nursing home or aged care facility or whatever you want to call it, For Papa's Sake really cares and I would encourage you to give them a call. They have worked out great for us and I would not recommend them for something so important if they weren't doing a great job.

Then he ran out of money and prior to being approved by Medicaid and going to a nursing home he went to my sisters up north. We asked to transfer my Dads care from Gentiva to a branch that service the area. They did 2 evaluations and said he didn't meet the standard to receive the care. After being on hospice with the same company for 9 months. Said his dementia wasn't bad enough & he was walking fine. Well after just 10 days he fell last night and broke his hip. I do not understand how the same company could have such different standards. I would not recommend Gentiva if you are looking for compassionate care for your loved one.

My grandmother sadly needed to stay there for 4 days before she departed us. The facility is clean beyond belief. They worked on it every moment of every day. The staff took such good care of my grandmother. They continuously checked in on her to make sure she was comfortable and also rotated her and kept her clean. I have heard so many horror stories of facilities that just kept their patients drugged. Here, the continuously checked her need for medicine. The facility also cared for my entire family. They made sure we were okay and spoke to us in such a caring manner. They helped to keep us fed, comfortable, and sane during this horrible time. They had things to keep my children busy both inside and outside. I also thought the chapel was a calming place. The procession the staff shared in with my family as my grandmothers body was taken will be engrained in my memory. The staff really helped make this situation easier on all of us.

    Positives: The facility is nice and peaceful, depending on what other families are there…read more Nursing/PCN staff was mostly compassionate and kind. My mother-in-law was brought in on Mother's Day. We were originally told she was only approved for 14 days of hospice, no mention on what would happen if it took longer than 14 days. It took a week to hear from social worker regarding her situation because social worker was out of town. no on-site chaplain, but one was available upon request. Social worker promised support with FMLA and a note for work, never received either. My husband was promised bereavement/mental health support, but only after she passed. it's been over a week and nothing. We have to track down the social worker to do her job. The last week of her stay, several loud families were visiting patients in the hospice house, with no enforcement of rules, rowdy guests, or inappropriate behaviors after we were told the teenagers in our party need to be on good behavior. The last few days of her stay, visitors, nurses and staff were talking loudly outside of her room about their personal lives. Completely inappropriate. Have had to ask staff several times for pain meds for MIL, she would be moaning and saying "ow" - nobody would check on her. Would tell staff at desk to get her more meds, they said I had to tell the nurse in a closed-door office. Why is it so hard to identify who is actually providing care? We were told by staff to plan funeral arrangements and they would contact our provider. After she passed, we were told she was a coroner's case and had to go to the Cook County Medical Examiner. We were NOT told this and have no record of this conversation or procedures (but a handout with funeral homes/crematoriums). Staff insisted they told us she was a coroner's case. We have no record. She passed on Memorial Day, the nurse on-site was unprepared for this and kept us at the hospice after she passed to talk to coroner. Did not release us, PCN staff told me it was okay to clean out her fridge and go home. We did not know we would have to talk to the cops, nobody told us that anyone who is seen for a fall and passes within 1 year has to go to the ME. No clue what to expect. More trauma after a traumatic day. We have now had to contact the funeral home on our own and hope they transport my MIL from Cook County to their facility without incident. This has been botched from the get-go. No continuity in training, communication, or actual support for families. The social worker here does not care about the families at all.

    My aging parents moved here in 2014, and my dad is still there...at 94 and counting! They both…read morearrived with "mild cognitive deficits of the Alzheimer's type." Because the care and programming and staff are so family- and patient-centered, my brothers and sister and I have a kind of peace of mind and heart we never thought possible. All I can say is thank heavens they bought long-term care insurance (with no cap!) back in the late '90s. After mom passed in 2016, dad ultimately moved to the "Renaissance Care" floor, where every possible need he has is met. We even transferred his supplementary health insurance coverage plan to the Sunrise Advantage Plan, as it's far less costly than BCBS, and his care is even more comprehensive, with visiting providers instead of the disruption of him having to go to them, which is quite difficult in his current confused state. We were worried they wouldn't like the food, but they loved it from the start! I think the food at a place like this serves as a barometer for their feelings about being there, and from the start, although a bit bumpy at first (nobody wants to move away from home), they settled in nicely and made friends. I would highly recommend Sunrise to anyone considering assisted living.
